Neuropsychological features in post-stroke cognitive impairment with no dementia patients with different Traditional Chinese Medicine syndromes

OBJECTIVE: To investigate neuropsychological features of post-stroke cognitive impairment with no dementia(PSCIND) patients with different Traditional Chinese Medicine(TCM) ***: We recruited 50 patients with PSCIND between April 2012 and March 2013. Patients were divided into different groups according to TCM classifications. Patients were assessed using neuropsychological tests, including cognitive screening(mini-mental state examination), memory testing (auditory verbal learning test), executive/attention[shape trails test, stroop color-word test(SCWT),reading the mind in the eyes test, the digit ordering test-A(DOT-A), and symbol digit modalities test], language(action naming test, Boston naming test, famous face test, similarity test, and verbal fluency test), and visuospatial functioning [complex figure test(CFT)].RESULTS: We found no significant differences between patients with and without a diagnosis of turbid phlegm blocking the upper orifices on neuropsychological test performance. Patients diagnosed with upper hyperactivity of liver Yang syndrome scored significantly lower on the SCWT-C executive test and the CFT-delayed recall memory test. Patients with excess syndrome scored significantly lower on the SCWT-C executive test, and significantly higher on the DOT-A executive ***: Neuropsychological characteristics differ between PSCIND patients with different TCM classifications.

Identification of SNP-containing regulatory motifs in the myelodysplastic syndromes model using SNP arrays and gene expression arrays

Myelodysplastic syndromes have increased in frequency and incidence in the American population, but patient prognosis has not significantly improved over the last decade. Such improvements could be realized if biomarkers for accurate diagnosis and prognostic stratification were successfully identified. In this study, we propose a method that associates two state-of-the-art array technologies-single nucleotide polymorphism (SNP) array and gene expression array-with gene motifs considered transcription factor -binding sites (TFBS). We are particularly interested in SNP-containing motifs introduced by genetic variation and mutation as TFBS. The potential regulation of SNP-containing motifs affects only when certain mutations occur. These motifs can be identified from a group of co-expressed genes with copy number variation. Then, we used a sliding window to identify motif candidates near SNPs on gene sequences. The candidates were filtered by coarse thresholding and fine statistical testing. Using the regression-based LARS-EN algorithm and a level-wise sequence combination procedure, we identified 28 SNP-containing motifs as candidate TFBS. We confirmed 21 of the 28 motifs with ChIP-chip fragments in the TRANSFAC database. Another six motifs were validated by TRANSFAC via searching binding fragments on coregulated genes. The identified motifs and their location genes can be considered potential biomarkers for myelodysplastic syndromes. Thus, our proposed method, a novel strategy for associating two data categories, is capable of integrating information from different sources to identify reliable candidate regulatory SNP-containing motifs introduced by genetic variation and mutation.

Background Elderly patients with non-ST-segment elevation acute coronary syndromes(NSTE-ACS)may present delirium but its clinical relevance is *** study aimed at detennining the clinical associated factors,and prognostic implications of delirium in old-aged patients admitted for *** LONGEVO-SCA is a prospective multicenter registry including unselected patients with NSTE-ACS aged>80 *** variables and a complete geriatric evaluation were assessed during *** association between delirium and 6-month mortality was assessed by a Cox regression model weighted for a propensity score including the potential confounding *** also analysed its association with 6-month bleeding and cognitive or functional *** Among 527 patients included,thirty-seven(7%)patients presented delirium during the *** was more frequent in patients with dementia or depression and in those from nursing homes(27.0%vs.3.1%,24.3%vs.11.6%,and 11.1%V5.2.2%,respectively;all P<0.05).Delirium was significantly associated with in-hospital infections(27.0%vs.5.3%,P<0.001)and usage of diuretics(70.3%vs.49.8%,P=0.02).Patients with delirium had longer hospitalizations[median 8.5(5.5-14)vs.6.0(4.0-10)days,P=0.02]and higher incidence of 6-month bleeding and mortality(32.3%vs.10.0%and 24.3%vs.10.8%,respectively;both P<0.05)but similar cognitive or functional *** was independently associated with 6-month mortality(HR=1.47,95%CI:1.02-2.13,P=0.04)and 6-month bleeding events(OR=2.87;95%CI:1.98-4」6,P<0.01).Conclusions In-hospital delirium in elderly patients with NSTE-ACS is associated with some preventable risk factors and it is an independent predictor of 6-month mortality.

Signs and genetics of rare cancer syndromes with gastroenterological features

Although the genetic bases of most hereditary cancer syndromes are known,and genetic tests are available for them,the incidence of the most rare of these syndromes is likely underestimated,partially because the clinical expression is neither fully understood nor easily diagnosed due to the variable and complex expressivity. The clinical features of a small pool of rare cancer syndromes include gastroenterological signs,though not necessarily tumors,that could require the intervention of a gastroenterologist during any of the phases of the clinical management. Herein we will attempt to spread the knowledge on these rare syndromes by summarizing the phenotype and genetic basis,and revising the peculiar gastroenterological signs whose underlying role in these rare hereditary cancer syndromes is often neglected. Close collaboration between geneticists and gastroenterologists could facilitate both the early identification of patients or relatives at-risk and the planning of multidisciplinary and tailored management of these subjects.

The progressive ageing of the population and the high incidence of acute coronary syndromes(ACS)in the elderly is leading to an important increase in the number of elderly patients admitted for ACS.[1]Nevertheless,the information about the optimal clinical management in this age group is scarce due to the exclusion of older patients from clinical trials.[2]Higher comorbidity and frailty are common in this clinical setting and they are associated with higher rates of complications and consumption of healthcare resources.[3]These are the main reasons for launching this special issue about management of ACS in the elderly,in which several experts in the field address some of the most important questions about how to deal with ACS in patients at older ages.

What is the rationale for immunotherapies in Parkinsonian syndromes (PS)? PS are neurodegenerative diseases which are clinically characterized by a hypokinetic phenotype in combination with additional motor and non-motor symptoms. One major pathological hallmark of all PS consists of a non-physiological detrimental accumulation of protein aggregates which appear intracellularly in neurons and glial cells but also in the extracellular space (Wong and Krainc, 2017). Depending on the pathogenic protein, PS can be divided into synucleinopathies, characterized by aggregation of the protein alpha-Synuclein (aSyn), and tauopathies, characterized by aggregation of the protein Tau (Levin et al., 2016;Poewe et al., 2017). Clinical syndromes of synucleinopathies include Parkinson’s disease (PD), multiple system atrophy (MSA) and dementia with Lewy bodies, and tauopathies include progressive supranuclear palsy (PSP) and corticobasal degeneration.

Relationship between platelet P-selectin and severity of acute coronary syndromes

In-hospital outcome of elderly patients with acute coronary syndromes treated with platelet glycoproteinⅡb/Ⅲa blockers

Background and objective The safety of intravenous glycoproteinⅡb/Ⅲa inhibitors (GPI) in elderly patients admitted with acute coronary syndrome (ACS) has not yet been established. The purpose of this study was to evaluate the safety of GPI in elderly patients with ACS. Methods Ninety consecutive patients≥70 years of age admitted to a county hospital between 1999-2004 were included. All patients had typical ACS symptoms along with high-risk markers. Results There was no difference in the TIMI risk score between patients who received GPI (n=47) and those who did not (n=43). Patients who received GPI had a lower creatinine clearance (40 cc/min vs. 47cc/min, p= 0.04). Patients who received GPI had a lower incidence of death, reinfarction or major bleeding (19% vs. 4%, p=0.03). There was no significant difference in major bleeding between the 2 groups. None of the patients in either group developed thrombocytopenia. Conclusion This retrospective small study suggests that the use of GPI in a selected group of elderly patients with acute coronary syndrome may be safe. (J Geriatr Cardiol 2005; 2(4):203-205 )

We previously reported the discovery of 14 channels in the human body,which possessed thebiophysical property of high emission of *** this study we found the same property on theRen and Du channels in healthy ***,we discovered that the luminance of the relat-ed channels in rat models with different“syndromes”varied *** instance,a markedlylow luminance appeared on the Du channel in animals with experimental syndrome of Yang defi-ciency induced by hydrocortisone;while in animals with experimental syndrome of blood deft-ciency caused by bleeding,an apparently low luminance occurred on the Ren *** inten-sity of the emitted light on Du and Ren channels increased after acupuncture treatment,but *** phenomenon conforms to the theory in traditional Chinese medicine that theDu channel is the“sea of Yang channels”,while the Ren channel is the“sea of Yin channels”.
