Construction and validation of a neovascular glaucoma nomogram in patients with diabetic retinopathy after pars plana vitrectomy

BACKGROUND Neovascular glaucoma(NVG)is likely to occur after pars plana vitrectomy(PPV)for diabetic retinopathy(DR)in some patients,thus reducing the expected benefit.Understanding the risk factors for NVG occurrence and building effective risk prediction models are currently required for clinical research.AIM To develop a visual risk profile model to explore factors influencing DR after surgery.METHODS We retrospectively selected 151 patients with DR undergoing PPV.The patients were divided into the NVG(NVG occurrence)and No-NVG(No NVG occurrence)groups according to the occurrence of NVG within 6 months after surgery.Independent risk factors for postoperative NVG were screened by logistic regression.A nomogram prediction model was established using R software,and the model’s prediction accuracy was verified internally and externally,involving the receiver operator characteristic curve and correction curve.RESULTS After importing the data into a logistic regression model,we concluded that a posterior capsular defect,preoperative vascular endothelial growth factor≥302.90 pg/mL,glycosylated hemoglobin≥9.05%,aqueous fluid interleukin 6(IL-6)≥53.27 pg/mL,and aqueous fluid IL-10≥9.11 pg/mL were independent risk factors for postoperative NVG in patients with DR(P<0.05).A nomogram model was established based on the aforementioned independent risk factors,and a computer simulation repeated sampling method was used to internally and externally verify the nomogram model.The area under the curve(AUC),sensitivity,and specificity of the model were 0.962[95%confidence interval(95%CI):0.932-0.991],91.5%,and 82.3%,respectively.The AUC,sensitivity,and specificity of the external validation were 0.878(95%CI:0.746-0.982),66.7%,and 95.7%,respectively.CONCLUSION A nomogram constructed based on the risk factors for postoperative NVG in patients with DR has a high prediction accuracy.This study can help formulate relevant preventive and treatment measures.

Paraneoplastic retinopathies: an update on pathogenesis, diagnosis and management

The paraneoplastic retinopathies are a heterogeneous group of disorders with significant visual consequences occuring in the setting of a systemic malignancy.These conditions may be characterized by the presence of antiretinal antibodies and may predate or follow the diagnosis of an underlying malignancy.Herein I review the clinical findings,pathophysiology,laboratory testing and management of the paraneoplastic retinopathies:cancer-associated retinopathy(CAR),melanoma-associated retinopathy(MAR),bilateral diffuse uveal melanocytic proliferation(BDUMP)and paraneoplastic vitelliform maculopathy(PVM).The pathophysiology of the paraneoplastic retinopathies varies from molecular mimicry resulting in anti-retinal antibody production(CAR,MAR)to relases of soluble factors either by the primary tumor(BDUMP)and/or immune system in response to the primary tumor(PVM)which result in retinal and/or retinal pigment epithelium dysfunction.For each condition,structural and functional multimodal retinal testing can be helpful in establishing the diagnosis.Treatment for the paraneoplastic retinopathies involves a combination of treating the underlying malignancy plus additional local and/or systemic immunosuppressive agents though no systemic therapeutic protocols exist.Despite these interventions,the retinopathy may be progressive or the retinopathy may be a harbinger of poor survival.Nevertheless,prompt diagnosis may help identify an underlying malignancy earlier and may thus improve cancer-related survival.

Comparison of anterior segment parameters of the eye between type 2 diabetic with and without diabetic retinopathy and non-diabetic

AIM:To compare anterior segment parameters between two groups of type 2 diabetic with and without diabetic retinopathy(DR)and non-diabetic elderly subjects based on hemoglobin A1c(Hb A1c)levels and status of DR.METHODS:This study was conducted on 997 residents aged 60y or over in Tehran,Iran.Diabetic group had Hb A1c level≥6.4%with no other systemic problems.The non-diabetic participants had normal eye findings and no systemic diseases.K1,K2,mean K,Q-value,anterior,central,posterior,and total corneal densitometric findings,anterior chamber volume(ACV),anterior chamber depth(ACD),corneal volume(CV),and pachymetry were measured by Pentacam AXL.RESULTS:A total of 678 non-diabetic(39%male)and 319 diabetic(35%male)subjects with mean age of 66.31±5.23 and 67.22±4.96y were examined,respectively.No statistically significant difference was found in anterior segment parameters between non-diabetic and diabetic groups(all P>0.05).However,middle,posterior,and total corneal densitometric values were statistically different between two groups af ter controlling the ef fects of confounders(P=0.014,0.007,and 0.042,respectively).Corneal densitometric values in all layers,ACD,and ACV were different between diabetic subjects with and without DR(all P<0.05).In the diabetic group,only corneal densitometric values had a negative relationship with fasting blood sugar(P<0.001).ACD and ACV had a negative correlation with Hb A1c levels(all P<0.05,r=-0.129 and-0.146,respectively).However,the relationships were not observed after controlling the confounders(P=0.938,0.466,respectively).CONCLUSION:Considering the higher densitometric values of the cornea and lower ACD and ACV in diabetic subjects with DR,it is suggested that the examiners should perform comprehensive retinal examinations when faced with such conditions.

Plasma and aqueous humor levels of adiponutrin and pannexin 1 in patients with and without diabetic retinopathy

AIM: To evaluate plasma and aqueous levels of adiponutrin and pannexin 1 in patients with and without diabetic retinopathy.METHODS: The study included three age and gendermatched groups of 20 cataract patients with no diabetes or additional disease(Group C), 20 cataract patients with diabetes and no retinopathy(Group DM+C), and 20 cataract patients with diabetic retinopathy(Group DR+C).All the patients were examined with respect to body mass index(BMI), fasting plasma glucose, hemoglobin A1c(HbA1c), and lipid profile.Phacoemulsification and intraocular lens(Phaco+IOL) implantation were performed to all patients in all the groups, and aqueous samples were taken during the operation.The plasma and aqueous adiponutrin and pannexin 1 levels were analyzed using enzyme-linked immunosorbent assays.RESULTS: A statistically significant difference was determined between the groups with respect to BMI, fasting plasma glucose, and HbA1c levels(P<0.05 for all parameters tested).The plasma adiponutrin levels of Group DR+C were statistically significantly lower than those of Group C and Group DM+C(P<0.001, P=0.004).No statistically significant difference was determined in the aqueous adiponutrin levels in three groups.The plasma pannexin 1 levels of Groups DM+C and DR+C were statistically significantly lower than those of Group C(both P=0.001).The aqueous pannexin 1 levels of Group DR+C were statistically significantly higher than those of Group C and Group DM+C(P=0.001, P<0.001).CONCLUSION: Adiponutrin and pannexin 1, which play an important role in the pathophysiology of diabetes and obesity, and have a regulatory role in hyperglycemia and insulin resistance.The measurement of adiponutrin and pannexin 1 levels may support clinicians in determining the risk of DR development.

Ischemic retinopathy is a retinal disease caused by ischemic attacks.Ischemia is a common pathologic mechanism in various retinal disorders and diseases such as age-related macular degeneration,diabetic retinopathy,glaucoma,or vascular occlusion(Osborne et al.,2004).Although various murine models have been developed to understand a series of metabolic mechanisms induced by retinal ischemia and further used to test promising therapeutics,effective treatment in ischemic retinopathy has not been clearly suggested.This is associated with the notion that the contributing pathologic metabolic pathways might be enormously complex.

Computer-aided diagnosis of retinopathy based on vision transformer

Age-related Macular Degeneration(AMD)and Diabetic Macular Edema(DME)are two com-mon retinal diseases for elder people that may ultimately cause irreversible blindness.Timely and accurate diagnosis is essential for the treatment of these diseases.In recent years,computer-aided diagnosis(CAD)has been deeply investigated and effectively used for rapid and early diagnosis.In this paper,we proposed a method of CAD using vision transformer to analyze optical co-herence tomography(OCT)images and to automatically discriminate AMD,DME,and normal eyes.A classification accuracy of 99.69%was achieved.After the model pruning,the recognition time reached 0.010 s and the classification accuracy did not drop.Compared with the Con-volutional Neural Network(CNN)image classification models(VGG16,Resnet50,Densenet121,and EfficientNet),vision transformer after pruning exhibited better recognition ability.Results show that vision transformer is an improved alternative to diagnose retinal diseases more accurately.

Changes and significance of retinal blood oxygen saturation and oxidative stress indexes in patients with diabetic retinopathy

BACKGROUND Diabetic retinopathy(DR)is a diabetic complication that can severely affect the patients’vision,eventually leading to blindness.DR is the most important manifestation of diabetic micro-vasculopathy and is mainly related to the course of diabetes and the degree of blood glucose control,while the age of diabetes onset,sex,and type of diabetes have little influence on it.AIM To explore the changes in blood oxygen saturation and oxidative stress indices of retinal vessels in patients with DR.METHODS In total,94 patients(94 eyes)with DR(DR group)diagnosed at Jianyang people’s Hospital between March 2019 and June 2020,and 100 volunteers(100 eyes)(control group)without eye diseases,were included in this study.Arterial and venous blood oxygen saturation,retinal arteriovenous vessel diameter,and serum oxidative stress indicators in the two groups were compared.Based on the stage of the disease,the DR group was divided into the simple DR and proliferative DR groups for stratified analysis.RESULTS The oxygen saturation of the retinal vessels in the DR group was significantly higher than that in the control group(P 0.05). Dear Editor,Hydroxychloroquine(HCQ)is a drug widely used in the treatment of autoimmune diseases and oncologic disorders,and was even used during a brief period of time for the therapy of COVID-19 disease[1].HCQ retinopathy has a prevalence ranging from 4.3%to 13.8%.However,for a patient with a normal screening examination the risk of developing HCQ retinopathy is very low[2].The American Academy of Ophthalmology recommended visual field(VF)and optical coherence tomography(OCT)for the screening,whereas the Royal College of Ophthalmologists advised the use of OCT and fundus autofluorescence(FAF)[2-3].

MicroRNAs as biomarkers of diabetic retinopathy and disease progression

Diabetes mellitus, together with its complications, has been increasing in prevalence worldwide. Its complications include cardiovascular disease(e.g., myocardial infarction, stroke), neuropathy, nephropathy, and eye complications(e.g., glaucoma, cataracts, retinopathy, and macular edema). In patients with either type 1 or type 2 diabetes mellitus, diabetic retinopathy is the leading cause of visual impairment or blindness. It is characterized by progressive changes in the retinal microvasculature. The progression from nonproliferative diabetic retinopathy to a more advanced stage of moderate to severe nonproliferative diabetic retinopathy and proliferative diabetic retinopathy occurs very quickly after diagnosis of mild nonproliferative diabetic retinopathy. The etiology of diabetic retinopathy is unclear, and present treatments have limited effectiveness. Currently diabetic retinopathy can only be diagnosed by a trained specialist, which reduces the population that can be examined. A screening biomarker of diabetic retinopathy with high sensitivity and specificity would aid considerably in identifying those individuals in need of clinical assessment and treatment. The majority of the studies reviewed identified specific microRNAs in blood serum/plasma able to distinguish diabetic patients with retinopathy from those without retinopathy and for the progresion of the disease from nonproliferative diabetic retinopathy to proliferative diabetic retinopathy. In addition,certain microRNAs in vitreous humor were dysregulated in proliferative diabetic retinopathy compared to controls. A very high percentage of patients with diabetic retinopathy develop Alzheimer’s disease. Thus, identifying diabetic retinopathy by measurement of suitable biomarkers would also enable better screening and treatment of those individuals at risk of Alzheimer’s disease.

Long-lasting impairments in rodent oxygen-induced retinopathy measured by retinal vessel density and visual function

In mild or moderate retinopathy of prematurity(ROP), retinal vessels undergo obliteration, proliferation, and regression. Despite complete regression of vessel abnormalities, a variety of visual impairments have been reported. Rodent oxygen-induced retinopathy(OIR) is widely used as a model to study ROP. However, the long-term changes of OIR model remain unclear. The aim of this study is to examine long term changes of retinal vessel and visual function in a rodent OIR model resembling human mild or moderate ROP. In this study, after subjecting the animals to 80% oxygen(O_2) for 5–7 d, the retinal vessel density at postnatal day 12(P12) was approximately 30% lower than that in the age-matched control, but this difference was not significant between the groups. Vessel abnormalities, such as vessel tortuosity, neovascular tufts, and the number of vessels protruding into the vitreous, peaked between P17 and P20. Despite the regression of many abnormalities, vessel density in the OIR group was 36% and 32% lower than that in the control animals at 6 weeks and 4 months, respectively. The visual acuity and contrast sensitivity were impaired in the OIR group when measured at 2, 3 and 4 months. Therefore, the rodent OIR model exhibited long-lasting reduction in retinal vessel density and visual impairments, similar to those observed in mild or moderate human ROP. This study suggests that the rodent OIR model can be used to explore possible interventions for mild and moderate human ROP.
