Implant failure,which is commonly associated with failure of osseointegration and peri-implant infection,is a severe complication of orthopedic *** particular,the survival rate of implants is significantly decreased in patients using long-term glucocorticoids(GCs).However,the exact molecular mechanism underlying GCs-induced implant loosening,as well as preventive strategies for these patients,is *** address this problem,we performed RNA-sequencing and found that WNT16 was correlated with GCs-induced osteopenia(LogFC=-5.17,p<0.01).Inspired by the concept of“organic-inorganic”hybrid,we theorized to introduce a bioactive two-dimensional nanosheet into a layer-by-layer(LbL)self-assembly coating to construct a customized implant that targets *** screening commercially available nanosheets,laponite(LAP)was identified as a cost-effective rescuer for GCs-induced WNT16 inhibition,which was then intercalated into LbL deposition system consisting of quaternized chitosan(QCS)and hyaluronic acid(HA).The hybrid coating(QCS/HA/LAP)showed micrometer thickness and improved hydrophilicity and interface ***,QCS/HA/LAP coated polyetheretherketone(PEEK)implant enhanced cell viability,adhesion,and osteogenic differentiation of bone marrow mesenchymal stem cells(BMSCs),and promoted osteointegration of PEEK in GCs-treated rats by targeting the WNT16/β-catenin *** assembled QCS has proven antibacterial properties,and the hybrid coating exerted potent detrimental effects against methicillin-resistant Staphylococcus aureus(MRSA)and Escherichia coli(***),both in vitro and in *** together,these results suggest that QCS/HA/LAP coating has great potential for use in implants customization,and has synergistic pro-osteogenic and antibacterial effects that help prevent implant failure in GCs-treated patients.

Escin, as an internally applied anti-inflammatory agent, has been widely used in the treatment of inflammation and edema resulting from trauma or operation in the clinic. However, the effect of its external use on cutaneous inflammation and edema remains unexplored. In the present study, the anti-inflammatory and anti-edematous effects of external use of escin were studied in carrageenan-induced paw edema and histamine-induced capillary permeability in rats, paraxylene-induced ear swelling in mice, and cotton pellet-induced granuloma in rats. Effects of external use of escin gel on prostaglandin E2(PGE2), tumor necrosis factor-α(TNF-α), and interleukin-1β(IL-1β) were determined by ELISA. The anti-inflammatory mechanism was explored by detecting the expression of glucocorticoid receptor(GR) with Western blotting and Real-time PCR analyses, with further exploration of nuclear factor-κB(NF-κB), p38 mitogen-activated protein kinase(P38 MAPK) and activator protein-1(AP-1) expressions. We demonstrated that external use of escin showed significant anti-inflammatory effects on acute and chronic inflammation in different animal models and its anti-inflammatory effects might be related to down-regulation of PGE2, TNF-α, and IL-1β. The results also showed that escin exerted its anti-inflammatory effects by promoting the expression of GR, with the possible mechanism being inhibition of the expressions of GR-related signaling molecules such as NF-κB and AP-1.

glucocorticoids(GCs)have been widely used as immunosuppressants and antiinflammatory agents to treat a variety of autoimmune and inflammatory diseases,and they fully exert their anti-inflammatory and immune-regulating effects in the *** effect of GCs on white blood cells is an important part of their *** can cause changes in peripheral blood white blood cell counts by regulating the proliferation,differentiation,and apoptosis of white blood *** the total number of white blood cells,neutrophil counts,lymphocytes,and eosinophils increases,the counts of basic granulocytes and macrophages *** addition,GCs can regulate the activation and secretion of white blood cells,inhibit the secretion of a variety of pro-inflammatory cytokines,the expression of chemokines,and promote the production of anti-inflammatory *** patients on GC therapy,the effects of GCs on leukocytes were similar to the changes in peripheral blood caused by bacterial ***,we suggest that clinicians should be more cautious in assessing the presence of infection in children with long-term use of GCs and avoid overuse of antibiotics in the presence of elevated *** work through genomic and non-genomic mechanisms in the human body,which are mediated by GC *** recent years,studies have not fully clarified the mechanism of GCs,and further research on these mechanisms will help to develop new therapeutic strategies.

Pretibial myxedema(PTM), an uncommon manifestation of Graves' disease(GD), is a local autoimmune reaction in the cutaneous tissue. The treatment of PTM is a clinical challenge. We herein report on a patient with PTM who achieved complete remission by multipoint subcutaneous injections of a long-acting glucocorticoid and topical glucocorticoid ointment application for a self-controlled study. A 53-year-old male presented with a history of GD for 3.5 years and a history of PTM for 1.5 years. Physical examination revealed slight exophthalmos, a diffusely enlarged thyroid gland, and PTM of both lower extremities. One milliliter of triamcinolone acetonide(40 mg) was mixed well with 9 mL of 2% lidocaine in a 10 mL syringe. Multipoint intralesional injections into the skin lesions of the right lower extremity were conducted with 0.5 mL of the premixed solution. A halometasone ointment was used once daily for PTM of the left lower extremity until the PTM had remitted completely. The patient's PTM achieved complete remission in both legs afteran approximately 5-mo period of therpy that included triamcinolone injections once a week for 8 wk and then once a month for 2 mo for the right lower extremity and halometasone ointment application once daily for8 wk and then once 3-5 d for 2 mo for the left lower extremity. The total dosage of triamcinolone acetonide for the right leg was 200 mg. Our experience with this patient suggests that multipoint subcutaneous injections of a long-acting glucocorticoid and topical glucocorticoid ointment application are safe, effective,and convenient treatments. However, the topical application of a glucocorticoid ointment is a more convenient treatment for patients with PTM.
