Objective To investigate the clinical features and Tlymphocytes subsets in patients with acquired immunedeficiency syndrome ( AIDS ) and cytomegalovirus(CMV) infection. Methods A total of 48 hospitalizedpatients with human immunodeficiency virus (HIV)-1 /AIDS and CMV infections were recruited at Peking UnionMedical College Hospital from Jan 2010 to Aug 2017.Their clinical features and immune function were retrospectivelyanalyzed. Patients with only HIV/AIDS inprevious study were recruited as controls. Results All 48patients were at C3 stage, including 36 men and12 women. Five of them were younger than 30 years old,33 cases within 31 - 50 years old,and 10 cases olderthan 50 years old.

Objective To determine anti-cytomegalovirus(CMV) antibodies along with anti-Chlamydia pneumoniae(CP) antibodies in comparison with inflammatory markers and other risk factors of atherosclerosis in patients with selected cardiovascular diseases(CVD) . Methods A total of 228 patients with coronary heart disease(CHD) and/or hyperten-sion(HT) ,and those who underwent reconstructive vascular surgery(RVS) on carotids or abdominal aorta were tested for the presence of anti-CMV IgG and IgM antibodies as well as for anti-CP IgA antibodies,C-reactive protein(CRP) ,and interleukin-6(IL-6) . Other risk factors for atherosclerosis,namely age,gender,smoking,hypercholesterolemia,and diabetes mellitus were also analyzed. Results Anti-CMV IgG antibodies were found in 204 patients sera(89.5%) ,compared with 46 positive of 68 sera in the controls(67.6%) ,whereas anti-CMV IgM antibodies were detected in 4 of 54 sera of patients tested(7.4%) ,but not in the controls. The highest proportion of positive sera with not only anti-CMV IgG antibodies(95.6.7%) ,but also anti-CP IgA antibodies(78.3%) ,IL-6(84.8%) and CRP(97.8%) ,was observed in patients with RVS. The results obtained corresponded to age,hypercholesterolemia,and diabetes. Conclusions The presence of anti-CMV antibodies together with antibodies to CP and markers of inflammation(CRP and IL-6) in our study was associated with CVD,primarily in elderly patients who underwent RVS.

Objective To determine whether the low immunity AIDS patients have opportunistic infections. Methods Patients with blood culture bacteria and fungi, observe the colony growth and morphology, and the smears, staining was observed cell characteristics under the microscope. Using serology and PCR methods to determine whether the patients with complicated infection. Results Blood culture found that the positive fungi was biphasic fungi, yeast phase growth at 37℃ while penicillium phase growth and yield rose red pigment at 25, just like the broom-like sticks under the microscope. Both IgM CMV antibody and cytomegalovirus-DNA qualitative fluorescence of PCR were positive. Conclusion The AIDS patient coexisting with penicilliummar neffei and cytomegalovirus infection. Combined detection a variety of methods in early detection can help to early discovery and treatment of the opportunistic infections in AIDS patients.

IL28B polymorphism and cytomegalovirus predict response to treatment in Egyptian HCV type 4 patients

A 9-month-old infant presented with persistent fever and bloody diarrhea. Antibacterial and antifungal treatment failed to relieve the symptoms, and his condition continued to deteriorate. On the 23rd hospital day, gastroscopy and colonoscopy were performed under general anesthesia and the diagnosis was confirmed by histopathological analysis. Both the nonspecific clinical manifestations and weak diagnostic value of serologic antibody detection often make diagnosis difficult. Our experts examines the case and offers a differential diagnosis, pathological findings and treatment options for an infant presenting with persistent fever and bloody diarrhea.

AIM:To test whether the status of positive cytomegalovirus(CMV) DNA detection adds to the predictive value of IL28B and to further categorize C/T allele carriers.METHODS:This study included 166 chronic hepatitis C(CHC) patients who received combined interferon and ribavirin therapy for 48 wk,84 spontaneous hepatitis C virus(HCV) resolvers who were positive for IgG anti-HCV antibody and negative for HCV RNA,and 100 healthy subjects who were negative for both HCV antibodies and RNA as controls.Genomic DNA from peripheral blood was used for IL28B rs.12979860 single nucleotide polymorphism(SNP) and CMV DNA detection.A 139 bp fragment containing IL28B SNP was amplified in all subjects by polymerase chain reaction using a specifically designed primer.Then the IL28B rs.12979860 SNP was detected by restriction fragment length polymorphism(RFLP) genotyping.The presence of CMV DNA was tested by amplification of the gB1 gene using nested polymerase chain reaction.The role of CMV and IL28B rs.12979860 SNP genotypes in determining the response rate to combined interferon therapy and clinical status of patients were statistically analyzed.RESULTS:Current data showed that 67% of patients carrying the IL28B 12979860 C/C allele had a sustained viral response(SVR) while the genotypes C/T and TT were associated with lower SVR rates,50% and 48%,respectively.SVR rates for the C/C allele were lower than other HCV genotypes and/or other populations.Genotype CC was associated with the response to interferon(P = 0.025).Genotype C/C was reduced from 48% in controls to 14% in CHC patients suggesting its protective role against progression to chronicity.The majority of spontaneously cleared subjects(86%) were C/C,confirming its protective role.The C/T allele was present in 71% of CHC patients compared with 38% of controls,so the use of IL28B SNP genotyping only in these patients may be of little value as a predictor of response.CMV reactivation occurred in 40% of CHC patients.Co-infection with CMV seriously diminished the response to interf

Human cytomegalovirus （HCMV） late mRNA expression in megakaryoblast and in turn the pathogenesis of idiopathic thrombocytopenic purpura （ITP） patients with HCMV infection, and effectiveness of ganciclovir were investigated. Colony forming unit-megakaryocytes （CFU-MK） of 46 ITP patients with HCMV infection were incubated from patients＇ bone marrow mononuclear cells （MNC）. Reverse transcriptase-polymerase chain reaction （RT-PCR） was subsequently used for CFU-MK for HCMV-late mRNA detection, Ganciclovir therapy was given to both HCMV-late mRNA positive and negative groups for comparison of therapeutic effectiveness, The results in 19 of 46 CFU-MK culture cells specimens with positive HCMV-DNA by PCR or positive CMV-IgM by enzyme linked immunosorbent assay （ELISA） in the correspondent serum of peripheral blood were positive for HCMV-late mRNA, Sixteen out of 19 patients with positive HCMV-late mRNA CFU-MK had a positive response to ganciclovir. Amongst 27 patients with negative HCMV-late mRNA CFU-MK, only 4 positive responders to ganciclovir therapy were observed. Curative effectiveness of ganciclovir in HCMV-late mRNA positive group was significantly higher than that in HCMV-late mRNA negative group （P〈0.01）, It was suggested that HCMV could directly infect CFU-MK, which might be one of the mechanisms responsible for HCMV related ITE The ganci- clovir is an effective therapy in resulting in the increases in thrombocyte in the ITP patients whose HCMV- late mRNA was positive in their CFU-MK.

cytomegalovirus(CMV) is one of the most common viral pathogens causing clinical disease in liver transplant recipients, and contributing to substantial morbidity and occasional mortality. CMV causes febrile illness often accompanied by bone marrow suppression, and in some cases, invades tissues including the transplanted liver allograft. In addition, CMV has been significantly associated with an increased predisposition to acute and chronic allograft rejection, accelerated hepatitis C recurrence, and other opportunistic infections, as well as reduced overall patient and allograft survival. To negate the adverse effects of CMV infection on transplant outcome, its prevention, whether through antiviral prophylaxis or preemptive therapy, is an essential component to the management of liver transplant recipients. Two recently updated guidelines have suggested that antiviral prophylaxis or preemptive therapy are similarly effective in preventing CMV disease in modest-risk CMV-seropositive liver transplant recipients, while antiviral prophylaxis is the preferredstrategy over preemptive therapy for the prevention of CMV disease in high-risk recipients [CMV-seronegative recipients of liver allografts from CMV-seropositive donors(D+/R-)]. However, antiviral prophylaxis has only delayed the onset of CMV disease in many CMV D+/Rliver transplant recipients, and such occurrence of lateonset CMV disease was significantly associated with increased all-cause and infection-related mortality after liver transplantation. Therefore, a search for better strategies for prevention, such as prolonged duration of antiviral prophylaxis, a hybrid approach(antiviral prophylaxis followed by preemptive therapy), or the use of immunologic measures to guide antiviral prophylaxis has been suggested to prevent late-onset CMV disease. The standard treatment of CMV disease consists of intravenous ganciclovir or oral valganciclovir, and if feasible, reduction in pharmacologic immunosuppression. To explore the effect of [-Ca^2＋]i and neuronal mitochondria transmembrane potentials in hippocampus of murine cytomegalovirus （MCMV） infected mice, newborn Balb/c mice were randomly divided into two groups： a virus inoculated group and a control group. After 56 days, single cell of hippocampus was isolated, and mitochondria transmembrane potentials and the intracellular free calcium level [-Ca^2＋]i in hippocampus were measured by means of flow cytometry （FCM）. Compared with the control group, the mitochondria transmembrane potentials was decreased （P〈0. 01 ） and the intracellular free calcium level [-Ca^2＋]i was increased （P〈0. 01） in inoculated group. The dysfunction of [-Ca^2＋]i and mitochondria transmembrane potentials in hippocampus may play an important role in the functional disorders in CMV-infected CNS.

Objective:To determine the prevalence of rubella and cytomegalovirus(CMV)among women in Tabriz City,Iran in 2013.Methods:This study was carried out in numerous laboratories in city of Tabriz.In 2013,26618 women aged 18-35 years were enrolled in this study.Serum samples were analyzed with chemiluminescence apparatus and SPSS software was used to conduct the statistical analysis.Results:A total of 26618 individuals were examined(10598 cases for anti-CMV and 16020 cases for anti-rubella).About 98.9%and 1.1%samples were reported positive and negative for CMV immunoglobulin G(IgG),respectively.The positivity for anti-CMV immunoglobulin M(IgM)antibody was found in 0.75%samples,while 99.25%were negative for the anti-CMV IgM antibody.Anti-IgG against rubella seropositivity was found in 95.8%and rubella IgM seropositivity in 0.93%.Anti-IgG against rubella seronegativity was found in 4.2%and rubella IgM seronegativity in 99.07%.Conclusions:According to the results,primary and secondary infections are usually symptomless or inconspicuous,but they can cause serious fetal damage.Routine antenatal care should therefore include serological testing to check the immune status of the woman and to diagnose any fresh virus infections that may arise.
