Introduction: Foreign bodies (FB) in the lower airways (LAs) constitute a potentially life-threatening emergency requiring immediate management. The objective of our study was to describe the patient pathway and management of foreign bodies in the lower airways. Methodology: This was a descriptive study with retrospective data collection conducted over a 6-year period (January 1, 2014, to December 31, 2019) in the ENT and Head and Neck Surgery Department of Donka National Hospital. Results: We observed a prevalence of 1.79%. The average age was 5.71 years, with a sex ratio of 1.2. However, 82.61% of the cases originated from rural areas. We noted that 78.83% of patients were referred after visiting 1 to 2 healthcare facilities (75.36%). The consultation delay was 3 days. Penetration syndrome was present in 98.56% of cases. The foreign body incidents occurred during play in 54.84% and during meals in 29.09% of cases. Radiographs were performed in 93.9% of cases. Endoscopy was used for both diagnostic and therapeutic purposes in all patients. The foreign bodies were located in the trachea (37.68%), larynx (26.08%), and bronchi (21.73%). Non-organic foreign bodies were predominant (52.17%). Postoperative outcomes were uncomplicated in 95.98% of cases, and the lethality rate was 2.85%. Conclusion: The pathway of patients with foreign bodies in the airways remains unpredictable in our context due to parental hesitations. Early management reduces morbidity and mortality rates.

Mucosal Prolapse of the Urethra in Little Girls about 11 Cases at the Urology Department of the Sino-guinean Friendship Hospital in Conakry (guinea)

Introduction: Urethral prolapse, a rare benign lesion of the female urethra, is considered unusual in children, but it is far from exceptional in girls aged 18 to 24. African origin. The diagnosis is clinical but can be overlooked or confused with other pathologies. Treatment remains controversial. The aim of this work is to study the epidemiological, clinical and therapeutic data of urethral mucosal prolapse in young girls in our department. Materials and methods: Retrospective and descriptive study on urethral mucosal prolapse in little girls identified from the hospitalization records of the Sino-guinean Friendship Hospital in Conakry. The study period was 5 years. Patients were selected randomly. Outcome assessment focused on the occurrence of recurrences and urinary incontinence. The mean follow-up duration was 24 months. The parameters studied were: Age, clinical and histological findings, therapeutic data and patient outcomes. Results: The incidence of urethral mucosal prolapse in girls in the urology department was 2.2 cases/year. The mean age of the patients was 6.7 years (with extremes of 5 months and 10 years). The age group of 7 to 10 years was the most represented, observed in 63.63% of patients. The most frequent reason for consultation was vulvar bleeding in 7 patients, or 63.63%. Malpighian hyperplasia with a congested chorion was the most observed histological type, in 4 patients. Surgical treatment consisting of complete excision of the prolapsed part had given excellent results. Conclusion: Urethral prolapse is a rare pathology, observed mainly in young black girls. Its diagnosis is based mainly on clinical examination. Surgical treatment gives satisfactory results, both clinically and aesthetically.

Background:Cholera is endemic in guinea,having suffered consecutive outbreaks from 2004 to 2008 followed by a lull until the 2012 *** we describe the temporal-spatial and behavioural characteristics of cholera cases in Conakry during a three-year period,including the large-scale 2012 ***:We used the national and African Cholera Surveillance Network(Africhol)surveillance data collected from every cholera treatment centre in Conakry city from August 2011 to December *** prevalence of suspect and confirmed cholera cases,the case fatality ratio(CFR),and the factors associated with suspected cholera were described according to three periods:pre-epidemic(A),epidemic 2012(B)and post epidemic(C).Weekly attack rates and temporal-spatial clustering were calculated at municipality level for period *** was confirmed by culture at the cholera national reference ***:A total of 4559 suspect cases were reported:66,4437,and 66 suspect cases in periods A,B and C,*** the 204 suspect cases with culture results available,6%,60%,and 70%were confirmed in periods A,B,and C,*** 0.3%,the CFR was significantly lower in period B than in periods A(7.6%)and C(7.1%).The overall attack rate was 0.28%in period B,ranging from 0.17%to 0.31%across ***,a cluster of cases was identified in two districts in the northern part of *** 14%,rice water stools were less frequent in period A than in period B and C(78%and 84%).Dehydration(31%vs 94%and 89%)and coma(0.4%vs 3.1%and 2.9%)were lower during period B than in periods A and *** treatment of drinking water was less frequent in period A,while there were more reports of recent travel in period ***:The epidemic dynamic and the sociological description of suspect cases before,during,and after the large-scale epidemic revealed that the Vibrio cholerae was already present before the ***,it appeared that infected individuals react

Nematode distribution in cultivated and undisturbed soils of guinea Savannah and Semi-deciduous Forest zones of Ghana

Climate change affects air temperature, sea levels as well as the soil and its ecosystem. The guinea Savannah and Semi-deciduous Forest zones of Ghana are characterized by different climatic conditions and vegetative cover. Annual average temperature has been steadily increasing whilst annual total rainfall has been decreasing in both zones, and this has been causing a southward shift of the Savannah into the Forest zone. Soil organisms provide crucial ecosystem services which are required for sustainable agriculture and food production yet crop cultivation disturbs the soil ecosystem. The harsh conditions associated with the Savannah further expose the soil ecosystem to disturbance and loss of biodiversity which threatens food production and security. Soil nematodes are the most abundant animals in the soil and play a central and critical role in the soil food web complex. Studying the nematode community structure gives a reflection of the status of the entire soil ecosystem. Soil samples were taken from cultivated and natural landscapes in the guinea Savannah and Semi-deciduous Forest agroecological zones to analyse the nematode community. Results from the study showed the guinea Savannah zone recording warmer soil temperatures, lower organic matter percentage and lower nematode diversity(Genus Richness) as compared to the Semi-deciduous Forest zone. If the Savannah continues to shift southward, the Forest zone soil ecosystem risks disturbance and loss of biodiversity due to the harsh Savannah conditions. Our findings indicate that prevailing crop cultivation practices also disturb soil ecosystem in the two ecological zones which span across West Africa. A disturbed soil ecosystem endangers the future of food production and food security.

Effect of emodin on mobility signal transduction system of gallbladder smooth muscle in guinea pig with cholelithiasis

Objective: To study the effect of emodin on protein and gene expressions of the massagers in mobility signal transduction system of cholecyst smooth muscle cells in guinea pig with cholesterol calculus. Methods: The guinea pigs were randomly divided into 4 groups, such as control group, gall-stone(GS) group, emodin group and ursodesoxycholic acid(UA) group. Cholesterol calculus models were induced in guinea pigs of GS, emodin and UA groups of induced models by lithogenic diet, while emodin or UA were given to the corresponding group for 7 weeks. The histomorphological and ultrastructure change of gallbladder were detected by microscope and electron microscope, the content of plasma cholecystokinin(CCK) and [Ca^(2+)]i were analyzed successively by radioimmunoassay and flow cytometry. The protein and mR NA of Gsα, Giα and Cap in cholecyst cells were determined by western blotting and real time polymerase chain reaction(RT-PCR). Results: Emodin or UA can relieve pathogenic changes in epithelial cells and muscle cells in gallbladder of guinea pig with cholesterol calculus by microscope and transmission electron microscope. In the cholecyst cells of GS group, CCK levels in plasma and [Ca^(2+)]i decreased, the protein and m RNA of GS group were downregulated,the protein and m RNA of Gi and Cap were up-regulated. Emodin significantly decreased the formative rate of gallstone, improved the pathogenic change in epithelial cells and muscle cells, increased CCK levels in plasma and [Ca^(2+)]i in cholecyst cells, enhanced the protein and mR NA of Gs in cholecyst cells, reduced the protein and mR NA of Gi and Cap in cholecyst cells in guinea pig with cholesterol calculus. Conclusion: The dysfunction of gallbladder contraction gives rise to the disorders of mobility signal transduction system in cholecyst smooth muscle cells, including low content of plasma CCK and [Ca^(2+)]i in cholecyst cells, abnormal protein and mRNA of Gs, Gi and Cap. STUDIES ON RELATIONSHIP BETWEEN Na, K-ATPase ACTIVITY AND SPERM CAPACITATION IN guinea PIG

In order to elucidate the biochemical mechanism of sperm capaeitation, the relationship between plasmalemma Na, K-ATPase, capacitation and acrosome reaction was *** of guinea pig spermatozoa was isolated by sucrose gradient centrifugation for the determination of Na, K-ATPase activity. As far as the authors are aware, the Na, K-ATPase activity in plasmalemma of the guinea pig has never been detected. By treating sperm plasmalemma with 0.05% DOC (deoxycholate), enzyme activity could be quantitatively measured. After spermatozoa were incubated in capacitation medium for 8 h, Na, K-ATPase activity was found to be decreased to 35.6% as compared with that before incubation. The spermatozoa incubated for 10.5h in capacitationmedium containing 1 and 5μmol/L ouabain showed 46.5% and 64.4% acrosome reactions respectively, while the acrosome reaction of the control group was only 27.4%. The above results suggest that the decrease in the Na, K-ATPase activity of guinea pig spermatozoa may be a prerequisite for *** results demonstrated for the first time that Na, K-ATPase activity exists in the sperm plasmalemma of the guinea pig. It was further found that the decrease of Na, K-ATPase activity of plasmalemma enhances sperm *** is suggested that sperm capacitation in the guinea pig is possibly induced by the decrease in plasmalemma Na, K-ATPaseand, as a consequence, the intracellular Na^+ is increased, which would benefit the exchange of Na_(out)^+/Ca_(in)^+ and the onset of acrosome reaction.

Effects of aqueous extracts of Irvingia gabonensis seeds on the hormonal parameters of male guinea pigs

Objective:To investigate the effects of the aqueous extracts of Irvingia gabonensis(Irvingaceae) seeds on the hormonal parameters of male guinea ***:The hormonal effects of Irvingia gabonensis on hormonal parameters of male guinea pigs were investigated and compared with that of proviron using enzyme immuno assay method,which was done by reaction of antibody with serum testosterone and testosterone label,magnetic solid phase separation and colour development *** phytochemical screening of Irvingia gabonensis seeds was also carried out using standard ***:The aqueous extract of the Irvingia gabonensis seeds(50-400 mg/kg) caused a statistically significance increase(Pguinea pigs,from (2.70±0.26) ng/mL to(3.10±0.42) ng/mL on the 7th day and to(3.30±0.48) ng/mL on the 28th day of the administration of the *** highest increase was(3.30±0.48) ng/mL,being obtained after 28 days of *** effects were similar to that of proviron,which was (2.80±0.28) ng/mL and(3.00±0.41) ng/mL on the 7th and 28th day of treatment respectively. The phytochemical screening of Irvingia gabonensis seeds revealed the presence of flavonoids. tannin,alkaloids,carbohydrate,volatile oils,terpenoids,saponins and cardiac glycosides. Conclusions:This study supports the claims on the use of the seeds of Irvingia gabonensis by traditional medicine practitioners as a fertility ***,further studies need to be done to isolate and characterize the active principle(s) responsible for this activity in this plant.
