BACKGROUND Since Heald proposed the total mesorectal excision(TME)procedure,the prognosis of patients with rectal cancer has been significantly improved.But Heald did not specifically describe the anterior surgical plane in female patients.And the surgical plane for mobilizing the anterior rectal wall during TME surgery in female patients remains controversial.AIM To investigate the anatomy of the female pelvis and identify the optimal plane for mobilizing the anterior rectal wall.METHODS We retrospectively collected surgical procedure videos and clinical data of female patients diagnosed with middle or low rectal cancer who underwent the TME procedure between January 2020 and October 2022 across six hospitals.The patients were divided into two groups based on the surgical approach used to mobilize the anterior rectal wall:The experimental group was to open the peritoneum at the lowest point of the peritonea reflection and enter the plane for mobilizing,while the control group was cut at 0.5-1 cm above the peritoneal reflection and enter another plan.Then,we compared the preoperative and postoperative information between the two groups.We also dissected and observed ten adult female pelvises to analyze the anatomic structure and compare the entry plane between the two approaches.Finally,we researched the pathological structure between the rectum and the vagina.RESULTS Finally,77 cases that met the criteria were included in our study.Our observations revealed that the experimental group underwent a smooth procedure,entering the plane amidst the mesorectal fascia and adventitia of the vagina,whereas the control group entered the plane between the vaginal adventitia and muscle layers.Compared to the control group,the experimental group showed a significant decrease in intraoperative bleeding[22.5(19.5-50)mL vs 17(5-20)mL,P=0.01],as well as a shorter duration of hospitalization[9(7-11.25)d vs 7(6-10)d,P=0.03].Through the examination of surgical videos and cadaveric studies,we discovered that Denonvilliers'fascia is a

Update Knowledge Assessment and Influencing Predictor of female Fertility Preservation in Oncologists

Objective:This study aims to offer an update assessment of the knowledge of Chinese oncologists on female fertility preservation,and identify the determinants that influence the implementation of fertility preservation.Methods:A total of 713 Chinese oncologists with different specialties completed the online self-report questionnaire to assess their understanding of fertility risks in cancer treatment,knowledge on female fertility preservation,and perceptions on the barriers in referring patients for fertility preservation.Results:Although most oncologists were familiar with fertility risk in cancer treatment,half of them lacked the knowledge for reproduction and preservation methods.In the multivariable model,oncologists in a hospital with a specialized reproductive institution,positive precaution for fertility risk,and fertility preservation discussion with patients were significantly correlated with the possibility of fertility preservation referral.Conclusions:The intervention targets based on the update evaluation and identified influencing determinants will be helpful for all the oncofertility researchers,oncologists and institutions in future efforts for well-established female fertility preservation services.

BACKGROUND In China,it has been well recognized that some female patients with esophageal squamous cell carcinoma(ESCC)have different overall survival(OS)time,even with the same tumor-node-metastasis(TNM)stage,challenging the prognostic value of the TNM system alone.An effective predictive model is needed to accurately evaluate the prognosis of female ESCC patients.AIM To construct a novel prognostic model with clinical and reproductive data for Chinese female patients with ESCC,and to assess the incremental prognostic value of the full model compared with the clinical model and TNM stage.METHODS A new prognostic nomogram incorporating clinical and reproductive features was constructed based on univariatie and Cox proportional hazards survival analysis from a training cohort(n=175).The results were recognized using the internal(n=111)and independent external(n=85)validation cohorts.The capability of the clinical–reproductive model was evaluated by Harrell’s concordance index(C-index),Kaplan–Meier curve,time-dependent receiver operating characteristic(ROC),calibration curve and decision curve analysis.The correlations between estrogen response and immune-related pathways and some gene markers of immune cells were analyzed using the TIMER 2.0 database.RESULTS A clinical–reproductive model including incidence area,age,tumor differentiation,lymph node metastasis(N)stage,estrogen receptor alpha(ESR1)and beta(ESR2)expression,menopausal age,and pregnancy number was constructed to predict OS in female ESCC patients.Compared to the clinical model and TNM stage,the time-dependent ROC and C-index of the clinical–reproductive model showed a good discriminative ability for predicting 1-,3-,and 5-years OS in the primary training,internal and external validation sets.Based on the optimal cut-off value of total prognostic scores,patients were classified into high-and low-risk groups with significantly different OS.The estrogen response was significantly associated with p53 and apoptosis pathways in esophageal cancer.CONCLUSION T

Dear Editor, In nature or during artificial breeding,evolutionary interven-tion or desirable trait selection could be achieved by means of gender control in an organism.Previously reported approaches including sex-sorted semen and genetic manipulation of a certain gene(Holden and Butler,2018;Yosef et al.,2019).In mice,a recent study demonstrated a genetic system for biasing sex ratio through crossing two genetically engineered mouse lines(Yosef et al.,2019).The maternal line encodes functional Cas9 protein on an auto-somal chromosome,whereas the paternal encodes guide RNAs on Y chromosome targeting vital genes.After fertil-ization,males carrying both Cas9 and guide RNAs were self-destructed,resulting in a female biased sex ratio.However,the litter size is proximate half of normal size since half of the progeny are eliminated.Moreover,a reproductive reservoir of males and females must be maintained for crossing.Here,we established a mouse line carrying Hspa2 promoted Cas9 and constitutively expressed sgRNA targeting repetitive sequences on Y chromosome.In those mice,spermatogenic cells carrying Y chromosome were eliminated during sper-matogenesis,resulting in female biased sex ratio in proge-nies whereas the total number was unaffected.

To the Editor:female infertility due to reproductive aging has become a prominent concern in the population.Evidence indicates that oxidative stress is one of the dominant mechanisms underlying reproductive aging.[1,2]Although female reproductive aging involves gradual changes to the hypothalamic-pituitary-ovarian(HPO)axis,previous studies support a key involvement of the hypothalamus.[3,4]In our recent dataset,it was shown that estrogen-induced oxidative stress led to the senescence of hypothalamic astrocytes that predated reproductive dysfunction.[3]Growing evidence has demonstrated that antioxidants might delay reproductive aging by reducing oxidative damage to ovaries.[1,2]However,the effect of antioxidants on the hypothalamus has not been reported.Astaxanthin(AST),3,3'-dihydroxy-β-carotene-4,4'-dione,is a xanthophyll carotenoid found predominantly in marine animals such as salmon,trout,lobster,and shrimp.Ginseng is a tonic drug in traditional Chinese medicine.Ginsenoside Rg1,a class of steroidal glycosides,is a prominent active anti-aging ingredient within ginseng responsible for these effects.Notably,the two antioxidants freely cross the blood-brain barrier.In this study,we evaluated the effects of these two natural compounds on the hypothalamus and ovaries in protecting female mice from reproductive aging.

To the Editor:Choledochocele,also known as type III choledochal cysts in the classification by Todani et al.[1],is a congenital abnormality of the biliary system.It is characterized by a cystic dilation of intramural segment of the distal common bile duct(CBD)protruding into the descending duodenum.Choledochocele makes up about 0.5%−4%of choledochal cysts[1,2].Compared with other subtypes,the incidence of choledochocele is extremely low and it frequently presents in adults at a relatively older age,with an average age of 51 years[3].The diagnosis and treatment are challenging.Here,we present a case of a young female patient with this rare disease entity,who recovered after effective endoscopic retrograde cholangiopancreatography(ERCP)management.

To the Editor:Non-alcoholic fatty liver disease(NAFLD)comes to prevail in Asia and,the prevalence in China is reported tobe24%to36%.lilThe prevalence ofNAFLD and its associated medical burden might be underestimated.The diagnosis of NAFLD led to the exclusion of other chronic liver diseases including“excess”alcohols,thus the population with alcoholic consumptions or other liver diseases might be neglected.In March 2020,the international expert consensus proposed a new concept,“metabolic associated fatty liver disease(MAFLD),”“and replaced the earlier term” of NAFLD.2As themost prevalent female malignancy,breast cancer(BC)shares similar risk factors to MAFLD.Higher levels of obesity and physical inactivity are closely correlated to BC and are significant drive for the increasing trends in the prevalence of BC.Some BC survivors may receive chemotherapy and endocrine therapy,both of which are reported to increase the risk of hepatic steatosis(HS),which is the fundamental criteria of the diagnosis of MAFLD.

female scientists are playing more and more important roles in science-related fields,although they just account for a minority(only 30%)of the scientific community worldwide,according to the 2020 data from the Institute for Statistics of UNESCO.In fact,female researchers have been the mainstay in organizing and guiding the scientific research in many fields.Showing the growing impact of female scientists is of significance as role models to enlighten younger generations and particularly encourage young girls to dedicate in science.The Special Issue of“female scientists in nano research”publishes such a collection that shares the research outcomes from those labs led by female scientists in nano-related research fields and showcases their high impacts on many aspects of nanomaterials and applications.This special issue collects 58 papers,including 16 reviews and 42 original research articles,which present the latest advances in area across diverse compelling topics in nano-science and technology,such as nanoscience for fluorescence and bioimaging,nanomaterials for energy,electronics and catalysis,sensing platforms based on nanoscience,nanobiotechnology and nanomedicine.

The gender gap in the practice of intensive care medicine has increasingly been recognized as a problem in recent years.Despite limited information,the available data suggest that representation of women in the physician workforce,academic positions,and leadership roles in intensive care is inadequate globally.In this article,we describe the situation of female intensive care unit(ICU)physicians from the perspective of Chinese intensivists.The proportion of female ICU physicians in China has been increasing in recent years.The biggest challenges faced by female ICU physicians include balance of work and life,difficulties in career planning,and lack of academic influence.More attention and policy support should be provided to help them play a better role in intensive care medicine.

Effect of date palm pollen supplementation on female sexual function in non-menopausal women:A double blind randomized clinical trial

Objective:Despite numerous experimental studies in the literature,there are few clinical trials regarding the effect of date palm pollen(DPP)supplementation on sexual function improvement.In the present study,we sought to evaluate the impact of DPP on female sexual function in Iranian non-menopausal women.Methods:Between October 2019 and December 2019,health centers in the city of Khalkhal,volunteers meeting the inclusion criteria were recruited in randomized clinical trials.Sixty-eight women were randomly stratified and assigned to one of the two study groups:placebo group(n=35)and palm pollen group(n=35),and received a starch or palm pollen capsule(300 mg per day),respectively,for 35 d.The female Sexual Function Index(FSFI)instrument was used to assess female sexual function.Results:After DPP supplementation,the increase in desire,lubrication,and the overall score,was statistically significant compared to the placebo group(P=0.002,P=0.000,and P=0.042;respectively);Whilst there was no significant differences in the remaining domains(arousal:P=0.763;orgasm:P=0.370;satisfaction:P=0.474;pain:P=0.259).There was a statistically significant positive correlation between the coitus and preintervention levels of desire(r=0.298,P=0.038),arousal(r=0.328,P=0.021),lubrication(r=0.361,P=0.011),orgasm(r=0.320,P=0.025),satisfaction(r=0.327,P=0.022),and overall scores(r=0.338,P=0.018).Conclusion:This study suggests that DPP(300 mg supplementation for 35 d),given to non-menopausal women,could improve the lubrication and desire domains of FSFI.
