Diagnostic pitfalls in malignant gliomas： the analysis of misdiagnosis and current recommendations

The malignant glioma is characterized by intrinsic aggressiveness and carries a dismal prognosis. Thecurrent standard treatment regimen for patients with malignant gliomas, specifically glioblastoma, is a combined therapy comprised of surgical resection followed by adjunctive radiation and chemotherapy. Yet, even with this new multimodality treatment, glioblastoma （GBM） recurs after a median time of 7 months following diagnosis, requiring a second-line treatment. But the clinical decisions are always difficult.

Introduction: Malignant gliomas refer to grade III or IV brain tumors defined according to the World Health Organization (WHO) classification. They are a heterogeneous group of pathologies and represent a serious health problem by their frequency, severity and treatment difficulties. The prognosis of malignant gliomas remains poor despite all the medical advances. Materials and Methods: It is a retrospective study included 20 cases of malignant glial tumors treated at the medical oncology department, Fattouma Bourguiba hospital in Monastir between 2012 and 2016, according to the STUPP protocol. Results: These were 12 men and 8 women with a median age of 43. Clinical signs were not very specific, dominated by intracranial hypertension and deficit signs. Imagery referred to the diagnosis of malignant gliomas in 1st intention. Surgery consisted of a macroscopically complete exeresis in (15%) cases, a partial exeresis in (50%), the rest of the patients had a stereotactic biopsy. Histology found GBM in 16 patients (80%), 2 cases of Grade III anaplastic astrocytoma (10%), 1 case of anaplastic oligodendroglioma (5%), and 1 case of Grade III anaplastic eppendymoma (5%). ＜正＞ The oncogenic role of AKT2 in the development of malignant gliomas was examined by using antisense approach. AKT2 expression was significantly inhibited in rat C6 glioma cells transfected with antisense AKT2 cDNA construct （ LXSN - AS -AKT2）. In addition, the transfected cells proliferated at a lowered level and apoptosis was induced. Forin vivo studies, pa-rental C6 cells and C6 cells transfected with LXSN - AS - AKT2 were implanted stereotactically into the right caudate nucleus of SD rats （control C6 group and transfected group）. The rats bearing well established C6 gliomas were treated with LXSN -AS - AKT2 DNA or LXSN （ empty vector） - lipofectamine complexes intratumorally （treated group and control treated group） . The mean survival of the rats of control C6 group and treated control group was 17. 8 0. 92 days and 17. 5 1. 10 days, respectively. The mean survival of the rats of transfected and treated group was significantly prolonged. MR images revealed distinct cerebral tumor foci in all of the control rats, whereas four rats in transfected group did not develop tumors and the tumor foci in five rats of treated group were regressed and disappeared. The expression of AKT2, PCNA, MMP2/9, and cyc-lin D were inhibited in the tumors of rats in transfected and treated groups while GFAP expression was increased. These re-sults suggest that AKT pathway may play an important role in the development and progression of gliomas. Anti - AKT ap-proach will open a new perspective for a targeted molecular therapy of malignant gliomas.

Objective:To analyze the efficacy and side-effect of teniposide plus cisplatin regimen on malignant glioma ***:A total of 56 patients with histologically confirmed malignant gliomas（WHO grade III/IV） were *** patients were received radiotherapy following *** were 24 patients received adjuvant chemotherapy and the other 32 patients received salvage *** chemotherapy regimen comprised of VM-26 300mg/m2 plus DDP 80 mg/m2,which were divided for 3-5 days intravenous administration,every 3-4 weeks as a *** clinical efficacy and survival time of the patients were evaluated if the patients who were completed more than two cycles ***:Forty-nine patients were included for evaluation:complete remission（CR） was achieved in 2 cases（4.1%）and 6 cases（12.2%） were partial remission（PR）,while stable disease（SD）were found in 30 patients（61.3%）,and progressive disease（PD） were in 11 cases（22.4%）.Objective response rate（CR+PR） was 16.3%,and disease control rate（CR+PR+SD） was 77.6%.The progression-free survival（PFS） rate at 6 months was 55.1%.The survival rate at 1 year,2 years and 3 years was 63.3%,34.7%,and 20.4%,*** median overall survival time（OS） was 20.8 *** side-effect of the regimen was mainly hematological,with grade 3 or 4 neutropenia in twelve patients（24.5%）,and which was recovered in several days after receiving G-CSF ***:VM-26 plus DDP chemotherapy regimen can achieve fairly high response rate and disease control rate for patients with malignant gliomas,and the toxicity is acceptable.
