BACKGROUND To retrospectively report the safety and efficacy of renal transcatheter arterial embolization for treating autosomal dominant polycystic kidney disease(ADPKD)patients with gross hematuria.CASE SUMMARY The purpose of this study is to retrospectively report the safety and efficacy of renal transcatheter arterial embolization for treating ADPKD patients with gross hematuria.Materials and methods:During the period from January 2018 to December 2019,renal transcatheter arterial embolization was carried out on 6 patients with polycystic kidneys and gross hematuria.renal arteriography was performed first,and then we determined the location of the hemorrhage and performed embolization under digital subtraction angiography monitoring.Improvements in routine blood test results,routine urine test results,urine color and postoperative reactions were observed and analyzed.Results:renal transcatheter arterial embolization was successfully conducted in 6 patients.The indices of 5 patients and the color of gross hematuria improved after surgery compared with before surgery.No severe complication reactions occurred.CONCLUSION For autosomal dominant polycystic kidney syndrome patients with gross hematuria,transcatheter arterial embolization was safe and effective.

Background:The assessment of renal function is important to the prognosis of patients needing Fontan palliation due to the reconstructed compromised circulation.To know the relationship between the kidney perfusion and hemodynamic characteristics during surgical design could reduce the risk of acute kidney injury(AKI)and the postoperative complications.However,the issue is still unsolved because the current clinical evaluation methods are unable to predict the hemodynamic changes in renal artery(RA).Methods:We reconstructed a three-dimensional(3D)vascular model of a patient requiring Fontan palliation.The technique of computational fluid dynamics(CFD)was utilized to explore the changes of RA hemodynamics under different possible blood flow rates.The relationship between the kidney perfusion and hemodynamic characteristics was investigated.Results:The calculated results indicated the declined tendency of the pressure and pressure drop as the flow rate decreased.When the flow rate decreased to two-thirds of its baseline,both the pressure of left renal artery(LRA)and the pressure of right renal artery(RRA)dipped below 50%,and the pressure of RRA fell more quickly than that of LRA.Uneven distribution of WSS was observed on the trunk of RA,and the lowest WSS was found at the distal of RA.The average WSS in RA dropped to around 50%as the flow rate reached one-third of its baseline.Conclusions:As a promising approach,CFD can be utilized to quantitatively evaluate the hemodynamic characteristics of RA and contribute to offsetting the drawbacks of clinical assessments of renal function,to help realize better prognosis for the patients with Fontan palliation.

AIM: To investigate the effect of cyclin-dependent kinase 5 (Cdk5) inhibitor roscovitine on the morphine antinociceptive tolerance development in rats. METHODS: Tail-flick test as pain threshold measurement and intrathecal injection techniques were used. RESULTS: Intrathecal roscovitine infusion alone produced an antinociceptive effect. Tolerance was induced by continuous intrathecal infusion of morphine 5μg/h for 5 d. Co administration of intrathecal roscovitine 1μg/h for 5 d enhanced the morphine antinociceptive effect in tolerant rats. It also caused a shift in the morphine antinociceptive dose"Cresponse curve to the left when co-administered with morphine during tolerance induction, and caused a 67 % reduction in the increase in the ED50 of morphine (dose producing 50 % of the maximum response). CONCLUSION: Cdk5 modulation is involved in the antinociceptive tolerance of morphine. Intrathecal roscovitine administration could attenuate this tolerance development.

Background Acute kidney injury （AKI） is associated with poor prognosis after cardiopulmonary bypass. The aim of this retrospective study was to investigate whether stent implantation before cardiopulmonary bypass has beneficial effect on development of AKI in renal artery stenosis （RAS） patients.Methods In this retrospective study, patients with abnormal baseline serum creatinine （SCr, 〉106 μmol/L） were not included. Included patients （n=69） were divided into two groups. Group 1 included 31 RAS patients receiving no stent implantation before cardiopulmonary bypass. Group 2 included 38 RAS patients having received stent implantation just before cardiopulmonary bypass. To assess AKI after cardiopulmonary bypass, serum urea nitrogen, SCr and creatinine clearance were recorded at baseline, at the end of operation, during the first and second postoperative 24 hours.Results Baseline characteristics were similar between groups. Serum urea nitrogen, SCr, creatinine clearance before and after cardiopulmonary bypass were also similar class groups. Incidence of AKI in group 1 was not significantly different from group 2. In group 1, AKI defined by RIFLE between occurred in 7 （22.6%） patients： 5 （16.1%） with RIFLE-R,2 （6.5%） with RIFLE-I, and no patients with RIFLE-F. In group 2, 10 patients （26.3%） had an episode of AKI during hospitalization： 6 （15.8%） had RIFLE-R, 4 （10.5%） had RIFLE-I, and no patients had RIFLE-F.Conclusions There are no data suggesting that it is necessary to stent RAS patients with normal SCr before cardiopulmonary bypass. However, it cannot be concluded that RAS is not associated with AKI after cardiopulmonary bypass

BACKGROUND Acute renal artery thrombosis is a relatively rare disease.Early diagnosis and emergent treatment can prevent the loss of renal function and the development of hypertension.CASE SUMMARY We report a patient with acute renal artery thrombosis who presented to our hospital with acute-onset right flank pain and was treated by percutaneous mechanical thrombectomy using the Rotarex device.After 2 mo,right kidney function had recovered slightly.CONCLUSION renal artery thrombosis may lead to loss of renal function and the development of hypertension.Rotarex mechanical thrombectomy may be a viable treatment option for rapid recanalization of the renal artery in patients with renal artery thrombosis.

Accessory renal arteries(ARAs)are common and usually originate from the abdominal aorta and the renal artery.Inferior phrenic arteries(IPAs)can also arise from the abdominal aorta or its branches.In this paper,we present the first case of a common trunk of the right ARA and right IPA arising from the thoracic artery at the level of T10,which was discovered by multidetector-row computed tomography in pretherapeutic evaluation and clearly confirmed by selective angiography.It is important to recognize this anatomical variation when performing cardiovascular and interventional radiological procedures.

Endovascular aortic repair(EVAR)has been increasingly used to treat abdominal aortic aneurysm(AAA)due to its lower mortality and shorter recovery time.However,the complications caused by renal artery coverage remained a major concern.In this study,three types of renal orientation and five types of partial renal coverage degree were adopted to numerically investigate the flow pattern,wall shear stress distribution,and blood perfusion during a cardiac cycle.The results show that as for the mild partial coverage(=renal artery with a downward orientation suffered fewer unfavorable wall shear stress(WSS)distribution and more blood perfusion.It should be noted that the renal artery with horizontal orientation suffered more unfavorable WSS distribution.In terms of severe coverage(≥50%),all of the renal arteries displayed highly disturbed wall shear distribution,resulting in a significant reduction in blood perfusion It was interesting to note that the artery with an upward orientation had a smaller rate of reduction in blood perfusion.It can be concluded that as for the mild renal coverage after EVAR,renal arteries with downward orientation may have the advantage of reducing the possibility of renal occlusion,while renal arteries with horizontal orientation may increase the unfavorable wall shear distribution.On the other hand,as for serious coverage(≥50%),high risk of renal ischemia remains the main concern for all types of renal arteries due to highly disturbed wall shear distribution would significantly increase regardless of renal orientation.

Background:Takayasu arteritis-induced renal arteritis (TARA), commonly seen in Takayasu arteritis (TA), has become one of the main causes of poor prognosis and early mortality in patients with TA. TARA progressing into Takayasu arteritis-induced renal artery stenosis (TARAS), could lead to severe complications including malignant hypertension, cardiac-cerebral vascular disease, and ischemic nephropathy. Since there existed no guidelines on treatments, this study aimed to review the comprehensive treatments for TARA.Methods:We searched systematically in databases including PubMed, Ovid-Medline, EMBASE, Web of Science, China National Knowledge Infrastructure, Wanfang, and SinoMed, from inception to May 2018. Literature selection, data extraction, and statistical analysis were performed.Results:Eighty-two literatures were recruited focusing on medical treatments ( n = 34) and surgical treatments ( n = 48). We found that combined medical treatments of glucocorticoids and conventional synthetic disease-modifying anti-rheumatic drugs could reach high rates of remission in patients with TARA, and biological disease-modifying anti-rheumatic drugs were preferred for refractory patients. After remission induction, surgical treatment could help reconstruct renal artery and recover renal function partly. Percutaneous transluminal angioplasty was the first choice for patients with TARAS, while open surgery showed a good longterm survival. Conclusions:Patients with TARA should benefit both from medical treatments and from surgical treatments comprehensively and sequentially. Multidisciplinary team coordination is recommended especially in patients with severe complications.

Background： The offspring of women with gestational diabetes mellitus （GDM） are prone to macrosomia. However, birth weight is difficult to be correctly estimated by ultrasound because of fetal asymmetric growth characteristics. This study aimed to investigate the correlations between fetal hemodynamics, fetal growth indices in late pregnancy, and birth weight in GDM. Methods： A total of 147 women with GDM and 124 normal controls （NC） were enrolled in this study. Fetal hemodynamic indices, including the systolic/diastolic ratio （S/D）, resistance index （RI）, pulsatility index （PI） of umbilical artery （UA）, middle cerebral artery （MCA）, and renal artery （RA）, were collected. Fetal growth indices, including biparietal diameter （BPD）, head circumference （HC）, abdominal circumference （AC）, and femur length, were also measured by ultrasound. Birth weight, newborn gender, and maternal clinical data were collected. Results： The independent samples t-test showed that BPD, HC, and AC were larger in GDM than in NC （P 〈 0.05）. Fetal hemodynamic indices of the UA and MCA were lower （P 〈 0.05）, but those of the RA were higher （P 〈 0.001 ） in GDM than in NC. Birth weight was higher in GDM than in NC （P 〈 0.001）. Pearson＇s correlation analysis showed that hemodynamic indices of the UA were negatively correlated with birth weight, BPD, HC, and AC in both groups （P 〈 0.05）. MCA （S/D, PI, and RI） was negatively correlated with birth weight, HC, and AC in GDM （i- = -0.164, -0.206, -0.200, -0.226, -0.189, -0.179, -0.196, -0.177, and - 0.172, respectively, P 〈 0.05）, but there were no correlations in NC （P 〉 0.05）. RA （S/D, PI, and RI） was positively correlated with birth weight in GDM （r = 0.168, 0.207, and 0.184, respectively, P 〈 0.05）, but there were no correlations in NC （P 〉 0.05）. Conclusion： Fetal hemodynamic indices in late pregnancy might be helpful for estimating newborn birth weight in women with GDM.
