Summary What is already known about this topic?Coronavirus disease 2019(COVID-19)causes symptoms ranging from mild to severe.Indicators for identifying severe COVID-19 infection have not been well identified,especially for young patients.What is added by this report?Both neutrophil-lymphocyte ratio(NLR)[area under curve(AUC):0.80;the odds ratios(OR)and 95%confidence intervals(95%CI):1.30(1.13–1.50)]and platelet-lymphocyte ratio(PLR)[AUC:0.87;OR(95%CI):1.05(1.01–1.09)]were determined to be indicators for recognition of patients with severe COVID-19 in young patients less than age 40.What are the implications for public health practice?NLR and PLR are useful indicators for identifying patients with severe COVID-19,especially in young patients less than age 40.

Background:Although a variety of risk factors of pneumonia after clipping or coiling of the aneurysm(post-operative pneumonia[POP])in patients with aneurysmal subarachnoid hemorrhage(aSAH)have been studied,the predictive model of POP after aSAH has still not been well established.Thus,the aim of this study was to assess the feasibility of using admission neutrophil to lymphocyte ratio(NLR)to predict the occurrence of POP in aSAH patients.Methods:We evaluated 711 aSAH patients who were enrolled in a prospective observational study and collected admission blood cell counts data.We analyzed available demographics and baseline variables for these patients and analyzed the correlation of these factors with POP using Cox regression.After screening out the prognosis-related factors,the predictive value of these factors for POP was further assessed.Results:POP occurred in 219 patients(30.4%)in this cohort.Patients with POP had significantly higher NLR than those without(14.11±8.90vs.8.80±5.82,P10 had significantly worse POP survival rate than patients having NLR≤10.NLR at admission might be helpful as a predictor of POP in aSAH patients.

Different lipid sources differ in the fatty acid profiles and differently affect growth performance as well as immune function of broilers. The influences of different dietary lipid sources on growth performance, nutrient digestibility, blood T lymphocyte population, and cardiac antioxidant status were investigated of broilers. A total of 360 one-day-old male broilers(BW = 44 ± 3 g) were randomized into 3 treatment groups, consisting of 6 replicates with 20 birds in each group. Broilers received standard diets supplemented with 5%(wt/wt) of lard(LD, as a control diet), sesame oil(SO), or flaxseed oil(FO).Broilers in both SO and FO treatment groups had lower(P lymphocyte count and in CD4^+:CD8^+ ratio compared to LD treatment.Sesame oil and FO treatments increased cardiac glutathione peroxidase(P lymphocytes ratio of fast growing broilers.

Background:After neoadjuvant chemotherapy(NAC),non-pathological complete response of breast cancer patients can benefit from tailored adjuvant chemotherapy.However,it is difficult to select patients with poorer prognosis for additional adjuvant chemotherapy to maximize the benefits.Our study aimed to explore whether the subtypes of tumor-infiltrating lymphocytes(TILs)in residual tumors(RT)is related to the prognosis of triple-negative breast cancer(TNBC)after NAC.Methods:Data from patients with primary TNBC consecutively diagnosed at the Breast Disease Center of Peking University First Hospital from 2008 to 2014 were retrieved,and the cases with RT in the breast after NAC were enrolled.TILs subtypes in RT were observed by double-staining immunohistochemistry,and counted with the median TILs value per square millimeter as the cut-off to define high versus low TILs density in each subtype.The relationships between the TIL density of each subgroup and the clinicopathological characteristics of the RT after NAC patients were analyzed by Fisher exact test.Disease-free survival(DFS)and overall survival(OS)were analyzed by the Kaplan-Meier method and log-rank statistics.Results:A total of 37 eligible patients were included in this study,and the median follow-up period was 50 months(range 17–106 months).There was no significant correlation between the infiltrate density of CD4^+,CD8^+,CD20^+,and CD68^+lymphocytes and clinic-pathological characteristics.Significantly better prognosis was observed in patients with high CD4^+-TILs(DFS:P=0.005,OS:P=0.021)and high CD8^+-TILs(DFS:P=0.018)and low CD20^+-TILs(OS:P=0.042).Further analysis showed that patients with CD4^+/CD20^+ratio greater than 1(DFS:P=0.001,OS:P=0.002)or CD8^+/CD20^+ratio greater than 1(DFS:P=0.009,OS:P=0.022)had a better prognosis.Conclusions:Subtypes of TILs in RT is a potential predictive biomarker of survival in TNBC patients after NAC.

AIM: To investigate the infiltration and activation of lymphocyte in iris-ciliary body and anterior chamber after allogenic penetrating keratoplasty (PK), for further revealing the role of iris-ciliary body in corneal allograft immune rejection. METHODS: In the mice models of PK, BALB/C mice received orthotopic isografts (n =35) or C57BL/6 donor allografts (n=25). Grafts were examined daily for 3 weeks by slit-lamp microscopy and scored for opacity. The infiltration of CD4(+) T lymphocyte in iris-ciliary body and anterior chamber was examined by immunohistology and the mRNA of CD80 and CD86 in both cornea graft and iris-ciliary body by RT-PCR was analyzed in allograft recipient at days 3, 6, 10 and the day when graft rejection occurred. Isograft recipients were examined as control at the corresponding time points. Transmission electron microscope was used to study the ultrastructure, especially cell infiltration, of iris-cilary body and corneal graft at day 3, 7 and the day when rejection occurred after allogenic PK. RESULTS: Rejection was observed in all the allograft recipients followed more than 10 days, at a median time of 15 days (range 12-18 days), but not in any of isografts. CD4(+) T cells were first detected at day 6 after transplantation in limbus and Ciliary body, and then in the stroma of recipient, iris, anterior chamber and corneal allograft with an increased number until graft rejection occurred. CD80 and CD86 mRNA were detected under RT-PCR examination in both graft and iris-ciliary body of allograft recipient, but not in any of isograft recipient. Three days after operation, lymphocytes and monocytes macrophages were visible in iris blood vessels and the anterior chamber, and vascular endothelial cell proliferation and activation were significant under transmission electron microscopy examination. At day 7, corneal endothelial cells became thinner. lymphocytes and mononuclear macrophages were found with great number in the anterior chamber and adhered to the corneal endothelium. Objective: To investigate the potential efficacy of panaxadiol saponins component(PDS-C) in the treatment of aplastic anemia(AA) model mice. Methods: Totally 70 mice were divided into 7 groups as follows: normal, model, low-, medium-, high-dose PDS-C(20, 40, 80 mg/kg, namely L-, M-, H-PDS-C), cyclosporine(40 mg/kg), and andriol(25 mg/kg) groups, respectively. An immune-mediated AA mouse model was established in BALB/c mice by exposing to 5.0 Gy total body irradiation at 1.0 Gy/min, and injecting with lymphocytes from DBA mice. On day 4 after establishment of AA model, all drugs were intragastrically administered daily for 15 days, respectively, while the mice in the normal and model groups were administered with saline solution. After treatment, the peripheral blood counts, bone marrow pathological examination, colony forming assay of bone marrow culture, T lymphocyte subpopulation analysis, as well as T-bet, GATA-3 and Fox P3 proteins were detected by flow cytometry and Western blot. Results: The peripheral blood of white blood cell(WBC), platelet, neutrophil counts and hemoglobin(Hb) concentration were significantly decreased in the model group compared with the normal group(all P<0.01). In response to 3 dose PDS-C treatment, the WBC, platelet, neutrophil counts were significantly increased at a dose-dependent manner compared with the model group(all P<0.01). The myelosuppression status of AA was significantly reduced in M-, H-PDS-C groups, and hematopoietic cell quantity of bone marrow was more abundant than the model group. The colony numbers of myeloid, erythroid and megakaryocytic progenitor cells in the model group were less than those of the normal mice in bone marrow culture, while, PDS-C therapy enhanced proliferation of hematopoietic progenitor cells by significantly increasing colony numbers(all P<0.01). Furthermore,PDS-C therapy increased peripheral blood CD3^+ and CD3^+CD4^+ cells and reduced CD3^+CD8^+ cells(P<0.05 or P<0.01). The efficacy of salvage interferon-α(IFN-α) treatment was investigated in patients with unsatisfactory response to minimal residual disease (MRD)-directed donor lymphocyte infusion (DLI)(n=24). Patients who did not become MRD-negative at 1 month after DLI were those with unsatisfactory response and were eligible to receive salvage IFN-α treatment within 3 months of DLI. Recombinant human IFN-α-2b injections were subcutaneously administered 2–3 times a week for 6 months. Nine (37.5%), 6 (25.0%), and 3 (12.5%) patients became MRD-negative at 1, 2, and>2 months after the salvage IFN-α treatment, respectively. Two-year cumulative incidences of relapse and non-relapse mortality were 35.9% and 8.3%, respectively. Two-year probabilities of event-free survival, disease-free survival, and overall survival were 51.6%, 54.3%, and 68.0%, respectively. Outcomes of patients subjected to salvage IFN-α treatment after DLI were significantly better than those with persistent MRD without IFN-α treatment. Moreover, clinical outcomes were comparable between the salvage DLI and IFN-α treatment groups. Thus, salvage IFN-α treatment may help improve the outcome of patients with unsatisfactory responses to MRD-directed DLI and could be a potential salvage treatment for these patients after allogeneic hematopoietic stem cell transplantation.

AIM:To compare lymphocyte subsets between healthy controls and alcoholics with liver disease.METHODS:The patient cohort for this study included individuals who were suspected to have alcoholic liver disease(ALD) and who had undergone liver biopsy(for disease grading and staging,doubts about diagnosis,or concurrent liver disease;n = 56).Normal controls included patients who were admitted for elective cholecystectomy due to non-complicated gallstones(n = 27).Formalin-fixed,paraffin-embedded liver biopsy specimens were sectioned and stained with hematoxylin and eosin and Perls' Prussian blue.The non-alcoholic steatohepatitis score was used to assess markers of ALD.lymphocyte population subsets were determined by flow cytometry.T lymphocytes were identified(CD3+),and then further subdivided into CD4+ or CD8+ populations.B lymphocytes(CD19+) and natural killer(NK) cell numbers were also measured.In addition to assessing lymphocyte subpopulation differences between ALD patients and controls,we also compared subsets of alcoholic patients without cirrhosis or abstinent cirrhotic patients to normal controls.RESULTS:The patient cohort primarily consisted of older men.Active alcoholism was present in 66.1%.Reported average daily alcohol intake was 164.9 g and the average lifetime cumulative intake was 2211.6 kg.Cirrhosis was present in 39.3% of the patients and 66.1% had significant fibrosis(perisinusoidal and portal/periportal fibrosis,bridging fibrosis,or cirrhosis) in their liver samples.The average Mayo end-stage liver disease score was 7.6.No hereditary hemochromatosis genotypes were found.ALD patients(n = 56) presented with significant lymphopenia(1.5 × 109/L ± 0.5 × 109/L vs 2.1 × 109/L ± 0.5 × 109/L,P lymphocyte subpopulations,except for NK lymphocytes:CD3+(1013.0 ± 406.2/mm3 vs 1523.0 ± 364.6/mm3,P < 0.0001),CD4+(713.5 ± 284.7/mm3 vs 992.4 ± 274.7/mm3,P < 0.0001),CD8+(262.3 ± 140.4/mm3 vs 478.9 ± 164.6/mm3,P < 0.0001),and CD19+(120.6 ± 76.1/mm3 vs 264.6 ± 88.0/mm3,P < 0.0001).CD8+

Objective To study the association between the expression of microRNA-155(miRNA-155)in peripheral blood CD4^+T lymphocytes and the level of semrn interferon-7(IFN-7)concentration and the severity of coronary artery disease (CAD).Methods After coronary angiography,252patients with suspected unstable angina pectoris (UAP)were divided into the UAP group (128patients with CAD confirmed by angiography)and the control group (124patients without CAD confirmed by angiography).Fresh peripheral blood was extracted 16-24h before coronary angiography,CD4^+T lymphocytes was tested using immunomagnetic beads,the expression ofmiRNA-155was tested using quantitative PCR and the expression of IFN-7was tested using enzyme-linked immunosorbent assay (ELISA).According to the results of angiography,Gensini score of coronary artery lesions was analyzed.Furthermore,we also analysis the association between the level of miRNA-155in peripheral blood CD4^+T lymphocytes,the level of serum IFN-γand Gensini score of coronary lesion.Results The levels ofmiRNA-155(0.49±0.08vs.0.23±0.09)and IFN-7(227.58±26.01vs.141.23±17.89)in the UAP group were significantly higher than that of the control group,the difference was statistically significant.The level of miRNA-155and IFN-γwere positively correlated with Gensini score of CAD (r =0.534,r =0.713,respectively,all P lymphocytes and the level of IFN-γ are closely correlated with the severity of CAD.
