Connectome-based predictive modelling can predict follow-up craving after abstinence in individuals with opioid use Disorders

Background Individual differences have been detected in individuals with opioid use Disorders(OUD)in rehabilitation following protracted abstinence.Recent studies suggested that prediction models were effective for individual-level prognosis based on neuroimage data in substance use Disorders(SUD).Aims This prospective cohort study aimed to assess neuroimaging biomarkers for individual response to protracted abstinence in opioid users using connectome-based predictive modelling(CPM).Methods One hundred and eight inpatients with OUD underwent structural and functional magnetic resonance imaging(fMRI)scans at baseline.The Heroin Craving Questionnaire(HCQ)was used to assess craving levels at baseline and at the 8-month follow-up of abstinence.CPM with leave-one-out cross-validation was used to identify baseline networks that could predict follow-up HCQ scores and changes in HCQ(HCQtolow V-up-HCQpa baseline).Then,the follow-up aseline predictive ability of identified networks was tested in a separate,heterogeneous sample of methamphetamine individuals who underwent MRI scanning before abstinence for SUD.Results CPM could predict craving changes induced by long-term abstinence,as shown by a significant correlation between predicted and actual HCQ fllow-up(r=0.417,p<0.001)and changes in HCQ(negative:r=0.334,p=0.002;positive:r=0.233,p=0.038).Identified craving-related prediction networks included the somato-motor network(SMN),salience network(SALN),default mode network(DMN),medial frontal network,visual network and auditory network.In addition,decreased connectivity of frontal-parietal network(FPN)-SMN,FPN-DMN and FPN-SALN and increased connectivity of subcortical network(SCN)-DMN,SCN-SALNandSCN-SMN were positively correlated with craving levels.Conclusions These findings highlight the potential applications of CPM to predict the craving level of individuals after protracted abstinence,as well as the generalisation ability;the identified brain networks might be the focus of innovative therapies in the future.

Neurodegenerative Disorders are characterized by disruptions to neuronal function and circuitry,leading to a variety of clinical syndromes depending on the affected neuroanatomic regions(Geula,1998).Many proteinopathies implicated in neurodegenerative diseases are characterized by the pathologic accumulation of proteins into inclusions that are initially deposited in specific areas of the brain and spread widely with disease progression.

To the Editor:Placenta accreta spectrum(PAS)Disorders refer to a spectrum of abnormal placentation.PAS subtypes include placenta accreta(creta or adherenta,PA),placenta increta(PI),and placenta percreta(PP).The incidence of PAS has dramatically increased over the past four decades.A study using a nationally representative sample in the United States showed that the prevalence of PAS was 0.29%among women who underwent cesarean delivery with live birth.[1]Women with PAS are at a considerably higher risk of concurrent hemorrhage,bladder and urinary tract injury,and hysterectomy during delivery.[1]

Impact of adverse childhood experiences on the symptom severity of different mental Disorders: a cross-diagnostic study

Background Adverse childhood experiences have a significant impact on different mental Disorders.Objective To compare differences in adverse childhood experiences among those with different mental Disorders and their relationships in a cross-disorder manner.Methods The study included 1513 individuals aged≥18 years:339 patients with substance use Disorders,125 patients with schizophrenia,342 patients with depression,136 patients with bipolar disorder,431 patients with obsessive-compulsive disorder(OCD),and 140 healthy controls.The Early Trauma Inventory Self Report-Short Form was used to investigate childhood traumatic experiences,and the Addiction Severity Index,Positive and Negative Syndrome Scale,Hamilton Depression Scale,Young Mania Rating Scale,and Yale-Brown Obsessive-Compulsive Scale were used to assess mental disorder severity.Correlation and multivariate logistic regression were analysed between adverse childhood experiences and clinical features.Results Levels of adverse childhood experiences were significantly different among different mental Disorders.Moreover,25.8%of patients with substance use Disorders reported childhood trauma,which was significantly higher than found in the other four psychiatric disorder groups.Emotional abuse scores were positively correlated with disease severity:the higher the total trauma score,the more severe the mental disorder.Conclusions Adverse childhood experiences are a common phenomenon in those with mental Disorders,and the level of trauma affects mental disorder severity.Emotional abuse is closely related to many mental Disorders.The incidence or severity of mental Disorders can be reduced in the future by reducing the incidence of adverse childhood experiences or by timely intervention in childhood trauma.

Childhood maltreatment(CM)has been consistently linked with numerous detrimental outcomes concerning physical and psychological health.However,few studies have explored vulnerability to neurological Disorders after CM.Early life adversity,in the form of poverty,stress and abuse,has been associated with decline in cognitive function and dementia later in life(Short and Baram,2019).Robust preclinical data suggest that early life stress(ELS)may increase the risk and worsen the course of neurological Disorders such as Alzheimer’s(AD)and Parkinson’s(PD)diseases,and traumatic brain injury(TBI)(Lesuis et al.,2018;Short and Baram,2019;He et al.,2020;Catale et al.,2021;Sanchez et al.,2021).

Augmentation of transforming growth factor-βsignaling for the treatment of neurological Disorders

Members of the transforming growth factor-β(TGF-β)superfamily perform a wide range of essential functions during development and in adulthood,as well as in response to injury and inflammation(Luo,2022).In the adult central nervous system,TGF-βs and their receptors are widely expressed in all of the major neuronal,glial,and vascular cell types.Members of the TGF-βsuperfamily are pivotal responders to pathological insults to the brain.Dysfunction of TGF-βsignaling contributes to pathogenesis of neurological Disorders.Manipulation of TGF-βsignaling pathway alters pathological and functional outcomes in models of neurological diseases(Luo,2022).Therefore,the TGF-βsignaling pathway has emerged as a potential therapeutic target against neurological diseases.

Use of edaravone to decrease perioperative neurocognitive Disorders in elderly patients with hip replacement

To the Editor:By a randomized controlled trial,Xi et at[1]assessed the effects of edaravone on the development of postoperative delirium(POD)and perioperative neurocognitive Disorders(PND)in elderly patients with hip replacement and showed that edaravone significantly decreased the incidence of POD within 7 days after surgery and the incidences ofPNDat 1 and 12months after surgery.Given that both PODand PND are common complications after hip surgery in elderly patients and have been significantly associated with postoperative death,hospital-acquired complications,persistent cognitive impairments,poor postoperative functional recovery,prolonged duration of hospital stay,and increased healthcare costs,[2,3]their findings have potential implications.However,we noted several issues of this study on which we would like to invite authors’comments.
