Objective:To detect the methylation status of MGMT gene promoter in non-small cell lung cancer(NSCLC),and to analyze the relationship between methylation of MGMT gene promoter and the chemosensitivity and prognosis of adjuvant chemotherapy after NSCLC.Methods:Tumor tissues and adjacent non-tumor tissues of 92 patients with NSCLC who underwent surgery and adjuvant chemotherapy from January 2012 to June 2015 in the thoracic surgery department of our hospital were collected,methylation-specific PCR(MSP)was used to detect the methylation status of MGMT gene promoter in tissues,and the relationship between methylation of MGMT gene promoter and chemosensitivity,progression-free survival(PFS)and 3-year overall survival rate was analyzed.Results:The methylation rate of MGMT gene promoter in cancer tissue was 39.13%(36/92)higher than that in non-cancer tissue 3.26%(3/92)(Pnon-methylation group(Pnon-methylated group(P<0.05);multivariate COX regression analysis showed that lymph node metastasis and promoter methylation of MGMT gene were independent risk factors for prognosis of NSCLC patients.Conclusion:MGMT gene promoter methylation is closely related to the chemosensitivity and prognosis of platinum-based adjuvant chemotherapy after NSCLC.It may be an important target to reverse the chemosensitivity of platinum-based chemotherapy and improve the prognosis of NSCLC.

The advent of targeted molecular therapy against the EML4-ALK fusion gene is the latest therapeutic intervention for a subset of patients with non-small cell lung cancer (NSCLC). Crizotinib (Xalkori) is an orally available small molecule tyrosine kinase inhibitor proven in clinical trials to significantly impact progression free survival and overall response rate. We present a case of a 56-year-old male with NSCLC whose lack of a positive treatment response to this therapy led to the clinical suspicion and identification of the underdiagnosed entity known as synchronous multiple primary lung cancer (SMPLC).

The purpose of this study was to compare the clinical effects of pemetrexed and docetaxel combined with cisplatin in the treatment of patients with non-small cell lung cancer.A total of 58 patients with non-small cell lung cancer who were enrolled between January 2017 and January 2018 were enlisted into a randomized digital table.29 patients who have received treatment with combined pemetrexed and cisplatin were assigned to the pemetrexed group,whereas for the other 29 patients which were treated with docetaxel and cisplatin combined,were assigned to the docetaxel group to verify the calculated clinical treatment efficiency of the patients with non-small cell lung cancer,soluble vascular cell adhesion molecule 1(SVCAM-1),and activated leukocyte cell adhesion molecule-1(alCAM-1)concentrations and to evaluate the quality of life scores of the patients after half a year as well as the incidences of adverse reactions following the treatments provided.The differences in SVCAM-1 and alCAM-1 concentrations and incidence of adverse reactions in patients with non-small cell lung cancer in the docetaxel group as compared with patients in the pemetrexed group after the treatments were statistically significant(P0.05,demonstrated no analytical significance.Both pemetrexed combined with cisplatin and docetaxel in combination with cisplatin as forms of treatments demonstrated significant effects in patients with non-small cell lung cancer.However,based on our study,it was found that the combined treatment involving pemetrexed and cisplatin can further reduce a

To assses the effect of single nucleotide polymorphism of excision repair cross-complementation group 1 C8092A on the clinical outcome and toxicity in advanced stage non-small cell lung cancer patients receiving first line platinum based chemotherapy.MethodsThis article is a review of the current research on single nucleotide polymorphism and its effect on treatment outcome and toxicity of advanced stage lung cancer.Conclusion The observations indicate that more advanced studies and trials on C8092A SNPs are needed so as to assess if it could be used as a potential biomarker in the future.

Objective Previous studies have proven that cumulative smoking dose predicts the prevalence of epidermal growth factor receptor(EGFR) mutations. The aim of this study was to investigate the relationship between smoking-related factors and EGFR mutation status. Methods Samples were collected from 195 smokers with non-small cell lung cancer(NSCLC) who underwent surgical resection and the presence of EGFR mutations(exons 19 and 21) were determined by real-time polymerase chain reaction(RT-PCR).Results EGFR gene mutations were present in 33(16.9%) patients who were smokers; the patients were divided into three groups according to the smoking index(SI). The incidence of EGFR mutations decreased from 38.9% in mild smokers to 8.1% in severe smokers(P = 0.001). Compared to daily smoking dose(P = 0.547), initial smoking age(P = 0.085) and duration of smoking history had a larger effect on EGFR mutation status(P = 0.002).Conclusion Although there is a decrease in the incidence of mutations with increasing SI, there were still around 17% of smokers with NSCLC that harbored EGFR mutations, so it is necessary to test for EGFR mutation status in smokers with NSCLC.

Expression of STAT3 in non-small cell lung cancer and its clinical significance

Signal transducer and activator of transcription 3(STAT3) plays an important role in tumor occurrence and development. Here we attempt to evaluate the clinical diagnostic value of STAT3 as a potential tumor marker by detecting the expression of STAT3 in blood of non-small cell lung cancer (NSCLC) patients. Immunohistochemistry was applied to detect STAT3 protein in A549 cells and MRC-5 cell, real time PCR was applied to detect STAT3 mRNA and ELISA was applied to detect the expression of STAT3 protein in NSCLC patients, benign lung disease group and healthy controls. The expression levels of STAT3 mRNA and STAT3 protein in A549 cells and NSCLC patients were significantly higher (P 0.05). When comparing the Az of STAT3 protein with CEA, CA125 and CYFRA21-1 respectively, there was no significant difference (P > 0.05). And we compared the Az of the four joint detection (STAT3 + CEA + CA125 + CYFRA21-1) with the three joint detection (CEA + CA125 + CYFRA21-1), 0.930 and 0.841, respectively, P < 0.05. It appears that the diagnostic value and accuracy of STAT3 is equivalent with CEA, CA125, and CYFRA21-1 in NSCLC, and to joint with other three can achieve higher diagnosis value.

As an epidermal growth factor receptor(EGFR)tyrosine kinase lung cancer-targeted drug,gefitinib has been widely applied in the treatment of non-small cell lung cancer(NSCLC).The present clinical application has shown that EGFR genovariation might cause the abnormal expression of lung cancer cells,it makes its sensibility to gefitinib decrease,and the clinical efficacy will be decreasing accordingly.Based on these problems,the effects of EGFR genovariation on the clinical efficacy of gefitinib treatment of NSCLC was further analyzed,and the successive development was summarized.

Quantitative monitoring and mutations of ctDNA before and after non-small cell lung cancer radical surgery

Objective The aim of this study was to study the quantitative expression of circulating tumour DNA(ctDNA) in patients with non-small cell lung cancer(NSCLC) before and after radical operation and to explore the correlation between gene mutations in non-small cell lung cancer tissues and those in ctDNA.Methods We randomly assigned 5 NSCLC patients from the Department of Thoracic Surgery of Fujian Medical University Union Hospital. All the patients had undergone radical surgery. Venous blood samples were collected from the 5 NSCLC patients at two time points(before the operation and 21–37 days after the operation) for monitoring ctDNA levels. This was done by isolating plasma from venous blood using high velocity centrifugation, extracting DNA from the plasma using the QIAamp Circulating Nucleic Acid kit, and then quantifying the ctDNA levels. The results were analyzed using the Wilcoxon Rank Sum Test. Moreover, the ctDNA levels were compared with those of carcinoembryonic antigen(CEA), which was detected simultaneously with the ctDNA. Then, DNA samples from the tumor tissues and peripheral blood cells and ctDNA were sequenced using the Hiseq2000 sequencing platform(Illumina) and the mutant genes were screened out. Mutations that occurred within the tumor tissues were used as positive control, whereas those found in the pre-operative blood cells were used as a negative control. Based on the mutational analysis of ctDNA genes, a total of 508 cancer-related genes were screened. Results The median values of the pre-and post-operative ctDNA levels in the 5 patients with NSCLC were 0.612(0.518–0.876) and 0.430(0.372–0.612) ng/μL, respectively. There was a significant difference between the two groups(P 0.05). In one of the cases, LC tissues showed multiple mutations, consistent with pre-operative ctDNA. Background: Published data have shown that microRNAs (miRNAs) could play a potential role as diagnostic and prognostic indicators in cancers. Data for the predictive value of miRNA let-7, miR-21, and miR-205 are inconclusive. The aim of the present analysis was therefore to evaluate the expression and the prognostic role of the above mentioned miRNAs?in early-stage?(T1N0) NSCLC patients. Methods: Quantification of let-7g, miR-21, and miR-205 expression was carried out into 105 early-stage NSCLC by quantitative Real Time-PCR (qRT-PCR).?Results: a significant association between the low miR-205 expression and ADC histotype (p??0.0001) compared to SCC?was found;moreover, survival analysis showed thattumors with a high?miR-205 expression had a significantly shorter mean PFS and OS compared to the patients with a low expression of this miRNA (p = 0.02 and p = 0.03, respectively). No other statistically significant correlations were observed between the analysed miRNAs and the main clinico-pathological characteristics of the NSCLC patients. Conclusion: The results indicated that miR-205 could represent a useful marker in the prognostic management of the early-stage (T1N0) NSCLC patients.

Adjuvant Chemotherapy of Gemcitabine plus Carboplatin versus Paclitaxel plus Carboplatin in Patients with Resected non-small Cell Lung Cancer

Background: This retrospective study was to evaluate the efficacy and toxicity of gemcitabine plus carboplatin (GC regimen) and paclitaxel plus carboplatin (PC regimen) combination chemotherapy administered as an adjuvant therapy after complete resection of non-small cell lung cancer. Methods: Forty-four patients (GC regimen, n = 29;PC regimen, n = 15) received gemcitabine at a dose of 1000 mg/m2 on days 1 and 8, and carboplatin with the target dose of area under the curve (AUC) of 4 on day 8 every 28 days and paclitaxel at a dose of 70 mg/m2 on days 1, 8 and 15, and carboplatin with the target dose of AUC of 5 on day 1 every 28 days. Results: A total of 130 cycles of the treatment were administered (averaged, 3.1 in GC arm and 2.7 cycles in PC arm). Forty-three patients (97.7%) completed the scheduled cycles. One patient (2.3%) was discontinued due to grade 4 pneumonia. The dose was reduced in 2 patients (4.5%) due to grade 4 thrombocytopenia. Grade 3/4 neutropenia was significantly observed in the PC group (GC: 12/29, 41.4%;PC: 11/15, 73.3%, p = 0.0443). The nonhematological toxicities were mild. Grade 1/2 alanine aminotransferase and aspartate aminotransferase in the GC group was significantly observed higher compared to those of the PC group (GC: 20/29, 69.0%;PC: 4/15, 26.7%, p = 0.0076). Grade 1/2 alopecia was significantly observed in the PC group (GC: 0/25, 0.0%;PC: 13/15, 86.7%, p 0.0001). There was no treatment-related death. The median survival time (MST) of the entire GC group was 784 days, the 3-year overall survival (OS) was 75.9%, and 3-year recurrence-free survival (RFS) was 65.5%. The MST of the entire PC group was 963 days, the 3-year OS was 80.0%, and the 3-year RFS was 60.0%. Conclusion: These results demonstrate that the GC and PC combination chemotherapies are efficacious and feasible regimens, which should be considered as one of the standard therapies for adjuvant therapy.
