Round spermatid injection into human oocytes:a systematic review and meta-analysis

Many azoospermic men do not possess mature spermatozoa at the time of surgical sperm extraction.This study is a systematic review and meta-analysis evaluating outcomes following round spermatid injection(ROSI),a technique which utilizes immature precursors of spermatozoa for fertilization.An electronic search was performed to identify relevant articles published through October 2018.Human cohort studies in English involving male patients who had round spermatids identified and used for fertilization with human oocytes were included.fertilization rate,pregnancy rate,and resultant delivery rate were assessed following ROSI.Meta-analysis outcomes were analyzed using a random-effects model.Data were extracted from 22 studies involving 1099 couples and 4218 embryo transfers.The fertilization rate after ROSI was 38.7%(95%confidence interval[CI]:31.5%–46.3%),while the pregnancy rate was 3.7%(95%CI:3.2%–4.4%).The resultant delivery rate was low,with 4.3%of embryo transfers resulting in a delivery(95%CI:2.3%–7.7%).The pregnancy rate per couple was 13.4%(95%CI:6.8%–19.1%)and the resultant delivery rate per couple was 8.1%(95%CI:6.1%–14.4%).ROSI has resulted in clinical pregnancies and live births,but success rates are considerably lower than those achieved with mature spermatozoa.While this technique may be a feasible alternative for men with azoospermia who decline other options,couples should be aware that the odds of a successful delivery are greatly diminished and the prognosis is relatively poor.

Aim： To detect the expression of hepatitis B virus （HBV） genes （HB S and C genes） in early embryonic cells after introducing motile human sperm carrying HBV DNA into zona-free hamster oocytes via the in vitro fertilization （IVF） technique. Methods： Human sperm-mediated HBV genes were delivered into zona-free hamster oocytes by the IVF method. Polymerase chain reaction （PCR） was used to detect HB S and pre-Core/Core （pre-C/C） coding genes both in one- and two-cell embryos. Reverse transcription-PCR （RT-PCR） analysis was used to study the expression of the two genes. Fluorescence in situ hybridization （FISH） analysis using the full-length HBV DNA as the hybridization probe was performed to confirm the integration of viral DNA in the host embryonic genome. Results： Both HB S and pre-C/C coding genes are present and transcribed in one- and two-cell embryos originated from hamster ova IVF with human spermatozoa carrying HBV DNA sequences. Conclusion： Sperm-mediated HBV genes are able to replicate and express themselves in early embryonic cells. These results provide direct evidence that HBV DNA could transmit vertically to the next generation via the male germ line.

Alteration of ERβ gene Rsal polymorphism may contribute to reduced fertilization rate and embryonic developmental competence

This paper aims to determine the possible role of estrogen receptor-β （ERβ） gene Rsal polymorphism on sperm fertility and early embryonic development in humans. Three groups of Chinese men were recruited： in vitro fertilization （IVF） group, including 374 couples who underwent conventional IVF; intracytoplasmic sperm injection （ICSI） group, including 294 couples who underwent an ICSI procedure using ejaculated sperm; and azoospermic group, consisting of 197 couples who underwent ICSI using either testis or epididymis sperm. Rsal polymorphism in the ERβ gene was detected by polymerase chain reaction （PCR）-restriction fragment length polymorphism technique; fertilization and high-quality embryo rates were evaluated for each group. In each group, no significant differences were found in the overall rates of fertilization and high-quality embryos among GG, AG and AA genotypes. However, the proportion of cycles possessing a satisfactory high-quality embryo rate with the AA genotype was significantly lower than that in the wild-type GG genotype from each group. These results demonstrated that sperm possessing the ERβ RsalA genotype may have reduced fertilization ability and decreased early embryonic developmental potential, which could directly or indirectly contribute to the low fertilization rate and early embryonic developmental arrest in some cases.

Nomogram to predict pregnancy outcomes of emergency oocyte freeze-thaw cycles

Background:Existing clinical prediction models for in vitro fertilization are based on the fresh oocyte cycle,and there is no prediction model to evaluate the probability of successful thawing of cryopreserved mature oocytes.This research aims to identify and study the characteristics of pre-oocyte-retrieval patients that can affect the pregnancy outcomes of emergency oocyte freeze-thaw cycles.Methods:Data were collected from the Reproductive Center,Peking University Third Hospital of China.Multivariable logistic regression model was used to derive the nomogram.Nomogram model performance was assessed by examining the discrimination and calibration in the development and validation cohorts.Discriminatory ability was assessed using the area under the receiver operating characteristic curve(AUC),and calibration was assessed using the Hosmer–Lemeshow goodness-of-fit test and calibration plots.Results:The predictors in the model of“no transferable embryo cycles”are female age(odds ratio[OR]=1.099,95%confidence interval[CI]=1.003–1.205,P=0.0440),duration of infertility(OR=1.140,95%CI=1.018–1.276,P=0.0240),basal follicle-stimulating hormone(FSH)level(OR=1.205,95%CI=1.051–1.382,P=0.0084),basal estradiol(E2)level(OR=1.006,95%CI=1.001–1.010,P=0.0120),and sperm from microdissection testicular sperm extraction(MESA)(OR=7.741,95%CI=2.905–20.632,P<0.0010).Upon assessing predictive ability,the AUC for the“no transferable embryo cycles”model was 0.799(95%CI:0.722–0.875,P<0.0010).The Hosmer–Lemeshow test(P=0.7210)and calibration curve showed good calibration for the prediction of no transferable embryo cycles.The predictors in the cumulative live birth were the number of follicles on the day of human chorionic gonadotropin(hCG)administration(OR=1.088,95%CI=1.030–1.149,P=0.0020)and endometriosis(OR=0.172,95%CI=0.035–0.853,P=0.0310).The AUC for the“cumulative live birth”model was 0.724(95%CI:0.647–0.801,P<0.0010).The Hosmer–Lemeshow test(P=0.5620)and calibration curve showed good calibration for the prediction of cumulative live bi

Oviductal epithelial cells selected boar sperm according to their functional characteristics

The interaction of oviductal epithelial cells （OECs） with the spermatozoa has beneficial effects on the sperm functions. The aim of this study is to evaluate the in vitro fertilizing capacity of incubating spermatozoa previously selected by density gradient in OEC and determinate some sperm characteristics that could explain the results obtained. In this study, we assessed in vitro fertilization （IVF）, tyrosine phosphorylation, phosphatidylserine translocation, nuclear DNA fragmentation, and chromatin decondensation. Three experimental sperm groups, previously selected by Percoll gradient, were established according to the origin of the sperm used for IVF：（i） W30 group： spermatozoa were incubated with oocytes in the absence of OEC; （ii） NB group： after sperm incubation in OEC, the unbound spermatozoa were incubated with oocytes, in the absence of OEC; and （iii） B group： after sperm incubation with OEC, the bound spermatozoa were incubated with oocytes in the OEC plates. The results showed that sperm from the NB group led to a lower IVF yield, accompanied by low penetration rates （NB： 19.6%, B： 94.9%, and W30： 62.9%; P 〈 0.001） and problems of nuclear decondensation. Moreover, higher levels of tyrosine phosphorylation were observed in the NB group compared with the W30 and B groups （NB： 58.7%, B： 2.5%, and W30： 4.5%; P 〈 0.01）. A similar trend was observed in phosphatidylserine translocation （NB： 93.7%, B. 5.7%, and W30： 44.2%; P 〈 0.01）. These results demonstrate that the OEC exerts a rigorous degree of sperm selection, even within an already highly selected population of spermatozoa, and can capture the best functional spermatozoa for fertilization.

Sperm donation in China is different from that in other countries due to cultural, social and political factors. This research presents the current status of sperm donation in China's Mainland and highlights some problems. Between January 2003 and December 2009, 19 471 sperm donors were screened totally and 6467 donors （33.2%） were recruited. The primary reasons for non-recruitment were either inadequate semen parameters （55.0%） or positive results for sexually transmitted diseases （7.9%）. There were 327 （1.7%） qualified donors who withdrew from the program because of frustration related to failed semen parameters, participation merely for free medical tests or job transfer. A questionnaire investigating donor intention, as well as other concerns associated with sperm donation, was distributed to 516 potential donors. All potential donors indicated their primary motivation as altruism, while 90.9% mentioned monetary reward as a second motivating factor. Approximately 93.4% of donors expressed some apprehension about the risk of consanguineous mating and the protection of their identity. Over the past 7 years, 488 389 vials of donors＇ semen have been cryopreserved. In 36 438 artificial insemination with donor sperm （AID） cycles, the clinical pregnancy rate was 23.9% and the live birth rate was 16.6%, In 7148 in vitro fertilization cycles, the clinical pregnancy rate was 45.8% and the live birth rate was 35.2%. Human sperm banks have been strictly monitored to ensure that each sperm donor can only impregnate five women nationwide. There is still a large gap between the supply and demand for sperm donation which may be solved by updated guidelines.

A comparison of implantation, miscarriage and pregnancy rates of single and double day 3 embryo transfer between fresh and frozen thawed transfer cycles： a retrospective study

Background Reduced endometrial receptivity in hyperstimulated cycles may lead to a lower implantation rate and a lower clinical pregnancy rate,but it is unclear if it is also associated with an increase in pregnancy loss rate.The aim of this study was to compare the implantation,miscarriage,and pregnancy rates between fresh and frozen thawed transfer of one or two day-3 embryos,with a view to understanding whether or not reduced endometrial receptivity encountered in hyperstimulated cycles is associated with an increase in miscarriage rate.Methods This study involved a consecutive series of 1 551 single day-3 embryo transfer cycles and consecutive 5 919 double day-3 embryo transfer cycles in the Assisted Reproductive Unit of the Sir Run Run Shaw Hospital,Hangzhou,China,between January 2010 and December 2012.Results The implantation and clinical pregnancy rates （single embryo 30.7％ and double embryos 33.4％ and 51.4％）using fresh cycle were both significantly lower than that of frozen-thawed cycles （single embryo 35.8％ and double embryos 38.1％ and 57.8％）.There was no difference in biochemical loss or clinical miscarriage rates between the two groups.Conclusions Impairment of endometrial receptivity associated with ovarian hyperstimulation leads to implantation failure at a very early stage,resulting in an increased number of non-pregnancy.It does not lead to increase in biochemical or clinical losses.The significantly reduced ongoing pregnancy rates in both fresh single and double embryo transfer are therefore due to failure to achieve a pregnancy,rather than pregnancy loss after conception.

Decline of semen quality during IVF is not associated with subjective male stress

The aim of the present study was to assess if semen quality declines during in vitro fertilization （IVF） and whether or not this phenomenon is triggered by chronic male stress. In order to test this hypothesis, we first investigated a retrospective cohort of 155 male IVF patients （testing cohort）. Subsequently, we started a prospective cohort study in men undergoing their first IVF and assessed semen quality and subjective male chronic stress using a validated tool, i.e. the Fertility Problem Inventory （FPI） questionnaire. The association between stress and sperm quality decline measured 4-6weeks before the start of IVF （T1） and at the day of oocyte retrieval （T2） was the primary outcome. Live birth rate, first trimester abortion and rate of poor responders were secondary outcomes. In the testing cohort, mean progressive motility, but not mean sperm density significantly declined. There were 78/154 （51%） men who showed a decline in semen density and 50/154 （32%） men who showed a decline in progressive motility. In the validation cohort, progressive motility declined, whereas, sperm density increased from T1 to T2. Of 78 men, 27 men had increased stress （FPI-score 〉 146）. Sperm density and progressive motility were not significantly different in men with and without stress. However, in the presence of male stress, couples had a higher rate of poor responders, miscarriages and a lower rate of live births. Subjective stress is not associated with a decline in semen quality observed during IVF but may be associated with adverse ore^nancv outcome.

Being born with nonoptimal birth characteristics decreases the chance of becoming a father. Urogenital malformations as well as metabolic syndrome are more common in men born small for gestational age （SGA） and could be contributing factors to the reduced fertility rate seen in these men. It could imply that men becoming fathers by assisted reproductive technology （ART） more often are born with low birth weight （LBW）, preterm, and/or SGA than men conceiving without treatment and also that men where intracytoplasmic sperm injection （ICSI） had to be performed more often are born with nonoptimal birth characteristics than men where conventional in vitro fertilization （IVF） successfully could be used. In this retrospective, case-control study using Swedish national registers, we compared the birth characteristics of 1206 men who have become fathers by ART with a control group consisting of age-matched men who became fathers without treatment. The differences in birth characteristics between men becoming fathers by IVF and ICSI were also assessed. For men becoming fathers by ART, OR of being born with LBW was 1.66 （95% CI = 1.17-2.36） compared with fathers who conceived without treatment. OR of being born prematurely was 1.32 （95% CI = 1.00-1.77）. Men becoming fathers via ICSI had a doubled increased likelihood of being born SGA compared with men who became fathers via IVF （OR = 2.12; 95% CI = 1.17-3.83）. In conclusion, we have found that men becoming fathers by ICSI treatments had more often been born SGA than men becoming fathers by conventional IVF.

Free radical scavenging window of infertile patients with polycystic ovary syndrome： correlation with embryo quality

The activity of free radicals in follicular fluid was related to ovarian responsiveness, in vitro fertilization （IVF）, and embryo transfer success rate. However, studies analyzing the relationship between the free radical scavenging capacity and embryo quality of infertile women with polycystie ovarian syndrome （PCOS） were lacking. The aim of this study was to evaluate the relationship between the free radical scavenging window of women with PCOS and their embryo quality. The free radical scavenging capacity of follicular fluid from women with PCOS was determined by a,a-diphenyl-b-picrylhydrazyl （DPPH）, 2,2-azinobis （3-ethylbenzthiazoline-6- sulphonic acid） assay, superoxide radical, and reactive oxygen species （ROS） assay. In the DPPH and ROS assays, the follicular fluid from grades I and II embryos was significantly higher than the follicular fluid from grades III and IV embryos. The lower control limit of DPPH radical scavenging capacity and upper control limit of ROS level were 13.2% and 109.0 cps, respectively. The calculated lower control limit and upper control limit were further confirmed in the follicular fluid of embryos of all grades. These cut-off values of free radical scavenging activity of follicular fluid could assist embryologists in choosing the development of embryos in PCOS patients undergoing IVF.
