Using no-cost mobile phone reminders to improve attendance for hiv test results: a pilot study in rural Swaziland

Background:Mobile technology has great potential to improve adherence and treatment outcomes in healthcare settings.However,text messaging and phone calls are unaffordable in many resource-limited areas.This study investigates the use of a no-cost alternative mobile phone technology using missed calls(‘buzzing’)to act as a patient reminder.The use of missed calls as a patient reminder was evaluated for feasibility and effectiveness as an appointment reminder in the follow-up of newly-diagnosed human immunodeficiency virus(hiv)positive patients in an hiv testing and counselling department in rural Swaziland.Methods:This pilot study uses a before-and-after operational research study design,with all patients with mobile phones being offered the intervention.The primary outcome was the rate of attendance at the hiv testing and counselling department for collection of results in those with mobile phones before and after the introduction of the intervention.Results:Over two-thirds,71.8%(459/639),of patients had a mobile phone.All patients with a mobile phone consented to being buzzed.There was no difference in attendance for follow-up at the clinic before and after the intervention was implemented(80.1%versus 83.3%,p=0.401),or after adjusting for confounding factors(OR 1.13,p=0.662).Conclusion:This pilot study illustrates that mobile technology may be feasible in rural,resource-poor settings as there are high rates of mobile phone ownership and the intervention had a 100%uptake rate,with positive feedback from staff and patients.In this particular setting,the intervention did not improve attendance rates.However,further research is planned to investigate the impact on adherence to appointments and medications in other settings,such as hiv chronic care follow-up and as part of an enhanced package to improve adherence.

Association of HLA-B Alleles With Human Immunodeficiency Virus Type 1 Infection in the Yi Ethnic Group in Sichuan Province

Objective To determine the distribution of HLA-B alleles in the Chinese Yi ethnic group and its association with hiv infection. Methods One hundred and six unrelated healthy hiv negative and 73 hiv positive Chinese Yi ethnic individuals were typed by PCR-SSP. Results The frequency of alleles B*07, B*35, and B*46 were increased in HTV-1 -positive subjects, whereas the alleles B*55, B*44 and B*78 were absent in the hiv-infected persons studied. The B*46 allele was present in a significantly higher gene frequency among hiv-1-positive individuals (P=0.02, OR=3.32, 95% CI=1.13-9.78) compared with control subjects. Conclusion HLA-B*46 may be associated with its susceptibility to hiv-1 infections.

Distribution of natural killer cell receptors in hiv infected individuals

Natural killer （NK） cells are bone marrow derived, large granular lymphocytes, comprising approximately 10% to 20% of the mononuclear cell fraction in normal peripheral blood. They form a part of the first line defense mechanism against tumoural and viral spreading^1-4 Unlike T and B cells, NK cells do not require gene rearrangement for assembly of their receptor genes; rather, NK cells discriminate potential target cells based on the levels of self major histocompatibility complex （MHC） class I expression on such cells^5.6 There are two kinds of NK cell receptors^2.7.8 Inhibitory receptors recognize MHC class I molecules and deliver a downregulatory signal that inactivates the lytic machinery of NK cells. Stimulatory receptors expressed by NK cells deliver an activation signal.

Nineteen lymph nodes of a hiv-positive boywere studied histologically and immunohistologically.According to Stutte’s classincation of hiv-relatedlymphadenopathy,84% of the lymph nodes were at thethird or fourth stages in relation to clinical statusARC/AIDS.Lymph follicle atrophy,angiogenesis,histiocytic proliferation and destruction of the normalreticulum frame were observed.Immunohistochemicalstudies showed most of the remaining lymphocytes tobe T cells and changes in the distribution of S-100 pos-itive cells.Ki 1 positive cells existed mainly at the sec-ond stage.The significance of these changes is dis-cussed.

Antiretroviral therapy improves neurocognitive impairment in people living with hiv? A meta-analysis

Objectives:Although effective antiretroviral therapy(ART)has been used for more than two decades,hiv-associated neurocognitive disorder remains prevalent.Thus,whether ART can improve neurocognitive impairment is controversial.This review aims to explore the effects of ART on cognitive impairment in people living with hiv(PLWH).Methods:A systematic literature search was conducted in eight databases(PubMed,Embase,Web of Science,Cochrane Library,CNKI,VIP,China Biology Medicine disc,and WanFang)to identify studies that compare cognitive function between study groups who are administered and not administered ART.We searched for articles published up to April 2019.Article evaluation and data extraction were independently conducted by two reviewers.Results:Sixteen articles(6,694 participants)-14 cross-sectional studies and 2 cohort studies—were included in this meta-analysis.The cross-sectional studies demonstrated that ART group did not perform better than the non-ART group(OR=1.16;95%CI,1.03-1.30).However,the cohort studies reported a significant improvement in cognitive function at three months(OR=4.01;95%CI,2.35-6.85)and six months(OR=9.24;95%CI,1.71-49.96)after ART initiation compared with the baseline data.No significant cognitive improvement was found in participants younger than 55 years old,but the two crosssectional studies showed that ART may improve cognitive function in PLWH under 65 years old with poor physical condition and immune status.Conclusions:ART could improve cognitive function in PLWH with poor physical condition and immune status,but it does not considerably improve cognition in the entire PLWH population.

Background:Tuberculosis infection still places a great burden on hiv-infected individuals in China and other developing countries.Knowledge of the survival of hiv-infected patients with pulmonary tuberculosis(PTB)would provide important insights for the clinical management of this population,which remains to be well described in current China.Methods:hiv-infected patients with PTB admitted to Shanghai Public Health Clinical Center from January 2011 to December 2015 were retrospectively enrolled.In this cohort,the survival prognosis was estimated by the Kaplan-Meier method,while univariate and multivariate Cox proportional hazards models were used to determine the risk factors affecting mortality.Results:After reviewing 4914 admitted patients with hiv infection,359 PTB cases were identified.At the time of PTB diagnosis,the patients’median CD4+T cell count was 51/mm3(IQR:23-116),and 27.30%of patients(98/359)were on combination antiretroviral therapy(cART).For the 333 cases included in the survival analysis,the overall mortality was 15.92%(53/333)during a median 27-month follow-up.The risk factors,including age older than 60 years(HR:3.18;95%CI:1.66-6.10),complication with bacterial pneumonia(HR:2.64;95%CI:1.30-5.35),diagnosis delay(HR:2.60;95%CI:1.42-4.78),CD4+T cell count less than 50/mm3(HR:2.38;95%CI:1.27-4.43)and pulmonary atelectasis(HR:2.20;95%CI:1.05-4.60),might independently contribute to poor survival.Among patients without cART before anti-TB treatment,the later initiation of cART(more than 8 weeks after starting anti-TB treatment)was found to increase the mortality rate(OR:4.33;95%CI:1.22-15.36),while the initiation of cART within 4-8 weeks after starting anti-TB treatment was associated with the fewest deaths(0/14).Conclusions:The subjects in this study conducted in the cART era were still characterized by depressed immunological competence and low rates of cART administration,revealing possible intervention targets for preventing TB reactivation in hiv-infected individuals under current circumstances.F

Factors influencing the natural history of hiv-1 infection

The natural history of hiv is defined as the untreated progression from the time of hiv transmission to death. The typical course of infection is characterized by three phases： acute or primary infection phase, the asymptomatic phase, and AIDS. Primary infection is often accompanied by a symptomatic acute retroviral syndrome starting 2-3 weeks after transmission and lasting an additional 2-4 weeks. The asymptomatic phase, in contrast, is long with a duration that varies widely. While there is little consensus on how to divide patients based on their duration of asymptomatic phase, multiple clinical groups have been described in the literature including rapid progressors, typical progressors, long-term nonprogressors, and elite controllers. Approximately 5% to 15% of asymptomatic subjects considered as rapid progressors experience a sharp decline in CD4^＋ T cell counts progressing to AIDS within 2 to 5 years.

Cohort Study of hiv Infection among Drug Users in Ruili,Longchuan and Luxi of Yunnan Province,China

In March 1992, KAP investigation and hiv blood test were carried out for 860 drug users and 82 spouses in Ruili, Luxi, Longchuan of Yunnan Province, China.The results showed that there were 285 injecting drug users (IDUs) (33.1%) among 860 drug users. Among 282 blood samples of IDUs, the hiv infection rate was 49.0%, highest in Ruili (81.8%, 63/77), then Longchuan (44.6%, 74/166), lowest in Luxi county (5.1 %, 2/39). Twelve new hiv ( +) were found from 75 persons, who had been tested as hiv (-) in recent two years. Sixty-two blood samples were collected among 82 spouses of IDUs with HI V ( + ), and 6 were hiv ( + ) (9.8%), with an increase of 6.7% compared with results of the investigation two years ago (3.1%, 2/64).

Since the introduction of antiretroviral therapy, the life expectancy of people living with human immunodeficiency virus (PLWH) has extended. This extension has led to an increase in the aging population in Japan and globally. Providing appropriate long-term care (LTC) for PLWH has thus become increasingly critical. Our study aimed to describe LTC setting preferences and related factors among middle-aged and older Japanese people living with hiv. A cross-sectional survey was conducted at two hospitals in Tokyo. One hundred seventy-five outpatients aged 40 years and above participated in this study. Participants completed an anonymous self-administered questionnaire to assess where they wanted to live once they could no longer care for themselves. Approximately 52.0% preferred a designated facility for older adults or LTC, while 30.3% preferred their home or living with family, a partner, or a friend (“familiar housing”). Bivariate analyses revealed that LTC setting preference was significantly associated with marital status, whether or not the participant had at least one child, and household composition. Furthermore, logistic regression analysis revealed that participants living with non-kin were less likely to prefer living in designated housing facilities for older adults or LTC (adjusted odds ratio = 0.17, 95% confidence interval: 0.05 - 0.63). The study findings suggest that family make-up and composition of cohabiters are critical indicators for LTC setting preference in this population. These findings can be the foundation for future care planning and delivery to meet the unique LTC needs and expectations of the aging population with hiv in Japan and similar global settings.

The outbreak of coronavirus disease 2019(COVID-19)caused by the novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)has become a pandemic.SARS-CoV-2 vaccination is currently being actively promoted worldwide as a promising measure to combat the pandemic.However,human immunodeficiency virus(hiv)-infected individuals who manifest varying degrees of immunodeficiency and chronic inflammation may require special consideration with regards to vaccine types and the timing of immunization depending on specific clinical situations.The present recommendation provides a reference for SARS-CoV-2 vaccination in hiv-infected patients.
