Associations between parental adherence to healthy lifestyles and cognitive performance in offspring:A prospective cohort study in China

Background:Previous studies have reported associations of specific maternal and paternal lifestyle factors with offspring’s cognitive development during early childhood.This study aimed to investigate the prospective associations between overall parental lifestyle and offspring’s cognitive performance during adolescence and young adulthood in China.Methods:We included 2531 adolescents aged 10-15 years at baseline in 2010 from the China Family Panel Studies.A healthy parental lifestyle score(ranged 0-5)was constructed based on the following five modifiable lifestyle factors:Smoking,drinking,exercise,sleep,and diet.Generalized estimating equation models were used to examine the association between baseline parental healthy lifestyle scores and offspring’s fluid and crystallized intelligence in subsequent years(2012,2014,2016,and 2018).Results:Offspring in the top tertile of parental healthy lifestyle scores performed better in overall fluid intelligence(multivariable-adjusted β=0.53,95%confidence interval[CI]:0.29-0.77)and overall crystallized intelligence(multivariable-adjusted β=0.35,95%CI:0.16-0.54)than those in the bottom tertile of parental healthy lifestyle scores.The results were similar after further adjustment for the offspring’s healthy lifestyle scores and persisted across the subgroups of parental socioeconomic status.Additionally,maternal and paternal healthy lifestyle scores were independently associated with better offspring’s cognitive performance,with significant contribution observed for paternal never-smoking,weekly exercise,and diversified diet.When both parents and offspring adhered to a healthier lifestyle,we observed the highest level of the offspring’s overall crystallized intelligence.Conclusions:Our study indicates that parental adherence to a healthier lifestyle is associated with significantly better offspring’s cognitive performance during adolescence and early adulthood,regardless of socioeconomic status.These findings highlight the potential cognitive benefits of promoting healthy lif

Effect of Shenmai Injection on Long-Term Prognosis of Patients with Chronic Heart Failure:A Multicenter,Large Sample Capacity,Long-Term Follow-Up Retrospective cohort study

Objective:To explore the effect of Shenmai Injection(SMI)on the long-term prognosis of patients with chronic heart failure(CHF).Methods:The Hospital Information System was used to extract data of CHF patients,and the retrospective cohort study was conducted for analysis.In non-exposed group,standardized Western medicine treatment and Chinese patent medicine or decoction were applied without combination of SMI while in the exposed group,SMI were applied for more than 7 days.Evaluation indicators are followed with New York Heart Association functional classification(NYHA classification),left ventricular ejection fraction(LVEF),N-terminal brain natriuretic peptide precursor(NT-ProBNP),cardiogenic death and heart failure(HF)readmission.Statistical analysis includes Kaplan-Meier analysis and Cox regression which are used to explore the relationship between SMI and outcome events.Results:A total of 1,211 eligible CHF patients were involved and finally 1,047 patients were followed up successfully.After treatment,the cases of NYHA classification decline in the exposed and non-exposed groups accounted for 64.30%and 43.45%,respectively;the improvement values of LVEF were 8.89%and 7.91%,respectively;the improvement values of NT-ProBNP were 909 pg/mL and 735 pg/mL,respectively.After exposure on SMI,the rates of cardiogenic death and HF readmission reduced from 15.43%to 10.18%and 38.93%to 32.37%.According to Kaplan-Meier analysis,the log-rank P value of SMI and cardiogenic death was 0.014,while the counterpart of SMI and HF readmission was 0.025.Cox regression analysis indicated that for cardiogenic death,age,cardiomyopathy,diabetes,and NYHA classification were risk factors whileβ-blockers,aldosterone receptor antagonists,Chinese patent medicine/decoction and SMI were protective factors.Likewise,for HF readmission,age,cardiomyopathy,and NYHA classification were risk factors while SMI was a protective factor.Conclusion:Combination with SMI on the standardized Western medicine treatment can effectively reduce cardiogenic mortal

Background Previous studies have linked gestational diabetes(GDM)with allergies in offspring.However,the effect of specific glucose metabolism metrics was not well characterized,and the role of polyunsaturated fatty acids(PUFAs),a modifier of metabolism and the immune system,was understudied.We aimed to investigate the association between maternal GDM and allergic diseases in children and the interaction between glucose metabolism and PUFAs on allergic outcomes.Methods This prospective cohort study included 706 mother–child dyads from Guangzhou,China.Maternal GDM was diagnosed via a 75-g oral glucose tolerance test(OGTT),and dietary PUFAs were assessed using a validated food frequency questionnaire.Allergic disease diagnoses and the age of onset were obtained from medical records of children within three years old.Results Approximately 19.4%of women had GDM,and 51.3%of children had any allergic diseases.GDM was positively associated with any allergic diseases(hazard ratio[HR]1.40;95%confidence interval(CI)1.05–1.88)and eczema(HR 1.44;95%CI 1.02–1.97).A unit increase in OGTT after two hours(OGTT-2 h)glucose was associated with an 11%(95%CI 2%–21%)higher risk of any allergic diseases and a 17%(95%CI 1–36%)higher risk of food allergy.The positive associations between OGTT-2 h glucose and any allergic diseases were strengthened with decreased dietary a-linolenic acid(ALA)and increased n-6 PUFAs,linoleic acid(LA),LA/ALA ratio,and n-6/n-3 PUFA ratio.Conclusions Maternal GDM was adversely associated with early-life allergic diseases,especially eczema.We were the first to identify OGTT-2 h glucose to be more sensitive in inducing allergy risk and that dietary PUFAs might modify the associations.

Background:Little was known about the association among time in range(TIR),time above range(TAR),time below range(TBR),and cancer mortality among patients with type 2 diabetes.We aimed to investigate the association among TIR,TAR,TBR,and the risk of cancer mortality among patients with type 2 diabetes.Methods:A total of 6225 patients with type 2 diabetes were prospectively recruited in Shanghai,China.TIR was measured with continuous glucose monitoring at baseline and was defined as the average percentage of time in the target glucose range during a 24 h period.Cox proportion hazard regression analysis was used to determine the association between TIR and the risk of cancer mortality.Results:During a mean follow-up of 7.10 years,we confirmed 237 death events related to cancer.The multivariable-adjusted hazard ratio(HR)for cancer mortality was 1.32(95%confidence interval[CI]:1.01-1.75)in patients with TIR≤70%compared with those with TIR>70%.When TIR was considered as a continuous variable,the multivariable-adjusted HR for cancer mortality associated with each 10%decrease in TIR was 1.07(95%CI:1.02-1.14).In the site-specific analysis,a significant association between TIR as a continuous variable and the risk of hepatocellular cancer was found(HR:1.24;95%CI:1.09-1.41).However,no relationship between hemoglobin A1c and cancer mortality was observed(HR:1.04;95%CI:0.97-1.10).Conclusions:The present study found an inverse association of TIR with the risk of cancer mortality among patients with type 2 diabetes.New evidence of TIR was added into the clinical practice that TIR may be an optimal target of glycemic control among patients with type 2 diabetes.

BACKGROUND Within the normal range,elevated alanine aminotransferase(ALT)levels are associated with an increased risk of metabolic dysfunction-associated fatty liver disease(MAFLD).AIM To investigate the associations between repeated high-normal ALT measurements and the risk of new-onset MAFLD prospectively.METHODS A cohort of 3553 participants followed for four consecutive health examinations over 4 years was selected.The incidence rate,cumulative times,and equally and unequally weighted cumulative effects of excess high-normal ALT levels(ehALT)were measured.Cox proportional hazards regression was used to analyse the association between the cumulative effects of ehALT and the risk of new-onset MAFLD.RESULTS A total of 83.13%of participants with MAFLD had normal ALT levels.The incidence rate of MAFLD showed a linear increasing trend in the cumulative ehALT group.Compared with those in the low-normal ALT group,the multivariate adjusted hazard ratios of the equally and unequally weighted cumulative effects of ehALT were 1.651[95%confidence interval(CI):1.199-2.273]and 1.535(95%CI:1.119-2.106)in the third quartile and 1.616(95%CI:1.162-2.246)and 1.580(95%CI:1.155-2.162)in the fourth quartile,respectively.CONCLUSION Most participants with MAFLD had normal ALT levels.Long-term high-normal ALT levels were associated with a cumulative increased risk of new-onset MAFLD.

Background:Mushrooms are a good source of many nutrients which are potentially beneficial for chronic diseases.We speculated that due to its abundant nutrients edible mushrooms might have a beneficial effect on the prevention of subclinical thyroid dysfunction(SCTD).Therefore,we designed a large-scale cohort study to examine whether mushrooms consumption is a protective factor for SCTD in adults.Methods:This prospective cohort study investigated 6631 participants(mean age:(45.0±10.2)years;55.1%men).Edible mushrooms consumption was measured at baseline using a validated food frequency questionnaire.SCTD was defined as abnormal serum thyroid-stimulating hormone levels and normal free thyroxine.Cox proportional hazards regression models were used to examine the association of edible mushrooms consumption with incident SCTD.Results:During follow-up period,a total of 262 new cases of SCTD were identified,the incidence rate of subclinical hypothyroidism was 8.9/1000 person-years and subclinical hyperthyroidism was 7.2/1000 person-years.After adjusting potential confounding factors,the multivariable hazard ratios(95%confidence intervals)for subclinical hypothyroidism were 1.00(reference)for almost never,0.53(0.29,0.97)for 1-3 times/week and 0.30(0.10,0.87)for≥4 times/week(P for trend=0.02).It also showed edible mushrooms consumption was inversely associated with subclinical hypothyroidism in obese individuals but not non-obese individuals,the final hazard ratios(95%confidence intervals)were 0.14(0.03,0.73)(P for trendcohort study has firstly demonstrated that higher edible mushrooms consumption was significantly associated with lower incidence of subclinical hypothyroidism among general adult population,especially in obese individuals.

The sudden onset of the coronavirus disease 2019(COVID-19)in January 2020 has affected essential global health services.Cancer-screening services that can reduce cancer mortality are strongly affected.However,the specific role of COVID-19 in cancer screening is not fully understood.This study aimed to assess the efficiency of global cancer screening programs before and during the COVID-19 pandemic and to promote potential cancer-screening strategies for the next pandemic.Electronic searches in PubMed,Embase,and Web of Science,and manual searches were performed between January 1,2020 and March 1,2023.cohort studies that reported the number of participants who underwent cancer screening before and during the COVID-19 pandemic were included.The methodological quality of the included studies was assessed using the Newcastle-Ottawa Scale.Differences in cancer-screening rates were estimated using the incidence rate ratio(IRR).Fifty-five cohort studies were included in this meta-analysis.The screening rates of colorectal cancer using invasive screening methods(Pooled IRR=0.52,95%CI:0.42 to 0.65,p<0.01),cervical cancer(Pooled IRR=0.56,95%CI:0.47 to 0.67,p<0.01),breast cancer(Pooled IRR=0.57,95%CI:0.49 to 0.66,p<0.01)and prostate cancer(Pooled IRR=0.71,95%CI:0.56 to 0.90,p<0.01)during the COVID-19 pandemic were significantly lower than those before the COVID-19 pandemic.The screening rates of lung cancer(Pooled IRR=0.77,95%CI:0.58 to 1.03,p=0.08)and colorectal cancer using noninvasive screening methods(Pooled IRR=0.74,95%CI:0.50 to 1.09,p=0.13)were reduced with no statistical differences.The subgroup analyses revealed that the reduction in cancer-screening rates varied across economies.Our results suggest that the COVID-19 pandemic has had a noteworthy impact on colorectal,cervical,breast,and prostate cancer screening.Developing innovative cancer-screening technologies is important to promote the efficiency of cancer-screening services in the post-COVID-19 era and prepare for the next pandemic.

Emerging epidemiological studies suggest that long-term ozone(O_(3))exposure may increase the risk of mortality,while pre-existing evidence is mixed and has been generated predominantly in North America and Europe.In this study,we investigated the impact of long-term O_(3)exposure on all-cause mortality in a national cohort in China.A dynamic cohort of 20882 participants aged40 years was recruited between 2011 and 2018 from four waves of the China Health and Retirement Longitudinal study.A Cox proportional hazard regression model with time-varying exposures on an annual scale was used to estimate the mortality risk associated with warm-season(Aprile September)O_(3)exposure.The annual average level of participant exposure to warm-season O_(3)concentrations was 100 mg m^(-3)(range:61 e142 mg m^(-3)).An increase of 10 mg m^(-3)in O_(3)was associated with a hazard ratio(HR)of 1.18(95%confidence interval[CI]:1.13e1.23)for all-cause mortality.Compared with the first exposure quartile of O_(3),HRs of mortality associated with the second,third,and highest exposure quartiles were 1.09(95%CI:0.95e1.25),1.02(95%CI:0.88e1.19),and 1.56(95%CI:1.34e1.82),respectively.A J-shaped concentration eresponse associationwas observed,revealing a non-significant increase in risk below a concentration of approximately 110 mg m^(-3).Low-temperature-exposure residents had a higher risk of mortality associated with long-term O_(3)exposure.This study expands current epidemiological evidence from China and reveals that high-concentration O_(3)exposure curtails the long-term survival of middle-aged and older adults.

Association between Self-Rated Health and Depressive Symptoms in Rural Chinese Adults:A cohort study Based on Propensity Score Matching

Health status is widely regarded as a correlate of depressive symptoms.However,health assessments based on clinical diagnosis in rural areas with poor medical conditions are very limited.Self-rated health(SRH)serves as a simple and convenient evaluation indicator,which may be used as an independent predictor of depressive symptoms.To confirm the relationship between SRH and depressive symptoms in rural adults,a longitudinal survey of rural households in China was conducted using the China Family Panel Studies(CFPS)from 2012 to 2016.Propensity score matching and logistic regression analysis were used to explore the association.After data cleansing,3,127 pairs(6,254 participants)aged 16 and older followed for 4 years were enrolled,of which the average age was(50.02±14.19)years old,and the proportions of male and female were 48.64%and 51.36%,respectively.The incidence rate of depressive symptoms within 4 years was 30.86%(95%CI:29.24–32.48)in the group with fair or poor SRH,and 21.59%(95%CI:20.14–23.03)in the group with good SRH.The difference between the two groups was statistically significant(χ^(2)=69.51,P<0.001).The results of univariate unconditional logistic regression analysis showed that there was a correlation between SRH and depressive symptoms in rural adults aged 30 and above(OR=1.65,95%CI:1.46–1.85,P<0.001).Thus,a simple and practical assessment tool based on SRH and other indicators should be established for early prevention and intervention in rural primary mental health care.
