Introduction:Machine learning (ML)‐based facial nerve injury (FNI) forecasting grounded on multicentric data has not been released up to *** distinct ML models,random forest (RF),K‐nearest neighbor,and artificial neural network (ANN),for the prediction of FNI were evaluated in this ***:A retrospective,longitudinal,multicentric study was performed,including patients who went through parotid gland surgery for benign tumors at three different university ***:Seven hundred and thirty‐six patients were *** most compelling aspects related to risk escalation of FNI were as follows:(1) location,in the mid‐portion of the gland,near to or above the main trunk of the facial nerve and at the top part,over the frontal or the orbital branch of the facial nerve;(2) tumor volume in the anteroposterior axis;(3) the necessity to simultaneously dissect more than one level;and (4) the requirement of an extended resection compared to a lesser extended *** contrast,in accordance with the ML analysis,the size of the tumor (>3 cm),as well as gender and age did not result in a determining favor in relation to the risk of ***:The findings of this research conclude that ML models such as RF and ANN may serve evidence‐based predictions from multicentric data regarding the risk of ***:Along with the advent of ML technology,an improvement of the information regarding the potential risks of FNI associated with patients before each procedure may be achieved with the implementation of clinical,radiological,histological,and/or cytological data.

Predicting parotid Dose Changes in Head-and-Neck Radiotherapy Using Machine Learning: Leveraging Anatomical Variations

Purpose: This study describes a machine-learning approach utilizing patients' anatomical changes to predict parotid mean dose changes in fractionated radiotherapy for head-and-neck cancer, thereby facilitating plan adaptation decisions. Methods: parotid mean dose changes during treatment sessions are assumed to correlate with patients’ anatomical changes, quantified by 65 geometrical features in four sets. SET1 is the parotid volumetric changes;SET2 is the distance changes from the parotid to the PTV;SET3 is the length of beam path changes between the parotid and skin near the neck;SET4 is the distance changes from the parotid to the two bony landmarks—the dens of the C2 and tip of the basilar part of the occipital bone. The introduced landmarks in SET4 are used as surrogates for the PTV in SET2 due to PTV’s unavailability at the simulation stage. Signed Euclidean distance is applied to quantify the distance and beam path length. A decision tree classifier to predict an x% increase in parotid mean dose is developed. In a study involving 18 patients (36 parotids) previously treated with adaptive radiotherapy, a leave-one-out cross-validation combined with enumerating 4 combinations of the 65 geometrical features is used to find a feature subset maximizing classifier’s accuracy. The classifier’s accuracy, with and without SET2’s PTV features inclusion, is evaluated to determine the SET4’s bony landmark surrogate feasibility. Results: Under x = 5% (or x = 10%) parotid mean dose increase: without SET2’s PTV features inclusion, one beam path feature from SET3 and one bony landmark feature from SET4 yield maximal accuracy of 86.1%, which is a 30.5% (19.4%) improvement over prevalence = 55.6% (66.7%);TPR = 87.5% (75%), TNR = 85% (91.7%), PPV = 82.3% (81.8%) and NPV = 89.5% (88%). Adenoid Cystic Carcinoma of parotid Salivary Gland—A Case Study

Adenoid Cystic Carcinoma (ACC) is an infrequent slow growing epithelial tumour constituting for around less than 1% of all the oral and maxillo-facial malignancies and almost 10% of all the salivary gland tumors. parotid gland is the second most common site to be involved in the head and neck region along with submandibular gland, Palate being the most common site involved in the oral cavity. Key feature of these tumors include its asymptomatic presentation, indolent nature, typically showing infiltrative growth and peri-neural invasion. Herein, we report a case of adenoid cystic carcinoma of right parotid gland of a 33-year-old male who presented with complaint of painless slow enlargement of left parotid gland and facial muscle weakness. On Examination firm mass in the region of the left parotid gland as well as left facial paralysis was seen. Biopsy results and further management is discussed here within.

Different histological subtypes of parotid gland tumors: CT findings and diagnostic strategy

AIM:To present computed tomography(CT) findings of different histological subtypes of parotid gland masses in detail and to establish diagnostic ***:From January 2009 to November 2011,56 patients were collected through the histopathology and Picture Archiving and Communication Systems records,which revealed 5 basal cell adenoma(BCA),16 pleomorphic adenoma(PA),25 Warthin's tumor(War-T),3 Kimura's disease(KD) and 7 parotid carcinoma(PCa) *** the CT images were retrospectively analyzed by two radiologists in consensus,based on their description of morphology(location,number,size,margin and fibrous capsule) and enhancement patterns of *** addition,the diagnostic efficiency of diagnostic strategy is ***:War-T and BCA patients' mean age was 59.9 ± 12.6 years and 58.4 ± 18.2 years;the significant difference was seen in War-T vs PA and BCA vs *** 40% of War-Ts presented with bilateral multifocal lesions,a higher ratio than *** two percent of War-Ts were limited to the superficial lobe,followed by BCA 60% and PA 40%.Vessel facing sign and enlarged lymph nodes were both frequent in War-T,which respectively accounts for 84% and 76% of *** contrast enhancement and decreases were unique for *** and PA showed obvious delayed *** diagnostic strategy of parotid gland tumor had a good diagnostic efficiency,with high accuracy,sensitivity and ***:Determination of the histological subtypes of parotid gland masses might be possible based on CT findings and clinical data.A diagnostic strategy with high diagnostic efficiency was established.

Sclerotherapy for the treatment of Warthin’s tumour of the parotid

Warthin’s tumour (WT) is the second most common benign salivary gland tumour of the parotid gland. Several treatments have been proposed. Surgery is considered the treatment of choice. We report a case of an 83-year-old woman who was treated using sclerotherapy and discuss the possible benefits of this method. To our knowledge this is the first report on sclerotherapic treatment of WT.

A Rare Case of Synchronous Bilateral Pleomorphic Adenoma of the parotid Gland

Multiple parotid gland tumors are unusual. Warthin’s tumor is the most represented neoplasm in this condition. We present a case of bilateral synchronous pleomorphic adenoma affecting the parotid glands in a 28-year-old woman. Clinically, the lesions were similar to solitary pleomorphic adenoma. Both lesions were diagnosed by fine-needle aspiration cytology. A total parotidectomy for left tumor and surgical enucleation for the right tumor, respectively, were performed with no facial nerves injury. A review of the pertinent literature is included.

Malignant Myoepithelioma of the parotid Gland: Literature Review on the Occasion of Our Report of a Case

Malignant myoepitheliomas are rare malignant salivary gland neoplasms which usually occur in the parotid gland. Very little material on this topic has been published. This tumour represents a therapeutic challenge since there are no specific recommendations or guidelines. We present a case of malignant myoepithelioma arising de novo in the parotid gland. We discuss our therapeutic approach and considerations on appropriate post-surgical treatment. Then we provide a review of international literature on this interesting and rare oncological entity.

<sup>31</sup>P MR Spectroscopy with 3D Chemical-Shift Imaging Detects Changes in Levels of Phosphorus Metabolites Due to Saliva Secretion in Human parotid Glands

.Objectives: To measure phosphorus metabolites in human parotid glands by 31P-MRS using three-dimensional chemical-shift imaging (3D-CSI), and ascertain whether this method can capture changes in adenosine triphosphate (ATP) and phosphocreatine (PCr) levels due to saliva secretion. Study Design: The parotid glands of 20 volunteers were assessed by 31P-MRS using 3D-CSI on 3T MRI. After obtaining a first (baseline) measurement, the participants took vitamin-C tablets and measurements were obtained twice more, in a continuous manner. The peak area ratios of PCr and β-ATP were evaluated. Results: A high proportion of PCr (0.26 ± 0.08) and ATP (α: 0.16 ± 0.06;β: 0.27 ± 0.06;γ: 0.21 ± 0.03) was noted at baseline. A significant decrease in β-ATP was observed between baseline (“pre”;0.27 ± 0.06) and the first scan after vitamin-C stimulation (“post-1”;0.19 ± 0.07, p parotid glands and capture changes in their levels due to saliva secretion. This technique is simple, non-invasive, and provides new information regarding saliva secretion.

A Case of Pleomorphic Adenoma of the parotid Gland with Multiple Local Recurrences through Facial to Cervical Regions

Pleomorphic adenoma is the most common benign parotid gland tumor. Although its local recurrence rate is known to be high, the recurrence extending to the cervical region is rare. Here we report a case of a young female (25 years old) with pleomorphic adenoma of the parotid gland which showed multiple recurrences through facial to cervical regions over a span of eight years. We also discuss how this benign tumor with a high recurrence rate has been treated in other cases, and how it should be treated.
