AIM To investigate second primary malignancy(SPM) risk after radiotherapy in rectal cancer survivors METHODS We used Taiwan's National Health Insurance Research Database to identify rectal cancer patients between 1996 and 2011. Surgery-alone, preoperative short course, preoperative long course, and post-operative radiotherapy groups were defined. The overall and sitespecific SPM incidence rates were compared among the radiotherapy groups by multivariate Cox regression, taking chemotherapy and comorbidities into account. Sensitivity tests were performed for attained-year adjustment and long-term survivors analysis. RESULTS A total of 28220 patients were analyzed. The 10-year cumulative SPM incidence was 7.8% [95% confidence interval(CI): 7.2%-8.2%] using a competing risk model. The most common sites of SPM were the lung, liver, and prostate. Radiotherapy was not associated with increased SPM risk in multi-variate Cox model(hazard ratio = 1.05, 95%CI: 0.91-1.21, P = 0.494). The SPM hazard remained unchanged in 10-yearsurvivors. In addition, no SPM risk difference was found between the preoperative radiotherapy and postoperative radiotherapy groups.CONCLUSION In this large population-based cohort study, we demonstrated that radiotherapy had no increase in SPM.

Background:The predominant method for Manske type ⅢB and Ⅳ thumb hypoplasia is pollicization.However,for those who are not willing to sacrifice the index finger,a method that could reconstruct a functionally capable and aesthetically acceptable thumb remains desirable.This study aimed to investigate and assess the functional and radiographic outcomes of utilizing a reversed vascularized second metatarsal composite flap for thumb reconstruction as a new alternative.Methods:From May 2014 to January 2017,15 patients with Manske type ⅢB or Ⅳ thumb hypoplasia who were admitted to the Department of Hand Surgery,Beijing Jishuitan Hospital were included in this study.An osteocutaneous flap containing a section of second metatarsal and its distal head was transferred in reversed position to reconstruct carpometacarpal joint.The donor site was reconstructed by a split half of the third metatarsal.Various functional reconstructions were commenced at second stage.The reconstructed thumbs were evaluated using the Kapandji score,pinch force,and the capacities of performing daily activities through a detailed questionnaire.Results:Among these 15 patients(seven type ⅢB and eight type Ⅳ),there were ten boys and five girls with median age of 4.2 years(range:2.0–7.0 years).There were seven right,three left,and five bilateral thumbs for whom only the right thumb received surgery.There were 14 metatarsal flaps survived(14/15).With an average follow-up of 19.2 months,the reconstructed thumbs had acceptable functional and aesthetic outcomes and the donor foot presented in decent appearance without signs of impaired function.All 15 children have improved the Kapandji score(from 0 to an average of 6.7),pinch force(from 0 to an average of 1.5 kg),with ability of grip and pen holding.X-ray indicated continuous bone growth.Patients and parents had good acceptance of the new thumb.Conclusions:Reconstruction of an unstable hypoplastic thumb(Manske type ⅢB and Ⅳ)with use of a vascularized metatarsal is an effective strategy.It offers an altern

Thera no stic nano particles are integrated systems useful for simulta neous diag nosis and imaging guided delivery of therapeutic drugs, with wide ranging pote ntial applicati ons in the clinic. Here we developed a thera no stic nan oparticle (~24 nm size by dynamic light scatteri ng) p-FE-PTX-FA based on polymeric micelle encapsulating an organic dye (FE) fluorescing in the 1,000-1,700 nm second near-infrared (NIR-Ⅱ) window and an an ti-ca ncer drug paclitaxel. Folic acid (FA) was conjugated to the nan oparticles to afford specific binding to molecular folate receptors on muri ne breast can cer 4T1 tumor cells. In vivo, the nan oparticles accumulated in 4T1 tumor through both passive and active targeting effect. Under an 808 nm laser excitation, fluorescence detection above 1,300 nm afforded a large Stokes shift, allowing targeted molecular imaging tumor with high signal to background ratios, reaching a high tumor to normal tissue signal ratio (T/NT) of (20.0±2.3). Further, 4T1 tumors on mice were completed eradicated by paclitaxel released from p-FE-PTA-FA within 20 days of the first injection. Pharmacokinetics and histology studies indicated p-FE-PTX-FA had no obvious toxic side effects to major organs. This represented the first NIR-Ⅱ theranostic age nt developed.

To understand and control the interfacial properties of polydiacetylenes(PDAs)vesicles withπ-conjugated backbone is very important for their colorimetric sensing of chemical and biological targets.In this work,we adopted 10,12-pentacosadiynoic acid(PCDA)as the model molecule to prepare PDAs vesicles in aqueous solution with different forms(from monomer to blue-to-purple-to-red phase)by controlling the UV irradiation dose.The variations of the interfacial conformation of PDAs vesicles during chromatic transitions were inspected by the adsorption behaviors of probe molecules(4-(4-diethylaminostyry)-1-methylpyridinium iodide,D289)on vesicle surface with surface-specific second harmonic generation(SHG)and zeta potential measurements.Resonant SHG signal from D289 adsorbed on vesicle surface attenuated sharply,and the adsorption free energy as well as the corresponding two-photon fluorescence signal decreased slightly in chromatic transitions.While,the change in the surface density of the adsorbed D289 molecules for PDAs vesicles with different forms was relatively small as estimated from zeta potential measurements.The attenuation of the SHG intensity was thus attributed to the overall order-disorder transition and the changed orientation of D289 molecules caused by the gradual distortion of carboxyl head group driven by backbone perturbation.

The emerging technique of photoacoustic imaging,especially in the near infra-red(NIR)window,permits high resolution,deep-penetration,clinically reliable sensing.However,few contrast agents are available that can specifically respond to intricate biological environments,and which are biodegradable and biocompatible.Herein,we in troduce a new class of pH-sensitive orga nic photoacoustic con trast age nt that operates in the second NIR window(NIR-II,960-1,700 nm),which is derived from the self-assembled charge-transfer nanocomplex(CTN)by 3,3',5,5'-tetramethylbenzidine(TMB)and its dication structure(TMB++).The unique NIR-ll-responsive CTN can specifically respond to pH change in the physiological range and allows noninvasive and sensitive visualization of the tumor acidic microenvironment(e.***.at pH5)in mice with higher signal-to-noise ratio.The CTN is biodegradable under physiological conditions(e.g.pH 7.4),which alleviates the biosafety concern of nan oparticle accumulati on in vivo.These results clearly show the pote ntial of the TMB/TMB++-based CTN as a promisi ng pH-activated and biodegradable molecular probe for specific tumor photoacoustic imaging in the NIR-II region.

Motivated by problems arising from Arithmetic Geometry,in an earlier article one of the authors studied germs of holomorphic isometries between bounded domains with respect to the Bergman metric.In the case of a germ of holomorphic isometry f:(Δ,λ ds 2Δ ;0) → (Ω,ds 2Ω ;0) of the Poincar disk Δ into a bounded symmetric domain Ω C N in its Harish-Chandra realization and equipped with the Bergman metric,f extends to a proper holomorphic isometric embedding F:(Δ,λ ds 2Δ) → (Ω,ds 2Ω) and Graph(f) extends to an affine-algebraic variety V  C × C N.Examples of F which are not totally geodesic have been constructed.They arise primarily from the p-th root map ρ p:H → H p and a non-standard holomorphic embedding G from the upper half-plane to the Siegel upper half-plane H 3 of genus 3.In the current article on the one hand we examine second fundamental forms σ of these known examples,by computing explicitly σ 2.On the other hand we study on the theoretical side asymptotic properties of σ for arbitrary holomorphic isometries of the Poincar disk into polydisks.For such mappings expressing via the inverse Cayley transform in terms of the Euclidean coordinate τ=s + it on the upper half-plane H,we have φ(τ)=t 2 u(τ),where u t=0 ≡ 0.We show that u must satisfy the first order differential equation u t | t=0 ≡ 0 on the real axis outside a finite number of points at which u is singular.As a by-product of our method of proof we show that any non-standard holomorphic isometric embedding of the Poincar disk into the polydisk must develop singularities along the boundary circle.The equation φuφt | t=0 ≡ 0 along the real axis for holomorphic isometries into polydisks distinguishes the latter maps from holomorphic isometries into Siegel upper half-planes arising from G.Towards the end of the article we formulate characterization problems for holomorphic isometries suggested both by the theoretical and the computational results of the article.

The imaging theory of nonlinear second harmonic generation (SHG) and third harmonic generation (THG) in confocal microscopy is presented in this paper. The nonlinear effect of SHG and THG on the imaging properties of confocal microscopy has been analyzed in detail by the imaging theory. It is proved that the imaging process of SHG and THG in confocal microscopy, which is different from conventional coherent imaging or incoherent imaging, can be divided into two different processes of coherent imaging. The three-dimensional point spread functions (3D-PSF) of SHG and THG confocal microscopy are derived based on the nonlinear principles of SHG and THG. The imaging properties of SHG and THG confocal microscopy are discussed in detail according to its 3D-PSF. It is shown that the resolution of SHG and THG confocal microscopy is higher than that of single- and two-photon confocal microscopy.
