Intraoperative hypotension happens in everyday clinical practice. It was suggested to have a strong association with adverse postoperative outcomes. Hypotension prediction index(HPI) was developed to predict Intraoperative hypotension(mean arterial pressure Intraoperative blood pressure management. We found that cerebral oxygenation was maintained in the event of hypotension during surgery. The patient had no Intraoperative or postoperative adverse outcomes despite the hypotension. We believe this can provide an individualized Intraoperative blood pressure management to avoid over-or under-treating hypotension.

Neutrophil peptide 1 belongs to a family of peptides involved in innate immunity. Continuous intramuscular injection of neutrophil peptide 1 can promote the regeneration of peripheral nerves, but clinical application in this manner is not convenient. To this end, the effects of a single Intraoperative administration of neutrophil peptide 1 on peripheral nerve regeneration were experimentally observed. A rat model of sciatic nerve crush injury was established using the clamp method. After model establishment, a normal saline group and a neutrophil peptide 1 group were injected with a single dose of normal saline or 10 μg/mL neutrophil peptide 1, respectively. A sham group, without sciatic nerve crush was also prepared as a control. Sciatic nerve function tests, neuroelectrophysiological tests, and hematoxylin-eosin staining showed that the nerve conduction velocity, sciatic functional index, and tibialis anterior muscle fiber cross-sectional area were better in the neutrophil peptide 1 group than in the normal saline group at 4 weeks after surgery. At 4 and 8 weeks after surgery, there were no differences in the wet weight of the tibialis anterior muscle between the neutrophil peptide 1 and saline groups. Histological staining of the sciatic nerve showed no significant differences in the number of myelinated nerve fibers or the axon cross-sectional area between the neutrophil peptide 1 and normal saline groups. The above data confirmed that a single dose of neutrophil peptide 1 during surgery can promote the recovery of neurological function 4 weeks after sciatic nerve injury. All the experiments were approved by the Medical Ethics Committee of Peking University People's Hospital, China(approval No. 2015-50) on December 9, 2015.

Research into medical artificial intelligence(AI)has made significant advances in recent years,including surgical applications.This scoping review investigated AI-based decision support systems targeted at the Intraoperative phase of surgery and found a wide range of technological approaches applied across several surgical specialties.Within the twenty-one(n=21)included papers,three main categories of motivations were identified for developing such technologies:(1)augmenting the information available to surgeons,(2)accelerating Intraoperative pathology,and(3)recommending surgical steps.While many of the proposals hold promise for improving patient outcomes,important methodological shortcomings were observed in most of the reviewed papers that made it difficult to assess the clinical significance of the reported performance statistics.Despite limitations,the current state of this field suggests that a number of opportunities exist for future researchers and clinicians to work on AI for surgical decision support with exciting implications for improving surgical care.

With the development of color Doppler and laparoscopic ultrasound, now Intraoperative ultrasound(IOUS) plays an important role in liver surgery. Compared to percutaneous ultrasound(US), IOUS is conducted directly on the liver surface, with no blind spots or dead ends, thus improves the detection, localization and characterization of lesions without influencing factors, such as obesity.

Computer assisted surgical planning allowed for a better selection of patients,evaluation of operative strategy, appropriate volumetric measurements,identification of anatomical risks, definition of tumour resection margins and choice of surgical approach in liver oncologic resections and living donor liver transplantations. Although preoperative computer surgical analysis has been widely used in daily clinical practice, Intraoperative computer assisted solutions for risk analysis and navigation in liver surgery are not widely available or still under clinical evaluation. Computer science technology can efficiently assist modern surgeons during complex liver operations, mainly by providing image guidance with individualized 2 D images and 3 D models of the various anatomical and pathological structures of interest. Intraoperative computer assisted liver surgery is particularly useful in complex parenchyma-sparing hepatectomies, for Intraoperative risk analysis and for the effective treatment of colorectal metastases after neoadjuvant therapy or when they are multiple. In laparoscopic liver surgery, Intraoperative computer aid is definitively more important as, apart from a restricted field of view, there is also loss of the fine haptic feedback. Intraoperative computer assisted developments face challenges that prevent their application in daily clinical practice. There is a vast variety of studies regarding Intraoperative computer assisted liver surgery but there are no clear objective measurements in order to compare them and select the most effective solutions. An overview of up-to-date Intraoperative computer assisted solutions for liver surgery will be discussed.

Background Intraoperative hypothermia(core temperatureIntraoperative hypothermia and its risk factors in pediatric patients during burn surgery.Methods In the present study we enrolled 197 pediatric patients with burn injury undergoing surgical debridement and skin grafting.Factors,such as total burn surface area(TBSA),were collected and analyzed to identify the potential risk factors for Intraoperative hypothermia.Results The incidence of Intraoperative hypothermia among all patients was 17.8%.Compared with patients with normothermia,children with hypothermia were associated with larger TBSA(25%vs 15%,pIntraoperative active warming(34.28%vs 54.93%,pIntraoperative hypothermia[severe:odds ratio(OR)=3.805,95%confidence interval(CI)=1.396-10.368,p=0.009;extremely severe:OR=6.933,95%CI=2.604-18.462,pIntraoperative hypothermia(OR=1.068,p=0.001)and could be used to predict the occurrence of hypothermia when combined with other factors.TBSA for predicting Intraoperative hypothermia by receiver operating characteristic(ROC)curve analysis showed a good predictive ability with an area under the ROC curve of 0.758.Conclusion TBSA is an important risk factor for Intraoperative hypothermia in pediatric patients with burn.

With the development of endoscopic techniques and equipment,endoscopic surgery has been widely performed to treat many diseases,such as early gastrointestinal cancer,submucosal tumors,and achalasia.Endoscopic surgery can be classified as digestive endoscopic tunnel technique (DETT) and non-tunnel technique.The non-tunnel technique includes endoscopic mucosal resection,endoscopic piecemeal mucosal resection,endoscopic submucosal dissection (ESD),multi-band mucosectomy,endoscopic submucosal excavation,and endoscopic full-thickness resection.[1] Endoscopic submucosal tunnel dissection (ESTD),peroral endoscopic myotomy (POEM),and submucosal tunneling endoscopic resection (STER) are considered types of DETT.Endoscopic resection is safer and more minimally invasive than open surgery and video-assisted surge ry.[2,3] The complication rate of resection is regarded as the main standard to evaluate the safety of endoscopic surgery,while bleeding and “perforation” are reported to be the most common Intraoperative complications.[4,5]

Intraoperative consultation includes gross evaluation and frozen section,whitch is an integral component of surgical oncology for many reasons.First,it allows appropriate surgical procedures to be undertaken.Second,it can potentially protect the patient from a second procedure.Thitd,it helps towards counselling of paients and their family members and lasty,it faclilitates early management planning.Udlizaion of frozen section is justified by the reported sensi dvity,specificity,positive and negativwe predictive values all of which are as high as 90-100%in some studies.It is very impor tant for gynecologists to have good communication with pathologi sts,especially for cass with an indeterminate,frozen result.Discussion with the pathologist can help the gynecologist understand the diagnostic dilemma and the differential diagnosis,which can help them make best surgical decisions.

Objective: Fluid therapy is one of the key components of perioperative management. However, evidence of Intraoperative fluid(IOF) administration affecting clinical outcomes following McKeown esophagogastrectomy remains limited. This study investigated the impact of IOF on clinical outcomes after McKeown esophagogastrectomy.Methods: Patients who underwent McKeown esophagogastrectomy between July 2013 and July 2016 were identified. Preoperative, Intraoperative and postoperative variables for each eligible patient were retrospectively collected from our electronic medical records and anesthetic records. IOF rates were determined and their relationships to postoperative clinical outcomes were compared.Results: A total of 546 patients were enrolled in the analysis. The median IOF rate was 8.87 mL/kg/h. We divided the patients into two groups: a low fluid volume group(LFVG <8.87 mL/kg/h, n=273) and a high fluid volume group(HFVG ≥8.87 mL/kg/h, n=273). No statistically significant differences in postoperative clinical outcomes were found between LFVG and HFVG either before or after propensity score matching.Conclusions: No effect of IOF administration on clinical outcomes in patients undergoing McKeown esophagogastrectomy was identified. Further high-quality studies examining the influence of IOF administration on clinical outcomes following McKeown esophagogastrectomy are still needed.

AIM: To report the Intraoperative complications associated with small incision lenticule extraction(SMILE) and their management. METHODS: This was a retrospective consecutive interventional clinical study, carried out on patients with myopia and myopic astigmatism, who underwent SMILE procedure. Type of Intraoperative complications and their management were recorded. RESULTS: Our study comprised 282 eyes of 141 patients who were enrolled for SMILE surgeries. The Intraoperative complications included lost vacuum(18 eyes, 6.38%), treatment decentration(6 eyes, 2.12%), wound bleeding(21 eyes, 7.45%), incomplete bubble separation(black islands)(3 eyes, 1.06%), the epithelial defects(15 eyes, 5.32%). Incision tear(27 eyes, 9.57%), lenticule adherence to the cap(6 eyes, 2.12%), and cap perforation occurred in 2 eyes(0.7%). CONCLUSION: Although SMILE is a promising technique for the correction of myopia and myopic astigmatism with predictable, efficient, safe refractive and visual outcomes, complications can occur. However, most of them are related to inexperience and are included in the learning curve of the technique. More studies with a bigger number of eyes are required to efficiently evaluate the Intraoperative complications and standardize their management strategies.
