The percentage of peripheral blood blasts on day 7 of induction chemotherapy predicts response to therapy and survival in patients with acute myeloid leukemia

Background Rapid clearance of peripheral blood blasts （PBBs） predicts complete remission （CR） and survival in patients with acute myeloid leukemia （AML）.We aimed to explore the correlation between induction therapy response,outcome,and the PBB *** Forty-six consecutive patients with de novo AML （excluding acute promyelocytic leukemia） were enrolled in this *** cytometry was performed to identify cells with a leukemia-associated aberrant immunophenotype in the initial bone marrow aspirate and in peripheral blood on day 7 of induction *** The PBB percentage on day 7 （D7PBBP） was significantly lower in patients who achieved CR （0.03% （0.0%,0.45%）） than in those who did not （10.85% （1.13%,19.38%）; u =-3.92,P 〈0.001）.The CR rate was significantly higher among patients with a D7PBBP of 〈0.945% （84.62%,22/26） than among those with a D7PBBP of 〉0.945% （25.0%,5/20;Х^2 =16.571,P 〈0.001）.D7PBBP was significantly correlated with overall survival （OS; r=-0.437,P=0.003） and relapsefree survival （RFS; r=-0.388,P=0.007）.OS and RFS were significantly higher in patients with a D7PBBP of 〈0.43% than in those with a D7PBBP of 〉0.43% （P 〈0.001 and P=0.039,respectively）.D7PBBP was also found to be an independent prognostic indicator in multivariate analysis for both OS （P=-0.036） and RFS （P=0.035）.Conclusion D7PBBP may be an important risk factor for the achievement of complete remission,for overall survival,and for relapse-free survival.

COMBINATION CHEMOTHERAPY WITH COBDP IN MALIGNANT LYMPHOMA: AN ANALYSIS OF 64 CASES

From February 1986 to February 1989, 64 patients with malignant lymphoma were treated with COBDP regimen. Of these patients, there were 7 cases with Hodgkin's disease and 57 cases with non Hodgkin's lymphoma. Clinical staging showed 7 stage Ⅰ , 5 stage Ⅱ, 22 stage Ⅲ and 30 stage Ⅳ . The COBDP regimen was carried out as : Cyclophosphamide 600 mg iv on days 1,8; vincristine 2 mg iv on days 1,8; Pingyanymycin long im on days 1, 4, 8, 11; cis-dichlorodiamineplatinum 20 mg iv drip on day 1 - 5 and prednisone 10 mg po tid on day 1 -14. Treatment results showed 39% (25 cases) achieving complete remission (CR) , 52% (33 cases) partial remission (PR) , giving an over-all response rate of 91 % (CR+PR). There was a significant difference in the CR of the untreated patients (56%) and that of the treated ones (28%) (P 12 months, vs >9 months) and the median surrivals of the CR patients (> 16. 5 months, vs > 15 months) (P< 0.05). The median survival after CR was significantly longer than that after PR (P<0. 05). The side effects were: anorexia, nausea; vomiting, alopecia and leucopenia without montality or pulmonary complication. The authors believe that COBDP regimen mayserve as the first line attack against walignant lymphomas.

Positive rate of serum CAE-Ab(anti-citric acid extract antibody of human skeletal musie) was used tc find small thymoma early Rabbit Anti-serum against human CAE was labeled to determine thymoma AgNOR(argyrophilic stain of nuclear organizer region)was used to differentiate bemgn and maligant thymoma .As Muller-Hermlink' s *** of cortical type occurred more common and reliable to infiltration Ⅱ Treatment of MG with thymus pathoiogic lestons Except for symptomatic treatment with anti-acytlecholine *** or CTX intravenous perfusion had effect to relieve the myasthenic cnsis Using thymus radiation therapy to 134 cases, the complete remission and remarkable improvement rates were 66.5%.Rhe long-term effect followed up was rather stable. Using percutanous paracentesis into thymus gland for intervention treatment to 13 cases, all of them showed effect within one week.10 cases followed up more than 3 years,9 cases were complete remission or remarkable improvement Thymectmy is still a basic therapy for *** 102 cases with thymectomy,the complete remission and remarkable improvement rates were 59.8%,mortanty was 176%.Continual observation of CAE-Ab could contribute to determine the pateint' s prognosis,9 patients in severe condition followed up within 3 years,their serum CAE-Ab antibody titers persisted at high lever,3 Of them died,but 7 patients followed up within I year,those of their serum antibodies decreased quickly,5 cases of them got

Myasthenia gravis with thymomas: treatment and prognosis

Objective to evaluate the treatments and prognostie factots of myasthenia gravis(MG) with thymomas. Methods: Our study was based on 103 cases of MG with surgically proved thymomas We analysis the subtypes of MG, the types and invasiveness of thymoma and evaluated the long-term effective rate and survival rate Ossermarl degree of MG has been scored as following Grade I :19(18.4%):ⅡA:15(14.6%);ⅡB:48(46.6%):Ⅲ11(10.7%); Ⅳ:8(7.8%). There were 50 patients with lymphocytic predominant 23 the mixed 29 the epithelial predominant I spinal cell thymoma Stage was classified as suggested by Masaoka as following Stage I 57 cases(55.3%). Ⅱ cases(22.3%). Ⅲ 21 cases(20.4%). Ⅳ 2 cases(1.9%)。 Seventy-six patients were administered with steroids (dexamethasone or prednisone). Seven patients were administered with chemothecapy with cyclophosphamide 59 patients were received radiation therapy to the mediastinum as postopenative treatment or beforee opetation. Results: Eighty-one patients with Masaoka Ⅰ and Ⅱ stages fifteen patients with Ⅲ stage were resected totally. 5 patits were with stage Ⅲ and Ⅳ resected subtotally and 2 patients with stage m received only biopsy. The symptoms of MG were improved in 69 patients postopentively exacerbated in 26 (18 cases with myasthenia crisis), unchanged in 5 and sppcared postoperatively in 2 cases. One casc died of operative complications. The rates of postoperative myasthenia cnsis were 13/46(28.3%) in 1970s and 1980s and 5/57(8.8%) in 1990s. local relapse occurred in 9 of 103 (8.7%)of patients liwenty three patients obtained complete remission (22.3%), 26 pharmacological remission (25.2%), 13 marked improvement(12.6%), 19 improvement (18.4%), 4 no change (3.9%) and 18 death (17.5%). The overall effective rate was 78.6%. The overall survival rates were 77.8% and 48.5% at 5 and 10 years respectively Among the three lustologie types, the surviaval rates at 5 years of 10 years of lymphyocytie thymoma were 88.9% and 72.7% respectively mixed thymoma 83.

A Controlled Clinical Study between Hepatic Arterial Infusion with Embolized Curcuma Aromatic Oil and Chemical Drugs in Treating Primary Liver Cancer

Objective: To evaluate the effectiveness, toxicity and prospective application of hepatic arterial infusion (HAI) with embolized Curcuma Aromatic oil (CAO) in treating primary liver cancer (PLC). Methods: In the treated group, 32 patients with PLC were treated by HAI with 1-3 ml embolized CAO and oral administration of Chinese herbal medicine. In the control group, 32 patients with PLC were treated with transcatheter artery chemoembolization (TACE). Results: In the treated group, one patient attained complete remission (CR) and 13 partial remission (PR), the total effective rate being 43.75%. The level of alpha fetal protein (AFP) turned to normal range in 7 cases and decreased in other 7. In the control group, 10 obtained PR and the total effective rate being 31.25%, AFP level turned to normal in 5 and decreased in 2. There was no statistical significance between the two groups. The incidences of post-embolism Sydrome, such as fever, abdominal pain and vomiting were similar between the two groups but no myelosuppression occurred in the treated group with significant difference as comparing with that in the control group (P<0.01). The mean survival time, median survival time, 1-, 2- and 3-year survival rate in the treated group were 11.5 months, 10 months, 37.5%, 13.3% and 6.9% respectively, while in the control group they were 7.25 months, 6 months, 15.6%, 3.2% and 0% respectively. Better result in mean survival time, median survival time and 1-year survival rate was found in the treated group than that of the control group (P<0.05). Conclusion:HAI with embolized CAO showed a similar good effect in treating PLC as that of TACE, but superior to TACE with longer survival time and milder myelosuppression.
