Clinical,laboratory,and therapeutic differences between immunoglobulin e-mediated and non-immunoglobulin e-mediated cow’s milk protein allergy in children

BACKGROUND Although breast milk is ideal for newborns,in some cases,it is replaced with cow’s milk,which contains proteins that increase the risk of cow’s milk protein allergy(CMPA).AIM To evaluate CMPA prevalence in Bahrain and compare clinical characteristics of children with immunoglobulin e(Ige)-and non-Ige-mediated *** This retrospective cohort study examined children with CMPA diagnosed at the pediatric gastroenterology outpatient clinic of the Salmaniya Medical Complex,Bahrain,between 2014 and 2022,and assessed CMPA *** presentations,laboratory findings,dietary modifications,and outcomes were compared between children with Ige-mediated and non-Ige-mediated *** variables were compared using Fisher’s exact test or Pearson’sχ2 test,whereas continuous variables were compared using Student’s t-test or the Mann-Whitney U *** Of 8332 patients,6022(72.3%)adhered to their *** them,618(10.3%)were suggested of having CMPA and 595(96.3%)were included;CMPA prevalence was 2%.Most patients were Bahraini(93.8%)and males(55.3%).Non-Ige-mediated type accounted for 77.3%cases and Ige-mediated type,22.7%.Ige-mediated CMPA presented more in boys(P=0.030),and later in life(5.1 months±4.5 months vs 4.2 months±4.2 months,P=0.016,95%CI:0.08-1.73),had more associated diseases(Pesented with more cutaneous(P=0.024)and respiratory(P=0.003)manifestations,severe symptoms[rash/dry skin(P=0.031),facial swelling/angioedema(P=0.003),failure to thrive(P=0.013),apparent life-threatening event(Pe physical findings(P=0.002)than non-Ige-mediated *** patients were exclusively fed cow milk formula(50.3%).The amino acid-based formula(AAF)was most prescribed(60.5%)with no difference between the two types(P=0.173).Although breastfeeding was recommended to 49.6%,only 8.2%were exclusively ***-mediated CMPA was associated with a longer follow-up duration than non-Ige-mediated CMPA(17.3 months±14.0 mo

Risk of connective—tissue disease in men with testicular or penile prostheses：a preliminary study

AIM: To help clarifying the possibility of connective-tissue diseases in men with penile or testicular prostheses. MeTHODS: eight patients underwent inflatable penile prostheses and 15, testicular prostheses consented to the study. Their medical records were reviewed and a follow-up interview and physical and serological examinations were performed. ReSULTS: In patients with penile prostheses, there was no abnormal antinuclear antibody (ANA) or IgM elevation. The serum levels of the rheumatoid factor (RF), C4, IgA and IgG were abnormal in one patient, and the levels of erythrocyte sedimentation rate (eSR) and C3, abnormal in two. Four had elevated Ige. In patients with testicular prostheses, there was no abnormal RF, ANA or IgM. The serum levels of eSR and IgA were abnormal in two, and three had abnormal C4, ten abnormal C3, and eleven decreased IgG. All had increased Ige. Men with penile prostheses had higher serum levels of IgG and IgM than those with testicular prostheses (P=0.001, P=0.016, respectively). The rates of abnormal values of Ige and IgG were higher in men with testicular prostheses than in men with penile prostheses (P=0.008, P=0.009, respectively). Physical examination was normal in all patients and nobody had documented symptoms pertinent to connective-tissue diseases. CONCLUSION: Our findings suggest that the risk of connective-tissue diseases is not higher in patients wearing prostheses as the ANA is negative and there is no apparent manifestation suggestive of connective-tissue diseases.

Acupoint Autohemotherapy Attenuates Atopic Dermatitis Lesions by Regulating Th1/Th2 Balance in DNCB-Induced BALB/c Mice

Objective: To evaluate the therapeutic effects of acupoint autohemotherapy(A-AHT) on 1-chloro-2,4-dinitrobenzene(DNCB)-induced atopic dermatitis(AD) in mice focusing on regulating T helper 1/T helper 2(Th1/Th2) immune responses. Methods: Thirty BALB/c mice were divided into 5 groups by a random number table, including normal control(NC), AD model(AD), A-AHT, sham A-AHT(sA-AHT), and acupoint injection of normal saline(A-NS) groups, 6 mice per group. Mice were challenged by DNCB for the establishment of experimental AD model. On the 8th day, except for the NC and AD groups, the mice in the other groups received management once every other day for a total of 28 days. For the A-AHT and sA-AHT groups, 0.05 mL of autologous whole blood(AWB) was injected into bilateral Zusanli(ST 36) and Quchi(LI 11) and sham-acupoints(5 mm lateral to ST 36 and LI 11), respectively. The A-NS group was administrated with 0.05 m L of normal saline by acupoint injection into ST 36 and LI 11. Dermatitis severity for dorsal skin of mice was determined using the Severity Scoring of Atopic Dermatitis(SCORAD) every week. The total immunoglobulin e(Ige), interleukin-4(IL-4), and interferon-gamma(IFN-γ) cytokine levels in serum were examined by enzyme-linked immunosorbent assay(eLISA). Spleen Th1/Th2 expression were analyzed via flow cytometry and immunohistochemical assay was used to detect T-box expressed in T cell(T-bet) and GATA-binding protein 3(GATA3) expressions in skin lesions of mice. Results: Compared with the AD group, both A-AHT and sA-AHT reduced the SCORAD index and serum Ige level(Pegulated serum IL-4 level and upregulated IFN-γ/IL-4 ratio(Pegulated the Th1/Th2 shift specifically and increased the related transcription factors such as T-bet expression and T-bet/GATA3 ratio(Ped significant effectiveness on the AD model mice, through regulating Th1/Th2 immune responses.

Inhibition of histamine release from human mast cells by natural chymase inhibitors

AIM: To investigate the ability of natural chymase inhibitors to modulate histamine release from human mast cells. MeTHODS: enzymatically dispersed cells from human lung, tonsil, and skin were challenged with anti-Ige or calcium ionphore A23187 in the absence or presence of the natural chymase inhibitors secretory leukocyte pro- tease inhibitor (SLPI) and α1-antitrypsin, then histamine release was determined. ReSULTS: Ige-dependent hista- mine release from lung, tonsil, and skin mast cells were inhibited by up to 70%, 61%, and 62%, respectively following incubation with α1-antitrypsin (5000 nmol/L).SLPI 5000 nmol/L was also able to inhibit anti-Ige- dependent histamine released from lung, tonsil and skin mast cells by up to approximately 72%, 67%, and 58%, respectively. While neither α1-antitrypsin nor SLPI by themselves altered histamine release from lung, tonsil and skin mast cells, they were able to inhibit calcium ionophore-induced histamine release from lung and tonsil mast cells. CONCLUSION: Both α1-antitrypsinand SLPIcould potently inhibit Ige-dependent and calcium ionophore- induced histamine release from dispersed human lung, tonsil, and skin mast cells in a concentration-dependent manner, which suggested that they were likely to play a protective role in mast cell associated diseases including allergy.

OBJeCTIVe: To quantitatively evaluate the therapeutic effects of Red Liriope platyphylla(RLP) on atopic dermatitis(AD), alterations in the luciferase(Luc) signal and general phenotype biomarkers were compared in phthalic anhydride(PA) treated Interleukin-4(IL-4)/Luc/Consensus non-coding sequence-1(CNS-1) transgenic(Tg) mice following treatment with aqueous extract of RLP(Aet RLP) for4 ***: Alterations in AD phenotypes were measured in IL-4/Luc/CNS-1 Tg mice following treatment with Aet RLP using inflammation parameter analysis, bioluminescence imaging analysis, histological analysis, reverse transcription-polymerase chain reaction, enzyme linked immunosorbent assay and Western blot ***: RLP contained high concentrations of total phenolic compounds, total flavonoid compounds and 5-HNe related to AD therapy. The Luc signal was only detected in the abdominal region and the submandibular lymph node(SL), mesenteric lymph node(ML), thymus and pancreas of the PA treated group. This signal was significantly decreased by 28%-73% throughout the body and in the four organs in PA + Aet RLP treated group. Furthermore, the lymph node weight, immunoglobulin e concentration and dermal thickness were decreased by 37%-67% in the PA + Aet RLP treated ***: Our findings suggest that the therapeutic effect of Aet RLP on PA induced AD could be successfully quantified by comparison of Luc signals and AD phenotype markers in IL-4/Luc/CNS-1Tg mice, and that the Luc signal was as sensitive as the general AD phenotypes, enabling detection of effects without euthanasia.

BACKGROUND Dedicator of cytokinesis 8(DOCK 8) deficiency,also known as autosomal recessive hyper immunoglobulin e(Ige) syndrome,is a combined immunodeficiency disease that was first recognized in *** is caused by genetic alterations(mutations or deletions) in the DOCK 8 gene and is characterized by multiple allergies,elevated Ige levels,and susceptibility to viral and bacterial *** diagnosis is critical to optimize the success of stem cell *** SUMMARY This study reports the case of a pediatric patient with DOCK 8 deficiency who had negative genetic testing using multiplex primary immunodeficiency(PID) panel and whole-exome sequencing(WeS) with a next-generation sequencing *** presented with chronic diarrhea and was managed as celiac disease based on previous negative workup for immunodeficiency and duodenal *** developed a generalized vesicular rash which was thought to be dermatitis herpetiformis associated with celiac ***,it turned out to be eczema herpeticum based on positive herpes simplex virus from blood and *** diagnosis was re-evaluated after the child was found to have multiple viral,bacterial,and parasitic co-infections(herpes simplex virus,cytomegalovirus,epstein-Barr virus,Salmonella,and cryptosporidiosis).Re-evaluation with target gene testing with copy number variation(CNV) analysis and Multiplex Ligation Probe Amplification(MLPA) showed a large homozygous deletion in the DOCK 8 gene,confirming the diagnosis of DOCK 8 *** Targeted gene testing with CNV analysis might detect deletions that can be missed by WeS for diagnosing patients with PID.
