Effect of lens surgery on health-related quality of life in preschool children with congenital ectopia lentis

AIM:To evaluate the effect of lens surgery on health-related quality of life(HRQoL)of preschool children with congenital ectopia lentis(CEL).METHODS:A prospective self-controlled study was conducted in Zhongshan Ophthalmic Center.Children aged from 5 to 7y whom were diagnosed with CEL and underwent phacoemulsification with scleral-fixated posterior chamber intraocular lens implantation and their parents were enrolled in this study.All of them completed the child and proxy(parental)PedsQL™4.0 before and after the surgery.Their preoperative scores were compared to their postoperative ones.Subgroup analyses were performed based on gender and preoperative bilateral presenting visual acuity of the children.RESULTS:Thirty-two children with CEL successfully underwent surgery without any complications,among whom 8 had monocular surgery and 24 had binocular surgery.Preoperative and postoperative questionnaires were completed by 32 child-parent pairs.Surgical intervention could significantly improve the vision of affected children(Phealth scores self-reported by the children were 68.75(62.50,81.25),65.00(60.00,80.00)and 67.39(60.87,78.26)preoperatively and were 93.75(87.50,100.00),90.00(83.33,96.67)and 89.13(85.32,95.65)postoperatively.The preoperative scores of the affected children were significantly lower in all scales than age-matched healthy children(Pquality of life in preschool children.Surgical intervention can significantly improve the HRQoL in affected children from both personal and family perspective.

BACKGROUND Patients with Hirschsprung disease(HD)are at risk of persistent constipation,fecal incontinence or recurrent enterocolitis after surgical treatment,which in turn may impact physical and psychosocial functioning.Generic health-related quality of life(HRQoL)and disease-specific health-related quality of life are relevant outcome measures to assess the impact of HD on the QoL of these patients.AIM To summarize all available evidence on HRQoL of patients with HD after surgery and the impact of possible moderating factors.METHODS Pubmed,Web of Sciences,Psyc Info and Embase were searched with search terms related to’Hirschsprung disease’,’Pediatrics’and’quality of life’.Mean and standard deviation of generic HRQoL overall and domain scores were extracted from each study,as well as data describing potential factors associated with QoL.Random effect models were used for meta-analytic aggregation of generic HRQoL scores.Meta-regression was used to assess the relationship between patient and clinical characteristics and generic HRQoL.Disease-specific HRQoL outcomes of patients with HD were systematically reviewed.RESULTS Seventeen articles were included in the systematic review(n=1137 patients)and 15 in the quantitative meta-analysis(n=1024 patients).Four studies reported disease-specific HRQoL.Patient’s age ranged between 0 and 21 years.Meta-analytic aggregation showed a nonsignificantly impaired generic HRQoL(d=-0.168[95%CI:-0.481;0.145],P=0.293,I^(2)=94.9)in patients with HD compared to healthy controls.Physical(d=-0.042[95%CI:-0.419;0.335],P=0.829,I^(2)=95.1),psychosocial(d=-0.159[95%CI:-0.458;0.141],P=0.299,I^(2)=93.6)and social HRQoL(d=-0.092[95%CI:-0.642;0.457],P=0.742,I^(2)=92.3)were also not significantly lower compared to healthy controls.There was no relation between health-related outcomes and the sex of the patients and whether generic HRQoL was measured by parental proxy or self-report.Disease-specific complaints of patients with HD impaired physical HRQoL,but not psychosocial and social HRQoL.CONC

Predictors of health-related quality of life in Children with Cyanotic Heart Disease Who Underwent Palliative and Total Repair

Background:Studies on predictors of health-related quality of life(HRQOL)in pediatric patients with cyanotic heart disease who are waiting for the next stage and those who have undergone total repair are scarce.Therefore,we aimed to identify such predictors in children who received the modified Blalock–Taussig shunt(MBTS)and those who underwent total repair.Methods:In this historical cohort and concurrent follow-up study,data of children who underwent MBTS at the age of 0–3 years between January 2005 and December 2016 at a super-tertiary care hospital in Southern Thailand were obtained.Children who were alive in December 2017 were recruited to evaluate the quality of life at least 1 year after their operation.Between January and December 2018,the“Pediatric quality of life Inventory 4.0 Generic Core Scales”with both child self-report and parent proxy-report scores were used to examine the HRQOL.Multivariate linear regression analysis was performed to identify independent predictors of HRQOL.Beta-coefficient(β)and 95% confidence intervals(95%CIs)were calculated and considered statistically significant at p<0.05.Results:Among the 380 enrolled children,148 died,122 survived and waited for total repair,and 110 survived after total repair.In the multivariate analysis,chronic lung disease was a common predictor of lower physical and psychosocial HRQOL reported by the parents(β[95%CI]:−0.42[−0.81,−0.03]and−0.49[−0.89,−0.09],respectively).Total repair was a predictor of higher physical HRQOL according to both parents and children(β[95%CI]:0.33[0.09,0.57]and 0.70[0.36,1.03],respectively).A predictor of higher psychosocial HRQOL reported by the parents was younger age during MBTS surgery compared with older age(β[95%CI]:0.012[0.001,0.022]).In the total repair subgroup,undergoing the Fontan procedure(vs.Glenn procedure)was a predictor for lower physical HRQOL reported by the parents(β[95%CI]:−0.82[−1.52,−0.13]).Higher socioeconomic status was a predictor of both physical and psychosocial HRQOL(β[95%CI]:0.018[0.001,0.034]and

Background:There are insufficient data regarding the impact of acute respiratory distress syndrome related to coronavirus disease 2019(C-ARDS)–caused by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)–on health-related quality of life(HRQoL)and the occurrence of stress-related disorders in coronavirus disease 2019(COVID-19)intensive care unit(ICU)survivors.The aim of this study is to assess HRQoL and the occurrence of stress-related disorders(acute stress disorder[ASD]and post-traumatic stress disorder[PTSD])in C-ARDS ICU survivors at 1 and 6 months following hospital discharge.Methods:This prospective observational study included 90 patients treated for C-ARDS between March and May 2020 in the ICU and discharged alive from the hospital.All patients included in the study were contacted by telephone 1 month and 6 months post-hospital discharge to assess the presence of symptoms of stress-related disorders and HRQoL using the 8-item Treatment Outcome Post-traumatic Stress Disorder scale(TOP-8)and 36-item Short Form survey(SF-36).We performed univariate analyses to evaluate differences between patients who developed stress and those who did not.We also compared SF-36 scores in our sample with data from the general Spanish population and from cohorts of non–C-ARDS and severe acute respiratory syndrome coronavirus-1(SARS-CoV-1)survivors.Results:There are 24.1%of patients showed symptoms of ASD;in 13.5%of cases the symptoms persisted 6 months later.Risk factors for the development of symptoms of ASD and PTSD are younger age,female sex,obesity,a previously diagnosed psychiatric disease and disease severity at ICU admission(P<0.05).HRQoL was greatly affected by C-ARDS;however,there was improvement on all scales of the SF-36 at the 6-month follow-up(P<0.05).The mean SF-36 score of our sample was higher than those previously reported in non–C-ARDS survivors(P<0.05)for physical functioning(78.0 vs.52.0),role functioning/physical(51.0 vs.31.0),bodily pain(76.1 vs.57.0),vitality(58.6 vs.48.0),social function(72.6

Profile and Correlates of health-related quality of life in Chinese Patients with Coronary Heart Disease

Background： Although coronary heart disease （CHD） is one of the major cardiovascular diseases, risk factors associated with the health-related quality of life （HRQoL） of CHD patients remain unclear. The present study was designed to determine the profile and significant factors of the HRQoL in CHD patients.Methods： A cross-sectional study was conducted in rural communities of Fangshan District, Beijing. China. Socio-demographic, lifestyle, and comorbidity information of CHD patients were collected by a structured questionnaire and medical records. HRQoL was measured using European quality of life 5-dimensions （EQ-5D） scale and EQ Visual Analog Scale （EQ-VAS）. Multiple linear and logistic regressions were performed to explore the association of potential risk factors with HRQoL scores and each EQ-SD, respectively.Results： Totally, 1928 CHD patients （mean age 61.64 ± 9.24 years; female：male = 2.4：1） were enrolled in the study. The mean score of EQ-5D index and EQ-VAS were 0.889 ±0.172 and 71.56 ±17.65, respectively. Multiple linear regression revealed that marital status, physical activity, moderate alcohol drinking, and family＇s population were positive independent correlates of EQ-VAS, whereas diabetes mellitus and stroke were negative independent correlates （all P 〈 0.05）. Age and stroke were negatively while physical activity, moderate alcohol drinking, family＇s population and household income were positively correlated with EQ-5D index （all P 〈 0.05） independently. In addition, each of the five HRQoL dimensions had various specific determinants, including obesity, underweight. smoking or education. Conclusions： Findings of the study highlight certain socio-demographic, lifestyle factors, and comorbid stroke or diabetes mellitus as correlates of HRQoL in Chinese CHD patients. Large-scale cohort studies should be carried out to confirm our results in the future.

Determinants of health-related quality of life worsening in patients with chronic obstructive pulmonary disease at one year

Background Chronic obstructive pulmonary disease （COPD） is a major cause of morbidity and mortality worldwide and has been the leading cause of death in China.Patients with COPD have significant decrements in their health-related quality of life （HRQL）.It is necessary to identify the factors involved in worsening HRQL in order to improve the HRQL of COPD patients.However,evidence from longitudinal studies is limited.The aim of the study was to evaluate the determinants of the deterioration of HRQL in patients with COPD.Methods At baseline,a total of 491 patients with stable COPD received comprehensive assessments,including psychosocial and clinical variables,six minutes walk distance （6MWD）,dyspnea grade measured by the 5-grade Medical Research Council （MRC） dyspnea scale,anxiety and depression measured by the hospital anxiety and depression scale and HRQL measured by St.George＇s Respiratory Questionnaire （SGRQ）.Patients were then monitored monthly for 12 months to document COPD exacerbations.At the end of the study period,the SGRQ values were reassessed.A 1-year change in SGRQ total score ≥4 was defined as a deterioration of the HRQL and as the outcome.A total of 450 patients completed the 12-month follow-up and were analyzed in the present study.Results The age （mean±SD） was （65.0±10.6） years and 68.7％ of subjects were men.The deterioration of the HRQL was 26.4％.In multivariate Logistic regression,independent and graded associations were found between the baseline MRC dyspnoea grade and the deterioration of HRQL （P=0.012）,OR 3.03 （95％ CI 1.11-8.24） for patients with MRC dyspnoea grade ≥4 versus patients with MRC dyspnoea grade =1.Similarly,the number of exacerbations during the follow-up was independently and gradually increased with the deterioration of HRQL （P ＜0.001）,OR 3.03 （95％ CI 1.9-5.6） for the participants with exacerbations ≥3 versus participants with no exacerbation.The 6MWD evaluated by quartiles was negatively associated with the deterioration of HRQL with borderline statistical significance.Conclu

Correlation between exercise, personal income level and health-related quality of life in patients with newly diagnosed stable angina

Background:Stable angina is one of the most common clinical types of coronary artery disease(CAD)and associated with poor health-related quality of life(HRQL).However,few studies have evaluated the risk factors associated with HRQL in patients with newly diagnosed stable angina.Methods:A cross-sectional survey was conducted with 342 consecutive outpatients with newly diagnosed stable angina from October 2017 to January 2018 at the Second Affiliated Hospital of Army Medical University,Chongqing,China.Eight dimensions of HRQL were evaluated via the 36-item Short-Form health Survey,including physical functioning,role limitation due to physical problems,bodily pain,general health,vitality,social functioning,role limitation due to emotional problems,and mental health.Physical and mental component summaries were calculated.Multiple stepwise regression was performed to determine the factors associated with HRQL.Results:Patients who were older,were females,did not exercise,had lower educational levels,had lower monthly incomes,had smoking/drinking habits,and had diabetes,hypertension,or hyperlipemia showed lower physical HRQL scores,while those who were older with lower educational levels and lower monthly incomes showed lower mental HRQL scores.The results of the multiple stepwise regression analyses showed that physical and mental HRQL were positively correlated with exercise and monthly income and negatively associated with age.Patients with monthly income≥5000 Yuan showed higher HRQL scores than those with monthly incomequality and drinking were negatively associated with physical,but not mental HRQL.Conclusions:Our findings indicated that exercise and personal income level,both modifiable factors,were positively associated with physical and mental HRQL.These findings could have implications for clinical suggestions and strategies to improve HRQL in patients with stable angina.

AIM: To evaluate intensity, localization and cofactors of pain in Crohn’s disease and ulcerative colitis patients in connection with health-related quality of life (HRQOL) and disease activity. METHODS: We reviewed and analyzed the responses of 334 patients to a specifically designed questionnairebased on the short inflammatory bowel disease questionnaire (SIBDQ) and the German pain questionnaire. Pain intensity, HRQOL, Crohn’s disease activity index (CDAI) and colitis activity index (CAI) were correlated and verified on a visual analog scale (VAS). RESULTS: 87.9% of patients reported pain. Females and males reported comparable pain intensities and HRQOL. Surgery reduced pain in both genders (P = 0.023), whereas HRQOL only improved in females. Interestingly, patients on analgesics reported more pain (P = 0.003) and lower HRQOL (P = 0.039) than patients not on analgesics. A significant correlation was found in UC patients between pain intensity and HRQOL (P = 0.023) and CAI (P = 0.027), and in CD patients between HRQOL and CDAI (P = 0.0001), but not between pain intensity and CDAI (P = 0.35). No correlation was found between patients with low CDAI scores and pain intensity. CONCLUSION: Most IBD patients suffer from pain and have decreased HRQOL. Our study reinforces the need for effective individualized pain therapy in IBD patients.

Mortality is a well-established patient-important outcome in critical care studies.In contrast,morbidity is less uniformly reported(given the myriad of critical care illnesses and complications of each)but may have a common end-impact on a patient’s functional capacity and health-related quality-of-life(HRQoL).Survival with a poor quality-of-life may not be acceptable depending on individual patient values and preferences.Hence,as mortality decreases within critical care,it becomes increasingly important to measure intensive care unit(ICU)survivor HRQoL.HRQoL measurements with a preference-based scoring algorithm can be converted into health utilities on a scale anchored at 0(representing death)and 1(representing full health).They can be combined with survival to calculate quality-adjusted life-years(QALY),which are one of the most widely used methods of combining morbidity and mortality into a composite outcome.Although QALYs have been use for health-technology assessment decision-making,an emerging and novel role would be to inform clinical decision-making for patients,families and healthcare providers about what expected HRQoL may be during and after ICU care.Critical care randomized control trials(RCTs)have not routinely measured or reported HRQoL(until more recently),likely due to incapacity of some patients to participate in patient-reported outcome measures.Further differences in HRQoL measurement tools can lead to non-comparable values.To this end,we propose the validation of a gold-standard HRQoL tool in critical care,specifically the EQ-5D-5L.Both combined health-utility and mortality(disaggregated)and QALYs(aggregated)can be reported,with disaggregation allowing for determination of which components are the main drivers of the QALY outcome.Increased use of HRQoL,health-utility,and QALYs in critical care RCTs has the potential to:(1)Increase the likelihood of finding important effects if they exist;(2)improve research efficiency;and(3)help inform optimal management of critically ill patients allowing fo

Background Studies investigating health-related quality of life(HRQoL)in youth with perinatally acquired HIV(PHIV+)are scarce.This study aimed to compare HRQoL of PHIV+to sociodemographic-matched youth not living with HIV(HIV-),Spanish general youth population,and to explore associations between sociodemographic variables,drug consumption,and HRQoL.Methods PHIV+youth were randomly selected from CoRISpe database(Cohort of the Spanish Pediatric HIV Network).HRQoL was evaluated by SF-12v2.Results Thirty-nine PHIV+youth(mean age:23.36 years,SD=3.83)and thirty-nine HIV-youth(mean age:22.97 years,SD=3.80)participated in this study.PHIV+obtained lower scores in SF-12 physical health subscale(PCS)than HIV-(P=0.001)and Spanish general youth population(P=0.006).PHIV+had lower scores on the mental health subscale(MCS)than the Spanish general youth population(P<0.001).PHIV+who were at school obtained better scores than those were not at school.PHIV+youth who had used cocaine and cannabis had lower scores in MCS(P=0.002).Conclusions There is a need for HRQoL management in the associated medical follow-up.
