Diagnostic value of Tumor marker pro-gastrin-releasing peptide in patients with small cell lung cancer： a systematic review

Background Lung cancer is one of the most common malignancies in the world and one of the leading cancers that result in death. The aim of this study was to evaluate and compare the diagnostic value of the serum Tumor marker pro-gastrin-releasing peptide 31-98 （ProGRP31-98） to pathological diagnosis as reference standard in patients with suspected small cell lung cancer （SCLC）.Methods Literature searches covering 1978 through to 2009 were performed in Pubmed, OVID, MEDLINE, EMbase,Cancerlit, China National Knowledge Infrastructure （CNKI）, and CBM using the key search words; ＇small cell lung cancer＇,＇Tumor marker＇, ＇ProGRP31-98＇ and ＇diagnostic tests＇, ＇ELISA＇, ＇EIA＇ and ＇diagnostic accuracy＇. Studies were collected and data analyzed to evaluate the diagnostic value of serum ProGRP31-98 levels for the diagnosis of SCLC compared with pathology. Eligibility criteria for inclusion in the analysis were based on criteria for diagnostic research published by the Cochrane Screening and Diagnostic Tests Methods Group （SDTMG）. The characteristics of the included articles were appraised and the data were extracted from the original articles for further statistical analysis of study heterogeneity using Review Manager 4.2 software. Based on study heterogeneity analysis, a suitable ＇effect＇ model was selected to calculate pooled sensitivity and specificity by meta-analysis. A Summary Receiver Operating Characteristic （SROC） curve and the area under the curve （AUC） were generated and sensitivity analysis conducted.Results A total of 22 articles were entered into this meta-review, including 11 English articles with a quality at level C. In total, the studies involved 6759 subjects, of which 1470 were diagnosed with SCLC by pathology, and 5289 subjects diagnosed with non-SCLC （NSCLC）. The meta-analysis showed that heterogeneity among studies was high （P=0.00001,（I2）=86.8%）. With ELISA, the pooled sensitivity was 0.72 （0.70 to 0.75 at 95% Cl） and the pooled specificity was 0.93 （0.92to 0.94 at 95% Cl）; the SROC and the AUC were 0.

Background: Outcome for non-small cell lung cancer(NSCLC) is still poor in the world. One of the reasons for therapy failure is escape of tumor cell from the immune surveillance. The emergence of novel anti-PD-1/PD-L1 checkpoint Inhibitors has shown prominent efficacy. Although overall survival(OS) has improved in the first-and second-line therapy settings for advanced NSCLC, the benefit is not universal.Objective: We intend to evaluate potential predictive biomarkers such as circulating immune cell ratio, Tumor marker and cytokine in the peripheral blood, and assess correlation with response to anti-PD-L1 inhibition, progression-free survival and OS.Method: We studied 20 patients that received anti-PD-L1 antibody immunotherapy. Tumor markers, circulating immune cells and cytokines were evaluated in peripheral blood samples at the time of diagnosis and 2 months after initiation of anti-PD-L1 inhibition. Plasma Tumor markers comprising CEA, CA125 and CA199 were assayed by the micro-particle chemiluminescence analysis. Circulating immune cells and serum cytokines were analyzed by flow cytometry.Results: We found that anti-PD-L1 antibody affects circulating lymphocyte ratio. The ratio increased slightly in disease controlled patients but decreased prominently in disease progressed patients. Moreover, decrease of CD4 and CD8 T cells were related with disease progression, while MDSC and Treg reduction is associated with better response. In addition, Tumor markers level changed after anti-PD-L1 antibody therapy. In disease controlled patients Tumor markers decrease or stay stable generally, while in progressed patients at least two kinds of Tumor markers increase over 50% from baseline. Increased cytokine values(IFN-α1, IFN-α2, TNF-α, IL-6, IL-8 and MCP-1) after treatment were correlated with improved response to immunotherapy and prolonged OS.Conclusion: The circulating immune cell ratios, plasma Tumor marker levels and serum cytokines were related with clinical efficacy of anti-PD-L1 antibody therapy. MicroRNA-371a-3p as a blood-based biomarker in testis cancer

MicroRNAs(miRNAs)are small noncoding RNAs involved in the regulation of mRNA transcription and translation,and possess all desirable features of an ideal Tumor marker.Of almost 31 different miRNA clusters identified in germ cell tumors(GCTs),miR-371a-3p has shown exceptionally high sensitivity and specificity for both seminomatous and nonseminomatous GCTs.It is easily obtainable and correlates well with tumor burden.Recent multiinstitutional prospective studies have shown promising test characteristics for miR-371a-3p as a diagnostic blood-based biomarker for GCT prior to orchiectomy including 80%e100%sensitivity and 90%e100%specificity.This accuracy may address other unmet needs in the management of patients with GCT.Early studies have suggested the utility of miR-371a-3p in detecting occult nodal metastasis in high-risk clinical stage I and early stage II disease.Ongoing clinical trials including SWOG 1823 and AGCT1531 are specifically designed to confirm the utility of miR-371a-3p in clinical stage I GCT.Despite its strong association with viable GCT after treatment with chemotherapy,miR-371a-3p does not seem to accurately predict the presence of teratoma in residual lesions.Also,standardization of extraction and interpretation methods is a necessary step to assure uniform results across different institutions.

Metastasis is a major challenge in the treatment of cancer.Exosomes are a class of small extracellular vesicles(EVs)that play critical roles in several human diseases,especially cancer,by transferring information(e.g.,DNA,RNA,and protein)via cell-to-cell communication.Numerous recent studies have shown that exosomal long non-coding RNAs(lncRNAs)play crucial regulatory roles in cancer metastasis in the tumor microenvironment by altering the expression of several key signaling pathways and molecules.Due to their specificity and sensitivity,exosomal lncRNAs have potential as novel Tumor markers and therapeutic targets in the treatment of cancer metastasis.In this review,we aim to summarize the roles of exosomal lncRNAs in cancer metastasis,the mechanisms underlying their roles,and their potential clinical applications.

Aptamer is an oligonucleotide chain with specific binding ability to protein and other targets,which is widely used in ma ny fields.Because of its ability to screen the premise of unknown targets,it can be used to discover some novel Tumor markers,i.e.,membrane proteins that are specifically highly expressed on the surface of tumor cells.Tumor markers can be used in many fields such as early diagnosis and treatment,and a new type of Tumor marker proved to be effective can significantly improve the therapeutic effect of such tumors.However,further characterization of newly acquired membrane proteins is essential for their clinical use as Tumor markers.This review first briefly introduced the process of obtaining novel Tumor markers from nucleic acid aptamers.Next,the commonly used protein characterization methods could be used as a technical means to identify membrane protein targets corresponding to tumor cell aptamers,to clarify the principles,advantages and disadvantages of various means,and to analyze the most suitable situations for various experimental methods.Finally,the outlook was made and the characterization methods that should be used in such experiments were summarized.

Abortive transcript(AT)is a 2-19 nt long non-coding RNA that is produced in the abortive initiation stage.Abortive initiation was found to be closely related to RNA polymerase through in vitro experiments.Therefore,the distribution of AT length and the scale of abortive initiation are correlated to the promoter,discriminator,and transcription initiation sequence,and can be affected by transcription elongation factors.AT plays an important role in the occurrence and development of various diseases.Here we summarize the discovery of AT,the factors responsible for AT formation,the detection methods and biological functions of AT,to provide new clues for finding potential targets in the early diagnosis and treatment of cancers.

Clinical Value of Tumor markers for Determining Cause of Pleural Effusion

Background： It is often challenging to distinguish tuberculous pleural effusion （TPE） from malignant pleural effusion （MPE）;thoracoscopy is among the techniques with the highest diagnostic ability in this regard.However, such invasive examinations cannot be performed on the elderly, or on those in poor physical condition.The aim of this study was to explore the differential diagnostic value of carbohydrate antigen 125 （CA 125）, carbohydrate antigen 199 （CA 199）, carcinoembryonic antigen （CEA）, neuron-specific enolase （NS E）, and squamous cell carcinoma （SCC） associated antigen in patients with TPE and MPE.Methods： Using electrochemiluminescence, we measured the concentration of Tumor markers （TMs） in the pleural effusion and serum of patients with TPE （n =35） and MPE （n =95）.We used receiver operating characteristic （ROC） curve analysis to evaluate the TMs and differentiate between TPE and MPE.Results： The cut-offvalues for each TM in serum were： CA125, 151.55 U/ml;CA199, 9.88 U/ml;CEA, 3.50 ng/ml;NSE, 13.27 ng/ml;and SCC, 0.85 ng/ml.Those in pleural fluid were： CA125, 644.30 U/ml;CA199, 12.08 U/ml;CEA, 3.35 ng/ml;NSE, 9.71 ng/ml;and SCC, 1.35 ng/ml.The cut-offvalues for the ratio ofpleural fluid concentration to serum concentration （P/S ratio） of each TM were： CA125, 5.93;CA199, 0.80;CEA, 1.47;NSE, 0.76;and SCC, 0.90.The P/S ratio showed the highest specificity in the case of CEA （97.14%）.ROC curve analysis revealed that, for all TMs, the area under the curve in pleural fluid （0.95） was significantly different from that in serum （0.85;P 〈 0.001）.Conclusions： TMs in TPE differ significantly from those in MPE, especially when detected in pleural fluid.The combined detection of TMs can improve diagnostic sensitivity.

Influence of age on seven putative prostate Tumor markers： a cohort study in Chinese men

The accuracy and sensitivity of prostate-specific antigen （PSA） for prostate cancer diagnosis is often poor; however, the reasons for its inaccuracy have rarely been investigated, especially with respect to age. In this study, 476 healthy males, aged 10-89 years, were stratified into eight age groups, and levels of seven markers were determined： total PSA （tPSA）, free PSA （fPSA）, %fPSA, isoform [-2]proPSA （p2PSA）, p2PSA/tPSA, %p2PSA, and the prostate health index （PHI）. Both tPSA and fPSA levels increased with age. The tPSA level was highest （1.39 ng m1-1） at 70-79 years; %fPSA was highest （0.57 ng ml-1） at 10-19 years; and %p2PSA was lowest （18.33 ng ml-1） at 40-49 years. Both p2PSA and p2PSA/tPSA had relatively flat curves and showed no correlation with age （P = 0.222）. PHI was a sensitive age-associated marker （P 〈 0.05）, with two peaks and one trough. The coverage rates and radiance graphs of PHI and %p2PSA were more distinctive than those of tPSA and the other markers. In subjects older than 69 years, PHI and %p2PSA both began to decrease, approximately 10 years earlier than the decrease in tPSA. Our results suggest that the clinical diagnosis of prostate cancer using PSA should be investigated more comprehensively based on patient age. Moreover, %p2PSA and PHI could be considered as earlier markers that may be more suitable than PSA alone.

Impact of chronic kidney disease on serum Tumor markers concentrations

Background Serum Tumor markers have always been of clinical importance in the diagnosis, monitoring disease progression and therapy efficacy for patients with malignant diseases. However, elevated serum Tumor markers are found in some benign conditions, especially in chronic kidney disease （CKD）. The elevation of them in CKD might cause confusion and misuse of these Tumor markers. We conducted this retrospective study to investigate which of the five widely used Tumor markers including carcinoembryonic antigen （CEA）, alpha-fetoprotein （AFP）, cytokeratin 19 fragment antigen 21-1 （Cyfra21-1）, squamous cell carcinoma antigen （SCC） and neuron specific enolase （NSE） are affected markedly by CKD, in order to use them more effectively. Methods Serum Tumor marker concentrations, biochemical, hematological parameters, and urinalysis were measured in CKD patients and healthy controls. The positive rate and median Tumor markers＇ level in CKD patients and controls, and those in CKD patients stratified by CKD grade were compared using nonparametric rank tests. Correlation analysis of serum Tumor markers and other parameters in CKD patients were performed using the Spearman correlation coefficient. Multivariate Logistic regression analysis was used to estimate the important variables that caused elevated serum concentrations of these markers in CKD patients. Results The overall positive rates and serum concentrations of Cyfra21-1, SCC, CEA in CKD group were significantly higher than those in control group. Positive rate and serum concentrations of those Tumor markers increased as kidney function decreased. Both univariate analysis and multivariate regression analysis showed that the elevations of those Tumor markers were not only associated with kidney function, but also with nutritional status. Conclusions Serum concentrations of Cyfra21-1, SCC, CEA are significantly influenced by kidney function, as well as nutritional status.
