Background:prospective analyses have yet to identify a consistent relationship between sleep duration and the incidence of gastrointestinal(GI)*** effect of changes in sleep duration on GI cancer incidence has scarcely been ***,we aimed to examine the association between baseline sleep duration and annual changes in sleep duration and GI cancer risk in a large population-based cohort ***:A total of 123,495 participants with baseline information and 83,511 participants with annual changes in sleep duration information were prospectively observed from 2006 to 2015 for cancer *** proportional-hazards models were used to calculate hazard ratios(HRs)and their confidence intervals(CIs)for GI cancers according to sleep duration and annual changes in sleep ***:In baseline sleep duration analyses,short sleep duration(≤5 h)was significantly associated with a lower risk of GI cancer in females(HR:0.31,95%CI:0.10-0.90),and a linear relationship between baseline sleep duration and GI cancer was observed(P=0.010),especially in males and in the>50-year-old *** the annual changes in sleep duration analyses,with stable category(0 to-15 min/year)as the control group,decreased sleep duration(≤-15 min/year)was significantly associated with the development of GI cancer(HR:1.29;95%CI:1.04-1.61),especially in the>50-year-old group(HR:1.32;95%CI:1.01-1.71),and increased sleep duration(>0 min/year)was significantly associated with GI cancer in females(HR:2.89;95%CI:1.14-7.30).Conclusions:Both sleep duration and annual changes in sleep duration were associated with the incidence of GI cancer.

Background:The serum vitamin D level varies widely by population,and studies have linked vitamin D levels with the risk of type 2 diabetes mellitus(T2DM).However,the relationship is inconsistent and the impact of vitamin D on T2DM among East Chinese adults is *** study aimed to investigate the association between serum 25-hydroxyvitamin D(25[OH]D)levels and the risk of T2DM and evaluated whether the association is modified by genetic ***:In the Survey on Prevalence in East China for Metabolic Diseases and Risk Factors(SPECT-China)cohort,1862 participants free of T2DM at baseline were included.A weighted genetic risk score was calculated with 28 variants associated with *** logistic models were used to examine the association of serum 25(OH)D and genetic risk with ***:After a 5-year follow-up,132 cases of T2DM were *** observed no significant association between quartiles of serum 25(OH)D and T2DM risk after multivariable adjustment(χ^(2)=0.571,P_(for trend)=0.426).Compared to those in the lowest quartile of 25(OH)D,the odds ratios(ORs)(95%confidence interval[CI])for participants with increased quartiles were 1.29(0.74-2.25),1.35(0.77-2.36),and 1.27(0.72-2.24),*** observed a positive association of glycated hemoglobin(HbA1c)with 25(OH)D at baseline(β=1.752,P=0.001)and after follow-up(β=1.385,P=0.003),and a negative association of ln conversion homeostasis model assessment(HOMA)-βwith 25(OH)D at baseline(β=-0.982,P=0.021).There was no significant interaction between 25(OH)D and diabetes genetic predisposition on the risk of T2DM(χ^(2)=2.710,P_(for interaction)=0.100).The lowest OR(95%CI)of T2DM was among participants with low genetic risk and the highest quartile of 25(OH)D(0.17[0.05-0.62]).Conclusion:Serum 25(OH)D may be irrelevant to the risk of incident T2DM among East Chinese adults regardless of genetic predisposition.

Objective:To study the transmission dynamics of coronavirus disease 2019(COVID-19)among 101 confirmed cases and their 387 household contacts and to determine risk factors associated with secondary attack among the household ***:A prospective cohort study was conducted from January 1st 2021 to February 28th 2021,among 101 SARS-CoV-2 cases and 387 household contacts who were followed up for 14 days from the last day of contact with the index case of *** dynamics of disease transmission was estimated,and factors affecting transmission risk were ***,the association between various factors and household secondary attack rate was ***:The median incubation period was found to be 5 days,and the observed reproductive number(R)was found to be 1.63(95%CI:1.28-1.98).The mean household secondary attack rate was 40.7%.Contacts with comorbidities like diabetes mellitus,hypertension,dyslipidemia,and hypothyroidism had significantly higher attack rates(P<0.05).Conclusions:As new variants of SARS-CoV-2 emerges,it is crucial to know the trasmission *** study shows a high secondary attack rate of COVID-19 among household contacts that must be closely monitored.

Objective:Epidemiological studies indicate that alcohol increases gastric cancer(GC)risk,yet most studies have focused on heavy alcohol intake,leaving other factors understudied.A comprehensive investigation of the effects of the frequency and amount of alcohol intake may help elucidate the GC risk associated with drinking ***:The Health Examinees-Gem(HEXA-G)study,a community-based large-scale prospective cohort study,enrolled Korean adults 40–69 years of age between the years 2004 and *** GC cases were identified through linkage to Korea Central Cancer Registry data until December 31,***-reported questionnaires were used to survey alcohol consumption-related factors(duration,frequency,amount,and type of alcoholic beverages).The frequency and amount of alcohol consumption were combined to explore GC risk according to 4 drinking patterns:“infrequent-light”,“frequent-light”,“infrequent-heavy”,and“frequent-heavy”.We used Cox proportional hazard models to estimate the adjusted hazard ratios(HRs)and 95%confidence intervals(CIs),and investigate the relationship between alcohol intake and GC ***:A total of 128,218 participants were included in the *** an average follow-up period of 8.6 years,462 men and 385 women were diagnosed with *** men,current drinkers showed a 31%greater risk of GC than non-drinkers(HR 1.31,95%CI 1.03–1.66),whereas no significant association was observed in *** men,GC risk was associated with a higher frequency(P trend 0.02)and dose of ethanol intake in grams(P trend 0.03).In men,the“frequent-light”(≥5 times/week and<40 g ethanol/day)drinking pattern was associated with a 46%greater risk of GC(HR 1.46,95%CI 1.02–2.07)than the“infrequent-light”pattern(<5 times/week and<40 g ethanol/day).Conclusions:This study suggests that frequent intake of alcohol,even in low quantities per session,increases GC *** research is warranted to evaluate the relationship between alcohol and GC in detai

Background： Despite the rapid growth in the incidence of acute myocardial infarction （AMI） in China, there is limited information about patients＇ experiences after AMI hospitalization, especially on long-tern1 adverse events and patient-reported outcomes （PROs）. Methods： The China Patient-centered Evaluative Assessment of Cardiac Events （PEACE）-prospective AMI Study will enroll 4000 consecutive AM I patients from 53 diverse hospitals across China and follow them longitudinally for 12 months to docunlent their treatment, recovery, and outcomes. Details of patients＇ medical history, treatment, and in-hospital outcomes are abstracted from medical charts. Comprehensive baseline interviews are being conducted to characterize patient demographics, risk factors, presentation, and healthcare utilization. As part of these interviews, validated instruments are administered to measure PROs, including quality of life, symptoms, mood, cognition, and sexual activity. Follow-up interviews, measuring PROs, medication adherence, risk factor control, and collecting hospitalization events are conducted at 1, 6, and 12 months after discharge. Supporting documents for potential outcomes are collected for adjudication by clinicians at the National Coordinating Center. Blood and urine samples are also obtained at baseline, 1 - and 12-month follow-up. In addition, we are conducting a survey of participating hospitals to characterize their organizational characteristics. Conclusion： The China PEACE-prospective AMI study will be uniquely positioned to generate new information regarding patient＇s experiences and outcomes alter AMI in China and seiwe as a foundation for quality improveinent activities.

Background： lmmunoglobulin G4-related disease （IgG4-RD） is a newly recognized systemic disease that can involve multiple organs and various clinical phenotypes. The purpose of this study was to analyze different types of organ involvement in IgG4-RD patients in China. Methods： We conducted a prospective cohort study on IgG4-RD patients to analyze the clinical manifestations and rare features oflgG4-RD. Patients were grouped into different types according to organ involvement regarding organ number and organ site. The constituent ratio in different types was also analyzed. Results： A total of 200 IgG4-RD patients, with a male：female ratio of 2.08：1, were grouped into different types, Cases having involvement of two or three organs were the most common whereas the fewest number of patients had multi-organ （≥4） involvement. Serum IgG4 and lgE levels, IgG4/IgG ratio, and percentage of eosinophils increased as the number of involved organs increased. In addition, constituent ratio analysis revealed that patients with salivary gland/lacrimal gland swelling, who also constituted the largest number of IgG4-RD patients, had higher serum IgG4 concentrations and IgG4/IgG values, had higher percentage of Eos, and were more likely to have had a history of allergies relative to patients with internal organ involvement. Conclusions： The characteristic feature of IgG4-RD is multiple organ involvement with various clinical manifestations and different types. Although serum IgG4 levels increased with the number of involved organs, serum IgG4 levels were higher for those patients with salivary gland/lacrimal gland swelling compared with those with internal organ involvement. Thus, valuable clues to the differential diagnosis of IgG4-RD could be obtained by examining the clinical patterns of organ involvement,
