Longitudinal analysis of immune reconstitution and metabolic changes in Women living with HIV:A real-world observational study

Background:Women comprise more than half of people living with human immunodeficiency virus/acquired immune deficiency syndrome(HIV/AIDS)worldwide and incomplete immune recovery and metabolic abnormalities affect them deeply.Studies of HIV antiretroviral therapy(ART)have a low female representation in China.We aimed to investigate immune reconstitution and metabolic changes of female HIV-positive cohort in China longitudinally.Methods:HIV-positive Women who initiated ART from January 2005 to June 2021 and were followed up regularly at least once a year were included in this study.Immunological indicators(cluster of differentiation 4[CD4]counts and CD8 counts),viral load(VL),and metabolic indicators were collected at follow-up.All data were collected from the China Disease Prevention and Control Information System(CDPCIS).VL was tested half a year,1 year after receiving ART,and every other year subsequently according to local policy.CD4/CD8 ratio normalization was considered as the primary outcome and defined as a value≥1.Incidence rate and probability of CD4/CD8 ratio normalization were estimated through per 100 person-years follow-up(PYFU)and Kaplan-Meier curve,respectively.Multivariate Cox regression was used to identify independent risk factors associated with CD4/CD8 ratio normalization.We further studied the rate of dyslipidemia,hyperuricemia,diabetes,liver injury,and renal injury after ART initiation with the chi-squared tests or Fisher’s exact probability tests,and a generalized estimating equation model was used to analyze factors of dyslipidemia and hyperuricemia.Results:A total of 494 female patients with HIV/AIDS started ART within 16 years from January 2005 to June 2021,out of which 301 Women were enrolled with a median duration of ART for 4.1 years(interquartile range,2.3-7.0 years).The overall incidence rate of CD4/CD8 ratio normalization was 8.9(95%confidence interval[CI],7.4-10.6)per 100 PYFU,and probabilities of CD4/CD8 normalization after initiating ART at 1 year,2 years,5 years,and 10 years fol

Intimate partner violence against Women. Does violence decrease after the entry of the alleged offender into the criminal justice system?

Intimate partner violence(IPV)is simultaneously assumed as a serious crime and a major public health issue,having recurrences as one of its main characteristics and,consequently,re-entries of some alleged offenders in the criminal justice system(CJS).The main goal of this study is to assess if in cases of female victims of IPV,violence decreases after the first entry of the alleged offender in the CJS.A retrospective study was performed based on the analysis of police reports of alleged cases of IPV during a 4-year period.The final sample(n=1488)was divided into two groups according to the number of entries in the CJS(single or multiple)followed by a comparative approach.Results suggest that violence decreases after the first entry of alleged offenders in the CJS.Re-entries were found in only 15.5%of the cases but they were accountable for 3.3 times more crimes on average.Besides,victims of recidivism presented more injuries and required more medical care.Thus,a small group of alleged offenders seems to be more violent and accountable for most of the IPV crimes registered in the CJS suggesting that regardless of legal sanctions aiming to deter violence,these measures may not be enough for a certain group of offenders.This study sustains the need for a predictive model to quantify the risk of repeated IPV cases within the Portuguese population.

Prioritization of factors related to mental health of Women during an earthquake:A qualitative study

BACKGROUND: According to the fact that Women account for the highest rate of victims of mental health loss during disasters, prioritization of their requirements is of remarkable signifi cance. Therefore, the present study was carried out with the aim to prioritize factors related to Women's mental health during an earthquake. METHODS: This study was conducted using the Delphi method in 2017. Participants consisted of experts including psychologists, counselors and psychiatrists, social medicine specialists, and psychiatric-mental health nurses with experience in service and disaster awareness, especially earthquake. They were selected using purposive and snowball sampling methods. The Delphi method was used in 2 rounds with 21 components and the fi nal attendance of 19 participants and the collective agreement of 75%. RESULTS: Of the 21 factors related to Women's mental health during the earthquake, the following components were, respectively, preferred by the participants: psychological well-being training, group relationships and support of Women in emergency situations, fair treatment in the provision of rights and services to Women, crisis awareness and management of behavior and diffi cult conditions, and social security during disasters with the total mean standard deviation (SD) of 13.4 (2.4), 13.0 (2.4), 12.7 (2.5), 12.5 (2.4), and 12.3 (2.2), respectively. CONCLUSION: Training in the fi eld of psychology and disasters, and social and cultural factors were prioritized among factors related to Women's mental health during an earthquake. Therefore, the results of this study can be applied by the planners and executives of mental health, Women and disasters, and the Women's community itself.

Protocol of Jidong Women Health Cohort Study: Rationale, Design, and Baseline Characteristics

Objective The Jidong Women Health Cohort Study is a prospective cohort study on female-specific characteristics and risks of chronic diseases in Chinese Women and focuses on the potential association between menopause and risks of cardiovascular disease(CVD). Methods The study includes 4,179 female participants with an age of older than 18 years from Caofeidian district, Tangshan city, northern China. Baseline information on female-specific characteristics and potential cardiovascular risk factors was collected and all the participants underwent a physical examination with blood samples collected in 2013. To establish a better risk assessment tool of female CVD, updated information from questionnaire investigation, physical examinations and occurrence of outcome events will be collected through a longitudinal follow-up annually up to the year 2024. Results At baseline, Mean age of the participants was 42.3 ± 12.8 years. Reproduction occurred in 2,948 participants(70.5%), menopausal transition in 173(4.3%), and postmenopause in 1,058(25.3%). The incidence of arterial hypertension, dyslipidemia, and diabetes showed significant difference across different groups stratified by Stage of Reproductive Aging Workshop(STRAW) system(P Women Health Cohort Study will contribute to the scientific evidence on association between female-specific characteristics and cardiovascular risks, and will also be helpful to provide a new path for early detection and prevention of CVD.

What is already known about this topic?Anemia during pregnancy is a global public health problem affecting both maternal and children’s health.The“National Nutrition Plan(2017–2030)”and“Healthy China Action(2019–2030)”issued by the State Council of China in 2017 and 2019,respectively,specified nutrition targets:by 2030,the anemia rate in pregnant Women should be reduced to less than 10%.The anemia prevalence of pregnant Women reported by the Chinese Nutrition and Health Surveillance in 2006 and 2010–2012 was 42.0%and 17.2%,respectively.What is added by this report?Past surveillance in 2010–2012 did not divide pregnant Women by gestation week,and the sample size was only 4,315 cases.In this study,the information of 206,753 registered pregnant Women from their first antenatal care(ANC)examination to childbirth was collected from 2014 to 2018.The overall prevalence of anemia among pregnant Women was 41.98%.What are the implications for public health practice?The overall prevalence of anemia among pregnant Women in the monitoring areas was high,far from the target of 10%.Anemia remains a serious health problem among pregnant Women in China.It is urgent to develop effective strategies and take measures to reduce the prevalence of anemia in China.

Does empowering Women benefit poverty reduction?Evidence from a multi-component program in the Inner Mongolia Autonomous Region of China

Ending poverty is a top priority of the international development agenda,and governments worldwide have attached great importance to poverty alleviation measures.However,poverty reduction policies have mostly focused on men,which has widened the gap in productivity and income between men and Women and increased gender inequality.This paper aims to determine the impacts of a multi-component program on Women's empowerment and poverty reduction,and explore the role empowered Women play in poverty reduction.The dataset used in this study was collected in nine poor counties of Ulanqab City in the Inner Mongolia Autonomous Region of China at the end of 2014,yielding a sample of 900 households.Recall questions were used to reconstruct the baseline data and build a panel dataset.Smaller groups of rural households were further identified to better target the Women in the beneficiary group.To control the selection bias,propensity score matching,inverse probability weighting,and the difference-in-differences matching method were used to analyze the effect of the program and undertake robust checks.The results show that the program has positive effects on Women's empowerment and poverty reduction simultaneously.Empowering Women also has positive effects on poverty reduction,and the Women who were the beneficiaries have contributed to increasing the incomes and living standards of households.Training,microfinance,and associations are common means or strategies to empower Women to address poverty.This paper provides new empirical evidence that Women can benefit from a gender-focus program through portfolio intervention such as training,cooperatives,and credit.Empowered Women further improve the livelihoods of poor households and help lift them out of poverty.The results suggest that researchers and policymakers need to pay more attention to poverty issues from the perspective of gender.

Objective:The aim of this study is to compare the effect of Vitamin D and Vitamin E supplementations,alone,and in combination,on reducing the intensity and duration of dysmenorrhea in Women who were referred to the Kowsar gynecological clinics of Shahid Motahari Hospital of Urmia University of Medical Sciences.Materials and Methods:A double-blinded clinical trial was conducted on 112 Women with dysmenorrhea who were randomly allocated into the four study groups.Finally,100 Women complete the study and received capsules containing a placebo(n=25),1000 IU of Vitamin D(n=25),400 mg of Vitamin E(n=25),and 1000 IU of Vitamin D+400 mg of Vitamin E(n=25)every 24 h for 2 consecutive months at the beginning of the menstrual period.The pain intensity and duration among groups were compared before and after the intervention.Results:The mean menstrual pain duration in all three experimental groups(Vitamin E,Vitamin D,and the combination of them)was lower than the placebo group(all P<0.05),and the mean pain intensity scores in all three experimental groups were significantly lower than the placebo group(all P<0.05).The results(means)for all of the drugs administered at the end of the 2nd month were better than the 1st month(all P<0.01).Conclusions:Vitamin E and Vitamin D,as well as their combination,are recommended as an effective and safe treatment for the management of the complications of dysmenorrhea.

Women on the liver transplantation waitlist are at increased risk of hospitalization compared to men

BACKGROUND Hospital admissions are common among patients with cirrhosis,but patient factors associated with hospitalization have not been well characterized.Given recent data suggesting increased liver transplant waitlist dropout among Women,we hypothesized that Women on the liver transplant waitlist would have increased rates of hospitalization compared with men.AIM To evaluate the role of gender on risk of hospitalization for patients on the liver transplant waitlist,in order to help explain gender disparities in waitlist outcomes.METHODS Patients listed for liver transplant at a single center in the United States were prospectively enrolled in the Functional Assessment in Liver Transplantation Study.Patients included in this retrospective analysis included those enrolled between March 2012 and December 2014 with at least 12 mo of follow up and without hepatocellular carcinoma.The primary and secondary outcomes were hospitalization and total inpatient days within 12 mo,respectively.Logistic and negative binomial regression associated baseline factors with outcomes.RESULTS Of the 392 patients,41%were female,with median(interquartile range)age 58 years(52-63)and model for end-stage liver disease 18(15-22).Within 12 mo,186(47%)patients were hospitalized≥1 time;48%were readmitted,with a median of 8(4-15)inpatient days.More Women than men were hospitalized(54%vs 43%;P=0.03).In univariable analysis,female sex was associated with an increased risk of hospitalization[odds ratios(OR)1.6,95%confidence interval(CI)1.0-2.4;P=0.03],which remained significant on adjusted multivariable analysis(OR 1.6,95%CI:1.1-2.6;P=0.03).Female gender was also associated with an increased number of inpatient days within 12 mo in both univariable and multivariable regression.CONCLUSION Women with cirrhosis on the liver transplant waitlist have more hospitalizations and inpatient days in one year compared with men,suggesting that the experience of cirrhosis differs between men and Women,despite similar baseline illness severity.Future studies
