Barosinusitis due to routine weather changes:A cross-sectional analysis of public websites

Background:Sinusitis is a common diagnosis that can be erroneously associated with routine weather‐related barometric pressure changes.In actuality,these pressure changes likely exacerbate migraine headaches,which can cause facial pain and pressure rather than true sinus inflammation.Objective:The present study sought to characterize the representation of both sinusitis and migraine in association with barometric pressure changes across websites on the Internet.Methods:An Internet search for relevant terms was conducted,and content of the resulting pages was assessed for associations between weather‐related pressure changes and either sinusitis or migraine.Variations in reported results across different subtypes of Internet sources were analyzed.The primary outcomes measured were(1)whether a given media source associated barometric weather changes with sinusitis,(2)whether that source associated barometric weather changes with migraine,and(3)treatment options offered by that source.Results:Of the 116 included webpages,36(31.03%)associated sinusitis and routine barometric pressure changes.Of these,10(27.77%)were otolaryngology practice sites.Sixty‐seven webpages(57.76%)associated migraine and routine barometric pressure changes.Of these,nonotolaryngology webpages were more likely to report this link.Conclusions:Otolaryngology practice sites were observed to be the most frequent professional medical resource reporting the unsubstantiated claim that routine barometric pressure changes are associated with sinusitis.Nonotolaryngology sources were more likely to link weather‐related pressure changes to migraine.These results suggest that opportunities exist for otolaryngology practice sites to educate patients about nonrhinogenic headache etiologies.

Dear Editor,The mpox(formerly known as monkeypox)disease caused by the mpox virus(MPXV)has infected more than 89,700 people and is linked to 157 deaths in 114 countries worldwide as of September 2023(www.***.int).Mpox is a zoonotic disease,the first human case of which was reported in 1970(Breman et al.,1980).Common symptoms in mpox-infected individuals include fever,myalgia,fatigue,headache,and a characteristic rash(Isidro et al.,2022).Current treatments for mpox remain limited,and an in-depth investigation of essential viral protein(s)will help to explore novel drug targets.

Rickettsia felis is an exclusively cytozoic Gram-negative prokaryote with cat fleas as the major vectors.[1]As early as 1918,Rickettsia felis was detected in cat fleas in Europe and named Rickettsia ctenocephali.[2]Symptoms of fever,malaise,headache,maculopapular rash and eschar are observed in patients with Rickettsia felis infection.

Radiotherapy for hyoid bone metastasis from lung adenocarcinoma:A case report

BACKGROUND Metastasis to the hyoid bone is an exceptionally rare occurrence,with documented cases limited to breast,liver,colon,skin,lung,and prostate cancers.This report highlights an unusual case involving the metastasis of lung adenocarcinoma to the hyoid bone,accompanied by a distinctive headache.Previous documentation involved surgical resection of the hyoid mass.We present a case displaying the benefits of palliative radiotherapy.CASE SUMMARY A 72-year-old non-smoking,non-alcoholic woman,initially under investigation for a year-long elevation in absolute lymphocyte count,presented with a monthlong history of intermittent throat pain.Despite negative findings in gastroenterological and otolaryngologic examinations,a contrast-enhanced chest computed tomography scan revealed a mediastinal mass and questionable soft tissue thickening in her left anterolateral neck.Subsequent imaging and biopsies confirmed the presence of lung adenocarcinoma metastasis to the hyoid bone.The patient was treated with platinum-based chemo-immunotherapy along with pembrolizumab.Ultimately,the lung cancer was unresponsive.Our patient opted for palliative radiation therapy instead of surgical resection to address her throat pain.As a result,her throat pain was alleviated,and it also incidentally resolved her chronic headaches.This is the second documented case of lung adenocarcinoma metastasizing to the hyoid bone.CONCLUSION Palliative radiotherapy may add to the quality of life in symptomatic patients with cancer metastatic to the hyoid bone.

To the Editor:New-onset headache within a week after craniotomy is commonly recognized as acute postcraniotomy headache(PCH).This headache pain mainly occurs within the first few post-operative days and is remarkably associated with a younger age,the female sex,and the retrosigmoid approach.[1]The mechanisms of PCH mainly involve mechanical and chemical irritation and aseptic inflammation of the scalp muscles,periosteum of the skull,dura mater,and trigeminal nerves.[2]The frequent onset of PCH leads to impaired daily activities,decreased quality of life,prolonged post-operative recovery,and worsened social functioning.The differential diagnosis of acute PCH may also lead to the patient receiving many unnecessary tests,including invasive tests such as a lumbar puncture.

Full recovery from chronic headache and hypopituitarism caused by lymphocytic hypophysitis:A case report

BACKGROUND Lymphocytic hypophysitis(LYH)is an important condition to consider in the differential diagnosis of patients with a pituitary mass.The main clinical manifestations of LYH include headache,symptoms related to sellar compression,hypopituitarism,diabetes insipidus and hyperprolactinemia.headache,which is a frequent complaint of patients with LYH,is thought to be related to the occupying effect of the pituitary mass and is rapidly resolved with a good outcome after timely and adequate glucocorticoid treatment or surgery.CASE SUMMARY Here,we report a patient with LYH whose initial symptom was headache and whose pituitary function assessment showed the presence of secondary hypoadrenalism,central hypothyroidism and hypogonadotropic hypogonadism.Pituitary magnetic resonance imaging showed symmetrical enlargement of the pituitary gland with suprasellar extension in a dumbbell shape with significant homogeneous enhancement after gadolinium enhancement.The size of the gland was approximately 17.7 mm×14.3 mm×13.8 mm.The pituitary stalk was thickened without deviation,and there was an elevation of the optimal crossing.The lesion grew bilaterally toward the cavernous sinuses,and the parasternal dural caudal sign was visible.The patient presented with repeatedly worsening and prolonged headaches three times even though the hypopituitarism had fully resolved after glucocorticoid treatment during this course.CONCLUSION This rare headache regression suggests that patients with chronic headaches should also be alerted to the possibility of LYH.

Myeloperoxidase-antineutrophil cytoplasmic antibody-associated vasculitis with headache and kidney involvement at presentation and with arthralgia at relapse:A case report

BACKGROUND Patients with proteinase 3-antineutrophil cytoplasmic antibody associated vasculitis(AAV)experience different manifestations at the initial onset and relapse.However,such cases of different initial and relapse manifestations have not been reported in myeloperoxidase(MPO)-AAV patients.CASE SUMMARY A 52-year-old woman was admitted to our hospital because of headache.Laboratory findings indicated nephrotic range proteinuria and microscopic hematuria,serum creatinine of 243μmol/L,anti-MPO antibody titer of>400 RU/mL,and positive perinuclearantineutrophil cytoplasmic antibody.Renal biopsy showed pauci-immune crescentic glomerulonephritis.The cerebrospinal fluid examination and brain magnetic resonance imaging did not show any abnormality.Therefore,MPO-AAV was diagnosed.Corticosteroids,plasmapheresis,and cyclophosphamide as induction therapy and mycophenolate mofetil(MMF)as maintenance therapy were administered.The patient’s headache disappeared;serum creatinine returned to normal;complete remission of microscopic hematuria and proteinuria was observed.Anti-MPO antibody titer reached normal limits after immunosuppressive treatment.Twenty-five months after stopping the immunosuppressive treatment,the patient relapsed with arthralgia,without neurological or renal involvement.The patient’s arthralgia improved after treatment with prednisone and MMF.CONCLUSION We have reported a rare case of MPO-AAV who initially presented with headache and kidney involvement.However,relapse presented with only arthralgia,which was completely different from the initial manifestations.This case suggests that AAV relapse should be highly suspected in MPO-AAV patients after remission,when clinical manifestations at relapse are different from those at onset.Prednisone and MMF may provide a good choice for refractory arthralgia during relapse in MPO-AAV patients.

Background and purpose Medullary infarction(MI)caused by spontaneous vertebral artery dissection(sVAD)is an important type of stroke.It is important to distinguish sVAD from other causes of stroke since the treatment strategies and prognosis were different between them.In this study,we aimed to explore the clinical and radiological features of MI in patients with acute MI caused by sVAD.Methods Patients with acute MI caused by sVAD and non-sVAD in a single tertiary hospital were enrolled from 2010 to 2020.Epidemiologic,clinical and image features were collected and analysed.MI lesions were categorised into three levels rostrocaudally and four arterial groups:anteromedial,anterolateral,lateral and posterior.Results A total of 128 patients with MI were enrolled with 47 cases of sVAD and 81 cases of non-sVAD.Patients with sVAD were younger than those with non-sVAD(med 44 years old vs 58 years old).The sVAD group was less likely to have hypertension(44.68%vs 67.90%;p=0.010)and diabetes(19.15%vs 45.69%;p=0.003),but more likely to have non-sudden onset(27.66%vs 9.87%,p=0.009),minor neck injury(19.15%vs 1.23%;p=0.001)and headache(46.81%vs 7.41%;p=0.000).Vertically,sVAD became more common in caudal medulla than in rostral medulla.Horizontally,the sVAD group was more likely to have lateral MI(91.48%vs 2.96%,p=0.000).In multivariable logistic regression analysis,age,non-sudden onset and headache were independently associated with sVAD with ORs of 0.935(95%CI 0.892 to 0.981,p=0.006),3.507(95%CI 1.060 to 11.599,p=0.040)and 5.426(95%CI 1.673 to 17.599,p=0.005).Conclusion sVAD was not uncommon in patients with MI,especially in patients with lateral MI.Young patients with headache and non-sudden onset should remind clinician the possibility of sVAD.

BACKGROUND Sick sinus syndrome is a common disease in cardiology.Typical symptoms include palpitations,dizziness,shortness of breath,chest tightness,and amaurosis.However,to date,there are no known reports of sick sinus syndrome presenting with headache.Whether there is a correlation between headache and sick sinus syndrome merits further research.In this report,we describe a case of headache induced by sick sinus syndrome.CASE SUMMARY A 73-year-old female patient presented to our department with the chief complaint of recurrent paroxysmal headache for more than 7 years.The patient described paroxysmal palpations,usually headache occurring after palpitation.Her blood pressure was normal when the most recent headache occurred.A magnetic resonance imaging study and magnetic resonance angiography of the head at another center were normal.A clinical neurological examination was negative.A 24-h Holter electrocardiogram monitoring study showed sick sinus syndrome.The patient received dual chamber pacing implantation and was administered drug therapy to control ventricular rate.The patient’s paroxysmal headaches and palpitations had resolved within 1 year,confirmed via a follow-up telephone call.CONCLUSION After dual-chamber pacing implantation and drug therapy administration to control the ventricular rate,the patient’s paroxysmal headaches and palpitations had resolved within 1 year,confirmed via a follow-up telephone call.We believe that the headaches were related to the patient’s sick sinus syndrome.
