Background:Andersson lesions(ALs)are not uncommon in ankylosing spondylitis(AS).Plain radiography(PR)is widely used for the diagnosis of ALs.However,in our practice,there were some ALs in AS patients that could not be detected on plain radiographs.This study aimed to propose the concept of Occult ALs and evaluate the prevalence and radiographic characteristics of the Occult ALs in AS patients.Methods:A total of 496 consecutive AS patients were admitted in the Affiliated Drum Tower Hospital,Medical School of Nanjing University between April 2003 and November 2019 and they were retrospectively reviewed.The AS patients with ALs who met the following criteria were included for the investigation of Occult ALs:(1)with pre-operative plain radiographs of the whole-spine and(2)availability of pre-operative computed tomography(CT)and/or magnetic resonance imaging(MRI)of the whole-spine.The Occult ALs were defined as the ALs which were undetectable on plain radiographs but could be detected by CT and/or MRI.The extensive ALs involved the whole discovertebral junction or manifested as destructive lesions throughout the vertebral body.Independent-samples t test was used to compare the age between the patients with only Occult ALs and those with only detectable ALs.Chi-square or Fisher exact test was applied to compare the types,distribution,and radiographic characteristics between detectable and Occult ALs as appropriate.Results:Ninety-two AS patients with a mean age of 44.4±10.1 years were included for the investigation of Occult ALs.Twenty-three patients had Occult ALs and the incidence was 25%(23/92).Fifteen extensive ALs were Occult,and the proportion of extensive ALs was significantly higher in detectable ALs(97%vs.44%,χ^(2)=43.66,P<0.001).As assessed by PR,the proportions of osteolytic destruction with reactive sclerosis(0 vs.100%,χ^(2)=111.00,P<0.001),angular kyphosis of the affected discovertebral units or vertebral body(0 vs.22%,χ^(2)=8.86,P=0.003),formation of an osseous bridge at the intervertebral space adjacent t

AIM To examine the relationship between the single nucleotide polymorphism CXCL10 rs1439490 and seronegative Occult hepatitis C virus(HCV) infection(OCI).METHODS One hundred and three cases of seronegative OCI and 155 cases of seropositive chronic HCV infection(CHC) were diagnosed at five Liver Centers in Northeastern China, from 2012 to 2016. CXCL10 rs1439490, rs1440802, and IL-28 B rs12979860 were analyzed by sequencing. Serum CXCL10 was measured by ELISA. Intrahepatic CXCL10 was determined by quantitative PCR and immunohistochemical semi-quantitative scoring. Liver necroinflammation and fibrosis were scored according to the METAVIR system.RESULTS CXCL10 rs1439490 G/G was more prevalent in OCI patients(n = 93/103; 90.3%) than in CHC patients(n = 116/155; 74.8%; P = 0.008). OCI patients had lower serum CXCL10 levels than CHC patients(192.91 ± 46.50 pg/mL vs 354.78 ± 102.91 pg/mL, P < 0.0001). Of IL-28 B rs12979860 C/C patients, OCI patients with rs1439490 G/G had lower serum and liver levels of CXCL10 and lower levels of liver necroinflammation and fibrosis than non-G/G patients. OCI patients had higher alanine aminotransferase normalization rates after Peginterferon treatment than CHC patients(P < 0.05) and serum CXCL10 decreased significantly(P < 0.0001). Liver necroinflammation and fibrosis were alleviated in 8 OCI patients after treatment. Multivariate analysis indicated that rs1439490 G/G significantly influenced the occurrence of OCI in HCV infection(OR = 0.31, 95%CI: 0.15-0.66, P = 0.002).CONCLUSION CXCL10 rs1439490 G/G is positively associated with OCI in HCV infection and antiviral outcome.

Prediction of flare-ups of ulcerative colitis using quantitative immunochemical fecal Occult blood test

AIM:To examine the feasibility of predicting the flareup of ulcerative colitis (UC) before symptoms emerge using the immunochemical fecal Occult blood test (IFOBT).METHODS:We prospectively measured fecal hemoglobin concentrations in 78 UC patients using the I-FOBT every 1 or 2 mo.RESULTS:During a 20 mo-period,823 fecal samples from 78 patients were submitted.The median concentration of fecal hemoglobin was 41 ng/mL (range:0-392 500 ng/mL).There were three types of patients with regard to the correlation between I-FOBT and patient symptoms;the synchronous transition type with symptoms (44 patients),the unrelated type withsymptoms (19 patients),and the flare-up predictive type (15 patients).In patients with the flare-up predictive type,the values of I-FOBT were generally low during the study period with stable symptoms.Two to four weeks before the flare-up of symptoms,the I-FOBT values were high.Thus,in these patients,I-FOBT could predict the flare-up before symptoms emerged.CONCLUSION:Flare-up could be predicted by I-FOBT in approximately 20% of UC patients.These results warrant periodical I-FOBT in UC patients.

AIM:To identify blood donors with Occult hepatitis B virus(HBV) infection(OBI) to promote safe blood donation.METHODS:Descriptive cross sectional study was conducted on 3167 blood donors negative for hepatitis B surface antigen(HBsAg),hepatitis C antibody(HCV Ab) and human immunodeficiency virus Ab.They were subjected to the detection of alanine aminotransferase(ALT) and aspartate transaminase(AST) and screening for anti-HBV core antibodies(total) by two different techniques;[Monoliza antibodies to hepatitis B core(Anti-HBc) Plus-Bio-Rad] and(ARC-HBc total-ABBOT).Positive samples were subjected to quantitative detection of antibodies to hepatitis B surface(anti-HBs)(ETI-AB-AUK-3,Dia Sorin-Italy).Serum anti-HBs titers > 10 IU/L was considered positive.Quantitative HBV DNA by real time polymerase chain reaction(PCR)(QIAGEN-Germany) with 3.8 IU/mL detection limit was estimated for blood units with negative serum anti-HBs and also for 32 whose anti-HBs serum titers were > 1000 IU/L.Also,265 recipients were included,34 of whom were followed up for 3-6 mo.Recipients were investigated for ALT and AST,HBV serological markers:HBsAg(ETI-MAK-4,Dia Sorin-Italy),anti-HBc,quantitative detection of anti-HBs and HBV-DNA.RESULTS:525/3167(16.6%) of blood units were positive for total anti-HBc,64% of those were antiHBs positive.Confirmation by ARCHITECT anti-HBc assay were carried out for 498/525 anti-HBc positive samples,where 451(90.6%) confirmed positive.Reactivity for anti-HBc was considered confirmed only if two positive results were obtained for each sample,giving an overall prevalence of 451/3167(14.2%) for total anti-HBc.HBV DNA was quantified by real time PCR in 52/303(17.2%) of anti-HBc positive blood donors(viral load range:5 to 3.5 x 105 IU/mL) with a median of 200 IU/mL(mean:1.8 x 104 ± 5.1 x 104 IU/mL).AntiHBc was the only marker in 68.6% of donors.Univariate and multivariate logistic analysis for identifying risk factors associated with anti-HBc and HBV-DNA positivity among blood donors showed that age above thirty a

Background Occult stress urinary incontinence may lead to de novo stress urinary incontinence after pelvic floor repair surgery. A measurement of pudendal nerve terminal motor latency can reflect the integrity of the nerves. We aimed to explore the value of pudendal nerve terminal motor latency in the diagnosis of Occult stress urinary incontinence in pelvic organ prolapse patients. Methods Ten patients with stress urinary incontinence （SUI group）, 10 with SUI and uterine or vaginal prolapse （POP＋SUI group） and 10 with uncomplicated uterine or vaginal prolapse （POP group） were evaluated for their pudendal nerve terminal motor latency using a keypoint electromyogram. Results The amplitude of positive waves was between 0.1 and 0.2 mV. The nerve terminal motor latency was between 1.44 and 2.38 ms. There was no significant difference in the wave amplitudes of pudendal nerve evoked action potential among the three different groups （P 〉0.05）. The pudendal nerve latency of the SUI group, POP＋SUI group and POP group were （2.9_＋0.7） seconds, （2.8_＋0.7） seconds and （1.9_＋0.5） seconds respectively. The difference between the SUI group and POP＋SUI group was not statistically significant （P 〉0.05）, whereas the difference between the SUI and POP groups and between the POP＋SUI and POP groups were statistically significant （P 〈0.05）. There was a positive correlation between pudendal nerve latency and the severity of SUI; the correlation coefficient was 0.720 （P 〈0.01）. Conclusions Patients with SUI may have some nerve demyelination injuries in the pudendal nerve but the damage might not involve the nerve axons. The measurement of pudendal nerve latency may be useful for the diagnosis of SUI in POP patients.

BACKGROUND Occult hepatitis C virus(HCV) infection(OCI) may be associated with extrahepatic diseases and it is known that the patients with chronic kidney disease(CKD) who are on hemodialysis(HD) present a higher prevalence of this type of infection than the general population, with a worse clinical outcome.However, there are no data in the literature to assess the presence of OCI in patients prior to the initiation of renal replacement therapy(RRT). Therefore, this study aimed to evaluate the occurrence and epidemiological aspects of OCI in patients with Predialysis CKD. We hypothesize that this infection could occur before RRT initiation.AIM To research the status in predialysis patients when HD patients have high prevalence of OCI.METHODS A cross-sectional study was conducted between 2015 and 2017. Adults with creatinine clearance < 60 mL/min·1.73 m^2(predialysis patients) were recruited to the study. Pregnant and postpartum women, patients with glomerulopathies,and patients showing positivity for serological markers of hepatitis B virus(HBV), HCV or human immunodeficiency virus infection were excluded. Patients were diagnosed with OCI according to test results of anti-HCV antibody negativity and HCV RNA positivity in either ultracentrifuged serum or, if serumnegative, in peripheral blood mononuclear cells.RESULTS Among the 91 total patients included in the study, the prevalence of OCI was 16.5%. Among these 15 total OCI patients, 1 was diagnosed by 14 ultracentrifuged serum results and 14 were diagnosed by peripheral blood mononuclear cell results. Compared to the non-OCI group, the OCI patients presented higher frequency of older age(P = 0.002), patients with CKD of mixed etiology(P = 0.019), and patients with markers of previous HBV infection(i.e.,combined positivity for anti-hepatitis B core protein antibody and anti-hepatitis B surface protein antibody)(P = 0.001).CONCLUSION Among predialysis patients, OCI involved the elderly, patients with CKD of mixed etiology, and patients with previous HBV infection.

Gastrointestinal bleeding can be obscure or Occult(OGIB),the causes and diagnostic approach will be discussed in this editorial.The evaluation of OGIB consists on a judicious search of the cause of bleeding,which should be guided by the clinical history and physical findings.The standard approach to patients with OGIB is to directly evaluate the gastrointestinal tract by endoscopy,abdominal computed tomography,angiography,radionuclide scanning,capsule endoscopy.The source of OGIB can be identified in 85%-90%,no bleeding sites will be found in about 5%-10% of cases.Even if the bleedings originating from the small bowel are not frequent in clinical practice(7.6% of all digestive haemorrhages,in our casuistry),they are notoriously difficult to diagnose.In spite of progress,however,a number of OGIB still remain problematic to deal with at present in the clinical context due to both the difficulty in exactly identifying the site and nature of the underlying source and the difficulty in applying affective and durable diagnostic approaches so no single technique has emerged as the most efficient way to evaluate OGIB.
