Pulmonary drugs and genes delivery systems have become an attractive approach in the treatment of lung diseases with the advantages of minimal drug loss,precise quantization,high local drugs concentration and low side ***,special attention should be paid to the biological barriers of the *** delivery systems need to be designed carefully in order to overcome these biological barriers and maximize the drugs and genes deposition to the *** drugs and genes delivery systems can offer some encouraging results for delivering drugs and genes to the ***,some lipid-based and polymer-based delivery systems could control and sustain the release of drugs and genes with minimal or negligible *** review focuses on the latest development of pulmonary delivery of drugs and genes for the treatment of various lung *** barriers to the lung,the delivery devices,the route of the administration and the delivery systems for drugs and genes pulmonary delivery are *** on the rapid development of clinical requirement,novel approaches using delivery systems for pulmonary delivery drugs and genes need to be constructed,which is believed to play an important role in the therapy of lung diseases in the future.

Objective:To investigate the constituent ratio and clinical features of diffuse interstitial lung disease(DILD)in Tianjin,and to identify the existing problems of its diagnosis and ***:Data of all patients diagnosed as DILD in ten comprehensive hospitals in Tianjin from 2003 to 2009 was *** disease constituent ratio of DILD to all kinds of diseases in the respiration department or in the whole hospital,the clinical manifestation,lung function,the result of blood gas analysis,and biopsy were ***:A total of 804 DILD patients were included in the study aged(69.28±12.09)years with a male/female ratio of *** disease constituent ratios of DILD were 4.52‰in respiratory department and 0.31‰in the whole hospital in 2003,which increased to 35.9‰and 1.51‰respectively in *** abnormality(43.1%),ground glass(25.6%)and honeycombing(11.4%)were the predominant imaging manifestations.26.7%patients’lung function were tested,and 10.3%had diffusion function *** patients presented with hypoxemia,and the average oxygenation index was(281±98.3)*** 4.0%of patients underwent bronchoalveolar lavage,and 3.5%had pathologic ***:The disease constituent ratio of DILD increased progressively from 2003 to 2009.A definite diagnosis of DILD demands cooperation of physicians,radiologists and *** should attach more importance on lung function test,bronchoalveolar lavage and biopsy.

Diffuse cystic lung diseases are uncommon but can present a diagnostic challenge because increasing number of diseases have been associated with this *** in the lung is defined as a round parenchymal lucency with a well-defined thin wall(disease.“Diffuse”distribution in the lung implies involvement of all *** lung involvement with cystic lesions can be seen in pulmonary lymphangioleiomyomatosis,pulmonary Langerhans’cell histiocytosis,lymphoid interstitial pneumonia,Birt-Hogg-Dubésyndrome,amyloidosis,light chain deposition disease,honeycomb lung associated with advanced fibrosis,and several other rare causes including metastatic ***-resolution computed tomography of the chest helps define morphologic features of the lung lesions as well as their distribution and associated features such as intrathoracic *** the tempo of the disease process and clinical context with chest imaging findings serve as important clues to defining the underlying nature of the cystic lung disease and guide diagnostic evaluation as well as management.

Purpose:To explore the effects of self-management education on the quality of life of patients living with chronic obstructive pulmonary disease(COPD).Methods:Eighty-four stable or discharged COPD subjects were recruited from April 2011 to January 2012 following treatment at Beijing Hospital or Peking Union Medical College *** were divided into an intervention group who underwent selfmanagement education or a control group who received usual care without additional education(n=42 each).The St George’s Respiratory Questionnaire(SGRQ)was used to measure quality of life at three and six ***:SGRQ impact domain scores revealed significant differences between the groups(t=2.167,p<0.05)at ***,impact,activity domain and total scores revealed significant differences between groups(t=3.482 to2.530,p<0.05)at six ***:A nurse-led,simple,structured self-management education program provided an effective method for the management of patients with COPD.

Objective To study changes in plasma fibrinolysis and their relationship with pulmonary artery pressures in neonates with lung diseases. Methods Plasma activities of the tissue plasminogen activator (TPA) and tissue plasminogen activator inhibitor (PAI) were measured by chromogenic substrate methods in 27 newborns with lung diseases [10 with the respiratory distress syndrome (RDS), 12 with the meconium aspiration syndrome (MAS) and 5 with pneumonia] and in 25 normal newborns (controls). The ratio of the time of peak velocity (TPV) to the right ventricular ejection time (RVET) was determined using an Aloka SSD650 ultrasound system incorporating pulse waves to evaluate pulmonary artery pressures. Results PAI activity was significantly higher in infants with lung diseases [( 9.3 ± 4.1 ) AU×10 -1 /ml] than in the controls [( 5.5 ± 3.0 ) AU×10 -1 /ml] (Plung diseases ( 0.29 ± 0.05 ) than in the controls ( 0.34 ± 0.08 )(Plung diseases. In 10 infants who developed pulmonary hemorrhage, TPA activity [( 1.8 ± 0.7 ) IU×10 -1 /ml)] and platelet counts [( 98 ±39)×10 9/L] in the acute phase of injury were significantly lower than those recorded during the recovery stage [( 3.7 ± 1.7 ) IU×10 -1 /ml and (180±30)×10 9/L, respectively] (P< 0.01 ). Conclusions Following pulmonary hypertension, endothelial cell injury results in a lower release of TPA and increased PAI activity.

Development and Validation of A Patient Reported Outcome Instrument for Chronic Obstructive Pulmonary diseases

Objective： To develop and validate a specific patient reported outcome （PRO） for chronic obstructive pulmonary disease （COPD） patients （COPD-PRO） at a set of standardized procedures. Methods： Literature analysis, interview and group discussion were performed to draft an initial model of COPD-PRO. Thereafter, 65 clinicians and experts throughout China reviewed the draft scale, Then cognitive debriefing interviews with 40 patients were conducted to assess respondent comprehension of the scale. After that, the revised scale was validated through pre-testing and field-testing. Finally, the psychometric properties of the COPD-PRO were evaluated by indicators such as validity, reliability and responsiveness based on the data from 230 patients. Results： The COPD-PRO contained 17 items in 3 domains： amelioration of clinical symptoms, satisfaction of health condition and satisfaction of treatment effect. The Cronbach＇s ~, Split-half coefficient and test-retest coefficient were 0.806, 0.744, 0.703, respectively; the correlation coefficients between domains and overall scale were 0.835-0.963; 5 factors were extracted according to the conceptual model. The differences of the scale scores before and after treatment were statistically significant （P=0.000）. Conclusions： The COPD- PRO has good validity, reliability and responsiveness. The COPD-PRO could provide patients＇ response to the treatments and then evaluate the effect of treatment in a standardized way.

The lungs, as a place of gas exchange, are continuously exposed to environmental stimuli, such as allergens, microbes, and pollutants. The development of the culture-independent technique for microbiological analysis, such as 16 S rRNA sequencing,has uncovered that the lungs are not sterile and, in fact, colonized by diverse communities of microbiota. The function of intestinal microbiota in modulating mucosal homeostasis and defense has been widely studied; however, the potential function of lung microbiota in regulating immunity and homeostasis has just begun. Increasing evidence indicates the relevance of microbiota to lung homeostasis and disease. In this review, we describe the distribution and composition of microbiota in the respiratory system and discuss the potential function of lung microbiota in both health and acute/chronic lung disease. In addition, we also discuss the recent understanding of the gut-lung axis, because several studies have revealed that the immunological interaction among the gut, the lung, and the microbiota was involved in this issue.
