本文报导了取正常猪脾脏用冻融—透析法制备转移因子(TF)的操作方法。通过系列试验TF含有15种氨基酸,蛋白反应阴性,无热源无抗原性,可通过半透膜,是一种多肽和单核苷酸物质。TF能激发和增强机体细胞免疫功能。自制了数批TF,对鸡传染性喉气管炎、传染鼻炎进行了治疗试验,1988～1989年在河南省五个鸡场共治疗12,720只病鸡,治愈率为85％以上。在常发此病的疫场预防了2,900只鸡,结果表明肌内注射较饮水预防效果好。TF原料来源广,成本低,制备简单,安全有效,是一种很有前途的生物制剂。

目的探讨儿童过敏性鼻炎(allergic rhinitis,AR)的发病及特异性免疫治疗(specific immunotherapy,SIT)的治疗机制。方法采用流式细胞术检测56例AR患者SIT治疗前、治疗1年后外周血中CD4+CD25+CD127lo/-Treg表达水平:采用ELISA法检测血清中IL-10、TGF-β1,并与30例健康儿童进行对比。结果 56例AR患儿SIT治疗前外周血中CD4+CD25+CD127lo/-Treg占CD4+T细胞的百分比为(5.36±1.31)%,显著低于对照组(10.23±2.26)%,P<0.05,但SIT治疗1年后其数值为(9.37±2.15)%,与对照组无明显差异。AR患儿治疗前血清中IL-10值为(3.68±1.21)pg/ml,显著低于对照组的(12.37±2.18)pg/ml,P<0.05;TGF-β1表达水平与对照组比较差异无统计学意义。SIT治疗1年后血清中IL-10水平亦与对照组差异不明显(P<0.05);而TGF-β1表达水平分别为(20.16±4.45)pg/ml及(9.27±2.38)pg/ml,其差异有统计学意义(P<0.05)。结论儿童AR患者外周血中CD4+Treg表达水平低,且存在功能缺陷,导致体内诱导免疫耐受机能不足,SIT治疗可以明显调节免疫免疫细胞的功能。

Allergen-specific immunotherapy （IT） is an effective treatment for allergic diseases. Although subcutane-ous and sublingual ITs are currently used, safer, easier, and more effective IT is under development. Induction of immune tolerance by oral administration of allergen has been proven, though oral IT has not been applied clinically. It is mainly because a large amount of puri-fied allergen is required to induce oral tolerance. To overcome this problem, plants, peculiarly rice, have been investigated as allergen vehicles for oral IT. Rice can store a considerable amount of expressed allergen in its seeds and the accumulated allergen is stable and resistant to gastrointestinal digestion. Therefore, we have developed transgenic rice seeds （Tg rice） in which major epitopes of cedar pollen or house dust mites are expressed. We are establishing Tg rice with dem-onstrated effcacy in murine models of allergic rhinitis and bronchial asthma by oral administration at practical doses. In addition, the amount, distribution, and aller-genicity of the expressed allergen have been improved in our Tg rice. Rice-based oral IT is a promising new concept in IT for the treatment of allergic diseases.

Allergies in Children: What’s New?—A Cross-Sectional Descriptive Study

Background: The prevalence of respiratory allergies is increasing worldwide, with important consequences especially for little children. Objective: The aim of this study was to assess the prevalence of respiratory allergies, such as rhinitis and asthma, and to point out the risk factors and their relationship with allergic diseases in a specific area of Northern Italy. Methods: 110 children, male and female, from our outpatient service for allergic children, between 3 and 17 years old, were examined. After a skin prick test and a nasal cytology, the written questionnaire of the International Study of Asthma and Allergies in Childhood was filled by parents together with their children. Results: 110 children were examined. 74% of children had rhinitis and 71% asthma. 88 patients were allergic, grass pollen and house dust mite was the most frequent allergens. A family history of atopy, family background, geographic area, active and passive smoking and home pets were associated to allergies. Older children (6 - 15 years old) had more often rhinitis associated with asthma and conjunctivitis as compared to younger. 21 Children were also affected by non allergic rhinitis. Conclusions: Respiratory allergies are widespread and associated to a low quality of life among little children. Sensitization to Ragweed is increasing with important consequences. rhinitis precedes the onset of asthmatic symptoms. Moreover non allergic rhinitis is increasing and frequently underdiagnosed.

Though symptoms of allergic diseases can be reduced by the use of drugs such as corticosteroids, antihista-mines or leukotrien antagonists, the only treatment di-rected to change the natural course of allergic disease is allergen-specific immunotherapy （SIT）. Its efficacy can last years after the cessassion of the treatment. SIT brings on regulatory T cells with the capacity to generate interleukin-10 and transforming growth fac-tor-b, restricts activation of mast cells and basophils, and shifts antibody isotype from IgE to the noninfam-matory type immunoglobulin G4. Subcutaneous （SCIT） and sublingual （SLIT） immunotherapy are the two most used ways at the present for applying SIT. These two treatments were demonstrated to be effective on re-ducing symptoms and medication use, in prevention of new sensitizations and in protecting from progression of rhinitis to asthma. The safety of SLIT appears to be better than SCIT although there have been a few head to head comparisons. In order to overcome compliance problems or possible systemic side effects which may be faced during this long-term treatment, recent inves-tigations have been focused on the implementation of allergens in quite effcacious and safer ways.

Prevalence of the Respiratory Allergies among Adult Population in the City of Skopje in Relation to Climatic Change and Change in Pollen Micro Flora

Introduction: Climate change may have an impact of prevalence and severity of respiratory allergies causing changes in the concentrations, distribution, dispersion patterns, and allergenic potential of aeroallergens in the environment. Objective: To assess the prevalence of atopy, sensitization to common pollen allergens, and respiratory allergies (allergic rhinitis and asthma) among adult population in the city of Skopje in relation to changes in pollen distributions caused by climate change and particularly maximum temperature, in the period 1996-2010. Methods: Aeropalinological measurements in the city of Skopje were performed by volumetric method in the period 1996-2009. Serial cross-sectional studies on the prevalence of atopy, sensitization to common pollen allergens, allergic rhinitis and asthma in adults by standardized methodology were conducted in defined critical periods (1996, 2003, 2007/2008 and 2009/2010). Evaluation of examined subjects included completion of a questionnaire, skin prick tests (SPT) to common inhalant allergens, and lung function tests. Results: Data from aeropalinological measurements showed differences in the pollen concentrations, as well as in the duration of pollen season for some pollen types. The prevalence of atopics was similar in all critical periods, and the prevalence of sensitization to common pollen allergens was slightly higher in the last study than in the study performed in 1996. We registered increase in the prevalence of allergic rhinitis in the examined period (11.5% in 1996 to 17.4% in 2009/2010) followed by increase in the prevalence of rhinitics sensitized to common pollen allergens. The prevalence of allergic asthma was significantly higher in the study conducted in 2009/2010 than in the study conducted in 1996 (5.1% vs. 2.1%, P = 0.037) followed by slightly higher prevalence of asthmatics sensitized to common pollen allergens in the same period. Introduction: The pathogenesis of nasal polyposis and nasal hyperplasia is still unknown. The localization of caspases in nasal polyps and nasal hyperplasia of patients with and without allergic rhinitis was studied. Methods: Sections of human nasal polyps (n = 5) and hyperplastic nasal turbinates (5 with, 5 without allergy) were stained for active caspase-3 and caspase-8. Double immunofluorescence was used to evaluate colocalization of the caspases with Ki-M1P and tryptase. TUNEL was performed. Results: Active caspase-3 and caspase-8 were seen in nearly all nasal polyps and hyperplastic nasal turbinates. Active caspase-3 was predominantly localized in stromal cells, identified as mast cells. Caspase-8 was localized in mast cells with the pattern similar to active caspase-3 and additionally found in epithelial cells at the nasal and polyp surface and in epithelial cells of glands. Conclusion: Our results suggest that mast cell apoptosis may be involved in the pathological mechanisms which characterize and sustain chronic inflammatory disorders of the nasal mucosa with and without allergy.
