Effects of self-administered exercises based on Tuina techniques on musculoskeletal disorders of professional orchestra musicians: a randomized controlled trial

BACKGROUND： Musicians are frequently affected by playing-related musculoskeletal disorders （PRMD）. Common solutions used by Western medicine to treat musculoskeletal pain include rehabilitation programs and drugs, but their results are sometimes disappointing. OBJECTIVE： To study the effects of self-administered exercises based on Tuina techniques on the pain intensity caused by PRMD of professional orchestra musicians, using numeric visual scale （NVS）. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： We performed a prospective, controlled single-blinded, randomized study with musicians suffering from PRMD. Participating musicians were randomly distributed into the experimental （n = 39） and the control （n = 30） groups. After an individual diagnostic assessment, specific Tuina self-administered exercises were developed and taught to the participants. Musicians were instructed to repeat the exercises every day for 3 weeks. MAIN OUTCOME MEASURES： Pain intensity was measured by NVS before the intervention and after 1, 3, 5, 10, 15 and 20 d of treatment. The procedure was the same for the control group, however the Tuina exercises were executed in points away from the commonly-used acupuncture points. RESULTS： In the treatment group, but not the control group, pain intensity was significantly reduced on days 1, 3, 5, 10, 15 and 20. CONCLUSION： The results obtained are consistent with the hypothesis that self-administered exercises based on Tuina techniques could help professional musicians controlling the pain caused by PRMD. Although our results are very promising, further studies are needed employing a larger sample size and double blinding designs.

The lipid extract of mussel(MLE) contains a high proportion of n-3 polyunsaturated fatty acids(PUFA), mainly docosahexaenoic acid(DHA, 22:6 n-3) and eicosapentaenoic acid(EPA, 20:5 n-3). Studies have shown that fish oil containing n-3 PUFA can improve the glycemic traits of Chinese patients with type 2 diabetes(T2D). This study investigated whether mussel lipid could improve glycemic traits of T2D patients, and explore the difference between the effects of mussel lipid and fish oil on T2D patients and its possible reasons. Methods In a double-blind randomized controlled trial, 133 recruited Chinese T2D patients were randomized to either fish oil(FO, n=44), MLE(MLE, n=44), or corn oil group(CO, n=45) for 60 days. The patients were asked to take corresponding oil capsules(2 capsules/day), which provided 1.6 g/day of both FO, MLE and CO, each FO capsule provided 38% of EPA + DHA, MLE capsule provided 41% of EPA + DHA. Results For glucose traits, MLE significantly decreased insulin(P = 0.005) and homeostasis model of insulin resistance(HOMA-IR)(P = 0.026) after the 60-day intervention, whereas no significant difference was found in FO or CO(P = 0.431 and P = 0.165, P = 0.61 and P = 0.109, respectively). For lipid profiles, the total cholesterol(TC) was significantly lower in the MLE and the CO groups after the 60-day intervention(P < 0.001 and P < 0.001), but not in the FO group(P = 0.245). The triacylglycerol(TG) was significantly lower in the MLE group after the 60-day intervention(P = 0.007), but not in the FO group or CO group(P = 0.460 and P =0.062). HDL-C, HDL-C/TC and HDL-C/LDL-C significantly decreased and LDL-C significantly increased in the three groups after the 60-day. There was a significant difference in HDL-C among the three groups at the end of the intervention(P = 0.003): compared with the FO group, HDL-C was significantly lower in the MLE group and CO group after intervention. Background: Traumatic brain injury is a major health problem that frequently leads to deficits in executive function. Self-regulation processes, such as goal-setting, may become disordered after traumatic brain injury, particularly when the frontal regions of the brain and their connections are involved. Such impairments reduce injured veterans' ability to return to work or school and to regain satisfactory personal lives. Understanding the neurologically disabling effects of brain injury on executive function is necessary for both the accurate diagnosis of impairment and the individual tailoring of rehabilitation processes to help returning service members recover independent ***/design: The COMPASSgoal(Community Participation through Self-Efficacy Skills Development) program develops and tests a novel patient-centered intervention framework for community reintegration psychosocial research in veterans with mild traumatic brain injury. COMPASSgoal integrates the principles and best practices of goal self-management. Goal setting is a core skill in self-management training by which persons with chronic health conditions learn to improve their status and decrease symptom effects. Over a three-year period, COMPASSgoal will recruit 110 participants with residual executive dysfunction three months or more post-injury. Inclusion criteria combine both clinical diagnosis and standardized scores that are >1 SD from the normative score on the Frontal Systems Rating Scale. Participants are randomized into two groups: goal-management(intervention) and supported discharge(control). The intervention is administered in eight consecutive, weekly sessions. Functional and psychosocial effects of pulmonary Daoyin on patients with COPD in China:study protocol of a multicenter randomized controlled trial

BACKGROUND： Chronic obstructive pulmonary disease （COPD） is a major public health problem worldwide. Pulmonary rehabilitation （PR） is an established intervention for the management of patients with COPD. Exercise training is an important part of PR, and its effectiveness in patients with COPD is well established. However, alternative methods of PR training such as Daoyin have not been appropriately studied. Hence, alternative forms of exercise training that require less exercise equipment and no specific training place should be evaluated. This paper describes the study protocol of a clinical trial that aims to determine if pulmonary Daoyin training will improve the exercise capacity and psychosocial function of patients with COPD in China. METHODS AND DESIGN： A multicenter, randomized, controlled trial will be conducted. A total of 464 patients meeting the inclusion criteria will be enrolled into this study with 232 patients in each of the trial group and the control group. Based on patient education, patients in the trial group will receive pulmonary Daoyin and continue with their usual therapy for three months. In the control group, patients will continue with their usual therapy. The primary outcome measures are exercise capacity assessed by the six-minute walking distance test and lung function. Secondary outcomes include dyspnea and quality of life. Measurements will be taken at baseline （month 0） and after the study period （month 3）. DISCUSSION： It is hypothesized that pulmonary Daoyin will have beneficial effects in improving exercise capacity and psychosocial function of patients with stable COPD, and will provide an alternative form of exercise training that is accessible for the large number of people with COPD. trial REGISTRATION： This trial has been registered in ***. Objective: The objective of this randomized controlled trial study was to evaluate the education training in relation to lifestyle improvement in patients with type 2 diabetes through its influence in the levels of glycated hemoglobin(HbA1 c), blood pressure, triglyceride, cholesterol levels, and body mass index(BMI).Methods: The study included patients with type 2 diabetes randomly selected from 20 residential areas in Tirana, Albania where family physicians provide services. The sample size in total was 200 patients in both groups(control and intervention). The education training(four sessions) was conducted by trained nursing staff for 6 months. Patients were screened for the biochemical profile before and after the intervention. To compare the groups with respect to the interest outcomes, the t-test was used. Objective： To evaluate the efficacy and safety of a combination therapy using Chinese medicine （CM） Shenzhu Guanxin Recipe （参术冠心方, SGR） and standard Western medicine treatment （SWMT） in patients with angina pectoris after percutaneous coronary intervention （PCI）. Methods： Double-blind randomized controlled trial was used in this experimental procedure. One hundred and eighty-seven patients with coronary heart disease receiving SWMT after PCI were randomly assigned to the treatment （SGR） and control （placebo） groups. Outcome measures including angina pectoris score （APS）, CM symptom score, and Seattle Angina Questionnaire （SAQ） score were evaluated in 1, 2, 3 and 12 months, and the death rate, restenosis and other emergency treatments were observed. The mixed-effects models were employed for the data analysis. Results： In the treatment group, a larger within-treatment effect size （d=1.74） was found, with a 76.7% reduction in APS from pretreatment to 12-month follow-up assessment compared with the control group （d=0.83, 53.8% symptom reduction）; between- treatment （BT） effect size was d=0.66. CM symptom scores included an 18.3% reduction in the treatment group （d=0.46）, and a 16.1% decrease in the control group （d=0.31）; d=0.62 for BT effect size. In regard to scores of SAQ, the BT effect size of cognition level of disease was larger in the treatment group （d=0.63）, followed by the level of body limitation of activity （d=0.62）, condition of angina pectoris attacks （d=0.55）, satisfaction level of treatments （d=0.31）, and steady state of angina pectoris （d=0.30）. Two cardiovascular related deaths and one incidental death were recorded in the control and treatment groups, respectively. No significant difference in any cardiovascular event （including death toll, frequency of cardiovascular hospitalization or emergency room visits） was found between the two groups.
