small bowel adenocarcinoma(SBA)is a rare malignancy of the gastrointestinal ***,these tumors are among those with worst *** clinical signs and symptoms and radiological diagnostic challenges often delay treatment,which negatively impacts the prognosis of the ***,recent advances in imaging technology,like multidetector computed tomography,magnetic resonance imaging,and capsule endoscopy,have made earlier and accurate diagnosis *** is the treatment of choice followed by adjuvant ***,there are no strict treatment guidelines available for the management of *** of the available evidence from colorectal and gastric carcinoma has been extrapolated to adequately manage *** for SBA is better than gastric carcinoma but worse than colorectal ***,there is not enough information on the molecular characteristics and tumor *** the incidence of SBA is very low,there is a need for further studies to evaluate the possible application of newer investigative agents and strategies to obtain a better outcome within the framework of international collaborations.

Gastrointestinal bleeding accounts for a drastic negative impact on the quality of the patients’lives as it requires multiple diagnostic and therapeutic interventions to identify the source of the *** bowel bleeding is the least common cause of gastrointestinal ***,it is responsible for the majority of complaints from patients with persisting or recurring bleeding where the primary source of bleeding cannot be identified despite investigation.A somatostatin analog known as octreotide is among the medical treatment modalities currently used to manage small bowel *** medication helps control symptoms of gastrointestinal bleeding by augmenting platelet aggregation,decreasing splanchnic blood flow,and antagonizing *** this review article,we will highlight the clinical efficacy of octreotide in small bowel bleeding and its subsequent effect on morbidity and mortality.

AIM:To evaluate the clinical presentation,treatment and survival of patients with primary malignant tumor of small bowel(PMTSB).METHODS:Clinicopathologic data about 141 surgically treated PMTSB patients(91 males and 50 females) at the median age of 53.5 years(range 23-79 years) were retrospectively ***:The most common initial clinical features of the patients were intermittent abdominal discomfort or vague abdominal pain(67.4%),abdominal mass(31.2%),bowel obstruction(24.1%),hemotochezia(21.3%),jaundice(16.3%),fever(14.2%),coexistence of bowel perforation and peritonitis(5.7%),coexistence of gastrointestinal bleeding and shock(5.0%),and intraabdominal bleeding(1.4%).Ileum was the most common site of tumor(44.7%),followed by jejunum(30.5%)and duodenum(24.8%).PMTSB had a nonspecific clinical *** bowel resection(n=81)was the most common surgical procedure,followed by right hemicolectomy(n=15),pancreaticoduodenectomy(n=10),and others(n=19).Twenty-seven adenocarcinoma patients and 13 malignant lymphoma patients received adjuvant chemotherapy with 5-fluorouracil and cyclophosphamide,adriamycin,vincristine and prednisone,*** about 120 patients was obtained during the *** median survival time of PMTSB patients was 20.3 *** 1-,3-and 5-year survival rate was 75.0%(90/120),40.0%(48/120)and 20.8%(25/120),*** was found in 73.7%(42/57),21.1%(12/57)and 15.8%(9/57)of the patients,*** stromal tumor was observed in 80.0%(20/25),72.0%(18/25)and 36.0%(9/25)of the patients,*** was detected in 100.0%(15/15),80.0%(12/15)and 46.7%(7/15)of the patients,*** lymphoma was demonstrated in 69.2%(9/13),30.8%(4/13)and 0%(0/13)of the patients,***:En bloc resection is the principal therapy for most PMTSB and chemotherapy is the important treatment modality for malignant lymphoma and other malignant tumors of small bowel which ca

AIM: To investigate the prevalence of proximal small bowel (SB) lesions detected by wireless capsule endoscopy (WCE) in Crohn's disease (CD). METHODS: WCE was performed in 64 patients: 32 with CD of the distal ileum, and 32 controls with iron-deficiency anemia (IDA) or diarrhea. WCE was performed using the Given SB-WCE, followed by small intestine contrast ultrasonography (SICUS). Findings compatible with CD by using WCE included erosions, aphthoid or deep ulcers, and strictures/stenosis. RESULTS: WCE detected proximal SB lesions in 16/32 (50%) patients (14 aphthoid ulcers, 2 deep ulcers, one stricture), which appeared not to be related to clinical parameters [epigastric pain, age, smoking, non-steroidalanti-inflammatory drugs (NSAIDs), IDA]. Among patients with proximal SB lesions, 6 (37%) were smokers, 3 (19%) NSAID users, 3 (19%) had epigastric pain and 4 (25%) had IDA. SICUS detected proximal SB lesions in 3/32 patients (19%) also showing lesions with WCE. No correlations were observed between proximal SB lesions assessed by WCE or by SICUS (χ2 = 1.5, P = 0.2). CONCLUSION: The use of WCE allows the detection of previously unknown upper SB lesions in a high proportion of patients with a previous diagnosis of CD involving the distal ileum.

The small bowel represents one of the main sites for cutaneous melanoma metastasis;however, numerous cases of primary intestinal melanoma have recently been described. In view of this, we present the case of a 39-year-old woman admitted for nausea, heartburn, abdominal pain, change in bowel habits and weight loss. Contrast-enhanced CT revealed a small bowel mass. Surgical resection of a 6 cm ileal tumour with regional mesenteric lymphadenectomy and end-to-end anastomosis was performed. Histopathological findings indicated the presence of an ileal melanoma metastasis. Subsequent dermatological examination identified a cutaneous lesion on the right forearm, however no malignant cells were found at the histopathological exam. Whole body PET CT with FDG identified multiple frontal and parietal lesions. Genetic testing was positive for BRAF gene V600 E mutation. The patient underwent multiple neurosurgical procedures for the resection of cerebral metastases. Palliative external radiation and chemotherapy was also attempted. After approximately 2 years after the diagnosis, the patient died following multiple episodes of intracranial hypertension.

BACKGROUND Obscure small bowel bleeding is defined as gastrointestinal bleeding(GIB)that is unidentifiable with esophagogastroduodenoscopy and a colonoscopy with video capsule endoscopy(VCE)being the next gold standard step for *** bowel transit time(SBTT)is a metric of a VCE study that is defined as the time the capsule takes to travel through the small *** To determine if SBTT within the VCE study,correlates to overall detection of obscure small bowel ***,we attempted to identify any existing correlation between SBTT and re-bleeding after a negative VCE *** This is a single center retrospective analysis of VCE studies performed for overt and occult GIB at Einstein Medical Center,Philadelphia,between 2015 and *** criteria primarily consisted of patients 18 years or older who had a VCE study done as part of the workup for a *** with incomplete VCEs,poor preparation,or with less than 6 mo of follow up were excluded.A re-bleeding event was defined either as overt or occult within a 6-mo *** rebleeding was defined as Visible melena or hematochezia with>2 gm/dL drop in hemoglobin defined an overt re-bleeding event;whereas an unexplained>2 gm/dL drop in hemoglobin with no visible bleeding defined an occult *** Results indicated that there was a significant and positive point biserial correlation between SBTT of 220 min and detection of a bleeding focus with a statistically significant p value of ***,the area under the curve was negligible when trying to identify a threshold time for SBTT to discriminate between risk of re-bleeding events after a negative *** In terms of SBTT and association with accuracy of VCE finding a bleeding focus,220 min was found to be adequate transit time to accurately find a bleeding focus,when *** was found that no threshold SBTT could be identified to help predict re-bleeding after a negative VCE.

This review provides an update on the epidemiology,pathophysiology,symptoms,diagnosis and treatment of neuroendocrine neoplasms(NENs)of the small bowel(SB).These NENs are defined as a group of neoplasms deriving from neuroendocrine *** are currently the most common primary tumors of the SB,mainly involving the ileum,making the SB the most frequently affected part of the gastrointestinal *** NENs by definition are located between the ligament of Treitz and the ileocecal *** are characterized by small size and induce an extensive fibrotic reaction in the small intestine including the mesentery,resulting in narrowing or twisting of the *** manifestations of bowel functionality are related to the precise location of the primary *** majority of them are non-functional NENs and generally asymptomatic;in an advanced stage,NENs present symptoms of mass effect by non-specific abdominal pain or carcinoid syndrome which appears in patients with liver metastasis(around 10%).The main manifestations of the carcinoid syndrome are facial flushing(94%),diarrhea(78%),abdominal cramps(50%),heart valve disease(50%),telangiectasia(25%),wheezing(15%)and edema(19%).Diagnosis is made by imaging or biochemical tests,and the order of request will depend on the initial diagnostic hypothesis,while confirmation will always be *** patients with a localized SB NEN with or without near metastasis in the mesentery are recommended for curative *** and distant spread may be susceptible to several therapeutic strategies,such as chemotherapy,somatostatin analogs and palliative resection.
