Effects of a Plantago ovata-based herbal compound in prevention and treatment of oral mucositis in patients with breast cancer receiving chemotherapy: A double-blind, randomized, controlled crossover trial

Background: Oral mucositis(OM) is one of the most common complications of mucotoxic cancer therapy.Mucositis induces clinically significant pain, increases the risk of infections and affects the patients’ quality of life.Objective: This study investigated the effects of an herbal preparation from Plantago ovata hydrocolloid in the prevention and treatment of OM, in breast cancer patients undergoing chemotherapy with a regimen including adriamycin.Design, setting, participants, and interventions: This research was a double-blind, randomized, controlled crossover trial. The herbal compound consisted of a mixture of 500 mg of P. ovate husk in 30 m L water plus three drops of vinegar per dose, which was used as a mouthwash. Phytochemical and physicochemical tests of the compound were also performed. Twenty-eight patients who developed mucositis during the chemotherapy screening cycle were randomized to the herbal compound(n = 14) and placebo(n = 14) groups. They received herbal compound or placebo three times per day during their next chemotherapy cycle(cycle 1 of treatment). Patients were crossed over during cycle 2 of treatment and received the alternative therapy. An oral care protocol was prescribed to all patients in cycles 1 and 2 of the treatment.Main outcome measures: The patients were visited at baseline, the end of the first and second weeks of the screening cycle, and the end of the first and second weeks of each of two treatment cycles. The degree of mucositis was used as the main treatment outcome. Other indexes, such as the severity of pain, xerostomia grade and the quality of life were also measured.Results: Compared with the placebo, the herbal compound significantly reduced the degree of mucositis,the severity of pain and the xerostomia grade;it also improved the patients’ quality of life(P < 0.05).Comparison between the screening cycle and placebo treatment group showed that the oral care protocol had a significant effect in the reduction of OM(P < 0.05).Conclusion: The oral care protocol and the he

番茄冬春季设施生产中为提高坐果率而采用的2,4-D蘸花措施会导致畸形果率增加,其转录层面的机理尚不明确。本研究中对以醋栗番茄LA1589(Solanum pimpinellifolium accession LA1589)为背景的果形近等基因系WT、sun和ovate花蕾喷施2,4-D,而后对花蕾转录组与成熟果实果形进行分析。结果发现,3个近等基因系在2,4-D处理3周后所形成的子房及其发育而来的成熟果实的果梗端均伸长,并最终导致子房与果实的形状指数增大,而在此过程中两个果形位点与2,4-D间存在互作。转录组分析发现,2,4-D处理可能通过激活生长素信号通路,促进生长素极性运输与细胞分裂素合成以及增加ATHB和bHLH转录因子的积累来改变果形;sun与ovate位点通过与2,4-D互作进一步增强了生长素积累、信号转导与极性运输,扩大了受影响的转录因子范围,最终强化了2,4-D对果形的影响;同时该互作还在sun与ovate近等基因系中分别上调了BRX与CKX5基因的表达来进一步增强上述过程;最后,ovate与2,4-D的互作导致两个SWEET基因的表达下调并可能影响糖类向子房中的运输。

Pharmacognostic standardisation of Hilleria latifolia(Lam.) H.Walt.(Phytolaccaceae)

Objective:To establish the pharmacognostic characters for the correct identification and quality control of Hilleria latifolia(H.latifolia),an important herb in Ghanaian folklore medicine,for the treatment of infections,pain and inflammation.Methods:The macro-morphological,qualitative and quantitative microscopic features,physicochemical and phytochemical features of the medicinally used parts of H.latifolia were evaluated using standard methods.Results:The plant has simple,alternate leaves with entire margin.The lamina is ovate to broadly lanceolate with an acuminate apex.It is hypostomatic with anomocytie stomata.The plant contains abundant prismatic crystals in all parts.Starch grains abound in the roots.The quantitative indices of the leaf and physicochemical parameters have also been established.Conclusions:The pharmacognostic features established in this study may be used as part of the pharmacopoeia! standard for the correct identification and quality control of H.latifolia.

Open Surgery for Retrieval of Patent Foramen Ovale Closure Device Embolized into Right Pulmonary Artery

Percutaneous Patent Foramen ovate PFO/Atrial Septal Defect (ASD) closure has become an increasingly simplified procedure over the past decade. The main advantages of a percutaneous approach include avoidance of surgery, short procedure time and hospital stay. Device embolization is seen rarely but it can be fatal. We report this complication following a percutaneous PFO closure in a 44-year-old man. The device was embolized into the distal part of the right pulmonary artery. We removed the device surgically and closed the PFO/ASD.
