Prolonging dual antiplatelet therapy improves the long-term prognosis in patients with diabetes Mellitus undergoing complex percutaneous coronary intervention

OBJECTIVE To investigate the optimal duration of dual antiplatelet therapy(DAPT)in patients with diabetes Mellitus(DM)requiring complex percutaneous coronary intervention(PCI).METHODS A total of 2403 patients with DM who underwent complex PCI from January to December 2013 were consecutively enrolled in this observational cohort study and divided according to DAPT duration into a standard group(11-13 months,n=689)and two prolonged groups(13-24 months,n=1133;>24 months,n=581).RESULTS Baseline characteristics,angiographic findings,and complexity of PCI were comparable regardless of DAPT duration.The incidence of major adverse cardiac and cerebrovascular event was lower when DAPT was 13-24 months than when it was 11-13 months or>24 months(4.6%vs.8.1%vs.6.0%,P=0.008),as was the incidence of all-cause death(1.9%vs.4.6%vs.2.2%,P=0.002)and cardiac death(1.0%vs.3.0%vs.1.2%,P=0.002).After adjustment for confounders,DAPT for 13-24 months was associated with a lower risk of major adverse cardiac and cerebrovascular event[hazard ratio(HR)=0.544,95%CI:0.373-0.795]and all-cause death(HR=0.605,95%CI:0.387-0.944).DAPT for>24 months was associated with a lower risk of all-cause death(HR=0.681,95%CI:0.493-0.942)and cardiac death(HR=0.620,95%CI:0.403-0.952).The risk of major bleeding was not increased by prolonging DAPT to 13-24 months(HR=1.356,95%CI:0.766-2.401)or>24 months(HR=0.967,95%CI:0.682-1.371).CONCLUSIONS For patients with DM undergoing complex PCI,prolonging DAPT might improve the long-term prognosis by reducing the risk of adverse ischemic events without increasing the bleeding risk.

Background Individuals with type 2 diabetes Mellitus(T2DM)are more vulnerable to social disconnection compared with the general population;however,there are few relevant studies investigating this issue.Aims To investigate whether social isolation or loneliness may be associated with subsequent risk of developing major adverse cardiovascular events,whether these associations vary according to fatal and non-fatal outcomes and how behavioural,psychological and physiological factors mediate these associations.Methods This longitudinal analysis included data from 19360 individuals with T2DM at baseline(2006-2010)from the UK Biobank.Social isolation and loneliness were measured using self-report questionnaires.The study outcomes included the first events of myocardial infarction(MI)or stroke(n=2273)and all-cause(n=2820)or cardiovascular disease-related mortality through linked hospital data ordeath registries.Results Over a median follow-up of 12.4 years(interquartile range(IQR):11.6-13.3 years),participants who were more socially isolated(most social isolation vs least social isolation)experienced increased risks for all-cause(hazard ratio(HR):1.33,95%confidence interval(Cl):1.19 to 1.47)and cardiovascular disease(HR:1.36,95%Cl:1.17 to 1.59)mortality but not first Ml or stroke.Loneliness(yes vs no)was associated with a greater risk for a composite of incident MI or stroke(HR:1.37,95%Cl:1.19 to 1.57)but not mortality.Social isolation was associated with fatal Ml and stroke,whereas loneliness was associated with non-fatal Ml and stroke.The significant associations of social isolation and loneliness with outcomes were mainly mediated by behavioural factors(mediating proportion:17.8%-28.2%and 17.6%-17.8%,respectively).Conclusions Among individuals with T2DM,social isolation and loneliness are associated with a greater risk of developing major adverse cardiovascular events,with differences in both risks stratified according to fatal and non-fatal events and underlying mediating factors.

To the Editor:Gestational diabetes Mellitus(GDM)is a pathologic condition in which glucose intolerance and insulin resistance develop to different degrees during pregnancy.Women with GDM have higher odds of adverse maternal and fetal outcomes.Patients with a history of GDM were at a significantly increased risk for recurrence of GDM in subsequent pregnancies.Our previous and some other studies found an increased incidence of adverse perinatal outcomes in pregnant with recurrent GDM.^([1])So far,women with a prior history of GDM are not recommended to refrain from subsequent pregnancy in light of the limited available data.What about those women with a history of recurrent GDM?Should women with recurrent GDM be recommended to avoid further pregnancy?Currently,studies on perinatal outcomes of women with re-recurrent GDM and their prognostic factors are very rare.Therefore,this retrospective cohort study was performed to explore perinatal outcomes of pregnant woman with re-recurrent GDM and their prognostic factors.

To the Editor:Type 1 diabetes Mellitus(T1DM)is a progressive disease caused by severe islet b-cell function impairment,resulting in high glucose variability(GV)in patients.[1]While intensive insulin treatment enabled T1DM patients to achieve near-normal glucose,reflected by reduced hemoglobin A1c(HbA1c)levels,stricter HbA1c targets were associated with an increased risk of hypoglycemia,which would compromise patients’quality of life and survival.[2]Recent studies suggest that GV reflects hypoglycemia better than HbA1c alone,and continuous glucose monitoring(CGM)provides a more comprehensive glycemic profile for better glucose indices assessment.[3]Therefore,we aimed to explore the relationship between the coefficient of variability(CV)of blood glucose and hypoglycemia in T1DM patients with different HbA1c levels and to identify the optimal cut-off values of CV for reducing the hypoglycemia risk in Chinese T1DM patients.

To the Editor:Diabetic kidney disease(DKD)is the leading cause of the end-stage renal disease nowadays.[1]The etiology of DKD is complex and remains largely unknown.[2]A genome-wide association study showed that a common missense variant in COL4A3,rs55703767,played a protective role in DKD in type 1 diabetes Mellitus in Europeans,and this effect depended on blood glucose levels.[3]However,data from type 2 diabetes Mellitus(T2DM)are insufficient.

Type 1 diabetes Mellitus(T1DM),an autoimmune disorder,is marked by T cell-mediated destruction of theβcells of the pancreas,which leads to an almost complete loss of the ability to synthesize insulin.[1]This insulin deficiency results in loss of the ability to regulate blood sugar,therefore,exogenous insulin administration is necessary for patients to control blood sugar and to reduce the incidence of related chronic diabetic complications.[2]In addition,a positive correlation between diabetes Mellitus and hypertension has been detected.[3]

Type 2 diabetes Mellitus(T2DM),a worldwide epidemic disease,has caused tremendous economic and social burden,but the pathogenesis remains uncertain.Nowadays,the impact of unrhythmic circadian clock caused by irregular sleep and unhealthy diet on T2DM has be increasingly studied.However,the contribution of the endogenous circadian clock system to the development of T2DM has not yet been satisfactorily explored.It is now becoming clear that the gut microbiota and the circadian clock interact with each other to regulate the host metabolism.Considering all these above,we reviewed the literature related to the gut microbiota,circadian clock,and T2DM to elucidate the idea that the gut microbiota is closely tied to the regulation of the circadian clock in the development of T2DM,which provides potential for gut microbiota-directed therapies to ameliorate the effects of circadian disruptions linked to the occurrence and development of T2DM.
