Impact of continuous local lavage on pancreatic juice-related postoperative complications: Three case reports

BACKGROUND Postoperative pancreatic leakage readily results in intractable pancreatic fistula and subsequent intraperitoneal abscess.This refractory complication can be fatal;therefore,intensive treatment is important.Continuous local lavage (CLL) has recently been reevaluated as effective treatment for severe infected pancreatitis,and we report three patients with postoperative intractable pancreatic fistula successfully treated by CLL.We also discuss our institutional protocol for CLL for postoperative pancreatic fistula.CASE SUMMARY The first patient underwent subtotal stomach-preserving pancreaticoduodenectomy,and pancreatic leakage was observed postoperatively.Intractable pancreatic fistula led to intraperitoneal abscess,and CLL near the pancreaticojejunostomy site was instituted from postoperative day (POD) 8.The abscess resolved after 7 d of CLL.The second patient underwent distal pancreatectomy.Pancreatic leakage was observed,and intractable pancreatic fistula led to intraperitoneal abscess near the pancreatic stump.CLL was instituted from POD 9,and the abscess resolved after 4 d of CLL.The third patient underwent aneurysmectomy and splenectomy with wide exposure of the pancreatic parenchyma.Endoscopic retrograde pancreatic drainage was performed on POD 15 to treat pancreatic fistula;however,intraperitoneal abscess was detected on POD 59.We performed CLL endoscopically via the transgastric route because the percutaneous approach was difficult.CLL was instituted from POD 63,and the abscess resolved after 1 wk of CLL.CONCLUSION CLL has therapeutic potential for postoperative pancreatic fistula.

A novel aeration strategy in repeated-batch fermentation for efficient ethanol production from sweet sorghum juice

To improve the efficiency of ethanol production in a batch fermentation from sweet sorghum juice under a very high gravity(VHG)condition(~290 g/L of total sugar)by Saccharomyces cerevisiae NP01,repeatedbatch fermentation under an aerated condition(2.5 vvm for the first 4 h during every cycle)was done in a5-L fermenter.The average ethanol concentration(P),productivity(Qp)and yield(Yp/s)for five successive cycles were 112.31 g/L,1.55 g/L·h^-1 and 0.44,respectively with 80.97%sugar consumption.To complete sugar consumption,the total sugar of the juice was reduced to a high gravity(HG)level(~240 g/L).The results showed that yeast extract was not necessary for ethanol production,and aeration during every other cycle i.e.,alternating cycles,was sufficient to promote both yeast growth and ethanol production.The average P,Qpand Yp/svalues for eight successive cycles with aeration during alternating cycles were97.58 g/L,1.98 g/Láh and 0.41,respectively with 91.21%sugar consumption.The total fatty acids in the yeast cells under the aerated condition were^50%higher than without aeration,irrespective the initial sugar concentration,whereas the ergosterol contents under aeration condition were^29%to 49%higher than those without aeration.

Effects of tomato juice on the pharmacokinetics of CYP3A4-substrate drugs

We previously demonstrated that tomato juice(TJ) contains potent mechanism-based inhibitor(s) of CYP3A4. In this study, we investigated the effects of TJ and grapefruit juice(GFJ)on the pharmacokinetics of the CYP3A4-substrate drugs, nifedipine(NFP) and midazolam(MDZ), in male Wistar rats. Oral administration of GFJ 90 min before the intraduodenal administration of NFP or MDZ increased the area under the concentration–time curves(AUCs)of NFP and MDZ by 32.4% and 89.4%, respectively. TJ increased MDZ blood concentrations and AUC after intraduodenal MDZ administration;however, it had no effect on NFP. When MDZ and NFP were intravenously administered, GFJ significantly increased the AUC of MDZ,but only slightly increased that of NFP. In contrast, TJ only slightly increased the AUC of MDZ.These results suggest that, similar to GFJ, TJ influences the pharmacokinetics of CYP3A4-substrate drugs;however, it may be a drug-dependent partial effect.

Chemical composition, antioxidant activity and GC-MS analysis of juice and peel oil of grapefruit varieties cultivated in India

Citrus family especially Grapefruit,has attained considerable attention due to the presence of a number of essential components that have cardiovascular and anti-hypertensive properties.The juice and essential oil extracted from eight recently released grapefruit cultivars were used to study physicochemical and antioxidant properties.The total soluble solids(TSS),titratable acidity(TA) and pH of juice samples extracted from various grapefruit varieties differed significantly.The refractive index,specific gravity and optical rotation values for the oil varied from 1.473 to 1.396,0.863 to 0.847 and +93 to +86,respectively.The percent 2,2-diphenyl-1-picrylhydrazyl radical activity(% DPPH activity) and ferric reducing antioxidant power(FRAP) values for grapefruit juice and peel oil varied from 24.06 to 18.79,2.91 to 1.44 mmol g^–1 and 84.87 to 74.73,7.76 to 5.73 mmol g^-1,respectively.There were significant differences in physicochemical,antioxidant properties and volatile profiles of extracted juice and oil.The oil exhibited higher DPPH and FRAP values than the juice.Among different components identified which accounted for over 99% of the volatile fraction,limonene,myrcene,and benzopyran were major components in all oil samples.

Potential prognostic biomarkers in pancreatic juice of resectable pancreatic ductal adenocarcinoma

Despite potentially curative surgery pancreatic cancer has a dismal prognosis.Serum cancer antigen 19-9(CA 19-9) correlates with tumor burden,resectability and survival in patients with pancreatic ductal adenocarcinoma.Identification of novel biomarkers may facilitate early diagnosis of pancreatic cancer and improve survival.Pancreatic juice is a rich source of cancer-specific proteins rendering it a promising tool for identifying biomarkers.Recent proteomic and micro RNA expression analyses have identified several biomarkers of potential diagnostic and prognostic value.Tumor markers CA 19-9 and carcinoembryonic antigen(CEA) are widely used in the characterization of premalignant and malignant lesions of the pancreas.Elevated level of CEA in bile is a marker for malignancy and a predictor of hepatic recurrence.The potential value of CA 19-9,CEA and lactate dehydrogenase as prognostic biomarkers in pancreatic juice and bile is unknown.Specimens of pancreatic juice and bile can be readily collected during surgical resection of the tumor.Profiling of pancreatic juice and bile to identify novel prognostic biomarkers may improve selection of patients for adjuvant therapy with a favorable impact on overall survival in patients diagnosed with pancreatic cancer.

The preparation,characterization,and application of silica-coated magnetic nanoparticles for papain immobilization is reported.Papain was covalently attached onto the(3-chloropropyl) trimethoxysilane-modified silica-coated magnetic nanoparticles. The enzyme-immobilized nanoparticles were characterized by Fourier transform infrared spectroscopy,X-ray powder diffraction,scanning electron microscopy,and vibrating sample magnetometry techniques.Response surface methodology combined with statistical analyses using Minitab were employed to evaluate optimum operating conditions to immobilize papain on the magnetic nanoparticles.The optimum conditions were: temperature = 27.3℃,pH of the enzyme solution = 7.1,concentration of papain = 3.3 mg/mL,and immobilization time = 10 h.Compared with the free papain,the immobilized papain displayed enhanced enzyme activity,better tolerance to variations in the medium pH and temperature,improved storage stability,and good reusability.Both the free and immobilized enzymes were effective for the clarification of pomegranate juice.

Optimization of Fermentation Process of Carrot juice by Probiotics and Analysis of Aroma Components

Carrot juice was fermented by probiotic Lactobacillus casei( CTCF-LC1) and Lactobacillus reuteri( CTCF-LT1) with sensory score as the main evaluation index,and the fermentation process was optimized by single factor and response surface experiments.The optimum ratio of fermentative strains CTCF-LC1∶ CTCF-LT1 was 1∶ 1,fermentation time was 8 h,inoculum size was 3.1% and fermentation temperature was26.6℃.Under the optimum conditions,the sensory score of fermented carrot juice was 91.3,pH was 4.50,total acid content was 3.73 g/L,total sugar content was 54.25 g/L,and reducing sugar content was 28.41 g/L.The carrot juice has bright orange color and stable state.It not only has the fresh flavor of fruit and vegetable juice,but also has the unique aroma from lactic acid bacteria fermentation,as well as soft and even taste,moderate acidity and sweetness.The changes of aroma components of carrot juice before and after fermentation were analyzed by headspace solid phase microextraction( HSPME) coupled with gas chromatography-mass spectrometry( GC-MS).After fermentation,substances with medicinal flavor components were reduced,and a lot of floral and fruit-scented substances were produced,such as methyl myristate,β-pinene,phenylethanol,and geraniol.Therefore,the results of this study can provide a theoretical basis for the development of fermented fruit and vegetable juice products with unique flavor substances.

Exposure to gastric juice may not cause adenocarcinogenesis of the esophagus

AIM: To determine the effects of gastric juice on the development of esophageal adenocarcinoma (EAC). METHODS: A animal model of duodenogastroesophageal reflux was established in Sprague-Dawley rats undergoing esophagoduodenostomy. The development of EAC and forestomach adenocarcinoma was investigated 40 wk after the treatment. Intraluminal pH and bile of the forestomach were measured. RESULTS: There were no significant differences in pH (t=0.117, P=0.925) or bile (χ2=0.036, P=0.85) in the forestomach before and 40 wk after esophagoduodenostomy. There were also no significant differences between the model and controls during esophagoduodenostomy or 40 wk after esophagoduodenostomy. The incidence of intestinal metaplasia (88%) and intestinal metaplasia with dysplasia and adenocarcinoma (28%) in the esophagus in the model was higher than in the controls 40 wk after surgery (χ2=43.06, P juice exposure may not cause intestinal metaplasia with dysplasia or adenocarcinoma of the forestomach and may not be related to EAC.

Effect of Combining Ultrasound and Mild Heat Treatment on Physicochemical, Nutritional Quality and Microbiological Properties of Pineapple juice

Increasing consumer awareness regarding the health benefits of different nutrients in food has led to the requirement of assessing the effect of food processing approaches on the quality attributes. The present work focuses on understanding the effects of ultrasound (US) processing, mild heat pasteurization (65°C for 15 min), thermal pasteurization (80°C for 15 min) and their combination on physicochemical, microbiological properties and nutritional quality of pineapple juice through 60 days of storage at room temperature. Ultrasound treatment showed significantly lower browning degree. Ultrasound followed by ultrasound combined with mild heat pasteurization (UMP) treatments was effective in retaining the total phenolic content of pineapple juice as compared to the thermal treatment or the untreated juice sample at room temperature during 60 days of storage. Thermal pasteurization (TP) followed by ultrasound combined with mild heat pasteurization (UMP) and ultrasound (US) treatment, in increasing order, was found to be effective in delaying microbial growth in pineapple juice. This study demonstrates that ultrasound combined with mild heat pasteurization treatments could be able to effectively inactivate the microorganisms and pectin methylesterase in pineapple juice whilst preserving relatively high amount of phenols.
