Effects of Malonaldehyde Contents in eschar Extract on Mitochondric Respiratory Function and Energization of the Inner Membrane

Extract of the eschar tissue taken from the rat which had been given full-thickness burn of20% TBSA was found to be inhibitory to the respiratory function of the mitochondria and theenergization of the inner membrane. The inhibition was also demonstrated by the 1: 2 and 1: 4and 1: 8 dilutions of the eschar extract. The extract of normal skin only showed weak inhibitory cf-fect. The 1: 4 dilution of normal skin had no (?) on mitochondrial functions. There is a widegap between the malonaldehyde (MDA) contents of these two kinds of extracts. An apparent posi-tive correlation was found between MDA content and degree of inhibition. However, on close ex-amination, it was found that the MDA content is not the direct cause of the decrease of therespiratory control ratio (RCR). The increase of MDA content is probably related to the destruc-tion of the lipid bilayer of the mitochondria. Data from the energization experiment show that theenergization process is compromised when the destruction of the membrane has proceeded to a cer-tain degrce. The inhibition of mitochondrial respiratory function becomes evident when encrgizationfunction is still normal. Therefore at least two mechanisms are operating in the inhibition *** work is needed to elucidate the problem.

The effects of D1 , an extract of burn eschar , and D'1 , an extract of normal skin of rats , on the induction of chemiluminescense in heparocytic submitochondrial particles were studied. It was found that both D1 and D'1 were able to induce chemiluminescence in hepatocytic submitochondrial particles in a dosage-dependent manner and D1 was more potent than D'1 in this function when they were in the same dosage. When superoxide dismutase (SOD) and beta-carotene were in dosage smaller than 10μg/ml and 3. 2 mmol/L respectively, they were able to suppress the D1's capacity to induce chemiluminescence in the submitochondrial particles in a dosage-dependent manner , too. These findings suggest that there is the occurrence of free radicals in the interaction between D1' and submitochondrial particles and superoxide anions and siglet oxygen seem to be involved.

The effects of toxic substance D1,extracted from burn eschars,on the hepatic mitochondrial respiratory function and its damages to the respirtatory chain were observed in the *** was found that the rate of stste 3respiration was inhibited and that of state 4 respiration stimulated by *** was an obvious dosage-effect relationship between D1 dosage and the rate of oxygen consumption, that is,the changes of the rate was slow in the range of low dosage and fast in the range of high dosage. The point of change was at 1 mg/ml of D1 *** ratio of FCCP-stimulated respiration rate/state 4 respiration rate was decreased when D1 dosage was *** results of experiments were similar whether glutamate+malate or succinate were used as the substrate. The point of change was also at 1 mg/ml of D1 concentration. The decrease of FCCP-stimulated rate/state 4rate implied that the respiratory chain was *** complex Ⅳ-the terminal segment of the chain-remained intact. Thus, the site of damage must be somewhere in complexesⅠ￣Ⅲ and/or complexes Ⅱ￣Ⅲ. Since NADH dehydrogenase and succinate dehydrogenase-the main enzymes in complexesⅠand Ⅱ-were not affected by D1 but NADH: cytochrome c reductase and succinate:cytochrome c reductase markedly inhibited, the site of damage was most likely located in complex Ⅲ-the bc1 complex.

When febrile illnesses develop a rash in children, parents are very concerned about a serious disease. Many rashes associated with fever are caused by infectious diseases. Rashes are generally nonspecific and play a supportive role in differential diagnosis, but for some diseases, the appearance of the rash is essential in making a diagnosis. Here is presented the case of a 4-year-old boy with high fever, headache, abdominal pain, vomiting, diarrhea, and a generalized maculo-papular rash including palms and soles. On physical examination were found a black eschar, cervical lymphadenopathy, and hepatosplenomegaly. Laboratory findings resulted in moderate leukocytosis and moderate involvement of the liver and renal function. Based on this finding, a diagnosis of Mediterranean Spotted Fever was performed. The child recovered after medication with azithromycin. Because there is no reliable test that can confirm MSF in its early stages, the diagnosis is commonly made on the basis of clinical findings, so a high index of suspicion should be maintained while evaluating a child with fever and rash.

Surgical treatment of deep partial thickness to full thickness burn wounds by knife has been the undisputed standard of care and was one key point in surgical burn medicine for decades. Recently, it gets more and more challenged by Bromelain-based enzymatic burn wound debridement (ED) as technique for non-surgical, selective eschar removal. Although the literature on ED is increasing constantly it cannot comprise the rapid progress that is made in clinical application of ED. To outline the current state of art in ED, recent literature as well as clinical experience is summarized and the main steps in clinical application including indications, wound preparation, application of the enzyme, wound bed assessment and further treatment after ED are discussed. Initial indications and limitations in application of ED could be gradually extended to increase versatility of ED as tool in burn surgery. Several randomized controlled trials compared ED to standard of care (SOC). They could show significant shorter time to complete burn wound debridement and wound closure, reduced need for surgery, reduced blood loss, reduced area of burns that needed surgical excision and need for autograft as well as an improved scar quality. Further research is necessary to justify an extensive use of ED as tool for burn eschar removal. Especially a robust comparison to surgical burn wound excision by knife as SOC is required to facilitate evidence-based burn surgery.

BACKGROUND Scrub typhus is a naturally occurring acute infectious disease that is primarily transmitted through the bites of chiggers or larval mites infected by Orientia tsutsugamushi(***).Omadacycline,a novel tetracycline,exhibits potent antibacterial efficacy against both typical bacteria and atypical ***,omadacycline application in the treatment of scrub typhus remains *** SUMMARY In the present work,we report several cases of scrub typhus,with the main clinical symptoms being fever,the formation of eschars or ulcers,local or systemic lymphadenopathy,headache,myalgia and *** samples were collected before omadacycline was administered,and *** infection was confirmed through targeted next-generation sequencing(tNGS).After two days of treatment,the patients’symptoms,including fever,were alleviated,with no adverse drug *** tNGS is an effective method for diagnosing scrub *** can be considered an alternative option for antiinfective therapy in patients with *** infections.

AIM To evaluate the efficacy of Nexobrid~? in the initial management of burns and lessons learned with the *** From January 27^(th) 2015 until January 25^(th) 2016, 25 patients aged between 18-94 years old with deep partial and full thickness burns were treated with Nexobrid~? covering 1%-30% of their total body surface area(TBSA). The debridement was applied in the first 96 h post-injury following the protocol suggested for Nexobrid~?. In patients with burns of more than 15% TBSA a second application of Nexobrid~? was performed. After the removal of the product-4 h post application and after a 2 h period of wet dressing-we used several products to cover the wound like Suprathel~?, Biobrane~?, Mepitel~? with wet dressing, silver sulphadiazine 1% cream, and in some cases even autografts. We treated patients with inhalation injury as well. All the procedures were done under deep sedation, regional blocks in extremities or general anaesthesia in the intensive care unit room or in the operating *** After these first 25 cases, we have observed that patients with partial thickness burns treated with Nexobrid~?, experienced great benefits in the reduction of the need for autografting compared with the standard of care. This isbecause after selective enzymatic debriding of the burn scar we can distinguish different wound beds, which can coexist in the same patient, and we also managed to associate each one to its ability to epithelize. In major burns, besides the improvement in wound healing, we observed an important improvement in their general state. This may be because SIRS significantly improved through a bloodless debridement of necrotic tissue, decreasing the requirements of vasoactive drugs and fluid resuscitation. Circumferential burns also benefited from enzymatic debridement, observing a decrease in the number of compartment syndromes and the need for escharotomies.
