metastatic melanoma to the common bile duct is very rare with only 18 cases reported so far. We report a 46 year old women who, 18 mo after excision of a skin melanoma, developped a painless progressive obstructive jaundice. At operation a melanoma within the distal third of the common bile duct was found. There were no other secondaries within the abdomen. The common bile duct, including the tumor, was resected and anastomosed with Roux-en-Y jejunal limb. The patient survived 31 mo without any sign of local recurrence and was submitted to three other operations for axillar and brain secondaries, from which she finally died. Radical resection of metastatic melanoma to the common bile duct may result in lifelong relief of obstructive jaundice. It is safe and relatively easy to perform. In other cases, a less aggressive approach, stenting or bypass procedures, should be adopted.

BACKGROUND Immunotherapy has revolutionized the treatment of metastatic melanoma,but a significant proportion of patients still experience treatment *** microbiota transplantation(FMT)has emerged as a potential strategy to overcome immunotherapy resistance by modulating the gut *** SUMMARY We present a case report of a 57-year-old male with metastatic melanoma refractory to immunotherapy who received FMT in combination with antiprogrammed death-ligand 1(PD-L1)immunotherapy(pembrolizumab).After failing multiple lines of treatment,the patient underwent a single FMT procedure by colonoscopy using fecal material from a female metastatic melanoma donor who successfully responded to *** FMT,the patient demonstrated a response with decreased subcutaneous disease and subsequently underwent surgery to remove the residual *** a subsequent recurrence in the small bowel that was resected,the patient remained on pembrolizumab without evidence of melanoma recurrence at the time of *** The favorable clinical and long-lasting effect we saw in our patient without significant toxicity suggests that this procedure should be considered in similar patients with immunotherapy refractory melanomas.

Introduction melanoma is the fifth most common cancer in the United *** estimated number of new cases of melanoma in 2012 is 76,250, with 9,180 melanoma-related death[1,2].Moreover,the incidence of melanoma is rising faster than any other cancer in man and second only to lung cancer in women[3].Although only 5% of patients had metastatic disease on diagnosis, the median overall survival (OS) is less than one-year and the twoyear survival rate is between 10~20%[4,5].Compared to the white population, Asian population is less prone to this condition because of the protective effect of melanin[6].The overall incidence of melanoma in China has not been reported.A recent single institution study in China reported 522 melanoma cases during only four years from 2006~2010[7].

A 54-year-old man diagnosed with metastatic melanoma of the right inguinal node with occult primary developed liver and bone metastases. The combination of dabrafenib plus trametinib was initiated, and a complete response (CR) was achieved 24 months after starting treatment. One month later, the target therapy was discontinued at the patient’s decision, and he has remained free from progression for 21 months since discontinuation. To the extent of our knowledge, real-world data in Asian melanoma concerning the discontinuation of dabrafenib plus trametinib after achieving CR have not been published;therefore, our case is a meaningful one for considering to cease target drugs and to rescue their financial toxicity.

Objective:Vitamin D receptor(VDR)mediates vitamin D *** examined whether VDR expression in excised melanoma tissues is associated with VDR gene(VDR)***:We evaluated VDR protein expression(by monoclonal antibody immunostaining),melanoma characteristics,and carriage of VDR-Fok I-rs2228570(C>T),VDR-Bsm I-rs1544410(G>A),VDR-ApaI-rs7975232(T>G),and VDR-TaqI-rs731236(T>C)polymorphisms(by restriction fragment length polymorphism).Absence or presence of restriction site was denoted by a capital or lower letter,respectively:"F"and"f"for Fok I,"B"and"b"for Bsm I,"A"and"a"for ApaI,and "T"and"t"for TaqI ***-four Italian cutaneous primary melanomas(52.1±12.7 years old)were studied;51.4% were stage Ⅰ,21.6% stage Ⅱ ,13.5% stage Ⅲ,and 13.5% stage Ⅳ *** expression was categorized as follows:100% positive vs.melanomas,Breslow thickness ofmelanomas with high *** VDR *** genotypes BbAA,bbAa,AATt,BbAATt,and bbAaTT were more frequent in 100%vs.melanomas lacking VDR expression(odds ratio=14.5;P=0.025).VDR expression was not associated with metastasis,ulceration,mitosis>1,regression,tumor-infiltrating lymphocytes,tumoral infiltration of vascular tissues,additional skin and non-skin cancers,and melanoma ***:We highlighted that VDR polymorphisms can affect VDR expression in excised melanoma *** VDR expression in AATT carriers is a new finding that merits further *** expression possibly poses implications for vitamin D supplementation against *** expression and VDR genotype may become precise medicinal tools for melanoma in the future.

BACKGROUND Uveal melanoma is the most common primary intraocular malignant tumor affecting the eyes in *** half of all primary uveal melanoma tumors metastasize;yet,there are currently no effective treatments for metastatic uveal *** the time of diagnosis,less than 4%of patients with uveal melanoma have detectable metastatic *** melanoma disseminates hematogenously,with the most common site of metastasis being liver(93%),followed by lung(24%)and bone(16%).CASE SUMMARY A 57-year-old woman was diagnosed with a dysplastic nevus on her eyelid,which was histologically confirmed as malignant melanoma after *** patient had no evidence of metastasis to other organs and received both radiation therapy and *** systemic treatment,a metastatic left neck lymph node was found and another round of chemotherapy was performed after *** emission tomography-Computed Tomography tracking after completion of chemotherapy revealed two metastatic liver *** patient underwent partial liver resection and showed no signs of recurrence at 1 year after *** Surgery is an effective treatment for metastatic uveal *** patients with liver metastatic lesions,hepatectomy improves outcome.

With the occasion of a case of malignant melanoma of the gallbladder, which appeared to be primary, we have reviewed the literature and the result of this research was that primary melanoma of the gallbladder remains a questionable medical entity. Only few cases of both primary and metastatic gallbladder melanoma have been reported so far, and the only agreement is that surgery is the mainstaytreatment. The role of adjuvant chemotherapy, hormonotherapy or immunotherapy for both primary and metastatic disease remains undefined.
