Transcoronary ablation of septal hypertrophy compared with surgery in the treatment of hypertrophic obstructive cardiomyopathy

Transcoronary ablation of septal hypertrophy ( TASH ) can lead to sustained improvement in both hemodynamics and symptoms in patients with hypertrophic obstructive cardiomyopathy ( HOCM ) . However, there havebeen few reports about its efficacy and safety compared with traditional surgical procedures. This study sought to compare TASH with surgery in the treatment of HOCM.

Cardiac amyloidosis is an infiltrative and restrictive cardiomyopathy caused by the extracellular deposition of amyloid fib-rils within the heart as systemic amyloidosis,lead-ing to heart failure,reduced quality of life,and death.[1]There are two major amyloid fibril proteins that af-fect the heart:amyloid immunoglobulin light chain(AL)and amyloid transthyretin(ATTR).The latter is further subdivided into wild-type ATTR and variant types based on the presence of a mutation in the transthyretin gene.

Calcineurin (CaN) is a unique calcium (Ca2+) and calmodulin (CaM)-dependent serine/threonine phosphatase that becomes activated in the presence of increased intracellular Ca2+ level. CaN then functions to dephosphorylate target substrates including various transcription factors, receptors, and channels. Once activated, the CaN signaling pathway participates in the development of multiple organs as well as the onset and progression of various diseases via regulation of different cellular processes. Here, we review current literature regarding the structural and functional properties of CaN, highlighting its crucial role in the development and pathogenesis of immune system disorders, neurodegenerative diseases, kidney disease, cardiomyopathy and cancer.

Background:Multiple mitochondrial dysfunction syndromes(MMDS)presents as complex mitochondrial damage,thus impairing a variety of metabolic pathways.Heart dysplasia has been reported in MMDS patients;however,the specific clinical symptoms and pathogenesis remain unclear.More urgently,there is a lack of an animal model to aid research.Therefore,we selected a reported MMDS causal gene,Isca1,and established an animal model of MMDS complicated with cardiac dysplasia.Methods:The myocardium-specific Isca1 knockout heterozygote(Isca1 HET)rat was obtained by crossing the Isca1 conditional knockout(Isca1 cKO)rat with theαmyosin heavy chain Cre(α-MHC-Cre)rat.Cardiac development characteristics were determined by ECG,blood pressure measurement,echocardiography and histopatho-logical analysis.The responsiveness to pathological stimuli were observed through adriamycin treatment.Mitochondria and metabolism disorder were determined by activity analysis of mitochondrial respiratory chain complex and ATP production in myocardium.Results:ISCA1 expression in myocardium exhibited a semizygous effect.Isca1 HET rats exhibited dilated cardiomyopathy characteristics,including thin-walled ventri-cles,larger chambers,cardiac dysfunction and myocardium fibrosis.Downregulated ISCA1 led to deteriorating cardiac pathological processes at the global and organiza-tional levels.Meanwhile,HET rats exhibited typical MMDS characteristics,including damaged mitochondrial morphology and enzyme activity for mitochondrial respira-tory chain complexesⅠ,ⅡandⅣ,and impaired ATP production.Conclusion:We have established a rat model of MMDS complicated with cardiomyopathy,it can also be used as model of myocardial energy metabolism dysfunction and mitochondrial cardiomyopathy.This model can be applied to the study of the mechanism of energy metabolism in cardiovascular diseases,as well as research and development of drugs.

Background:Drug-induced cardiomyopathy is a significant medical problem.Clinical diagnosis of myocardial injury is based on initial electrocardiogram,levels of circulating biomarkers,and perfusion imaging with single photon emission computed tomography(SPECT).Positron emission tomography(PET)is an alternative imaging modality that provides better resolution and sensitivity than SPECT,improves diagnostic accuracy,and allows therapeutic monitoring.The objective of this study was to assess the detection of drug-induced cardiomyopathy by PET using 2-deoxy-2-[^18F]fluoro-D-glucose(FDG)and compare it with the conventional SPECT technique with[^99m Tc]-Sestamibi(MIBI).Methods:cardiomyopathy was induced in Sprague Dawley rats using high-dose isoproterenol.Nuclear[^18F]FDG/PET and[^99m Tc]MIBI/SPECT were performed before and after isoproterenol administration.[^18F]FDG(0.1 mCi,200-400μL)and[^99m Tc]MIBI(2 mCi,200-600μL)were administered via the tail vein and imaging was performed 1 hour postinjection.Isoproterenol-induced injury was confirmed by the plasma level of cardiac troponin and triphenyltetrazolium chloride(TTC)staining.Results:Isoproterenol administration resulted in an increase in circulating cardiac troponin I and showed histologic damage in the myocardium.Visually,preisoproterenol and postisoproterenol images showed alterations in cardiac accumulation of[^18F]FDG,but not of[^99m Tc]MIBI.Image analysis revealed that myocardial uptake of[^18F]FDG reduced by 60%after isoproterenol treatment,whereas that of[^99m Tc]MIBI decreased by 45%.Conclusion:We conclude that[^18F]FDG is a more sensitive radiotracer than[^99m Tc]MIBI for imaging of drug-induced cardiomyopathy.We theorize that isoproterenolinduced cardiomyopathy impacts cellular metabolism more than perfusion,which results in more substantial changes in[^18F]FDG uptake than in[^99m Tc]MIBI accumulation in cardiac tissue.

To assess the left ventricular regional relaxation abnormalities in patients with hypertrophic cardiomyopathy (HCM) by quantitative tissue velocity imaging (QTVI), Doppler echocardiography and QTVI were performed in HCM (n=10) and healthy subjects (n=11) at apical long-axis, two-chamber and four-chamber views. Regional early diastolic velocity (rVe) and regional atrial contraction (rVa) were measured at each segment of ventricular middle, basal and annular levels. Mean rVe and mean rVa at three levels as well as mean rVe/rVa ratio were calculated. Our results showed that transmitral inflow peak velocities during early diastole (E) and atrial contraction (A) were also measured and E/A ratio was calculated. The rVe of all left ventricular segments in HCM were lower than those in healthy subjects (P<0.05), but compared with healthy subjects majority of rVa in HCM were not different except inferior wall and anterior wall. E between HCM and healthy subjects was different (P=0.036), while mean rVe between them was significantly different (P<0.0001). Mean rVa and mean rVe/rVa of three levels were lower in HCM than in healthy subjects (P<0.05), but there were no differences in A and E/A between them (P=0.22, P=0.101). Left ventricular regional myocardial relaxation is reduced in HCM. Transmitral inflow E and A are influenced by preload, relaxation of myocardium and atrial contraction, etc., while rVe and rVa reflect myocardial relaxation function independently. QTVI is more sensitive and more accurate than conventional Doppler imaging for characterizingregional diastolic properties in HCM.

The parathyroid gland is essential to maintaining the serum calcium level within the normal range by secreting the parathyroid hormone(PTH).Significant incidences of increased blood pressure,arrhythmia,left ventricular(LV)hypertrophy,and congestive heart failure(CHF)can occur in patients with parathyroid disease.Hypocalcemia can cause dilated cardiomyopathy,whether this is infrequent.Hypoparathyroidism caused by thyroidectomy and primary hypoparathyroidism are the greatest causes of hypocalcemic heart failure(HF).

Dear editor,In many cases,S T-segment changes on electrocardiogram(ECG)are suggestive of acute coronary syndrome,but there may be other causes,[12]such as stress induced cardiomyopathy.However,reports of such changes in patients with pneumothorax are very rare.[3-1]Here,we report a case of ST-segment elevation due to stress-induced cardiomyopathy in a patient with pneumothorax.

Objective:Late gadolinium enhancement(LGE) patterns of cardiovascular magnetic resonance (CMR) relying on PSIR(phase-sensitive inversion recovery sequence) techniques had been used to determine the characteristics of LGE in apical hypertrophic cardiomyopathy(ApHCM). Methods:Forty patients pure ApHCM[age,(60.2±10.4) years,31 men]were enrolled.LGE images were acquired using PSIR,and analyzed using a 17-segment model.Summing the LGE areas in all short axis slices yielded the total volume of late enhancement,which was subsequently presented as a proportion of total LV myocardium(%LGE).Results:Mean maximal apical wall thickness was(17.9±2.3) mm,and mean left ventricular(LV) ejection fraction was(67.7±8.0)%.LGE was detected in 130 segments of 30 patients(75.0%),occupying(4.9±5.5)% of LV myocardium.LGE was mainly detected at the junction between left and right ventricles in 12(30%) and at the apex in 28(70%),although LGE-positive areas were widely distributed,and not limited to the apex.Focal LGE at the non-hypertrophic LV segments was found in some ApHCM patients,even without LGE of hypertrophied apical segments.Conclusions:LGE was frequently observed not only in the thickened apex of the heart but also in other LV segments,irrespective of the presence or absence of hypertrophy.The simple presence of LGE on CMR was not representative of adverse prognosis in this population.
