Background: Accurate pathological assessment of breast specimens after NACT is crucial. So we should explore the relevance between molecular subtypes and efficacy of neoadjuvant chemotherapy in breast cancer as well as its prognostic factors, which about survival analysis and disease free survival involed, which was one part of contibuting for evaluating in terms of global survival and disease free survival. Methods: 264 patients with breast cancer who received neoadjuvant chemotherapy before surgery in Breast Center, the Fourth Hospital of Hebei Medical University, between January 2007 and May 2011. The relationship between molecular subtypes and neoadjuvant chemotherapy, and clinical pathological features were analyzed. Results: The total pCR rate was 12.50%（33/264）. The pCR rate were 3.03%（1/33）, 9.40%（14/149）, 17.39%（8/46）, 27.78%（10/36） in the subtype of Luminal A, Luminal B, HER2, and Basal-like, respectively. And that had predicted The pCR rate associated with breast cancer molecular subtypes,（P＜0.05）. The pCR rate of Basal-like subtype was highest, HER2 subtype was higher than Luminal A and Luminal B. Multiple factors analysis results showed that: the independent impact factors of 5-year overall survival rate in neoadjuvant chemotherapy breast cancer patients were clinical stage, tumor size, chemotherapy regimens, lymph node metastasis, ER status and pathological remission;the independent impact factors of 5-year disease free survival rate were tumor size, chemotherapy regimens, lymph node metastasis, ER status, PR status and pathological remission（P＜0.05）. Basal-like subtype breast cancer had shorter overall survival and diseasefree survival（P＜0.05）. Conclusions: The pCR rate was frequently observed in HER2 and Basal-like subtypes breast cancer. That could achieve a higher rate of pCR on（ AC→TH） chemotherapy. But Basal-like subtype breast cancer were showed worse prognosis, due to residual tumor after neoadjuvant chemotherapy, which could be in combination

Purpose:To investigate the neural mechanisms underlying attention deficits that are related to neoadjuvant chemotherapy in combination with cerebral perfusion changes based on arterial spin labeling(ASL) ***:A total of 31 young women with breast cancer who were scheduled to receive neoadjuvant chemotherapy and 34 matched healthy control subjects were *** patients completed two assessments of the attention network tasks(ANT),neuropsychological background tests,and a three-dimensional pseudocontinuous ASL scan,which were performed before neoadjuvant chemotherapy and within 1 month after completing *** controls underwent a single ***:After neoadjuvant chemotherapy,the patients exhibited reduced efficiencies of the alerting and executive control attention networks but not the orienting network(p chemotherapy and healthy controls.A correlation analysis revealed that a decrease in the performance of ANT was significantly correlated with an increase in CBF changes in some brain regions of the patients with breast ***:The present study represents a significant step toward understanding the potential neoadjuvant chemotherapy-induced changes in attention network and cerebral perfusion *** results demonstrated that neoadjuvant chemotherapy influences hemodynamic activity in different brain areas through increasing cerebral perfusion,which reduces the efficiency of attention networks in breast cancer patients.

Oxaliplatin plus S-1 or capecitabine as neoadjuvant or adjuvant chemotherapy for locally advanced gastric cancer with D2lymphadenectomy: 5-year follow-up results of a phase II-III randomized trial

Objective： To compare the effect of neoadjuvant chemotherapy （NACT） with adjuvant chemotherapy （ACT） using oxaliplatin plus S-1 （sex） or capecitabine （CapeOX） on gastric cancer patients with D2 lymphadenectomy. Methods： This was a two-by-two factorial randomized phase Ⅱ-Ⅲ trial, and registered on ISRCTN registry （No. ISRCTN12206108）. Locally advanced gastric cancer patients were randomized to neoadjuvant sex, neoadjuvant CapeOX, adjuvant sex, or adjuvant CapeOX arms. Primary analysis was performed on an intention- to-treat （ITT） basis using overall survival （OS） as primary endpoint. Results： This trial started in September 2011 and closed in December 2012 with 100 patients enrolled. Treatment completion rate was 56%, 52%, 38% and 30% in the four arms, respectively. NACT group had fewer dropouts due to unacceptable toxicity （P=0.042）. Surgical complication rate did not differ by the four groups （P=0.986）. No survival signifcant difference was found comparing NACT with ACT （P=0.664; 5-year-OS： 70% vs. 74% respectively）, nor between the sex and CapeOX groups （P=0.252; 5-year-OS： 78% vs. 66% respectively）. Subgroup analysis showed sex significantly improved survival in patents with diffuse type （P=0.048）. Conclusions： No significant survival difference was found between NACT and ACT. sex and CapeOX had good safety and efficacy as neoadjuvant regimens. Diffuse type patients may survive longer due to sex.

HER2 and topoisomerase Ⅱα： possible predictors of response to neoadjuvant chemotherapy for breast cancer patients

Background Surrogate markers may be used to assess the response to neoadjuvant treatment. The association between HER2 overexpression and favorable response to specific therapy in breast cancer is controversial, and the mechanism unclear. The purpose of the study was to evaluate HER2 and topoisomerase Ⅱα （Topo Ⅱα） as candidates for predicting the response to neoadjuvant chemotherapy in breast cancer patients.Methods Between 1999 and 2006, seventy-six breast cancer patients who had received neoadjuvant chemotherapy were studied. Regimens including either CEF （cyclophosphamide, epirubicin, 5-fluorouracil） or CMF （cyclophosphamide, methotrexate, 5-fluorouracil） were given in more than three cycles to this group of patients. Protein expression of HER2 and Topo Ⅱα were determined by immunohistochemistry. The primary endpoint was pathological and clinical response.Results Of 76 primary breast cancer samples, 27 （35.5%） showed overexpression of either HER2 （25%） or Topo Ⅱα protein （10.5%）, whereas in 7 tumors （9.2%） both proteins were found to be overexpressed. Ten patients （13.2%） had a clinical complete response and 21 （27.6%） had a clinical partial response. Five women （6.6%） had a pathological complete response, 5 （6.6%） had microscopic residual disease, and 46 （60.5%） had macroscopic residual disease. HER2 and Topo Ⅱα overexpression was significantly associated with a favorable response （P 〈0.001 and P=0.005 respectively）.Conclusion Our study suggests that HER2 and Topo Ⅱα overexpression could be predictors of the response to neoadjuvant chemothrapy in both the CEF and CMF arms.

APOPTOSIS AND PROLIFERATION OF TUMOR CELLS IN LOCALLY ADVANCED CERVICAL CANCER AFTER neoadjuvant INTRAARTERIAL chemotherapy

Objective: Through observing the clinical response toneoadjuvant intraarterial chemotherapy in locally advanced cervical cancer and investigating the changes of p53 proteinexpression, proliferation and apoptosis of tumor cells afterchemotherapy, to study the relationship between biological markers and chemotherapeutic response. Methods: 20women with locally advanced squamous cervical cancerreceived consecutive infusion chemotherapy of five days ofcisplatin and adriamycin via the superselective uterineartery. The response to chemotherapy was evaluated bygynecologic examination and ultrasonography 3 weeks after chemotherapy. The changes of apoptotic index (AI), proliferation index (PI) and p53 expression of tumor cellswere detected by immunohistochemical ***: The clinical response rate of locally advancedsquamous cervical cancer to uterine artery infusionchemotherapy was 70%. No change of PI was found 3 weeks after treatment, but AI significantly increased from2.79?.76 to 4.29?.13 (Pchemotherapy showed higher PI before chemotherapy and significantly increased AI and AI/PIafter chemotherapy than non-responders (Pneoadjuvantintraarterial chemotherapy. One more cycle ofchemotherapy should be given to those who have significantly increased AI or AI/PI after chemotherapy, while definite treatment such as surgery or/and radiotherapy should be immediately given to those patients without increased AI or AI/PI.

The clinical observation of neoadjuvant chemotherapy in locally advanced breast cancer with DX regimen

The recent clinical curative effect and adverse events of docetaxel and capecitabine （DX） of neo- adjuvant chemotherapy in patients with locally advanced breast cancer was discussed. Methods： The data of 72 cases of neoadjuvant chemotherapy （DX） in locally advanced breast cancer after 4 cycles were retrospectively analyzed. Docetaxel 75 mg/m^2 by infusion 1 h on dl, capecitabine 2000 mg/m^2 by oral for twice daily on d1-14, 21 days was a cycle. Results： All 72 patients were assessed for efficacy and adverse events. The total effective rate was 80.5% （58/72）, including pathological complete response （pCR） was 7 （9.7%）, clinical complete remission （cCR） was 15（20.8%）, clinical partial response （PR） was 43 （59.7%）, stable disease （SD） was 8 （11.1%） and progressive disease （PD） was 6 （8.3%）. The main adverse events were gastrointestinal reactions and bone marrow suppression. The 3 to 4 degrees of adverse reactions including granulocytopenia in 7 patients （20.6%）, hand-foot syndrome in 6 patients （15.2%）. Conclusion： The DX regimen provide a favorable efficacy and safety profile in patients with locally advanced breast cancer for neoadjuvant chemotherapy.

Phase III study of TAC and TP regimens as neoadjuvant chemotherapy in patients withtriple-negative breast cancer

Objective： This study aimed to compare the efficacy and safety of neoadjuvant chemotherapy with TAC and TP regimens of triple negative breast cancer （TNBC）. Methods： A total of 102 patients with TNBC were confirmed by histopathology. They were divided into TAC group （52 cases） and TP group （50 cases）. Group TAC： Docetaxel 75 mg/m2 or paclitaxel （taxol liposome） 135 mg/m2 on all, pirarubicin 40 mg/m2 or epirubicin 75 mg/m2 on d2, cyclophosphamide 600 mg/m2 on dl; Group TP： Docetaxe175 mg/m2 or paclitaxel （taxol liposome） 135 mg/m2 on dl, cisplatin 30 mg/m2 on d2-d4, with 21 days as a cycle. All patients underwent operation after 2-4 cycles of chemotherapy. The short-term effects and toxic and adverse effects were evaluated. Results： In TAC group, 5 cases （9.6%） had pathological complete release （pCR）, 35 cases （67.3%） partial release （PR）, 9 cases （17.3%） stable disease （SD）, and the response rate （RR） was 76.9%. In TP group, 4 cases （8%） had pCR, 32 cases （64%） PR, 5 cases （10%） SD, and RR was 72%. In 102 patients, 12 patients with tumor progression after 2 cycles of chemotherapy, included 3 cases in TAC group, 9 cases in TP group. In TAC group, 2 cases occurred atrial premature contraction; while 3 cases developed grade 2 renal injury in TP group. In TAC group, grade 3-4 hematologic toxicity and alopecia was significantly higher than that in TP group, but grade 3-4 gastrointestinal reaction rate in TP group was significantly higher than TAC group. Conclusion： TAC and TP regimens all had certain efficacy in the neoadjuvant chemotherapy for TNBC, and the toxicity reactions can be tolerated.

Relationship between expression of ER, PR, Her-2, Ki-67 and neoadjuvant chemotherapy effect in breast cancer

Objective: The purpose of the study was to investigate the relationship between the expression of estrogen receptor(ER), progestogen receptor(PR), human epidermal growth factor receptor(Her-2), Ki-67 and the effect of neoadjuvant chemotherapy in breast cancer. Methods: The expression of ER, PR, Her-2 and Ki-67 in 45 breast cancers which received neoadjuvant chemotherapy was detected by immunohistochemistry. Results: The effective rates in ER negative and PR negative groups were higher than those in ER positive and PR positive groups(83.3% vs 59. 4%, 82.4% vs 60.6%). There was no significant difference of the effective rate between Her-2 overexpressed group and Her-2 non-overexpressed group(81.8% vs 64.1%), and the same thing happened between Ki-67 negative group and Ki-67 positive group(67.7% vs 63.2%). Conclusion: In the patients with breast cancer, ER, PR negative ones were more sensitive to neoadjuvant chemotherapy. These patients may get more benefits from chemotherapy. ER, PR could be feasible markers for predicting the effective rate of neoadjuvant chemotherapy.

Objective:The aim of this study was to study changes of HER-2 expression after neoadjuvant chemotherapy in the breast cancer ***:One hundred and thirty-seven female patients with primary breast cancers,who received neoadjuvant chemotherapy,underwent core needle puncture and Mammotome biopsy before chemotherapy,and the biopsy results were used as the basis of histological diagnosis,fluorescence in situ hybridization (FISH) was performed to test HER2 status of tumor tissues before and after *** patients underwent FEC,TE,or AC neoadjuvant chemotherapy of 2-6 cycles before ***:Twenty-two patients were positive according to FISH test among 137 preoperative patients,8 patients achieved pathological complete remission after chemotherapy (three HER-2 positive patients and five negative patients),91 patients achieved partial remission,24 patients were stable,and 14 cases were ***-two patients were positive according to FISH test (8 patients with pathological complete remission did not undergo test),and positive patients still expressed positively after chemotherapy before neoadjuvant *** negative patients were converted to be positive,and changes before and after chemotherapy had no statistical difference (P>0.05).Conclusion:neoadjuvant chemotherapy makes no influence on patients with HER-2 positive expression,while patients with negative expression can be converted to be positive,but without significant difference.
