Endoscopic assisted Microscopic Skull Base Surgery

Background: Skull base tumors presented great challenge for neurosurgeons since decades due to their deep location, associated morbidity and limitation of operative field;however modern neurosurgery using the endoscope and/or the microscope served in minimizing peri-operative morbidities and improving the clinical outcome. Objective: To demonstrate the value of endoscope-assisted microsurgical technique for resection of skull base tumors. Patients and Methods: 30 patients divided into 3 groups (10 patients had medial sphenoid wing meningioma constituted group 1, 10 patients had suprasellar meningioma constituted group 2 and 10 patients had Cerbello-Pontine Angle (CPA) epidermoids constituted group 3) were operated through Endoscope assisted Microscopic Skull Base Surgery technique at Al-Azhar University Hospitals during the period starting from January 2016 till the end of September 2017 using a rigid endoscope for inspection of tumor boundaries and neighboring vascularity in addition to confirm the extent of resection. Tumor resection was tried in all cases. Intra-operative resection rate and post-operative radiological outcomes were assessed. Results: Total gross resection was possible in 27 patients (90%). Subtotal resection was done in the other 3 cases (recurrent medial sphenoid wing meningiomas) due to excessive bleeding and adhesions of the tumors with vascular structures. Total resection of the tumor in post operative radiology was obtained in 24 patients (80%) and post-operative residual tumor was noticed in 6 patients in early post operative radiology and in only 3 cases at 3 months follow up radiology. Conclusion: Endoscopic-assisted microsurgical approach is a reliable, safe and effective option for adequate surgical resection of skull base tumors. The technique allowed proper inspection of the tumor relations and vascularity, detection of any residual portions, providing better chance for gross total resection with minimal tissue damage or vascular injury as well as convenient clinical outcome.

Autism spectrum disorder is a neurodevelopmental condition characterised by difficulties with communication, social interaction, and restricted and repetitive behaviours. For children with this disorder, the ability to engage with people and activities, and to participate in everyday occupations is compromised. Animal assisted therapy has been proposed as an intervention modality to facilitate development of an individual’s social behaviours such as engagement and attention. In spite of the increasing evidence showing the positive impact animal assisted therapy can have for children on the autism spectrum, there remain no universal standards or formalised guidelines for ethical practice with either the clients or the therapy dogs involved. The proposed pilot randomised control trial aims to determine the impact of canine assisted occupational therapy on the on-task behaviours and overall goal attainment of children on the autism spectrum, as well as consider the impact that being involved within therapy sessions has on the therapy dog.

Effect of bombarding energy on ion beam assisted deposited DLC film on UHMWPE

Diamond-like carbon (DLC) films on ultra-high molecular weight polyethylene substrates were prepared at CHn+ ion bombarding energies of 200-1000eV at room temperature using ion beam assisted deposition technique. The wear tests exhibited a high wear resistance for all DLC films. X-ray photoelectron spectfoscopy and Raman spectroscopy analysis indicated that DLC film was amorphous with a characteristic high fracdion of sp3 bonds in the structure of mixed sp:2+ sp3 bonding at an optimal bombardingenergy of 600eV.

Vacuum assisted Closure (VAC) and Platelet-Rich Plasma (PRP): A Successful Combination in a Challenging Case of Gastroschisis

Giant gastroschisis could be a surgical challenge concerning the abdominal wall reconstruction. Many techniques have been described for both primary or staged closure but sometimes neither of them is succesful in all patients. We are presenting the combined use of Vacuum assisted Closure (VAC) and Platelet-Rich Plasma (PRP) to improve the result in this difficult case. The use of VAC device is a well known procedure in the treatment of adult difficult wounds closure. It consists of a sponge applied directly on the abdominal wall defect, covered with a transparent dressing and connected to a controlled continuous negative pressure system [1]. Platelet-rich plasma (PRP) is an autologous concentration of human platelets in a small volume of plasma. Due to this combination it provides multiple growth and healing factors actively secreted by platelets which have been shown to begin and accelerate wound healing [2] [3]. The association between VAC and PRP was effective in the shrinkage and reduction of the abdominal defect. Fifteen months after the removal of the VAC device the fascia appears competent with a cutaneus scar that will need a plastic correction in the future. The VAC should be considered as a helpful and effective device in case of complicated giant gastroschisis or omphalocele when traditional treatment is not sufficient.

Modeling and Simulation of Laser assisted Turning of Hard Steels

This research work is focused on simulation of laser assisted turning as a new solution for machining of hard steels. A transient, three-dimensional model was developed to predict the temperature distribution of a rotated cylindrical steel workpiece subjected to a localized heating using a moving Gaussian laser beam. In this regard, a User-Defined Function was created to overcome the problem of a moving Gaussian heat source’ definition. This User-Defined Function was compiled into a finite volume software package (Fluent), where three-dimensional single precision solver was used for analysis. Based on this model, simulation of the surface temperature of 32 mm diameter workpiece of AISI51 50H steel was performed as a function of time at a specific distance behind the laser beam spot, which is corresponding to 30° angle from the laser beam. The simulation results were compared with other published data of the same steel type where a close agreement was obtained. The verified model was used for simulation of laser assisted turning of 20 mm diameter workpiece of AISI D2 tool steel. The cutting depth, behind the laser beam, was set at a distance corresponding to 60° angle from the laser beam for having sufficient access for handling both laser head and cutting tool. This cutting depth was studied as a function of different lasers and machining parameters. The results indicated that the optimum parameters for successful laser-assisted turning process of the concerned steels are 800 W laser power, 5 mm laser beam spot diameter, 20 sec preheating time, 0.8 mm/sec laser scanning speed, 300 rpm rotational speed and 0.8 mm/sec feed rate. These parameters ensure easy/successful cutting of 1 mm depth in one pass without deteriorating the properties of the remaining bulk material. It can be deduced that the developed model might provide a useful tool for online process control of different steel types regardless of their physical properties and geometries.

Outcome of assisted Reproduction Treatment in Women with Extremely Low Antimullerian Hormone (AMH) Levels

Purpose: To evaluate age related ovarian response to controlled ovarian hyperstimulation, clinical pregnancy and ongoing pregnancy rates beyond 20 weeks in women undergoing assisted reproduction treatment (ART) with antimullerian hormone (AMH) levels of 38 years of age. Non parametric variables were expressed as median (Interquartile range) and compared by Kruskal-Wallis test. Categorical variables were expressed as numbers with proportions (%) and compared by Fisher’s exact test. Results: There was no statistical difference in body max index, level of antimullerian hormone (AMH), follicle stimulating hormone (FSH), dose of gonadotrophins used and cycles cancellation rate in two groups. Although number of oocytes retrieved (median 5), clinical pregnancy (18.4%) and ongoing pregnancy rate beyond 20 weeks (18.4%) was higher in group 1, there was no statistical difference between the two groups. There was one miscarriage in group 2. Conclusion: Women with extremely low-serum AMH levels can still have clinical pregnancy and ongoing pregnancies beyond 20 weeks after ART, though chances will be lower than women with normal ovarian reserve.

Endoscopic assisted Microscopic Fenestration of Inner Membrane in Cases of Chronic Subdural Hematoma

Introduction: Chronic subdural hematoma (CSDH) is one of the most common types of intracranial hemorrhages and carries a significant morbidity;there is no clear optimal treatment. Recurrence, pneumocephaly and seizures are common complications. We will evaluate outcome after inner membrane fenestration in comparison to burr-hole evacuation only in cases of CSDH. Patients and Methods: Our work was conducted on 20 patients with CSDH from February 2017 to July 2017. Endoscopic-assisted microscopic fenestration technique was used to do inner membrane fenestration in ten patients and the other 10 patients operated upon by traditional burr-hole evacuation. Regular follow up was done up to one month. Results: Clinical outcome was nearly the same in the two groups. Recurrence rate was only noted in the non-fenestration group (20%). Midline shift was better in the membrane fenestration group after one month and we experienced no intraoperative surgical complications related to the fenestration technique. Postoperative complications, such as Pneumocephalus and seizures, were seen in both study groups. Conclusion: Endoscopic assisted microscopic technique decreases surgical risks to do inner membrane fenestration although there is no major difference between doing fenestration or not in clinical outcome, but it may decrease recurrence, but larger studies are needed.

Assessment of Risk of Carrier Waves in the assisted Reproductive Laboratory

assisted reproductive technology (ART) laboratories represent the marriage of the most basic of biological activities with the most cutting edge technologies. While this association has worked well, the mixture of biology and technology can create risks to normal embryo development. Recently a significant amount of literature has explored the risks of manmade, electrically induced magnetic fields and carrier waves on reproduction, which some studies have suggested will lower functional gamete numbers in the males and potentially induce genetic issues in embryos. However, little is known about these phenomena within the ART laboratory, a laboratory filled with electronic equipment. The object of the present study was to explore the potential exposure of gametes and early stage embryos to two of the most prevalent fields and waves utilized in manmade technologies seen in the general environment, electromagnetic fields (EMF) and radio frequency waves (RF), and determine the effect varying levels of these energetic forces had on gamete function and embryo development. Results indicated that while extremely high concentrations of EMF (approximately 50-100X of laboratory background) caused negative outcomes in both gametes and embryos, levels consistent will the majority of lab equipment did not appear to impact growth, or function. Further, even extremely high RF appeared to have no impact cellular function. Results suggest few issues with EMF or RF on gamete and embryo function at normal laboratory levels for the relatively short exposure times seen in the ART laboratory.

A Comparison Study between the Newly Developed Vertical Wells Steam assisted Gravity Drainage and the Conventional SAGD Process

A numerical simulation study using the CMG-STAR Simulator was performed to compare the performance of the newly developed process (VWSAGD) utilizing vertical wells to enhance heavy oil recovery during steam assisted gravity drainage against the conventional steam assisted gravity drainage process which utilized horizontal wells (HWSAGD) under the same operating conditions. Two identical reservoir models were simulated for the two processes using 3-Dimensional, black heavy oil model (14° API). Each reservoir type consists of 49 × 49 × 20 grid blocks on a 5-acre model, which incorporated a typical heavy oil reservoir rock and fluid properties taken from the SPE case study, stspe001.dat (CMG 2015 release). A sensitivity analysis for both processes was performed for the grid density, soaking time, steam quality, bottom hole producing pressure, steam injection rate, reservoir thickness, reservoir area, and horizontal to vertical permeability anisotropy. More preferable reservoir conditions are those such as high horizontal to vertical permeability ratio, thick reservoir oil zones, as well as improved reservoir recovery for the VWSAGD process. Under unfavorable conditions such as thin reservoir oil zones, an improved reservoir recovery response was limited for the VWSAGD process and could be uneconomical in real field cases. Finally, the simulation results from this study include cumulative recoveries, Steam oil ratios, produced water-oil ratios, pressure and temperature distributions, and production rates. In addition, the results from this study have shown that the new VWSAGD process is more favorable than the conventional HWSAGD process.

Salting-Out assisted Liquid-Liquid Extraction Combined with HPLC for Quantitative Extraction of Trace Multiclass Pesticide Residues from Environmental Waters

In this study, salting-out assisted liquid-liquid extraction combined with high performance liquid chromatography diode array detector (SALLE-HPLC-DAD) method was developed and validated for simultaneous analysis of carbaryl, atrazine, propazine, chlorothalonil, dimethametryn and terbutryn in environmental water samples. Parameters affecting the extraction efficiency such as type and volume of extraction solvent, sample volume, salt type and amount, centrifugation speed and time, and sample pH were optimized. Under the optimum extraction conditions the method was linear over the range of 10 - 100 μg/L (carbaryl), 8 - 100 μg/L (atarzine), 7 - 100 μg/L (propazine) and 9 - 100 μg/L (chlorothalonil, terbutryn and dimethametryn) with correlation coefficients (R2) between 0.99 and 0.999. Limits of detection and quantification ranged from 2.0 to 2.8 μg/L and 6.7 to 9.5 μg/L, respectively. The extraction recoveries obtained for ground, lake and river waters were in a range of 75.5% to 106.6%, with the intra-day and inter-day relative standard deviation lower than 3.4% for all the target analytes. All of the target analytes were not detected in these samples. Therefore, the proposed SALLE-HPLC-DAD method is simple, rapid, cheap and environmentally friendly for the determination of the aforementioned herbicides, insecticide and fungicide residues in environmental water samples.
