The Impact of breastfeeding Pattern on the Duration of Lactational Amenorrhoea in Chengdu, China

Objective To determine the relation between infant feeding practices (and other factors) and the duration of postpartum amenorrhoea in a sample of Chinese women Subjects &. Methods Infant feeding and the return of menses were measured longitudinally in 541 mothers and their infants in Pengxian County, Sichuan Province. The durations of breastfeeding and of lactational amenorrhoea were estimated through survival analysis. Cox's non-parametric hazards regression analysis was performed to determine the variables associated with the duration of lactational amenorrhoea. Results Mothers breastfed frequently per day, the mean frequency ranging from 6. 4 per day at 3 weeks postpartum to 5. 4 per day at 6 months. They breastfed for over 18. 3 months on average. The median duration of lactational amenorrhoea was 9. 5 months. The period from birth until the first breastfeed was a significant determinant of the duration of lactational amenorrhoea, P = 0. 038. Compared with women who ended full breastfeeding earlier, those who ceased full breastfeeding at 120 to 24C days returned to menses significantly sooner (risk ratio = 1. 34; 95% CI = 1. 06-1. 69). This finding is probably spurious since no relationship between full breastfeeding duration and the return of menses was found among women -who ended full breastfeeding after 240 days. Conclusion The most important determinant of the return of menses was the delay in the first breastfeed after birth, suggesting that the Ten Steps to Successful breastfeeding should be more precisely implemented.

Fracture Risks Related to Parity and breastfeeding Effect in Post-Menopausal Women Aged Sixty and Over: Results from the “Quality of Bone in Lorraine” Register

Introduction: The long term effect of accumulation of genital events as repeated pregnancy and longer breastfeeding in bone heath later in women’s life is still disputed. The objective was to assess the impact of parity and cumulated duration of breastfeeding on fracture risk in post-menopausal women aged sixty an over. Patients and Methods: It was a leading study from the register “Quality of Bone in Lorraine (QBL)” achieved in the department of endocrinology and osteoporotic disease of Nancy (France). This register included all patients sent for an assessment of the bone mass density from January 1, 2006 to December 31, 2014 (9 years). It was about post-menopausal women aged sixty an over suffering or not from osteoporosis fracture or bone fragility just after the age of 45. The genital events of patients to their age (from puberty to menopause) as well as the existence of hormone replacement therapy use, parity, and breastfeeding duration were taken into account. The assessment of bone fracture was clinical, radiological or by using the vertebral fracture assessment method. Results: 861 post-menopausal women were included. In comparison to the control group, the fractured population had a mean age of (74.3 ± 9 vs. 72 ± 8 years), a family history of fracture (32.1% vs. 26%), and an average input of calcium (2.4 ± 1 vs. 2.3 ± 0 portions per day). The age at menarche was of 12.8 ± 1 years in each group, a mean genital activity duration of (36.8 ± 3 vs. 37.2 ± 3 years), a parity of (2.1 ± 1 vs. 1.8 ± 1 children), a cumulated breastfeeding duration (4.2 ± 16 vs. 3.1 ± 5 months) and an age of menopause of (48.6 ± 4 vs. 48.6 ± 4 years) were respectively found in fractured and witness population. Overall, an osteoporotic fracture has been rediscovered in 50.9%. In multivariate analysis, only a cumulative duration of breastfeeding of 6 months and over was associated with a higher fracture risk (OR = 1.5 [1.1 - 2.2]). The

Effects of early breastfeeding education on maintenance of breastfeeding practice: A prospective observational study

Background: Breast feeding is an ideal feeding type for most infants and mothers. We evaluated the effect of early breast-feeding and prenatal education of breast- feeding in maintenance of breast-feeding practice after delivery. Methods: Eighty two pregnant women who experienced vaginal delivery were recruited from the department of obstetrics and gynecology of a university hospital between January 2006 and October 2006. These differences among variables were determined using SPSS ver. 10.0 (SPSS Inc., Chicago, IL, USA). Follow-up telephone interviews were conducted 1 week and 4 weeks after delivery to check feeding method for baby and other complications. Results: We calculated the correlations among the pregnant women’s age, height, and weight, abortion experience, the newborn’s weight, and social factors using ANOVA. There was no statistically significant relationship between early breast-feeding and maintenance of breast-feeding practice. Conclusions: Antenatal and post- partum period education about breast-feeding is necessary for breast-feeding practice maintenance. Further studies in developing educational programs are needed.

Objective:To investigate present breastfeeding risk factor of pregnant women bearing second child,to provide evidence for the effective intervention in promoting ***:Questionnaires were delivered to the pregnant women bearing second child at obstetric clinic in Nanjing Maternity and Child Health Care Hospital from August to October,*** questionnaire includes two sections,one is the retrospective research on breastfeeding history after their first delivery,second is the prospective research on their willingness of breastfeeding in their second *** variables that influence the breastfeeding intention were listed in the *** factors were analyzed using ANOVA analysis,chisquare test and logistic ***:Multivariate logistic regression analysis showed that the factors significantly associated with breastfeeding initiation and intention in first child were labor pattern(OR=1.840,95%CI=1.022-3.313),education experience at fetus university(OR=2.216,95%CI=1.208-4.064) and family income(OR=0.660,95%CI=0.465-0.937).The factors significantly associated with breastfeeding initiation and intention in second child were feeding pattern in first child(OR=0.314,95%CI=0.218-0.772) and breastfeeding duration in first child(OR=0.812,95%CI=0.699-0.945).Gravidity(OR=0.702,95%CI=0.503-0.981),breast anomalies(OR=0.190,95%CI=0.067-0.544) and the first child was taken care of by nanny(OR=5.341,95%CI=1.178-24.216)were strongly associated with pregnant woman bearing second child’s intention of getting help from the *** The breastfeeding experience in first child will influence the breastfeeding intention in the second *** who do have the successful breastfeeding experience and longer breastfeeding duration will much more likely to choose breastfeeding as the major feeding pattern in their second child.

A META-analysis of the association between breastfeeding and risk of infant hepatitis C virus infection

Objective: To investigate the association between breastfeeding and infant acquired infection of Hepatitis C Virus(HCV). Methods: To obtain studies eligible for META-analysis, China biological medicine discs(CBMD) and MEDLINE citations were surveyed. Criteria for enrollment of published studies for META-analysis were based on principle by Abdolmaleky HM[1]. Odd ratio(OR) was calculated and summarized by fixed effect model or random effects using RevMan software. The heterogeneity of ORs was assessed using an χ2 test of goodness of fit. The significance of the pooled OR was determined by the z-test. The strength of association was summarized usingOR. An OR>1.0 indicated a positive association between the risk factor and infants HCV infection. Results:After searching in Medline and CBMD, 120 articles were enrolled for further identification. Thirty-seven were of review on relative fields and were excluded. Abstracts of remaining articles were carefully read, and finally, only 6 articles met, the standards for enrollment criteria. After analysis of the factor concerned, no significant association was found between breastfeeding and infant HCV infection, with a pooled OR equal to 0.60(95%CI = 0.22-1.60), and proved not to be of risk factors on infant acquired infection of HCV. Conclusion: breastfeeding dose not increase the risk of infant acquired infection of HCV.

Risks of maternal prepregnancy overweight/obesity, excessive gestational weight gain, and bottle-feeding in infancy rapid weight gain: evidence from a cohort study in China

Rapid weight gain(RWG) in infants is associated with numerous health problems, and its risk factors are still unclear. We assessed 98,097 maternal-infant pairs from a population-based cohort study and followed up with them until the infants were6 months old. We assessed the associations between maternal prepregnancy weight status;gestational weight gain;feeding pattern;and infants’ RWG at 0–1, 0–3, 1–3, and 3–6 months using multivariate unconditional logistic regression models, with controlled confounders. We found that maternal prepregnancy weight status, gestational weight gain, and feeding pattern at the1 st, 3 rd, and 6 th months had significant impacts on the infants’ RWG at each time period(P<0.05). Infants with overweight/obese mothers had a higher risk of RWG after birth, whereas those of mothers who experienced excessive gestational weight gain had higher risks of RWG from birth than the other groups(P<0.01). Infants who were formula-fed had a higher risk of RWG than breastfed infants at the same time point(P<0.01). In conclusion, maternal prepregnancy obesity, excessive gestational weight gain, and formula-feeding were risk factors for infants’ RWG during the first 6 months of life.
