BACKGROUND Embedded foreign bodies in the tongue are rarely seen in clinical settings.An untreated foreign body can cause a granuloma which often presents as an enlarged tongue mass.However,if foreign body ingestion status is unknown,physical examination and magnetic resonance imaging(MRI)tend to lead to suspicion of tongue cancer,especially in older patients.Thus,differential diagnosis of an enlarged tongue mass is important,especially because it is closely related to the choice of treatment method.CASE SUMMARY A 61-year-old woman was admitted to the hospital with pain and noticeable swelling in the tongue that had persisted for over 1 mo.She had no previous medical history.MRI revealed abnormal signal intensities that were indicative of a neoplasm.Thus,the oral surgeon and radiologist arrived at a primary diagnosis of tongue cancer.The patient visited the Ear Nose and Throat Department for further consultation and underwent an ultrasound examination of the tongue.The ultrasonography was consistent with a linear hyperechoic foreign body which was indicative of an embedded foreign body(bone)in the tongue,even though the patient denied any history of foreign body ingestion.Complete surgical enucleation of the lesion was conducted.The mass which included a fish bone was completely removed.The post-operative pathological examination confirmed that the mass was a granuloma containing collagen fibers,macrophages and chronic inflammatory cells.The patient recovered without complications over a 2 mo follow-up period.CONCLUSION We report a rare case of foreign body granuloma in the tongue that was primarily diagnosed as tongue cancer.The MRI and ultrasound examinations revealed a piece of bone in the left lateral aspect of the tongue.The granuloma,which contained a fish bone,was completely removed via surgery and confirmed via biopsy.Differential diagnosis of the enlarged tongue mass was critical to the selection of treatment method.

Epstein-Barr virus(EBV),a double-stranded DNA virus with an envelope,is a ubiquitous pathogen that is prevalent in humans,although most people who contract it do not develop symptoms(Kerr,2019).While the primary cells EBV attacks are epithelial cells and B lymphocytes,its target range expands to a variety of cell types in immunodeficient hosts.Serological change occurs in 90%of infected patients.Therefore,immunoglobulin M(IgM)and IgG,serologically reactive to viral capsid antigens,are reliable biomarkers for the detection of acute and chronic EBV infections(Cohen,2000).Symptoms of EBV infection vary according to age and immune status.

The antitumor capabilities of agonistic anti-4-1BB mAbs have made them an attractive target for tumor immunotherapy.However,the adverse side effects associated with agonist antibodies have hindered their clinical development.Here,we aimed to study the immune-related adverse events of repeated doses and long-term use of agonistic anti-4-1BB mAbs.We show that chronic activation of 4-1BB signals induced the accumulation of IFN-γ-producing PD-1^(+)CD8^(+)T cells in the secondary lymphoid organs of tumor-bearing mice by increasing the number of dividing CD8^(+)T cells,which was beneficial for suppressing tumor growth in the early phase of anti-4-1BB induction.However,repeated exposure to anti-4-1BB mAbs led to granuloma development in tumor-draining lymph nodes(TDLNs)of mice due to recruitment and accumulation of macrophages via the CD8^(+)T cell-IFN-γaxis.This was accompanied by excessive lymph node swelling,which impaired the sequential activation of CD8^(+)T cells.Our data provide insights into the immune-related adverse events of long-term agonist 4-1BB antibody dosing,which should be considered during the clinical development of immunomodulating therapy.

This is a case report of mediastinal fungal granuloma in an immunocompetent host. The definite diagnosis was made by pathological biopsy via video-assisted thoracoscopy and silver methenamine staining showed aspergillus hyphae and spores in the epithelioid granuloma. In conclusion, opportunistic pathogenic fungi can cause granulomatous inflammation in mediastinal lymph nodes in an immunocompetent host, as it can do in an immunocompromised host. More attention should be paid on tissue biopsy and pathological examination to ensure a correct diagnosis for these kinds of cases.

A middle aged uncircumcised man presented with a painless growth on the glans penis which started 6 months back as a pinhead sized papule and rapidly progressed in size.The patient reported occasional bleeding from the lesion on minimal trauma.The patient was unable to perform sexual intercourse owing to the size of lesion and concern of bleeding.The patient was in monogamous relationship with his wife and denied any high risk sexual behaviour.On examination,there was a non-tender,oval,dark red,smooth,shiny,exophytic pedunculated growth of size 2.5 cm2 cm0.5 cm on the glans with a small but broad pedicle at the base of the lesion(Fig.1).Histopathological examination of the mucosal biopsy from the lesion revealed lobular proliferation of capillaries with thinned out epithelium(Fig.2).The diagnosis of penile pyogenic granuloma(PG)was reached on the basis of above findings.The lesion was referred to urology services and the lesion was surgically excised.

Summary: To investigate the etiology and pathogenesis of cholesteatoma otitis media accompanied by cholesterol granuloma and the relationship between cholesteatoma and cholesterol granuloma, 63 cases of middle ear cholesterol granuloma treated in our hospital during the period from March 1988 to May 2000 were retrospectively reviewed. All cases were surgically and pathologically verified. 15 cases of cholesteatoma coexisting with cholesterol granuloma were found among the 63 patients. All 15 cases had a long-term history of otitis media, such as otorrhea (sanguine purulent otorrhea and bloody otorrhea in 8 cases) and perforation of the eardrum (perforation of pars flaccida in 8 cases). Temporal bone CT scans showed cholesteatoma in 11 cases. All patients were treated surgically, and cholesteatoma and cholesterol granuloma were found coexisting alternately, the latter lying mainly in the tympanic antrum, attic and mastoid air cells. Chocolate-colored mucus was accumulated in well-developed mastoid air cells, and glistening dotty cholesterol crystals were also found. In most cases, enlarged aditus, destruction of lateral attic wall, erosion of ossicular chain, exposure of horizontal segment of facial nerve and tegmen of attic were observed. Occlusion of Eustachian tube was noted in 6 cases, and occlusion of tympanic isthmus was revealed in all cases. A post-operative dry ear was achieved in all patients, and hearing improvement was achieved in all 12 cases following tympanoplasty. Cholesteatoma and cholesterol granuloma in middle ear may share a common pathophysiological etiology: occlusion of ventilation and disturbance of drainage. The diagnosis should be considered when patients presented with chronic otitis media with bloody otorrhea. CT and magnetic resonance imaging are useful for the diagnosis before operation. The surgical approach depends on the location, extension and severity of the lesion, The purpose of surgery is to remove the lesion and create an adequate drainage.
