Significant risk factors for intensive care unit-acquired weakness:A processing strategy based on repeated machine learning

BACKGROUND Intensive care unit-acquired weakness(ICU-AW)is a common complication that significantly impacts the patient's recovery process,even leading to adverse outcomes.Currently,there is a lack of effective preventive measures.AIM To identify significant risk factors for ICU-AW through iterative machine learning techniques and offer recommendations for its prevention and treatment.METHODS Patients were categorized into ICU-AW and non-ICU-AW groups on the 14th day post-ICU admission.Relevant data from the initial 14 d of ICU stay,such as age,comorbidities,sedative dosage,vasopressor dosage,duration of mechanical ventilation,length of ICU stay,and rehabilitation therapy,were gathered.The relationships between these variables and ICU-AW were examined.Utilizing iterative machine learning techniques,a multilayer perceptron neural network model was developed,and its predictive performance for ICU-AW was assessed using the receiver operating characteristic curve.RESULTS Within the ICU-AW group,age,duration of mechanical ventilation,lorazepam dosage,adrenaline dosage,and length of ICU stay were significantly higher than in the non-ICU-AW group.Additionally,sepsis,multiple organ dysfunction syndrome,hypoalbuminemia,acute heart failure,respiratory failure,acute kidney injury,anemia,stress-related gastrointestinal bleeding,shock,hypertension,coronary artery disease,malignant tumors,and rehabilitation therapy ratios were significantly higher in the ICU-AW group,demonstrating statistical significance.The most influential factors contributing to ICU-AW were identified as the length of ICU stay(100.0%)and the duration of mechanical ventilation(54.9%).The neural network model predicted ICU-AW with an area under the curve of 0.941,sensitivity of 92.2%,and specificity of 82.7%.CONCLUSION The main factors influencing ICU-AW are the length of ICU stay and the duration of mechanical ventilation.A primary preventive strategy,when feasible,involves minimizing both ICU stay and mechanical ventilation duration.

A Study on the Values and strategies of English Picture Book Reading for Improving Pupils’ English Reading Literacy

Reading is one of the core skills in English learning.It is indispensable for EFL learners to develop good English reading literacy in primary schools.English picture books are consistent with pupils’cognitive and thinking modes,which is valuable to improve their English reading literacy.However,at present,most Chinese pupils have some problems in English reading.The paper aims at analyzing the current problems in Chinese pupils’English reading and digging out the values and strategies of English picture book reading,so as to stimulate their reading interests,cultivate their intercultural awareness,and improve their English reading literacy.

Colorectal cancer screening: Modalities and adherence

In the last decade,several studies have explored various modalities and strategies for colorectal cancer(CRC)screening,taking into account epidemiological data,individual characteristics,and socioeconomic factors.In this editorial,we comment further on a retrospective study by Agatsuma et al published in the recent issue of the World Journal of Gastroenterology.Our focus is on screening trends,particularly in relation to efforts to improve the currently suboptimal uptake among the general population worldwide,aiming to enhance early diagnosis rates of CRC.There is a need to raise awareness through health edu-cation programs and to consider the use of readily available,non-invasive screening methods.These strategies are crucial for attracting screen-eligible populations to participate in first-line screening,especially those in high-or average-risk groups and in regions with limited resources.Liquid biopsies and biomarkers represent rapidly evolving trends in screening and diagnosis;however,their clinical relevance has yet to be standardized.

Nurses'critical thinking disposition and professional commitment:a cross-sectional study

Objective:High-quality and successful nursing care delivery in a variety of situations requires critical thinking training and professional commitment.The present study aimed to investigate critical thinking disposition,professional commitment,and the relationship between these 2 variables among nurses.Methods:As a result of stratified random sampling,201 nurses were enrolled in the present cross-sectional study.Data were gathered using a socio-demographic form,the Critical Thinking Disposition Inventory(CTDI),and the Nurses'Professional Commitment Scale(NPCS).Results:Although the critical thinking disposition scores were at a moderate level(115.70±13.55),the mean score for professional commitment was high(91.37±11.77).A positive correlation was found between critical thinking disposition and professional commitment(r=0.67,P=0.001).Conclusions:Nursing managers are urged to employ strategies to enhance nurses'critical thinking disposition and professional commitment.

The integration of Artificial Intelligence(AI)in healthcare is setting the stage for a transformative shift in how patient care is delivered,research is conducted,and operations are managed.Propelled by the exponential growth of data,computational advancements,and AI innovations,this integration promises a new era of precision medicine with highly personalized and effective treatment strategies.However,the journey towards seamlessly embedding AI into healthcare systems is complex,marked by challenges such as ensuring data privacy and security,addressing ethical considerations,and overcoming barriers to technology integration and adoption.This paper delves into the current trends driving AI in healthcare,including machine learning,natural language processing,robotics,and the Internet of Medical Things,while also tackling the significant challenges these innovations present.It further explores strategies for navigating these obstacles,aiming to pave the way for the successful adoption of AI technologies that enhance healthcare delivery and patient outcomes.In doing so,this work underscores the critical role of collaborative efforts among stakeholders and the need for robust frameworks to ensure AI's ethical,secure,and effective integration into healthcare.

Exclusive breastfeeding greater than 50%, success of education in a university hospital in Bogotá: Case-control study

BACKGROUND Maintenance rates of exclusive breastfeeding(EBF)worldwide are low,thus,one of the objectives of the summary of policies on breastfeeding(BF)in world nutrition goals for 2025 are that at least 50%of infants under six months of age receive EBF that year.The Objective of this study is to document the rates of EBF in children born in San Ignacio University Hospital(HUSI)and identify factors associated with maintenance.AIM To document the percentages of EBF in those that were born at HUSI and identify factors associated to their maintenance.METHODS This is a study of cases and controls in an analytic,retrospective cohort that took children born alive between January 2016 and January 2019 at HUSI located in the city of Bogotá,Colombia.RESULTS Receiving information about BF at HUSI was able to maintain EBF up until 4 mo(OR=1.65;95%CI:1.02-2.66).The presence of gynecologic and obstetric comorbidities(OR=0.32;95%CI:0.12-0.83),having mastitis(OR=0.56;95%CI:0.33-0.94),and receiving information from mass media(OR=0.52;95%CI:0.31-0.84)are factors associated with not maintaining EBF.CONCLUSION Receiving education at a Women-and Child-Friendly Institution was the only significant factor to achieve EBF until 4 mo,with a frequency greater than the one reported in the country,which matches multiple studies where counseling and individualized support on BF achieve this purpose.Knowledge about BF and early detection of obstetric/gynecologic complications must be strengthened among the healthcare staff in charge of mothers during post-partum.Additionally,strategies must be promoted to continue BF such as creating milk banks with the objective of increasing BF rates even when mothers return to work.

With the advantages of metal conductivity,large specific surface area,and rich surface functional groups,two-dimensional(2D)MXenes have shown great potential in the field of gas sensing.However,gas sensors fabricated with pristine MXenes generally suffer from several problems such as low sensitivity,poor selectivity,significant baseresistance drift,and poor environment stability.Therefore,many efforts have been devoted to overcoming these problems.In this review,we review the progress on MXenebased gas sensors and summarize several efficient strategies(including structural design,surface modification,inorganic Schottky j unction/heterojunction sensitization,polymer addition,and metal-ion intercalation)to promote the gassensing performance.In addition,the major challenges and future development directions of MXene-based gas sensors are also outlined in the present review.

strategies to enhance Li^(+) transference number in liquid electrolytes for better lithium batteries

Growing market demand from portable electronics to electric automobiles boosts the development of lithium-ion batteries(LIBs)with high energy density and rate performance.However,strong solvation effect between lithium ions(Li^(+))and solvent molecules in common electrolytes limits the mobility of Li^(+)ions in electrolytes.Consequently,anions dominate the charge conduction in electrolytes,and in most cases,the value of Li^(+)transference number(T_(+))is between 0.2 and 0.4.A low T_(+)will aggravate concentration polarization in the process of charging and discharging,especially at high rate,which not only increases the overpotential but also intensifies side reactions,along with uneven deposition of lithium(Li)and the growth of lithium dendrites when lithium metal is used as anode.In this review,promising strategies to improve T_(+)in liquid electrolytes would be summarized.The migration of Li^(+)ions is affected directly by the types and concentration of lithium salts,solvents,and additives in bulk electrolytes.Besides,Li^(+)ions will pass through the separator and solid electrolyte interphase(SEI)when transferring between anodes and cathodes.With this in mind,we will classify and summarize threads of enhancing T_(+)from five aspects:lithium salts,solvents,additives,separators,and SEI based on different mechanisms,including covalently bonding,desolvation effect,Lewis acid-base interaction,electrostatic interaction,pore sieving,and supramolecular interaction.We believe this review will present a systematic understanding and summary on T_(+)and point out some feasible threads to enhance battery performance by enhancing T_(+).

Protein assembly is the structural basis for the collaboration between proteins to accomplish life activities due to its realization of the domain-limited and precise spatial arrangement of proteins.Therefore,artificial manipulation of protein selfassembly has profound implications in areas such as exploring the mysteries of life and developing biomaterials.In this review,we not only summarize the classical assembly strategies and the structures of exquisite protein assemblies,but also aim to generalize the flexible and controllable assembly tools and the“interaction”between the assembled protein-based materials and the external environment.On the basis of this,the application,challenges and further development of protein assembly in biology are reviewed.
