Background Tirofiban has been widely used as an adjunctive pharmacologic agent for revascularization in patients undergoing percutaneous coronary intervention, and the outcomes appear attractive. However, the potential benefits from early administration of tirofiban in patients with acute ST-segment elevation myocardial infarction(STEMI) undergoing primary percutaneous coronary intervention(PPCI) remain *** We conducted a search in MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials up to September 2012 without language restriction. A total of eight randomized trials(n=1 577 patients) comparing early(emergency department or ambulance) versus late(catheterization laboratory) administration of tirofiban in STEMI patients undergoing PPCI were included in this meta-analysis. Risk ratio(RR) was computed from individual studies and pooled with random- or fixed-effect *** There were no differences in post-procedural Thrombolysis In Myocardial Infarction(TIMI) flow grade 3 and Corrected TIMI Frame Count(RR=1.02, 95% confidence interval(CI): 0.99–1.05, P=0.18; weighted mean difference(WMD)=–0.93, 95% CI: –5.37–3.52, P=0.68, respectively) between the two groups. Similarly, there were no significant differences in the incidence of 30-day mortality(RR=1.69, 95% CI: 0.69–4.13, P=0.25) and re-myocardial infarction(RR=0.71, 95% CI: 0.21–2.35, P=0.57) between early and late administration of tirofiban. As to the safety end points, no significant difference was observed in hospital minor bleeding(RR=1.08, 95% CI: 0.54–2.14, P=0.83) and hospital and 30-day major bleeding between the two groups(RR=0.98, 95% CI: 0.46–2.10, P=0.96; RR=1.32, 95% CI: 0.59–2.97, P=0.49, respectively).Conclusions Early administration of tirofiban in patients undergoing PPCI for STEMI was safe, but no beneficial effects on post-procedural angiographic or clinical outcomes could be identified as compared with late administration. Background Cognitive impairment (CI) increases cardiac mortality among very elderly patients. Percutaneous coronary intervention (PCI) for ischemic heart disease (IHD) patients is considered a favorable strategy for decreasing cardiac mortality. Here, we investigated the influence of CI on cardiac mortality after PCI in very elderly patients. Methods We performed a retrospective observational analysis of patients who received PCI between 2012 and 2014 at the South Miyagi Medical Center, Japan. IHD patients over 80 years old who underwent the Mini-Mental State Examination for CI screening during hospitalization and/or who had been diagnosed with CI were included. Participants were divided into CI and non-CI groups, and cardiac mortality and incidence of adverse cardiac events in a 3-year follow-up period were compared between groups. Statistical analyses were performed using the t-test,χ^2 test, and multivariable Cox regression analysis, with major comorbid illness and conventional cardiac risk factors as confounders. Results Of 565 patients, 95 were included (41 CI, 54 non-CI). Cardiac mortality during the follow-up period was significantly higher in the CI group (36%) compared with the non-CI group (13%)(OR = 4.3, 95% CI: 1.56–11.82, P < 0.05). CI was an independent cardiac prognostic factor after PCI and, for CI patients, living only with a CI partner was an independent predictor of cardiac death within three years. Conclusions CI significantly affected cardiac prognosis after PCI in very elderly patients, particularly those living with a CI partner. To improve patients’ prognoses, social background should be considered alongside conventional medical measures.

Background: The patterns of nonadherence to antiplatelet regimen in stented patients(PARIS) thrombotic risk score are a novel score for predicting the risk of coronary thrombotic events(CTEs) after percutaneous coronary intervention(PCI) with drug?eluting ***, the prognostic value of this score has not been fully evaluated in non?Euro?American PCI ***: We performed a prospective, observational study of 10,724 patients who underwent PCI in Fuwai hospital, China and evaluated the PARIS thrombotic risk score’s predictive value of CTEs in the PCI population. The area under the receiver operating characteristic curve(AUROC) was used to assess the predictive value of the PARIS score for ***: Among 9782 patients without in?hospital events, a total of 95 CTEs occurred during the 2?year follow?up. The PARIS score was significantly higher in patients with CTEs(3.38 ± 2.04) compared with patients without events(2.53 ± 1.70, P < 0.001).According to the risk stratification of the PARIS thrombotic score, the risk of CTEs in the high?risk group was 3.14 times higher than that in the low?risk group(hazard ratio [HR], 3.14; 95% confidence interval [CI], 1.92–5.13; P < 0.001). However, the risk of CTEs in the intermediate?risk and low?risk groups was not significant(HR, 1.39; 95% CI, [0.86–2.24]; P = 0.184). The PARIS score showed prognostic value in evaluating CTEs in the overall population(AUROC, 0.621; 95% CI, 0.561–0.681), the acute coronary syndrome(ACS) population(AUROC, 0.617; 95% CI, 0.534–0.700; P = 0.003), and the non?ACS population(AUROC, 0.647; 95%CI, 0.558–0.736; P = 0.001).Conclusions: In a real?world Chinese population, the PARIS thrombotic risk score shows a modest prognostic value for CTEs in patients after PCI. This score also has a predictive value for CTEs in the ACS and non?ACS subgroup populations.
