BACKGROUND enhanced recovery after surgery(ERAS)started a revolution that changed age-old surgical stereotypical practices regarding the overall management of the surgical *** the last decade,ERAS has gained significant acceptance in the community of general surgery,in addition to several other surgical specialties,as the evidence of its advantages continues to *** of the last remaining fields,given its significant complexity and intricate nature,is liver transplantation(LT).AIM To investigate the existing efforts at implementing ERAS in *** We conducted a systematic review of the existing studies that evaluate ERAS in orthotopic LT,with a multimodal approach and focusing on measurable clinical primary endpoints,namely length of hospital *** All studies demonstrated a considerable decrease in length of hospital stay,with no readmission or negative impact of the ERAS protocol applied to the postoperative *** ERAS is a well-validated multimodal approach for almost all types of surgical procedures,and its future in selected LT patients seems promising,as the preliminary results advocate for the safety and efficacy of ERAS in the field of LT.

enhanced recovery after Cesarean Section (CS) versus Conventional Care in a Lower Middle-Income Country: A Randomized Controlled Trial

Background: enhanced recovery after surgery (ERAS) has been tested in a wide variety of surgeries with promising outcomes. However, there is a need for a standardized, evidence-informed approach to both the development of new ERAS Society guidelines, and the adaptation and revision of existing guidelines. Developing countries have limited resources and deserve every effort to improve economic status. Aim of the Study: to evaluate perinatal maternal outcomes of ERAS? versus routine care protocols in women undergoing elective cesarean section (CS) in a lower middle-income country with limited resources ranked as a third most country performing CS worldwide using a multidisciplinary team (MDT) management. Design: A prospective randomized Controlled Trial. Setting: Outpatient department (OPD) and labor ward at the Woman’s Health hospital, Assiut University, Egypt. Participants: Healthy pregnant women planned for elective CS (300 women) were randomly divided into a study group offered ERAS protocol and a control group offered regular care. Results: Repeat CS was the main indication of elective CS in both groups without significant difference (89 cases (59.3%) and 75 cases (50%) in both groups respectively). Other indications included cephalopelvic disproportion in 17 cases (11%) and 19 cases (12.6%), placenta previa in 28 (18.6%) and 34 (22.6%) cases, DM in 11 (7%) and 19 (12.6%) cases, and others in 5 (0.3%) and 3 (2%) cases in both groups respectively. The study group took much less time to eat and walk. It had significantly lower pain levels and postoperative problems, as well as much greater women’s satisfaction and a shorter hospital stay (p = 0.001). Conclusions: Collaboration of nursing, obstetricians and anesthesiologists is the cornerstone for a successful ERAS for CS. Significant better perinatal maternal outcomes encourage expansion of ERAS in lower middle-income countries with limited resources.

Mechanism of CO_2 enhanced CBM recovery in China: a review

Permeability of coal reservoirs in China is in general low. Injection of CO2 into coal seams is one of the potential ap-proaches for enhancing coalbed methane (CBM) production. The feasibility of this technology has been investigated in China since the 1990s. Advances in mechanism of CO2 enhanced CBM recovery (CO2-ECBM) in China are reviewed in light of certain aspects, such as the competitive multi-component gas adsorption, sorption-induced coal swelling/shrinkage and its potential effect on CBM production and numerical simulation for CO2-ECBM recovery. Newer investigations for improving the technology are discussed. It is suggested that a comprehensive feasibility demonstration in terms of geology, technology, economics and environment-carrying capacity is necessary for a successful application of the technology for CBM recovery in China. The demonstration should be car-ried out after more investigations into such facets as the control of coal components and structure to a competitive multi-component-gas adsorption, the behavior and essence of super-critical adsorption by coal of gas, environmental and safe feasi-bility of coal mining after CO2 injection and more extensive pilot tests for CO2-ECBM recovery.

Synergistic activation of smithsonite with copper-ammonium species for enhancing surface reactivity and xanthate adsorption

Copper ions(Cu^(2+))are usually added to activate the sulfidized surface of zinc oxide minerals to enhance xanthate attachment using sulfidization xanthate flotation *** adsorption of Cu^(2+)and xanthate on the sulfidized surface was investigated in various systems,and its effect on the surface hydrophobicity and flotation performance was revealed by multiple analytical methods and experiments.X-ray photoelectron spectroscopy(XPS)and time-of-flight secondary ion mass spectrometry(To F-SIMS)characterization demonstrated that the adsorption of Cu^(2+)on sulfidized smithsonite surfaces increased the active Cu—S content,regardless of treatment in any activation *** sulfidized surface pretreated with NH_(4)^(+)-Cu^(2+)created favorable conditions for the adsorption of more Cu^(2+),significantly enhancing the smithsonite *** potential determination,ultraviolet(UV)-visible spectroscopy,Fourier transform-infrared(FT-IR)measurements,and contact angle detection showed that xanthate was chemically adsorbed on the sulfidized surface,and its adsorption capacity in various systems was illustrated from qualitative and quantitative *** comparison to the Na2S–Cu^(2+)and Cu^(2+)–Na2S–Cu^(2+)systems,xanthate exhibited a higher adsorption capacity on sulfidized smithsonite surfaces in NH_(4)^(+)-Cu^(2+)–Na2S–Cu^(2+)***,activation with Cu^(2+)–NH4+synergistic species prior to sulfidization significantly enhanced the mineral surface hydrophobicity,thereby increasing its flotation recovery.

Most of the studies regarding the formation and stability of emulsions focus on the conditioning and management of crude oil on surface *** a great deal of the crude oil produced is in the form of stable emulsions,it is often claimed that these emulsions are formed through chokes and other flow constrictions in oil field ***,emulsions are produced in wells,which not only lack these constrictions but also are produced at low flow rates,demonstrating the fact that emulsions can be formed within the well *** present work reviews the literature regarding the formation and properties of heavy and extra-heavy oil emulsions in naturally fractured porous media due to the current relevance that these types of crude oil exploitation take,satisfying the hydrocarbon energy ***,emulsions have received more attention recently since they can be formed in-situ and improve oil *** understand the flow mechanics of emulsions in porous media,different models to describe their transportation are ***,the formation of emulsions in the reservoir for enhanced oil recovery purposes,including the use of nanoparticlestabilized emulsions is discussed.

Evidence or eminence in abdominal surgery: Recent improvements in perioperative care

Repeated surveys from Europe, the United States, Australia, and New Zealand have shown that adherence to an evidence-based perioperative care protocol, such as enhanced recovery After Surgery(ERAS), has been generally low. It is of great importance to support the implementation of the ERAS protocol as it has been shown to improve outcomes after a number of surgical procedures, including major abdominal surgery. However, despite an increasing awareness of the importance of structured perioperative management, the implementation of this complex protocol has been slow. Barriers to implementation involve both patient- and staff-related factors as well as practice-related issues and resources. To support efficient and successful implementation, further educational and structural measures have to be made on a national or regional level to improve the standard of general health care. Besides postoperative morbidity, biological and physiological variables have been quite commonly reported in previous ERAS studies. Little information, however, has been obtained on cost-effectiveness, long-term outcomes, quality of life and patient-related outcomes, and these issues remain important areas of research for future studies.

Fast-track protocols in laparoscopic liver surgery:Applicability and correlation with difficulty scoring systems

BACKGROUND Few series have reported the utility of fast-track protocols(FTP)in minimally invasive liver *** To report the applicability of FTP in minimally invasive liver surgery and to correlate with difficulty *** The series of patients undergoing minimally invasive liver surgery from 2014 was ***,Southampton and Gayet’s scores were compared as predictors of FTP *** of FTP was considered within 24-h,48-h and *** models were performed to define discharge<24 h,<72 h,complications and *** From 160 cases,78 were candidates for FTP,of which 22(28.2%),19(24.4%)and 14(17.9%)were discharged in<24-h,48-h and 72-h,respectively(total=71.5%).Iwate,Southampton and Gayet’s scores achieved area under the receiver operating characteristic values for<24-h stay of 0.780,0.687 and 0.698,*** and specificity values for the best score(Iwate)were 87.7%and 66.7%,respectively(cutoff=5.5).In multivariate models,<72 h stay and complications revealed body mass index as a risk factor independent from difficulty *** The development of aggressive FTP is feasible and<24-h stay can be achieved even in moderate and advanced complexity *** scores,including body mass index value,may be useful to predict which cases may adhere to these protocols.
