Alcohol,total sugar,total acid,and total phenol contents are the main indicators of wine quality *** study aims to establish simultaneous analysis models for the four indicators through near-infrared(NIR)spectroscopy with wavelength optimization.A Norris derivative filter(NDF)platform with multiparameter optimization was established for spectral *** optimal parameters(i.e.,derivative order,number of smoothing points,and number of differential gaps)were(2,9,3)for alcohol,(1,19,5)for total sugar,(1,17,11)for total acid,and(1,1,1)for total *** equidistant combinationpartial least squares(EC-PLS)was used for large-scale wavelength *** wavelength step-by-step phaseout PLS(WSP-PLS)and exhaustive methods were used for secondary *** final optimization models for the four indicators included 7,10,15,and 13 wavelengths located in the overtone or combination regions,*** an independent validation,the root mean square errors,correlation coefficient for prediction(i.e.,SEP and RP),and ratio of performance-to-deviation(RPD)were 0.41 v/v,0.947,and 3.2 for alcohol;1.48 g/L,0.992,and 6.8 for total sugar;0.68 g/L,0.981,and5.1 for total acid;and 0.181 g/L,0.948,and 2.9 for total *** results indicate high correlation,low error,and good overall prediction ***,the established reagent-free NIR analytical models are important in the rapid and real-time quality detection of the wine fermentation process and finished *** proposed wavelength models provide a valuable reference for designing small dedicated instruments.

Background:The quality of laboratory tests has a great impact on diagnosis and patient *** lines of evidence show that pre-analytical errors are the major source of mistakes in the total test process(TTP).The pre-analytical phase is the most vulnerable and complex step that involves many individuals,including clinicians,nurses,workers and laboratory staff,and it is difficult to ensure a sample’s quality in the *** recent years,many quality indicators have been recommended by the International Federation of Clinical Chemistry and Laboratory Medicine(IFCC) to improve the quality of the pre-analytical *** study aimed to assess the value of the monitoring and analysis of quality indicators(QIs) in the pre-analytical phase Methods:This study was conducted in the diagnostic laboratory of the Second Affiliated Hospital of the Zhejiang University School of Medicine Beginning in January 2013,the laboratory groomed the workflow of samples with improved softwarefor real-time monitoring,analyzed some QIs monthly(i.e,sample loss rate,sample transportation overtime rate and sample unqualified rate),and communicated with relevant individuals or departments in a timely ***:1) In 2013,when the sample workflow was subjected to real-time monitoring and the QIs were assessed monthly,compared with the same time in 2012,the sample loss rate,transportation overtime rate and rejected rate fell from 0.053‰,65.0‰ and 7.83‰ to 0.003‰,15.2‰ and3.84‰,respectively;these differences were statistically significant(P<0.01).2) A total of 1,939,042 blood samples were collected for clinical biochemistry,hematology,hemostasis and immunology analyses during January 2013 and December *** these samples,7,472(3.85‰)were rejected,and the three main reasons were clotted blood samples(26.57%,1,985/7472),hemolyzed samples(26.35%,1,969/7472) and samples collected with an insufficient volume(26.16%,1,955/7472).Monthly analysis about these QIs can provide some useful inf

On-table hepatopancreatobiliary surgical consults for difficult cholecystectomies:A 7-year audit

Background: Cholecystectomy is considered a general surgical operation. However, general surgeons are not trained to manage severe complications such as bile duct injury(BDI) and should refer to hepatopancreatobiliary(HPB) surgeons when difficulty arises. This study aimed to investigate the outcomes of patients who had on-table HPB consults during cholecystectomy. Methods: This is an audit of 50 patients who required on-table HPB consult during cholecystectomy from 2011 to 2017. Consultations were classified as “proactive” and “reactive”, where consults were made before or after surgical incision, respectively. Patient demographics and perioperative details were collected. Results: The median age of the patients was 62.5 years [interquartile range(IQR) 50.8–71.3 years]. Eight(16%) patients had underlying HPB co-morbidity. Gallbladder wall was thickened in all patients(median 5 mm, IQR 4–7 mm), and common bile duct was of normal caliber in all patients(median 5 mm, IQR 4–6 mm). Median length of operation and length of stay were 165 min(IQR 124–209 min) and five days(IQR 3–7 days), respectively. Subtotal cholecystectomy was performed in 18(36%) patients. Forty-eight patients were initially managed by laparoscopic approach, 15(31%) required open conversion;majority(9/15, 60%) were initiated before on-table consult. Majority of referrals(98%) were reactive. Common reasons for referral included unclear anatomy or anatomical variations(30%), presence of dense adhesions and/or contracted gallbladder(18%) and impacted stones in Hartmann’s pouch(16%). Three(6%) patients were referred for BDI(2 Strasberg D and 1 Strasberg E1), and two(4%) were referred for torrential bleeding from arterial injury(1 cystic artery and 1 right hepatic artery). Any morbidity and 30-day readmission were 22% and 6%, respectively. There was no 90-day mortality. Conclusions: Calling for help in BDI is obligatory, but in other instances is a personal choice. Objective: To establish a high-performance liquid chromatographic method(HPLC) for the simultaneous determination of 16 compounds from Artemisia ***: HPLC was used to analyze 16 quality indicators of A. ordosica. The HPLC conditions were as follows: Agilent Eclipse Plus C18column(250 mm × 4.6 mm, 5 μm) with acetonitrile(A)-water(B) as mobile phase, gradient elution: 0–10 min, 75%-65% B;10–30 min, 65%-35% B;and finally 30–40 min, 35%-15%B. The flow rate was 1.0 mL/min, the column temperature was 40 °C, the injection volume was 10 μL, and monitored by absorbance at 285 nm for compounds 1–10, 12 and 225 nm for compounds 11, 13–***: Under the selected experimental chromatographic conditions, compounds 1–16 showed good linearity(r > 0.9993) in a wide concentration range. Their average recoveries were 99.50%, 95.38%,97.75%, 96.00%, 98.20%, 97.50%, 95.50%, 99.33%, 96.75%, 96.50%, 98.50%, 97.83%, 99.20%, 95.33%, 97.33%and 96.30%, respectively, and the RSD were 1.99%, 1.81%, 1.63%, 1.98%, 1.67%, 1.92%, 1.74%, 1.67%,1.90%, 1.72%, 1.88%, 1.83%, 1.79%, 1.76%, 1.81% and 1.96%, ***: Based on the results of the HPLC analysis, it was concluded that p-hydroxycinnamic acid(1),O-hydroxycinnamic acid(2), coniferyl alcohol(5), 5,4’-dihydroxy-7,3’-dimethoxyflavanone(8), 5,4’-dihy droxy-7-methoxyflavanone(9), 5-hydroxy-7,4’-dimethoxyflavanone(12), dehydrofalcarindiol(13), arteordoyn A(14), dehydrofalcarinol(15) and capillarin(16) are best suited for the role of quality indicators of A. ordosica grown in different ecological environments.

quality of Maternal, Neonatal and Child (MNCH) care is an important aspect in ensuring healthy outcomes and survival of mothers and children. To maintain quality in health services provided, organizations and other stakeholders in maternal and child health recommend regular quality measurement. quality indicators are the key components in the quality measurement process. However, the literature shows neither an indicator selection process nor a set of quality indicators for quality measurement that is universally accepted. The lack of a universally accepted quality indicator selection process and set of quality indicators results in the establishment of a variety of quality indicator selection processes and several sets of quality indicators whenever the need for quality measurement arises. This adds extra processes that render quality measurement process. This study, therefore, aims to establish a set of quality indicators from a broad set of quality indicators recommended by the World Health Organization (WHO). The study deployed a machine learning technique, specifically a random forest classifier to select important indicators for quality measurement. Twenty-nine indicators were identified as important features and among those, eight indicators namely maternal mortality ratio, still-birth rate, delivery at a health facility, deliveries assisted by skilled attendants, proportional breach delivery, normal delivery rate, born before arrival rate and antenatal care visit coverage were identified to be the most important indicators for quality measurement.
