Electrospinning/3D printing-integrated porous scaffold guides oral tissue regeneration in beagles

The combined use of guided tissue/bone regeneration(GTR/GBR)membranes and bone filling grafts represents a classical therapy for guiding the regeneration and functional reconstruction of oral soft and hard ***,due to its displacement and poor mechanical support,bone meal is not suitable for implantation in the case of insufficient cortical bone support and large dimensional *** combination of GTR/GBR membrane with a three-dimensional(3D)porous scaffold may offer a resolution for the repair and functional reconstruction of large soft and hard tissue *** this study,a novel integrated gradient biodegradable porous scaffold was prepared by bonding a poly(lactic-co-glycolic acid)(PLGA)/fish collagen(FC)electrospun membrane(PFC)to a 3D-printed PLGA/nano-hydroxyapatite(HA)(PHA)*** consistency of the composition(PLGA)ensured strong interfacial bonding between the upper fibrous membrane and the lower 3D *** vitro cell experiments showed that the PFC membrane(upper layer)effectively prevented the unwanted migration of L929 *** in vivo investigations with an oral soft and hard tissue defect model in beagles revealed that the integrated scaffold effectively guided the regeneration of defective oral *** results suggest that the designed integrated scaffold has great potential for guiding the regeneration and reconstruction of large oral soft and hard tissues.

Background:oral inflammatory diseases,oral carcinoma,dental treatment procedures,and trauma are the main causes of oral tissue damage. These pathophysiological conditions cause damage in both soft and hard oral tissues such as epithelium,periodontal tissue,pulp/dentin,alveolar bone,and critical size defect in mandibular/maxilla. oral tissue regeneration is a complex process involving various biological and cellular activities,i.e.,osteogenesis,angiogenesis,inflammation,and immunomodulation. Recent advances in biomaterials and stem cells have revolutionized the various tissue regenerative approaches,including hard and soft tissue. However,the bioactive materials that can regulate biological and cellular activities required for oral tissue regeneration still need to be developed and optimized. Methods:Recently,we developed a series of nano-macro scale bioactive materials for oral tissue regeneration.(1) BMP2-functionalized calcium phosphate bone graft(BMP2-BioCaP) for small and large size bone defect reconstruction,(2) Hydroxypropyltrimethyl ammonium chloride chitosan(HACC) doped BMP2-BioCaP bone graft(HACC/BMP2-BioCaP) for infected bone defect healing,(3) BMP2 internally incorporated BioCaP bone graft(Biobone) for large size bone defect repair,(4) Mesoporous bioactive glass nanoparticles(BG-nanoparticles) for pulp/dentin regeneration,and(5) Strontiumdoped hydroxyapatite/silk fibroin(SrHA/SF) biocomposite nanospheres for bone regeneration. Results:All the BMP2-BioCaP-based bone grafts doped 10 times lower dose of BMP2 compared to clinical dose to induce bone regeneration. BMP2-BioCaP bone grafts showed less immunogenicity and an acceptable biodegradation rate. Most importantly,these bone graft showed robust bone regeneration potential without any noticeable adverse effects both in small(rat) and large animal bone defect models(rabbit/dog/sheep). Moreover,BioCaP graft robustly promoted bone regeneration and alveolar bone defect healing without affecting orthodontic too
