Effect of lipid extract from mussel on glycemic traits in Chinese type 2 diabetic patients: a randomized controlled trial

摘      要:The lipid extract of mussel(MLE) contains a high proportion of n-3 polyunsaturated fatty acids(PUFA), mainly docosahexaenoic acid(DHA, 22:6 n-3) and eicosapentaenoic acid(EPA, 20:5 n-3). Studies have shown that fish oil containing n-3 PUFA can improve the glycemic traits of Chinese patients with type 2 diabetes(T2D). This study investigated whether mussel lipid could improve glycemic traits of T2D patients, and explore the difference between the effects of mussel lipid and fish oil on T2D patients and its possible reasons. Methods In a double-blind randomized controlled trial, 133 recruited Chinese T2D patients were randomized to either fish oil(FO, n=44), MLE(MLE, n=44), or corn oil group(CO, n=45) for 60 days. The patients were asked to take corresponding oil capsules(2 capsules/day), which provided 1.6 g/day of both FO, MLE and CO, each FO capsule provided 38% of EPA + DHA, MLE capsule provided 41% of EPA + DHA. Results For glucose traits, MLE significantly decreased insulin(P = 0.005) and homeostasis model of insulin resistance(HOMA-IR)(P = 0.026) after the 60-day intervention, whereas no significant difference was found in FO or CO(P = 0.431 and P = 0.165, P = 0.61 and P = 0.109, respectively). For lipid profiles, the total cholesterol(TC) was significantly lower in the MLE and the CO groups after the 60-day intervention(P 0.001 and P 0.001), but not in the FO group(P = 0.245). The triacylglycerol(TG) was significantly lower in the MLE group after the 60-day intervention(P = 0.007), but not in the FO group or CO group(P = 0.460 and P =0.062). HDL-C, HDL-C/TC and HDL-C/LDL-C significantly decreased and LDL-C significantly increased in the three groups after the 60-day. There was a significant difference in HDL-C among the three groups at the end of the intervention(P = 0.003): compared with the FO group, HDL-C was significantly lower in the MLE group and CO group after intervention.
