Influence of omega 3 Fatty Acid Supplementation on Phase Angle Values of People Living with HIV/AIDS with Lipodystrophy Secondary to Antiretroviral Therapy: A Randomized Clinical Trial

Objective: The measurement of phase angles is an important monitoring parameter and supplementation with omega-3 could promote benefits by modulating the electrical potential of membranes and increasing body cell mass. This study aimed to evaluate the effectiveness of omega-3 fatty acid supplementation on the phase angle of people living with HIV/AIDS. Methods: In this study, 63 individuals of all genders who were undergoing outpatient follow-up and showed lipodystrophy due to highly active antiretroviral therapy were analyzed. Our sample consisted of two groups, one that received supplementation containing 2550 mg of omega-3/day (1080 mg of eicosapentaenoic acid and 720 mg of docosahexaenoic acid) for three months (n = 32) and another that underwent nutrition guidance (n = 31). Phase angle and body cell mass were assessed for both groups and compared at the beginning of research (T0) and after our intervention (T1) for each group separately. Results: Phase angle averaged 6.45° ± 1.06 SD. The comparison between T0 and T1 showed a significant increase in phase angle and body cell mass, whereas the guidance group showed a decrease in body cell mass at T1 in relation to T0, with a significant p-value. Variance in phase angle between moments showed significant values between T0 and T1 in the supplementation group for all genders. Conclusion: omega-3 positively modulated patients phase angle and body cell mass, but we emphasize the need for other studies that can solidify knowledge about supplementation dosage and intervention time.

Background:Dietary supplementation of omega-3 fatty acids can reduce the activation of the endocannabinoid system(ECS)by decreasing the availability of arachidonic acid,thus lowering endocannabinoids(e CBs)levels.The ECS is a modulator of energy metabolism,stress response and inflammation in mammals,yet there is little information on the roles of the ECS in transition dairy cows.During the periparturient period,the adipose tissue and liver are the main metabolic organs that participate in the adaptations of dairy cows to onset of lactation;however,exceeded adipose tissue lipolysis and accumulation of lipids in the liver have adverse effects on cows'physiology.Here we aimed to examine whether omega-3 supplementation during the transition period will modulate ECS activation and affect metabolic and inflammatory indices in postpartum dairy cows,by supplementing twenty-eight transition Holstein dairy cows with either saturated fat(CTL)or encapsulated flaxseed oil(FLX).Components of the ECS,metabolic and inflammatory markers were measured in blood,liver,and subcutaneous adipose tissue.Results:FLX supplementation reduced feed intake by 8.1%(P3)postpartum compared to CTL.The m RNA transcription levels of the cannabinoid receptor 1(CNR1/CB1)tended to be lower(2.5 folds)in white blood cells of FLX than in CTL(P=0.10),and protein abundance of ECS enzyme monoacylglycerol lipase was higher in peripheral blood mononuclear cells of FLX than in CTL(P=0.04).In adipose tissue,palmitoylethanolamide levels were lower in FLX than in CTL(by 61.5%;P=0.02),relative m RNA transcription of lipogenic genes were higher,and the protein abundance of cannabinoid receptor 2(P=0.08)and monoacylglycerol lipase(P=0.10)tended to be higher in FLX compared to CTL.Hepatic 2-arachidonoylglycerol tended to be higher(by 73.1%;P=0.07),and interlukin-6 m RNA transcription level was 1.5 folds lower in liver of FLX than in CTL(P=0.03).Conclusions:Nutritional sup

Effects of omega-3 Fatty Acids on Chinese Patients with Cardiovascular Risk Factors: A Systematic Review and Meta-Analysis

Objective:Fish oil(FO)contains omega-3 that inhibits inflammation and blood lipid metabolism,giving it a protective cardiovascular effect.Due to dietary habits,a majority of large-scale clinical trials examining FO and cardiovascular health have been conducted in the Caucasian populations.However,the effects of FO on cardiovascular inflammation indicators and blood lipid metabolism in the Chinese population remain unclear.This study aimed to perform a meta-analysis to elucidate the impact of FO on cardiovascular health in the Chinese population.Methods:Web searches were utilized to locate records of clinical trials related to cardiovascular health and consumption of FO capsules or fish containing omega-3 in several databases,including PubMed,Medline,Embase,Cochrane Library,CNKI,and ClinicalTrial.gov,etc.We obtained lipid metabolism and related proinflammatory markers as the study outcome.We used Review Manager 5.4 and Stata 16 for the statistical analysis.If the I^(2)≥30%,a random effects model was used,and if the I^(2)30%,a fixed effects model was used.Results:Twenty eligible trials were shortlisted from>1000 records.The meta-analysis revealed that supplementation with eicosapentaenoic acid and docosahexaenoic acid reduced systolic blood pressure by 1.88 mmHg(95%CI:4.97 to1.20,P=0.23),diastolic blood pressure by 0.86 mmHg(95%CI:1.79 to 0.06,P=0.07),fasting blood glucose by 0.05 mmol/L(95%CI:0.16 to 0.06,P=0.40),and low-density lipoprotein-cholesterol by 0.12 mmol/L(95%CI:0.23 to0.01,P=0.04),when compared to placebo.However,these supplements increased high-density lipoprotein-cholesterol by 0.03 mmol/L(95%CI:0.01 to 0.05,P34,P<0.001)demonstrated the best results.Further,results from dose subgroup analyses showed that the all-dose group demonstrated a decrease in tumor necrosis factor(TNF-a)levels among the study subjects,when compared to other groups.Conclusio

The aim of this article is to examine the effect of omega-3（ω-3） long-chain polyunsaturated fatty acids（LCPUFAs） intake on retinopathy of prematurity（ROP） by reviewing the experimental and clinical trials conducted on animal models and infants.LCPUFAs demonstrate cytoprotective and cytotherapeutic actions contributing to a number of anti-angiogenic and neuroprotective mechanisms within the retina.Their intake appears to have a beneficial effect on ischemia,oxidative stress,inflammation and cellular signaling mechanisms,influencing retinal cell gene expression and cellular differentiation.ω-3 LCPUFAs may modulate metabolic processes that activate molecules implicated in the pathogenesis of vasoproliferative and neurodegenerative retinal diseases such as ROP.

目的:探讨omega-3脂肪酸(ω-3 PUFA)对金黄地鼠痤疮模型皮脂腺斑、过氧化物酶体增殖物激活受体(PPARγ)及硬脂酰辅酶A去饱和酶(SCD-1)的影响。方法:将24只金黄地鼠随机分为空白对照组、ω-3 PUFA低剂量组、ω-3 PUFA高剂量组及阳性对照组,连续灌胃4周。测量各组金黄地鼠皮脂腺斑面积,观察其组织病理改变。采用免疫组化检测皮脂腺斑组织中PPARγ和SCD-1蛋白表达。结果:干预后,与空白对照组比较,ω-3 PUFA低剂量组、ω-3 PUFA高剂量组及阳性对照组金黄地鼠皮脂腺斑面积均较小,且各组皮脂腺斑PPARγ蛋白表达均升高(P3PUFA能够改善金黄地鼠痤疮模型皮脂腺斑大小和组织结构,其机制可能是通过调节PPARγ/SCD-1信号通路。

Nutrition plays a key role in brain development,mental health,and psychiatric disorders.The role of omega-3 polyunsaturated fatty acids in physical health is well established,and their role in mental health is becoming increasingly evident.omega-3 fatty acids are involved in a wide range of physiological functions that are related to neurogenesis,neurotransmission,and neuroinflammation;therefore,they play fundamental roles in the development,functioning,and aging of the brain.In humans,dietary deficiencies of omega-3 fatty acids are associated with an increased risk of developing various psychiatric disorders,including depression,bipolar disorder,schizophrenia,dementia,attention-deficit/hyperactivity disorder,and autism.In particular,eicosapentaenoic and docosahexaenoic acid have been linked to the maintenance of mental health,and their deficits have been implicated in the pathophysiology of mental disorders.This may be mediated by the modulation of inflammatory processes and their direct effects on neuronal membrane fluidity and receptor function.However,randomized clinical trials that have investigated the therapeutic effects of omega-3 fatty acids have yielded inconclusive results,thereby limiting the use of these nutrients in psychiatric practice.High-quality clinical trials should be conducted to examine the efficacy of omega-3 fatty acids in preventing and treating mental disorders.The undesirable side effects of omega-3 fatty acid supplementation should also be considered.These effects may become apparent after many years of administration,and therefore,they may not be detected in most cases.

背景:心境障碍是临床常见的一类重性精神疾病,具有反复发作的特点,主要包括抑郁障碍及双相障碍。目前,心境障碍发病趋于年轻化,社会影响大,而且存在治疗方法有限、治疗难度大的临床问题。近年来,营养干预对心境障碍的辅助治疗作用受到大家的关注。既往在成年人研究中发现,omega-3可缓解单双相抑郁障碍的抑郁症状。但关于omega-3在青少年抑郁人群的疗效研究仍缺乏充足的循证医学证据,且干预时间及干预剂量存在研究的异质性,难以为临床上解决青少年心境障碍的治疗难题提供有效建议。故而在青少年抑郁人群中开展omega-3增效治疗的疗效研究具有重要意义。目的:本研究的目的是探究omega-3干预对青少年单双相抑郁障碍患者的辅助治疗效果。方法:（1）研究对象:从2021年9月至2022年9月总共入组53例青少年抑郁发作患者。本研究入组基线处于抑郁发作的青少年单双相抑郁患者,采用学龄儿童情感障碍和精神分裂症问卷（The Schedule for Affective Disorders and Schizophrenia for School-Age Children,K-SADS）对入组患者进行临床症状诊断和评估。（2）干预方法:本研究为随机双盲安慰剂对照研究。对符合入组条件的患者釆用SPSS 22.0软件中的Uniform函数产生随机数字表形成随机分组序列的简单随机分组法,随机分为研究组和对照组,分别给予早晚各2粒的omega-3或玉米油安慰剂干预至少8周,最长36周。研究组服用二十碳五烯酸（Eicosapentenoic Acid,EPA）2000 mg/d,二十二碳六烯酸（Docose Hexaenoie Acid,DHA）1000 mg/d;安慰剂组为同等剂量的玉米油。两组均可接受临床常规的药物和心理治疗。（3）临床评估:在基线、4周、8周、16周及36周进行临床评估,采用汉密尔顿抑郁量表17项(Hamilton Depression Scale,HAMD)、汉密尔顿焦虑量表（Hamilton Anxiety Scale,HAMA）、杨氏躁狂评定量表（Young Manic Rating Scale,YMRS）、儿童功能大体评定量表（Children's Global Assessment Scale,CGAS）及社会和职业功能评估量表（Social and Occupational Functioning Assessment Scale,SOFAS）等,分别评估患者的抑郁、焦虑、躁狂和社会功能的改善情况。（4）疗效和安全性组间比较:首先比较两组在关键时间点的脱落情况。然后,把HAMD总分及在各症状维度分数作为我们的主要疗效指标,采用广义估算模型比较两组在关键时间点8周及36周的主要疗效指标的情况,计算两组的有效率。其次,采用广义估算模型比较两组在HAMA、YMRS、CGAS及SOFAS的总分情况。最后,比较基线、8周及36周的体重指数（Body Mass Index,BMI）,分析两组在8周时的血糖、血脂情况,以了解omega-3及玉米油的安全性。结果:（1）从2021年9月至2022年9月总共入组53例青少年抑郁发作患者。首先进行8周疗效分析。总共有42人完成了8周的干预,其中omega-3组27人,对照组15人。总共有13人完成36周的干预,其中omega-3组7人,玉米油组6人。（2）8周主要疗效指标分析:通过比较两组在8周的HAMD减分率发现omega-3组的8周有效率为14.8%（4/27）;而玉米油组有效率为26.7%（4/15）;但两组的8周HAMD减分率的差异无统计学意义（n=42,P>0.05）。采用广义估算模型分析两组的HAMD总分,结果显示omega组与玉米油组不存在明显统计学差异[（15.18±1.512）分比（15.75±3.517）分,Cohen's d=1.8447,P>0.05],但可以观察到患者的HAMD总分均较前下降;进一步分析HAMD的各症状维度,8周干预后omega-3组的3个症状维度改善,认知障碍（P3）8周次要疗效指标分析:omega-3组与玉米油组在躁狂症状及社会功能情况方面均较基线有明显下降,但在两组之间不存在显著的统计学差异（P>0.05）;而8周的BMI与基线变化不大。两组之间的血糖、血脂情况未见显著统计学差异。（4）36周主要疗效指标分析:两组的HAMD总分水平相近[（9.86±1.801）分比（10.50±1.712）分,Cohen's d=0.36424,P>0.05],在HAMD的各症状维度均未见显著的组间差异,但是omega-3组在认知障碍（P=0.042）、焦虑/躯体化（P

Role of perinatal long-chain omega-3 fatty acids in cortical circuit maturation:Mechanisms and implications for psychopathology

Accumulating translational evidence suggests that the long-chain omega-3 fatty acid docosahexaenoic acid(DHA) plays a role in the maturation and stability of cortical circuits that are impaired in different recurrent psychiatric disorders. Specifically, rodent and cell culture studies find that DHA preferentially accumulates in synaptic and growth cone membranes and promotes neurite outgrowth, dendritic spine stability, and synaptogenesis. Additional evidence suggests that DHA may play a role in microglia-mediated synaptic pruning, as well as myelin development and resilience. In nonhuman primates n-3 fatty acid insufficiency during perinatal development leads to widespread deficits in functional connectivity in adult frontal cortical networks compared to primates raised on DHA-fortified diet. Preterm delivery in non-human primates and humans is associated with early deficits in cortical DHA accrual. Human preterm birth is associated with longstanding deficits in myelin integrity and cortical circuit connectivity and increased risk for attention deficit/hyperactivity disorder(ADHD), mood, and psychotic disorders. In general, ADHD and mood and psychotic disorders initially emerge during rapid periods of cortical circuit maturation and are characterized by DHA deficits, myelin pathology, and impaired cortical circuit connectivity. Together these associations suggest that early and uncorrected deficits in fetal brain DHA accrual may represent a modifiable risk factor for cortical circuit maturation deficits in psychiatric disorders, and could therefore have significant implications for informing early intervention and prevention strategies.
