Rectal cancer staging: Multidetector-row computed tomography diagnostic accuracy in assessment of mesorectal fascia invasion

AIM: To assess the diagnostic accuracy of multidetectorrow computed tomography(MDCT) as compared with conventional magnetic resonance imaging(MRI), in identifying mesorectal fascia(MRF) invasion in rectal cancer ***: Ninety-one patients with biopsy proven rectal adenocarcinoma referred for thoracic and abdominal CT staging were enrolled in this study. The contrast-enhanced MDCT scans were performed on a 256 row scanner(ICT, Philips) with the following acquisition parameters: tube voltage 120 KV, tube current 150-300 m As. Imaging data were reviewed as axial and as multiplanar reconstructions(MPRs) images along the rectal tumor axis. MRI study, performed on 1.5 T with dedicated phased array multicoil, included multiplanar T2 and axial T1 sequences and diffusion weighted images(DWI). Axial and MPR CT images independently were compared to MRI and MRF involvement was determined. Diagnostic accuracy of both modalities was compared and statistically ***: According to MRI, the MRF was involved in 51 patients and not involved in 40 patients. DWI allowed to recognize the tumor as a focal mass with high signal intensity on high b-value images, compared with the signal of the normal adjacent rectal wall or with the lower tissue signal intensity background. The number of patients correctly staged by the native axial CT images was 71 out of 91(41 with involved MRF; 30 with not involved MRF), while by using the MPR 80 patients were correctly staged(45 with involved MRF; 35 with not involved MRF). Local tumor staging suggested by MDCT agreed with those of MRI, obtaining for CT axial images sensitivity and specificity of 80.4% and 75%, positive predictive value(PPV) 80.4%, negative predictive value(NPV) 75% and accuracy 78%; while performing MPR the sensitivity and specificity increased to 88% and 87.5%, PPV was 90%, NPV 85.36% and accuracy 88%. Autoimmune pancreatitis:Multimodality non-invasive imaging diagnosis

Autoimmune pancreatitis (AIP) is characterized by obstructive jaundice, a dramatic clinical response to steroids and pathologically by a lymphoplasmacytic infiltrate, with or without a pancreatic mass. Type 1 AIP is the pancreatic manifestation of an IgG4-related systemic disease and is characterized by elevated IgG4 serum levels, infiltration of IgG4-positive plasma cells and extrapancreatic lesions. Type 2 AIP usually has none or very few IgG4-positive plasma cells, no serum IgG4 elevation and appears to be a pancreas-specific disorder without extrapancreatic involvement. AIP is diagnosed in approximately 2%-6% of patients that undergo pancreatic resection for suspected pancreatic cancer. There are three patterns of autoimmune pancreatitis: diffuse disease is the most common type, with a diffuse, “sausage-like” pancreatic enlargement with sharp margins and loss of the lobular contours; focal disease is less common and manifests as a focal mass, often within the pancreatic head, mimicking a pancreatic malignancy. Multifocal involvement can also occur. In this paper we describe the features of AIP at ultrasonography, computed tomography, magnetic resonance and positron emission tomography/computed tomography imaging, focusing on diagnosis and differential diagnosis with pancreatic ductal adenocarcinoma. It is of utmost importance to make an early correct differential diagnosis between these two diseases in order to identify the optimal therapeutic strategy and to avoid unnecessary laparotomy or pancreatic resection in AIP patients. Non-invasive imaging plays also an important role in therapy monitoring, in follow-up and in early identification of disease recurrence.

Dual Radial-Resonant Wide Beamwidth Circular Sector Microstrip Patch Antennas

In this article, a design approach to a radial-resonant wide beamwidth circular sector patch antenna is advanced. As properly evolved from a U-shaped d ipole, a prototype magnetic dipole can be fit in the radial direction of a circular sector patch radiator, with its length set as the positive odd-integer multiples of onequarter wavelength. In this way, multiple TM0m(m = 1,2,...) modes resonant circular sector patch antenna with short-circuited circumference and widened E-plane beamwidth can be realized by proper excitation and perturbations. Prototype antennas are then designed and fabricated to validate the design approach. Experimental results reveal that the E-plane beamwidth of a dual-resonant antenna fabricated on air/Teflon substrate can be effectively broadened to 128°/120°, with an impedance bandwidth of 17.4%/7.1%, respectively. In both cases, the antenna heights are strictly limited to no more than 0.03-guided wavelength. It is evidently validated that the proposed approach can effectively enhance the operational bandwidth and beamwidth of a microstrip patch antenna while maintaining its inherent low profile merit.

Active shim coils design for Halbach magnet based on inverse boundary element method

With advantages of small volume,high field strength,and compact stray field,the Halbach array magnet is of great potential in desktop NMR ***,field inhomogeneity poses a main obstacle to its *** shimming is necessary for improving the field *** active shim coil design method for cylindrical Halbach magnet based on inverse boundary element method(iBEM)is introduced in this *** coils targeting 1st~3rd and Z4/Z5/Z6 spherical harmonic(SH)terms were designed with constraints of field accuracy,resistance,inductance,and current *** loops and symmetric property influence were discussed to further improve the performance of high-order shim ***,with the prototype of the shim coils constructed,field pattern and SH transformation proved that the coils designed could fit the target SH distribution,though contamination from low-order components existed in highorder terms.A practical shimming experiment on the Halbach magnet has improved the homogeneity from 40.3 ppm to 3.8 ppm in radius 2.5 mm,height 5 mm cylindrical sample space,verifying the coils'compensation *** work provides a flexible inverse coil design method for cylindrical Halbach magnet and discusses the high-order shim coil design and implementation,which is potential in nuclear magnetic resonance applications based on Halbach magnet arrays.

Autoimmune pancreatitis in the context of IgG4-related disease:Review of imaging findings

Current understanding of autoimmune pancreatitis (AIP) recognizes a histopathological subtype of the disease to fall within the spectrum of IgG4-related disease. Along with clinical, laboratory, and histopathological data, imaging plays an important role in the diagnosis and management of AIP, and more broadly, within the spectrum of IgG4-related disease. In addition to the defined role of imaging in consensus diagnostic protocols, an array of imaging modalities can provide complementary data to address specific clinical concerns. These include contrast-enhanced computed tomography (CT) and magnetic resonance (MR) imaging for pancreatic parenchymal lesion localization and characterization, endoscopic retrograde and magnetic resonance cholangiopancreatography (ERCP and MRCP) to assess for duct involvement, and more recently, positron emission tomography (PET) imaging to assess for extra-pancreatic sites of involvement. While the imaging appearance of AIP varies widely, certain imaging features are more likely to represent AIP than alternate diagnoses, such as pancreatic cancer. While nonspecific, imaging findings which favor a diagnosis of AIP rather than pancreatic cancer include: delayed enhancement of affected pancreas, mild dilatation of the main pancreatic duct over a long segment, the “capsule” and “penetrating duct” signs, and responsiveness to corticosteroid therapy. Systemic, extra-pancreatic sites of involvement are also often seen in AIP and IgG4-related disease, and typically respond to corticosteroid therapy. Imaging by CT, MR, and PET also play a role in the diagnosis and monitoring after treatment of involved sites.
