气候变化背景下,高温会影响职业人群的工作效率并造成劳动生产力损失.本研究基于区域气候模式并采用本土化的暴露-反应函数,预估了未来气候变化情景下中国因高温造成的劳动时间损失.研究发现,与基线期的213亿小时高温相关劳动时间损失相比,到21世纪末在RCP2.6、RCP4.5和RCP8.5情景下,损失将分别增加-17.8%、10.8%及121.1%.损失主要集中在华南、华中和华东地区,其中广东与河南将占到全国总损失的四分之一.因为这些区域的温度将更高、人口更密集、户外工作者在总人群中的比例较大.与RCP2.6、RCP4.5和RCP8.5情景相比,若能实现1.5℃温控目标,将分别能够避免11.8%、33.7%和53.9%的劳动时间损失,相当于避免了全中国21世纪中叶的0.1%、0.6%及1.4%的年均GDP损失.本研究显示未来气候变化会给中国造成严重的热相关劳动生产力损失,并强调亟需采取因地制宜的气候减缓及适应策略以保护职业人群健康.本研究对其他与中国相似的发展中国家同样具有重要意义.

Optimum interval time of programmed intermittent epidural bolus of ropivacaine 0.08%with sufentanyl 0.3 mg/mL for labor analgesia:a biased-coin up-and-down sequential allocation trial

Background:The programmed intermittent epidural bolus(PIEB)technique is widely used in labor analgesia,but the parameter settings of PIEB have not yet been standardized.We designed a study to identify the optimal interval duration for PIEB using 10 mL of ropivacaine 0.08%and sufentanyl 0.3 mg/mL,a regimen commonly used to control labor pain in China,to provide effective analgesia in 90%of women during the first stage of labor without breakthrough pain.Methods:We conducted a double-blind sequential allocation trial to obtain the effective interval 90%(EI90%)during the first stage of labor between April 2019 and May 2019.This study included the American Society of Anesthesiologists physical status II–III nulliparous parturients at term,who requested epidural analgesia.The bolus volume was fixed at 10 mL of ropivacaine 0.08%with sufentanyl 0.3 mg/mL.Participants were divided into four groups(groups 60,50,40,and 30)according to the PIEB intervals(60,50,40,and 30 min,respectively).The interval duration of the first parturient was set at 60 min and that of subsequent parturients varied according to a biased-coin design.The truncated Dixon and Mood method and the isotonic regression analysis method were used to estimate the EI90%and its 95%confidence intervals(CIs).Results:Forty-four women were enrolled in this study.The estimated optimal interval was 44.1 min(95%CI 41.7–46.5 min)and 39.5 min(95%CI 32.5–50.0 min),using the truncated Dixon and Mood method and isotonic regression analysis,respectively.The maximum sensory block level above T6 was in nearly 20%of parturients in group 30;however,5.3%,0%,and 0%of the parturients presented with sensory block level above T6 in groups 40,50,and 60,respectively.There were no cases of hypotension and only one parturient complained of motor block.Conclusion:With a fixed 10 mL dose of ropivacaine 0.08%with sufentanyl 0.3 mg/mL,the optimal PIEB interval is about 42 min.Further studies are warranted to define the efficacy of this regimen throughout all stages of labor.

Concerned topics of epidural labor analgesia:labor elongation and maternal pyrexia:a systematic review

Objective:labor is a complex process and labor pain presents challenges for analgesia.Epidural analgesia(EA)has a well-known analgesic effect and is commonly used during labor.This review summarized frequently encountered and controversial problems surrounding EA during labor,including the labor process and maternal intrapartum fever,to build knowledge in this area.Data sources:We searched for relevant articles published up to 2019 in PubMed using a range of search terms(eg,“labor pain,”“epidural,”“analgesia,”“labor process,”“maternal pyrexia,”“intrapartum fever”).Study selection:The search returned 835 articles,including randomized control trials,retrospective cohort studies,observational studies,and reviews.The articles were screened by title,abstract,and then full-text,with a sample independently screened by two authors.Thirty-eight articles were included in our final analysis;20 articles concerned the labor process and 18 reported on maternal pyrexia during EA.Results:Four classic prospective studies including 14,326 participants compared early and delayed initiation of EA by the incidence of cesarean delivery.Early initiation following an analgesia request was preferred.However,it was controversial whether continuous use of EA in the second stage of labor induced adverse maternal and neonatal outcomes due to changes in analgesic and epidural infusion regimens.There was a high incidence of maternal pyrexia in women receiving EA and women with placental inflammation or histologic chorioamnionitis compared with those receiving systemic opioids.Conclusions:Early EA(cervical dilation≥1 cm)does not increase the risk for cesarean section.Continuous epidural application of low doses of analgesics and programmed intermittent epidural bolus do not prolong second-stage labor duration or impact maternal and neonatal outcomes.The association between EA and maternal pyrexia remains controversial,but pyrexia is more common with EA than without.A non-infectious inflammatory process is an accepted mechanism of epidural-relat

Traditional medicine use during pregnancy and labor in African context:A scoping review

Traditional medicine(TM)has been more popular among pregnant women worldwide and has played a significant part in maternal health-care services in many nations.Herbs,herbal preparations,and finished herbal products all contain active substances that are derived from plant parts or other plant components that are thought to have medicinal advantages.To diagnose,prevent,and treat illnesses as well as to enhance general well-being,about 80%of people use a variety of TM,including herbal remedies.A systematic search of Google Scholar and PubMed was performed utilizing an established scoping review framework by Joanna Briggs Institute from January 2012 to December 2022.A consequent title and abstract review of articles published on TM in the African context were completed.Of over 15,000 published studies identified,15 meeting the inclusion criteria were integrated into the following seven categorical themes:prevalence of TM use,source of information on TM use,reasons for use of TM,route of administration,common herbs used in pregnancy and labor,the effect of herbs used in pregnancy and labor,and predictors of use of TM.The studies reviewed were primarily in the context of an African setting on the use of TM regarding herbal medicine.Of all the articles,the highest number of studies was conducted in Zimbabwe.This review shows increased use of TM by women during pregnancy and labor with a reported prevalence rate varying from 12%to 60%.However,a decrease in use in the third trimester of pregnancy was reported.The most frequent source of information on the use of TM was from family and friends,while age,parity,education,and income were factors affecting use.In conclusion,the participants do not often disclose the use of TM during their antenatal attendance and the reason for use was accessibility and cost.Therefore,there is a need for further study on the safety and efficacy of TM use in pregnancy and labor.

Traditional medicine(TM)has played an essential part in maternity services around the world,which has led to increased utilization among pregnant women.Herbs,herbal preparations,and completed herbal products are examples of TMs that contain active substances such as plant parts or other plant components that are thought to have therapeutic advantages.This study review aimed to identify the herbs commonly used,reasons for use,and effect of use,to make adequate recommendations on herbal medicine use as a remedy for pregnancy and labor.Incorporating evidence from reviews,personal correspondence,and diaries,this study demonstrates that about 80%of people used TM such as herbal remedies for sickness diagnosis,prevention,treatment,and promotion of general well‑being.Due to its accessibility,cost,and availability,TM is usually used by expectant mothers.Examples of TM used in pregnancy and labor include honey,aloe,raspberry,jute mallow,and hibiscus leaves.It is important to note that its use in pregnancy and labor can be beneficial or harmful to both mother and child.Lack of standardization,financial risk,lack of safety,and effectiveness are challenges to TM.There is a need of creating awareness of the safe use and effects of TM in pregnancy and labor through the provision of health education programs for women in the community.

Is the feminization of labor harmful to agricultural production? The decision-making and production control perspective

Even today,academics continue to debate the effect of feminization of agricultural labor force on agricultural output.By considering the dimensions of participation in decision-making and production,this study divides the various agricultural production models into three types:(i)the traditional model of decisions made either jointly by men and women or by men alone while both genders participate in production,(ii)complete feminization of agricultural decision-making and the production labor force,and(iii)feminization of the agricultural production labor force only.This study investigates the effects of combining or separating decision-making and production in regard to agricultural development in the context of feminization of the agricultural labor force.Using follow-up data collected from 2004–2008 by the Ministry of Agriculture of China,we built a comprehensive panel data model to test our hypotheses.Our research shows that in comparison to traditional agricultural households and fully feminized agricultural labor forces,partially feminized production resulted in lower grain yield and technological advancement.The feminization of agricultural labor does not necessarily have a negative impact on agricultural output,especially since heavy manual labor is being increasingly replaced by agricultural machinery and outsourcing of tasks.The degree of feminization of the decision-making and production processes should be an important consideration when evaluating the purported negative effects of the feminization of agricultural labor.

More children nutrition distribution, less labor incentive: Evidence from Chinese collective agriculture

Distribution system will affect the labor incentive that has been heatedly discussed by recent literatures.Using a unique micro dataset, this paper dem on strates that the equalitaria n distributi on system is one of the reas ons for the in sufficie nt labor incentive during the Chinese Collective Agriculture period. Specifically speaking, in the distribution of basic rations, the proportion for children (aged 1-3 and 4-7 years) was often beyond their nutrition demand, resulting the dissatisfaction of other families with more laborers and less children, thus these households will reduce their labor supply gradually. At the same time, the existence of outstanding accounts makes it a failure to use work points to buy distributions, which is the mechanism of the distribution system and insufficient labor incentive. All the results have been accepted by the robustness tests. The study will help to understand the distribution system and labor incentive, as well as the failure of the Chinese collective agriculture.

An Early Pregnant Chinese Woman with Cerebral Venous Sinus Thrombosis Succeeding in Induction of labor in the Second Trimester

Cerebral venous sinus thrombosis(CVST)is a rare condition in early pregnancy.A22-year-old Chinese woman at10weeks of pregnancy requested induced abortion and was diagnosed as CVST for a severe headache accompanying with nausea and vomiting.The patient was treated successfully with anticoagulation,followed by amniocentesis,and finally succeeded in induction of labor safely.The diagnosis,treatment and prognosis for this rare condition are discussed in this paper.

Establishment and verification of labor demand estimation model in planting industry

In view of the poor precision of the theoretical model of labor demand estimation,it is difficult to estimate and predict the actual production problems accurately.Based on the actual production conditions and the relationship between the degree of mechanization of planting and the demand of labor force,this study established an estimation model for the labor demand of planting industry considering the factors of planting structure and mechanization degree.In order to ensure high reliability of data,the method of checking out abnormal data was adopted to obtain the cultivated land area index when the mechanization degree is from 0 to 100%.Taking Suihua region(Heilongjiang Province,China)as an example,the theory of the research was analyzed and applied.This study accessed to the data of cultivated land area per labor can afford when the mechanization level in Suihua area were 0 and 100%respectively through the investigation,and the average cultivated land area data of each labor force in two cases were sorted out and the abnormal data were eliminated at the same time.Finally,using the derived model,the data obtained and the mechanization level and cultivated land area of Suihua in the future,the labor demand amount in Suihua area from 2015 to 2019 were predicted.The model established in this study can be used to calculate the quantity of both current labor demand in planting industry and the labor demand in the various moments in the future through forecasting the future mechanization level and cultivated area which are the two main factors influencing the quantity of labor demand in planting structure.

Preventive and therapeutic effect of N-Acetyl-L-cysteine on infection-associated preterm labor in mice

Objective:To study the preventive and therapeutic effect of N-Acetyl-L-cysteine on infectionassociated preterm labor in mice.Methods:A total of 66 C57BL/6 inbred strain pregnant mice were selected and randomly divided into groups A,B and C,with 22 cases in each group.Group A,B and C were regarded as model group,prevention group and treatment group,respectively.The model of infection-associated preterm labor was built by intraperitoneal injection of Escherichia coli.Ten mice of each group were taken and observed the preterm birth rates and live birth rates,respectively.Three mice of each group were killed at 3 h,6 h,12 h and 24 h after building the model.Their uterus tissues were collected and the expressions of the AP-1 and MCP-1 in those tissues were assayed with immunohistochemical method and the expressions of NF-κ Bp65 and TNF- protein in the placenta tissues of those mice were also detected with immunohistochemical method.Results:The preterm birth rates of mice in groups B and C were significantly lower than that in group A,while their live birth rates were distinctly higher than that in group A(P0.05).Conclusions:N-Acetyl-L-cysteine can lower the incidence rate of infection-associated preterm labor by prohibiting the activation of the protein AP-1/MCP-1and decreasing the expression of NF- κ Bp65 and TNF- in the pregnant tissues of premature mice to reduce the inflammatory reactions.
