Clinical Evaluation of moisturizers with Physiological Analysis of Stratum Corneum TARC and TSLP

To verify the usefulness of moisturizers, a multi-center study was conducted on patients with atopic dermatitis who visited 3 university hospitals in Japan between November 2009 and March 2012. Thirty-seven patients with dry skin and stable symptoms who were receiving topical and oral treatments were given moisturizers for 8 weeks. The moisturizers contained components such as glycerin and seawater minerals in 3 forms: lotion, emulsion, and cream. The moisturizers were chosen by physicians based on the degree of dryness of the patients’ skin. In addition to observing the skin condition of the entire body, high-frequency conductance as a parameter of skin surface hydration and transepidermal water loss (TEWL) in the buccal region and the medial side of the forearm were measured, and those of the back were used for comparison. Furthermore, stratum corneum levels of thymic stromal lymphopoietin (TSLP) and thymus and activation-regulated chemokine (TARC), both of which were objective parameters for atopic dermatitis skin lesion severity, were analyzed using the tape stripping method. At the beginning of the study, TSLP showed a correlation with skin symptoms (dryness, itching) and high-frequency conductance, whereas TARC showed a correlation with skin symptoms (erythema, dryness, itching) and TEWL. In addition, a correlation was noted among TSLP, TARC, and itching. At the end of the study, erythema, dryness, itching, TEWL,TSLP, and TARC were significantly reduced, whereas high-frequency conductance was significantly increased. The moisturizers clearly improved the dry skin symptoms of these patients with atopic dermatitis and improved the physiology of their sensitive and damaged skin. These findings also support the involvement of cytokines/chemokines in the pathogenesis of atopic dermatitis lesions.

Vulvar Contact Dermatitis: Effect of a Treatment with a Anti-Inflammatory, Moisturizing and Healing Product on the Course of the Disease

Background: Vulvar contact dermatitis is inflammatory disorders of genital skin causing itch, burning, and plaques with overlying excoriation and linear fissures. Objective: To assess the effectiveness of a soft foam based on Tiab® system (TF), complex of titanium micro crystals covalently bound with silver ions associated with hyaluronic acid, as a treatment of vulvar contact dermatitis. Method: 36 patients with the diagnosis of vulvar contact dermatitis received an 10-days treatment, during which the subjects applied TF, complex of titanium micro crystals covalently bound with silver ions associated with hyaluronic acid on the affected vulvar surfaces, used once a day. Clinical symptoms (itching and burning) and signs (vulvar surface disruption) were documented at baseline and after 1 months of follow-up. Results: There was a significant reduction of symptoms between the basal scores and the symptoms complaints 1 months of follow-up. Moreover a significant reduction of excoriations and linear fissures was observed. Conclusion: The present study shows that application of TF can be an effective agents for the reduction of symptom and sign of vulvar contact dermatitis, and the medical device can be used as a preventive agent in predisposing condition of vulvar discomfort.

Vulvar Dermatoses: Effect of a Synergistic Treatment with a Moisturizing and Healing Product on the Course of the Disease

Background: Vulvar dermatoses are inflammatory disorders of genital skin causing itch, burning, and plaques with overlying excoriation and linear fissures. Objective: To assess the effectiveness of a soft foam based on Tiab®system (TF), complex of titanium micro crystals covalently bound with silver ions associated with hyaluronic acid, in the supportive treatment of lichen sclerosus (LS) and lichen simplex chronicus (LSC), the two common vulvar dermatoses. Method: 20 patients with the diagnosis of LS or LSC received an 4-week treatment, during which the subjects applied Mometasone furoate in the form 0.1% cream were treated with Mometasone furoate 0.1% cream in combination with TF once a day. Clinical symptoms (itching and burning) and signs (vulvar suface disruption) were documented at baseline and at the end of the four-week treatment. Results: There was a significant reduction of symptoms between the basal scores and the symptoms complaints at 4-week treatment. Moreover, a significant reduction of excoriations and linear fissures was observed. Conclusion: The present study shows that daily application of TF can be an effective adjunct for the treatment of vulvar dermatosis. The foam had not only a moisturizer action, it may also be associated with a reduction in epithelial disruption in the form of excoriations and fissures.

Evaluation of the Efficacy and Safety for Atopic Dry Skin of a Medicinal Moisturizing Lotion (Mentholatum AP Soft Medicinal Moisture Lotion) Prepared by a High-Pressure Emulsification Method

In order to evaluate the efficacy and safety of Mentholatumò AP Soft Lotion (APA), which is a Vaseline-based preparation that has been converted to nanoparticles by high-pressure emulsification technology, we conducted a study of 4-week continuous use on the atopic dry skin of patients with minor to moderate atopic dermatitis (AD). The results showed that improvements in the skin findings (dryness and scaling) and itching were observed beginning one week after starting to apply APA and that the symptoms had almost completely resolved at 3 to 4 weeks after the start of application. In addition, a significant increase in the degree of high-frequency conductivity, which is an indicator of the amount of moisture in the stratum corneum, and significant decrease in transepidermal water loss (TEWL) were observed, and improvements in skin functions were observed. The results for overall degree of improvement showed “improvement” or better in 100% of the subjects, and no side effects were observed in any of the cases. Based on the above results, it was concluded that APA is a preparation that has excellent efficacy and safety as skin care for atopic dry skin, in which reduced stratum corneum function is observed in the skin, and as a supplemental method of treatment of AD or treatment method to maintain remissions.

Improvement of Scalp Condition and Quality of Life through Proper Skin Care of Dry Scalp

Introduction: Appropriate skin care for dry scalp includes treatment for dandruff and itchiness. However, little is known about the appropriate washing methods for subjects with dry scalp. Me- thods: A scalp moisturizing lotion was used for 8 weeks on 30 outpatients at the Tokushima University Hospital who had dry scalp. All subjects were given the same hair shampoo/conditioner and instructions on how to wash their hair. Scalp symptoms were observed before and after the testing period. Indigenous bacteria were collected from the scalp for quantitative assessment. A survey was conducted to assess the quality of life (QOL). Results: All scalp condition items (dryness, scales and desquamation, itchiness, excoriation, and erythema) improved significantly after the testing period;the levels of Malassezia and bacterial colonization also decreased significantly. The QOL evaluation indicated significant improvements in “symptoms”, “emotions”, and “global” measures. A correlation was found between the extent of dryness at the start of the test and the level of Malassezia colonization, as well as QOL scores. Conclusion: Using a scalp moisturizing lo- tion, washing hair with a shampoo and conditioner that cause low irritation levels, and using a hair washing method that avoids irritation led to improvement in scalp condition, QOL, and pa- tient satisfaction.

New innovation of moisturizers containing non-steroidal anti-inflammatory agents for atopic dermatitis

Atopic dermatitis is a chronic, relapsing and extremely pruritic eczematous disease which commonly affects children. The standard management consists of a combination of anti-inflammatory drugs in adjunctive with skin care management particular moisturizer application. A concern for the side effects associated with long term use of corticosteroids has also been considered. There has been an emerging interest in moisturizer containing non-steroidal anti-inflammatory agents such as herbal extracts, vitamins, mineral and lipids. The in vitro and the in vivo studies of each agent were reviewed. The clinical study on the efficacy of moisturizers containing these agents were also demonstrated including the author's studies and clinicalexperience. These moisturizers might be considered as an alternative treatment in acute flare of mild to moderate atopic dermatitis.
