Kenya, a lower-middle-income country in East Africa, faces a rising burden of chronic kidney disease (CKD), with an estimated 12,500 individuals suffering from end-stage renal disease (ESRD). Renal transplantation—the preferred treatment option for ESRD, remains underutilized. Since the first transplant in 1978, seven centers have been established, with 829 transplants performed by 2022. Living-related renal transplants (LRRT) dominate, while deceased donor renal transplantation (DDRT) is yet to be implemented. Recent data show improved outcomes, with one-year graft survival rates up to 96%, but challenges such as acute rejection rates (32.8%) and limited donor outcome data persist. Barriers include high costs, limited insurance coverage, inadequate laboratory infrastructure, and a transplant workforce shortage. Efforts to establish DDRT programs are underway but are hampered by the absence of organ procurement systems and insufficient laboratory capabilities. Future priorities include reducing costs and expanded insurance coverage for transplant care. Investments in laboratory infrastructure, local tissue typing, and surgical training are essential. Strengthening international collaborations and public education campaigns can improve donor pools and transplantation access. Strategic policy reforms and resource allocation are vital to scaling up Kenya’s kidney transplant program and addressing the unmet needs of its ESRD population.

FoxP3^(+)regulatory T cells(Tregs)play crucial roles in peripheral immune *** addition,Tregs that reside or accumulate in nonlymphoid tissues,called tissue Tregs,exhibit tissue-specific functions and contribute to the maintenance of tissue homeostasis and *** an experimental mouse model of crescentic glomerulonephritis induced by an anti-glomerular basement membrane antibody,Tregs started to accumulate in the kidney on day 10 of disease onset and remained at high levels(~30–35%of CD4^(+)T cells)during the late stage(days 21–90),which correlated with stable disease *** depletion on day 21 resulted in the relapse of renal dysfunction and an increase in Th1 cells,suggesting that Tregs are essential for disease control during the convalescence *** Tregs that accumulated in the kidney showed tissue Treg phenotypes,including high expression of GATA3,ST2(the IL33 receptor subunit),amphiregulin(Areg),and PPARγ.Although T-bet^(+) Tregs and RORγt^(+) Tregs were observed in the kidney,GATA3^(+) Tregs were predominant during the convalescence stage,and a PPARγagonist enhanced the accumulation of GATA3^(+) Tregs in the *** understand the function of specific genes in kidney Tregs,we developed a novel T cell transfer system to T cell-deficient *** experiment demonstrates that ST2,Areg,and CCR4 in Tregs play important roles in the accumulation of GATA3^(+) Tregs in the kidney and in the amelioration of renal *** data suggest that GATA3 is important for the recruitment of Tregs into the kidney,which is necessary for convalescence after renal tissue destruction.

Chronic kidney disease (CKD) is estimated to affect 10–14% of global population. kidney fibrosis, characterized by excessiveextracellular matrix deposition leading to scarring, is a hallmark manifestation in different progressive CKD;However, at present noantifibrotic therapies against CKD exist. kidney fibrosis is identified by tubule atrophy, interstitial chronic inflammation andfibrogenesis, glomerulosclerosis, and vascular rarefaction. Fibrotic niche, where organ fibrosis initiates, is a complex interplaybetween injured parenchyma (like tubular cells) and multiple non-parenchymal cell lineages (immune and mesenchymal cells)located spatially within scarring areas. Although the mechanisms of kidney fibrosis are complicated due to the kinds of cellsinvolved, with the help of single-cell technology, many key questions have been explored, such as what kind of renal tubules areprofibrotic, where myofibroblasts originate, which immune cells are involved, and how cells communicate with each other. Inaddition, genetics and epigenetics are deeper mechanisms that regulate kidney fibrosis. And the reversible nature of epigeneticchanges including DNA methylation, RNA interference, and chromatin remodeling, gives an opportunity to stop or reverse kidneyfibrosis by therapeutic strategies. More marketed (e.g., RAS blockage, SGLT2 inhibitors) have been developed to delay CKDprogression in recent years. Furthermore, a better understanding of renal fibrosis is also favored to discover biomarkers of fibroticinjury. In the review, we update recent advances in the mechanism of renal fibrosis and summarize novel biomarkers andantifibrotic treatment for CKD.

DNA tetrahedro n nano structure (DTN) is one of the simplest DNA nano structures and has bee n successfully applied for biose nsin g, imagi ng, and treatment of can cer. To facilitate its biomedical applications and pote ntial clinical tran slation, fun dame ntal un derstandi ng of DTN's transportation among major organs in living organisms becomes increasingly important. Here, we describe the efficient renal clearanee of DTN in healthy mice by using positron emission tomography (PET) imaging. The kidney elimination of DTN was later applied for renal function evaluation in murine models of unilateral ureteral obstruction (UUO). We further established a mathematical program of DTN to validate its changes of transportation pattern in healthy and UUO mice. We believe the establishment of pharmacokinetic profiles and mathematical model of DTN may provide in sight for future optimization of DNA nano structures for biomedical applications.

The biochemical effects of gadolinium chloride were studied using high-resolution IH nuclear magnetic resonance （NMR） spec- troscopy to investigate the biochemical composition of tissue （liver and kidney） aqueous extracts obtained from control and gadolinium chloride （GdCl3）（10 and 50 mg/kg body weight, intraperitoneal injection, i.p.） treated rats. Tissue samples were collected at 48, 96 and 168 h p.d. after exposure to GdCI3, and extracted using methanol/chloroform solvent system. ^1H NMR spectra of tissue extracts were analyzed by pat- tern recognition using principal components analysis. The liver damages caused by GdCl3 were characterized by increased succinate and decreased glycogen level and elevated lactate, alanine and betaine concentration in liver. Furthermore, the increase of creatine and lactate, and decrease of glutamate, alanine, phosphocholine, glycophosphocholine （GPC）, betaine, myo-inositol and trimethylamine N-oxide （TMAO） levels in kidney illustrated kidney disturbance induced by GdCl3.

Investigation on Acute Biochemical Effects of Ce（NO3 ）3 on Liver and kidney Tissues by MAS 1H NMR Spectroscopic-Based Metabonomic Approach

High resolution magic angle spinning （MAS）-^1H nuclear magnetic resonance （NMR） spectroscopic-based metabonomic approach was applied to the investigation on the acute biochemical effects of Ce（NO3）3. Male Wistar rats were administrated with various doses of Ce（NO3）3（2, 10, and 50 mg·kg^- 1 body weight）, and MAS ^1H NMR spectra of intact liver and kidney tissues were analyzed using principal component analysis to extract toxicity information. The biochemical effects of Ce（NO3）3 were characterized by the increase of triglycerides and lactate and the decrease of glycogen in rat liver tissue, together with an elevation of the triglyceride level and a depletion of glycerophosphocholine and betaine in kidney tissues. The target lesions of Ce（NO3）3 on liver and kidney were found by MAS NMR-based metabonomic method. This study demonstrates that the combination of MAS ^1H NMR and pattern recognition analysis can be an effective method for studies of biochemical effects of rare earths.

BACKGROUND Inflammatory myofibroblastic tumor(IMT) is a rare mesenchymal tumor that is characterized by spindle cells differentiated from muscle fibroblasts and infiltration of various types of inflammatory cells. IMT can occur at any age and at any anatomic site. The most common location of IMT is the bladder in the genitourinary tract. Only scarce cases of kidney IMT have been reported in the *** SUMMARY A 77-year-old woman, with a history of bilateral renal calculus for 15 years, was admitted to the Department of Urology of our hospital complaining of recurrent painless gross hematuria for one month. The treatment with cephalosporin was ineffective. Computed tomography imaging showed a mixed density and slightly heterogeneously enhanced lesion in the middle pole of the left kidney and ipsilateral adrenal enlargement. The patient underwent surgical treatment by retroperitoneoscopic left radical nephrectomy plus adrenalectomy. A large number of typical spindle cells surrounded by plasma cells and lymphocytes were observed microscopically. Immunohistochemical analyses indicated that these spindle cells were positive for vimentin, cytokeratin(CK), Ki-67, CK7,CD34, and CD31 and were focally positive for CD10 and anaplastic lymphoma kinase(ALK-1). Thus, a diagnosis of IMT was made definitively. The patient recovered well after operation, and no recurrence or metastasis was noted during the 22-mo *** Since kidney IMT is very rare and lacks characteristic clinical manifestation, it is easily misdiagnosed as a malignant tumor before operation. Surgery remains the best choice for diagnosis and treatment, and such cases must be followed carefully because of the uncertain biological behavior of this tumor. This report suggests that renal calculus may be one of the causes of IMT, but further investigation is necessary to prove it.
